OCT Bests Angiography Guidance for Improving PCI Outcomes

An 'optical biopsy' of intravascular plaque during stenting is more effective at reducing the rate of restenosis or thrombosis in people with moderately or severely calcified lesions than angiography guidance, a substudy of the ILUMIEN IV clinical trial has found.

'We found that in those calcified lesions there was a marked benefit of optical coherence tomography [OCT] guidance compared with angiography guidance in improving long-term outcomes, specifically in reducing the 2-year rate of target vessel failure and serious adverse cardiovascular events, as well as stent thrombosis,' study leader Gregg Stone, MD, director of academic affairs for Mount Sinai Heart Health System in New York City, told Medscape Medical News.
Results of the analysis were published in the European Heart Journal .
Substudy Results
The study included 2114 patients from the ILUMIEN IV clinical trial, which compared OCT- and angiography-guided percutaneous coronary intervention (PCI). The participants in the substudy had a single treated calcified lesion in a major epicardial vessel. On angiographic guidance, 1082 patients (51.2%) had a moderately or severely calcified lesion and 1032 (48.8%) had no or mild calcification.
Using angiographic guidance alone, the rate of target vessel failure at 2 years in the moderately or severely calcified lesions was 93% greater than that in the lesions with no or mild calcification (9.7% vs 5.2%).
However, with OCT guidance, the 2-year rates of failure were similar in both groups, 6.8% in the moderately and severely calcified lesions and 7.7% in the no or mildly calcified lesions.
With OCT guidance, the 2-year failure rates were 38% lower compared with angiography guidance in the patients with moderately and severely calcified lesions: 6.8% vs 9.7%. The differences in the patients with no or mild calcification were not statistically significant, the substudy found: 7.7% with OCT guidance and 5.2% with angiographic guidance ( P = .01).
The three major predictors of vessel failure after PCI, Stone said, are minimal stent area and a major dissection or untreated disease at the edge of the stent. 'The real advantage of optical coherence tomography is that it provides a much more accurate picture than angiography in determining the minimal stent area or in detecting edge-related dissections or significant untreated disease,' he said.
The 2024 European Society of Cardiology guidelines recommend intravascular imaging as a Class IA indication for anatomically complex lesions, Stone noted; but those guidelines do not specifically call out calcified lesions. 'That's the major gap this study fills,' he said.
The American College of Cardiology, in conjunction with the American Heart Association and the Society for Cardiovascular Angiography and Intervention, this year issued a Class 1A recommendation for intracoronary imaging, including OCT, during stenting of the left main artery or for other complex lesions.
'Directly Relatable to the Clinic'
While OCT operators need a high level of skill to perform the imaging and interpret the results, the substudy findings are 'quite generalizable to all settings,' Stone said.
'These findings are directly translatable to the clinic, but there's always more fine tuning that can be performed,' he added. That refining would include clarification of the characteristics of calcified plaque on OCT that are at highest risk for target vessel failure, and possibly the need for an advanced strategy of lesion preparation, using either lithotripsy, atherectomy or cutting or scoring balloons, he said.
Several calcium scoring classifications for OCT have been proposed, but none has been clinically validated. 'That would be a rich area for future research,' Stone said.

The new data underscore the value of intravascular imaging, and add to the data supporting OCT specifically, for characterizing plaque in percutaneous coronary intervention, Yader Sandoval, MD, an interventional cardiologist at the Minneapolis Heart Institute at Abbott Northwestern Hospital and cochair for the Center for Coronary Artery Disease at the Minneapolis Heart Institute Foundation, Minneapolis, told Medscape Medical News .
'This analysis of ILUMIEN IV complements and further adds that this is the right direction that intravascular imaging not only provides superior technical results but it improves outcomes for these patients,' Sandoval said.
The substudy clears up a finding from the original ILUMIEN IV study that, while OCT guidance resulted in a larger minimal stent area than angiography guidance, it did not necessarily reduce target vessel failure at 2 years.
A strength of the ILUMIEN IV trial was the 'meticulous core laboratory data analysis' used, along with its global scope, he said.
The substudy, also reported using OCT guidance, took about 16 minutes longer per procedure than angiography (67.7 minutes vs 51.5 minutes) and required longer fluoroscopy duration and greater total radiation dose, Sandoval said, which could raise questions about the practicality of OCT for this indication.
'The main message is, you are better characterizing calcium and therefore better preparing the lesion and providing a better technical result, better area, and better results for the patient,' he said. 'That comes with the investment that the procedure is little more complex, and you have to do a better job of preparing the lesion. And to me these data elegantly show that.'

