What you need to know about emergency medical care in New Zealand

Frontline ambulance crews responded to more than 450,000 incidents nationwide in 2024, according to latest data compiled by Hato Hone St John.
But do users need to pay for an ambulance if one is called to transport them to an emergency medical centre?
And why does it sometimes take so long to see a doctor after arriving at an emergency department?
In a medical emergency, it's good to know the answers to these questions and more ahead of the incident.
Here's what you need to know about New Zealand's emergency healthcare system.
Police advise people in a medical emergency to call 111 and ask for an ambulance if either someone has:
Communications centres located in various parts of the country receive 111 emergency calls and coordinate the deployment of air or ground resources.
In the event of a medical emergency, the communications centre will assign the most appropriate emergency unit to the scene.
In most parts of New Zealand, ambulance services are provided by Hato Hone St John. In Wellington, they are offered by Wellington Free Ambulance.

It's important to note that New Zealand's emergency system is primarily supported by the public healthcare system.
While some private hospitals do offer emergency medical services, insurance providers sometimes exclude emergency costs incurred at private facilities.
Emergency departments typically do not provide non-urgent medical treatment.
However, if a patient or injured person is near an emergency department and the condition is critical, they can be taken directly to the facility without waiting for 111 dispatch.
Critical conditions include such things as heavy bleeding, broken major bones, bad burns, chest pain, issues related to breathing or consciousness, mental health emergencies, severe allergic reactions and injuries due to an accident (for example, a car crash).
If an injured or ill person is a long way from an emergency department, police advise people to call 111 for an ambulance.
Yes - and it's not cheap.
For example, Hato Hone St John reports that each emergency deployment costs around $1000.
However, users of the service are not charged the full amount.
Citizens, residents or those with work visas exceeding two years are charged $125 per callout.
For ambulance calls responding to an injury, whether a patient pays depends on the timing the service is provided.
If the service is used within 24 hours of injury, Accident Compensation Corporation covers the cost.
If more than 24 hours have passed, the patient bears the cost.
It's worth noting that you might be charged a callout fee if someone else calls the ambulance on your behalf.
For overseas visitors or short-term visa holders who are not eligible for public healthcare, callout charges can be as high as $800.
Sometimes people question why they don't receive immediate treatment upon arrival at the emergency department.
Upon arrival, emergency departments use a triage scale to assess and prioritize patients based on urgency.
In New Zealand, emergency departments follow the Australian triage scale , which has five levels, with level one being the most critical and level five the least.
Level five includes non-urgent issues, sometimes even administrative matters (e.g. consultations, prescriptions), and wait times can be as long as two hours.
Level one includes life-threatening conditions that require immediate attention and take top priority.
Additionally, there is a distinction between "emergency care" and "urgent care".
Emergency care focuses on life-threatening conditions and often involves trauma cases (surgical).
Urgent care is for less critical medical issues, with some clinics equipped to provide this service.
How much one pays to receive emergency care depends on whether one qualifies for public healthcare.
National health services directory Healthpoint says emergency care fees for short-term visa holders start at $566.72 per visit. Stays exceeding 10 hours will cost short-term visa holders at least $1768.47.
Additional charges may also apply depending on the treatment provided, Healthpoint says.
Emergency departments prioritize saving lives.
Medical staff cannot verify a patient's public healthcare status during treatment.
If a patient is ineligible, they will receive an invoice for the medical care they receive after treatment.
Starship Hospital in Auckland provides a dedicated pediatric emergency department for children aged 0-14, although sometimes older teenagers with acute conditions may also be treated there.
In other cities, larger hospitals usually have specialized pediatric services within their individual emergency departments.
Emergency dental care is available, but not through an emergency department in most cases.
Instead, dental clinics handle this type of care separately.
Many offer after-hours emergency services, but routine dental treatment should be arranged with your regular dentist.
However, if an accident causes a dental, oral, or gum injury, the injury may be treated during a visit to a regular emergency department.
Some translation services are available at many hospitals nationwide.
Upon arrival at a hospital, patients or injured people should ask staff to request assistance from an interpreter or arrange translation services online or by phone.
If a person is ill or injured but does not need emergency care, Health New Zealand advises them to visit an urgent care clinic or consult their GP at the earliest appointment.
They can also contact Healthline for free advice on 0800 611 116.
Additionally, a pharmacy can provide advice and support for minor injuries.

Photo: 123rf Researchers have found that cardiovascular hospitalisations rise after tropical cyclones. Researchers at Monash University in Melbourne and Otago University assessed hospital data from Canada, New Zealand, South Korea, Taiwan, Thailand and Vietnam from 2000-19, as well as recorded data from 124 cyclones. They found hospitalisations for cardiovascular disease (CVD) tend to rise, with this rate peaking at 2 months after the event, and the increase lingered up to 6 months after a tropical cyclone. The team also looked at variables, including sex, age, socioeconomic status and geographic location. Results showed a statistically significant and prolonged association between cyclones and CVD hospitalisation for people of all sexes aged 20-59. The team also evaluated six different types of CVD based on hospital diagnostic codes - heart failure, ischemic heart disease, hypertensive heart disease, heart rhythm disturbances, peripheral vascular diseases and stroke. Ischemic heart diseases and stroke were the two most common subsets of cyclone-associated CVD, and the latter was especially common in males of all ages. Areas with higher levels of socioeconomic deprivation had higher rates of cyclone-associated CVD hospitalisations, but the opposite was the case in areas with stronger socioeconomic advantages. "The study demonstrates that, as well as directly causing immediate deaths and injuries, extreme climate events can have important indirect health impacts over following months," said Professor Simon Hales, an epidemiologist at the University of Otago and one of the study's authors. "These delayed, indirect health impacts are due to infrastructure damage and disruption to livelihoods in the aftermath of major storms. "It is important to understand these health effects and mechanisms in more detail in order to design adaptive responses, as well as to motivate climate mitigation measures. "Cyclones, storms and heavy rainfall events are projected to become more frequent, unless stronger efforts are made to reduce emissions of greenhouse gases," he said. Associate Professor George Laking - executive board member of OraTaiao, the New Zealand Climate and Health Council - said similar work was done after Cyclone Gabrielle for populations in Tai Rāwhiti and Hawke's Bay. "We weren't able to detect that signal in our own project. I think that is explained by the smaller populations we studied. "In this report, there is a higher level of uncertainty in the New Zealand country-specific data. I think this represents our overall smaller population. "Interestingly, the strongest signal in New Zealand was for the north of the South Island, not so strong for the East Coast North Island regions we studied." Laking said he had waited for this study, showing the adverse interaction of cyclones and cardiovascular health. "Under climate change, we have to expect an increased incidence and severity of adverse weather events. That is a consequence of the greater amount of energy and water in the atmosphere. "These incredibly destructive events are adverse for human health in all sorts of ways." Laking said climate change had been identified as the No.1 risk to human health this century. Photo: LDR / Supplied Blenheim seniors are flocking to Marlborough Lines Stadium 2000 and staff are starting to worry there may not be room for them all. Even first thing on a Monday, the changingrooms are packed before the Aquacise class, a hot favourite among the older demographic. "Don't suggest that people join our class, because there's no room," one participant said, only partly joking. Senior use of the aquatic, fitness and sport centre was expected to keep growing until 2038, while other demographics were expected to plateau, a report from the Marlborough Stadium Trust said. Seniors went to the stadium 25,555 times in the 2023/24 year, up 249 percent from 7333 visits in 2013/14. Aquacise instructor Alison 'Janey' Phipps said at least 50 people or even up to 90 people attended her class, which started at 9am, three times a week. An Aqua-gentle class ran twice a week. "I just love this demographic of people," Phipps said. "They just get on with it, they're just in there and I really like that." Aquacise was really good for seniors, as it helped with mobility, injury