On July 4, 2006, NASA's space shuttle Discovery launched on a "return to flight" mission that paved the way for it and its sister ships to fly for another five years. Now, a sprawling budget enacted on Independence Day will seemingly lead to Discovery lifting off again — though this time not into space, but rather from its place in the national collection. President Donald Trump signed into law the so-called "One Big Beautiful Bill" today (July 4), a day after the legislation was narrowly passed out of Congress with only Republican support. Deep within the 900-page bill is a provision added by Texas' senators to transfer a "space vehicle" to a NASA center "involved in the administration of the Commercial Crew Program" and "placed on public exhibition at an entity within the Metropolitan Statistical Area where such center is located." The vague language, written in such a way to skirt Senate restrictions on reconciliation bills, was aimed at achieving the "Bring the Space Shuttle Home Act" introduced by Senators Ted Cruz and John Cornyn in April. "It's long overdue for Space City to receive the recognition it deserves by bringing the space shuttle Discovery home," said Cornyn in a statement released after the Senate passed its version of the bill in a vote of 50 to 50, with Vice President J.D. Vance breaking the tie. "Houston has long stood at the heart of America's human spaceflight program, and this legislation rightly honors that legacy," said Cruz, who chairs the Senate Committee on Commerce, Science, & Transportation. "It ensures that any future transfer of a flown, crewed space vehicle will prioritize locations that have played a direct and vital role in our nation's manned space program, making Houston, Texas, a leading candidate." "Bringing such a historic space vehicle to the region would underscore the city's indispensable contributions to our space missions, highlight the strength of America's commercial space partnerships, and inspire future generations of engineers, scientists, and pioneers who will carry our legacy of American leadership in space," he said. The bill allocates $85 million to move Discovery from the its display home of the past 13 years, the Smithsonian National Air and Space Museum's Steven F. Udvar-Hazy Center in Chantilly, Virginia, to Space Center Houston, the official visitor center for NASA's Johnson Space Center in Texas. "No less than $5 million" is earmarked for the transportation of the winged orbiter, with the remainder going towards the "construction of a facility to house the space vehicle." Per the bill, the move of space shuttle Discovery must be completed by Jan. 4, 2027. It does not stipulate how the orbiter should be moved. It is not clear if the $85 million will be enough to cover the transfer and display, given that the first time the retired shuttles were brought to their museum homes in 2012, the cost for just the preparation and delivery of each vehicle was $28.8 million, which did not include ground transportation to the museum. Discovery is the United States' most flown spacecraft in history, with 39 missions between 1984 and 2011. In the process of retiring the shuttle fleet, Discovery was identified by NASA as the "vehicle of record," such that it was kept more intact than Atlantis or Endeavour for the purpose of serving as engineering example at the Smithsonian. Along with many other delights, springtime brings the beginning of the asparagus growing season. Regardless of whether you prefer the green, purple or white variety, asparagus provides a rich source of vitamins and minerals, and its consumption as part of a healthy diet may reduce risk of cancer and cardiovascular-related diseases. Despite the nutritional benefits of asparagus, many are opposed to eating the vegetable due to its pungent aftereffects. As Benjamin Franklin wrote in 1781, 'A few stems of asparagus eaten, shall give our urine a disagreable odour.' This odor has become so well known that post-consumption urine is now often referred to as 'asparagus pee.' Scientists believe the odor in question is due to two chemicals: methanethiol and S-methyl thioester. When enzymes in the human digestive tract break down the asparagusic acid that's naturally present in the vegetable, these volatile compounds are created. When voided from the body, they become foul-smelling gas, wafting up from your asparagus pee. And just because you don't smell it doesn't mean you're not making it. Two studies have shown that people who are unable to smell the odor in their own urine also don't detect it in the urine of known producers. Yes, volunteers sniffed samples of other people's asparagus pee. Though most everyone probably produces the scent to some degree, it seems not everyone's noses pick up on it. In fact, a study my colleagues and I conducted in 2017 found that only 40 percent of those surveyed reported detecting the odor in their urine. A lower proportion of women were able to detect the odor, compared to men, despite women being thought to have a more keen sense of smell. We asked almost 7,000 participants from two large cohort studies to respond to the prompt 'After eating asparagus, you notice a strong characteristic odor in your urine.' By linking the questionnaire data with genetic data, we were able to show that the ability to smell or not to smell depends on a person's genetic makeup. Hundreds of variants in the DNA sequence across multiple genes involved in sense of smell are strongly associated with the ability to detect asparagus metabolites in urine. Asparagus isn't the only food that has genetically linked controversial smell or taste effects. Some people avoid eating cilantro because they claim it has a 'soapy' aftertaste. A study using data from almost 30,000 users of 23andMe found genetic variants in olfactory receptors linked to people's perception of this adverse taste.