recovery and fall prevention, as well as just being fun, she said. "I really like to focus on balance and hip mobility. Working into the hip joint, just for trips and falls, getting that strength in the legs, so that they can correct quickly." As classic tunes from the 80s boomed through the aquatic area, Phipps had the group doing leg, arm and core exercises in a 45-minute workout that was not for the faint of heart, using pool noodles and foam dumb bells. An 80-year-old participant, having her ritual coffee after the class at the stadium cafe with the other participants, said she was dragged along by a friend, while she was recovering from an injury. "I was still on crutches and I was at the back of the pool, and I've gradually got myself fit enough and now I'm there doing it all." Aquacise class help seniors with mobility and fall prevention. Photo: LDR / Supplied Classes offered more than simply exercise, she said. "Don't discount all the social stuff. A lot of the people who come, it's the only people they see all day. "They come and they swim, and they talk to the people and they live alone. That one bit of socialisation they can get is fantastic." Her friend had joined after moving to Blenheim from Wellington. The stadium's pool facilities was one of the reasons she chose the town, she said. "It's a great facility. A lot of councils don't have the same facility to provide these services." All the demand had put the facilities under considerable strain, stadium trustee Anna Houkamau told the Marlborough District Council's assets and services committee on 15 July. "We have up to 90 seniors in some of the exercise classes in the mornings and associated capacity issues with things like changing," Houkamau said. The basketball and netball courts were also at capacity, particularly due to their use by groups, with not enough space to hold tournaments. The trust's new '10 + 10 committee' was working on a plan to future-proof the stadium and part one involved assessing the community's needs, likely working with the council to do that, she said. "[It's about] ensuring that our existing asset is in good form for the next 10 years and 10 beyond that," Houkamau said. "Step one is to be able to understand what we could afford to maintain in the future." In 2013, the council bought the neighbouring buildings at the corner of Redwood and Kinross streets, and leased them to Smith's City and the Marlborough Youth Trust's MySpace centre. The aim of the purchase was to ensure the stadium could expand in the future, if demand warranted it. Council's group manager for property and community facilities Jamie Lyall said it was now proposed for those buildings to be integrated into the stadium complex, but any decision to transfer the buildings to the trust would be done in collaboration with the Marlborough Youth Trust. - LDR is local body journalism co-funded by RNZ and NZ On Air

Photo: RNZ Health NZ is promising bonus payments to senior doctors who work in hard-to-staff areas, although the union is accusing it of failing to consistently pay allowances now. The Association of Salaried Medical Specialists, which represents 5500 senior hospital doctors and dentists, has criticised Te Whatu Ora's new offer to its members as amounting to "an effective pay cut" for 90 percent of them. Union head Sarah Dalton was also sceptical of Health NZ's proposal to ring-fence $32 million over two years for an incentive to doctors to work in areas or specialities with particular shortages . The agency was already paying $384m a year on contingency staffing which was "driving people out of salaried into locum work and temporary gigs", and current systems for paying allowances were ad hoc and "shambolic", she said. However, Health NZ's industrial relations lead, Robyn Shearer, said incentive funding showed the agency was listening to what the union had to say about the need to retain staff in those areas. Specific details on how to allocate that money were yet to be agreed, she said. "We need to get round the table and work ... through that in the bargaining process. "We are definitely keen to work on better consistency - there's a big piece of work around application of allowances up and down the country, and we're keen to work with the union on that." Health NZ would "much rather" employ permanent staff than have to find locums, she said. "But there are good reasons for employing locums, including covering staff sickness and unfilled posts. Photo: RNZ Insight/Karen Brown Should Health NZ's offer be accepted, first-year specialists would get an extra $17,000 in their pay packets over the 21-month term, and second year specialists more than $11,000. "Senior doctors at the top of the pay scale would receive an increase to base pay of more than $9000 to more than $277,000 over the course of the proposed settlement.
