
Air India Replaced Crashed Plane's Throttle Control Module Twice In 6 Years After Boeing Directive
The Throttle Control Module (TCM) of the ill-fated Air India plane, which crashed near the Ahmedabad airport on June 12, was replaced by the airline twice in the last six years, following a directive from Boeing in 2019, news agency PTI quoted sources as saying.
The TCM includes fuel control switches, the ones that feed fuel into the aircraft's engines, the focus to which was shifted in the ongoing probe into the crash.
The preliminary report on the crashed Air India plane, released by the Aircraft Accident Investigation Bureau (AAIB) on Saturday, noted that the TCM was replaced in 2019 and 2023.
However, it also stated that the TCM replacement was not linked to the fuel control switches.
According to PTI, sources said that a revised maintenance planning document (MPD) was issued by Boeing for all the operators of the Dreamliners in 2019, according to which, the operators have to change the TCM every 24,000 flight hours.
Specific details about the MPD could not be immediately ascertained, the report stated.
It also quoted an Air India spokesperson as saying that the airline was working closely with stakeholders, including regulators.
'We continue to fully cooperate with the AAIB and other authorities as their investigation progresses. 'Given the active nature of the investigation, we are unable to comment on specific details and refer all such enquiries to the AAIB," the spokesperson said.
AAIB, in its report, mentioned that at this stage of the investigation, 'there are no recommended actions to B787-8 and/or GE GEnx-1B engine operators and manufacturers".
Air India's Dreamliner VT-ANB, which crashed on June 12, was powered by GEnx-1B engines.
According to the report, the FAA issued a Special Airworthiness Information Bulletin on December 17, 2018, regarding the potential disengagement of the fuel control switch locking feature.
'This SAIB was issued based on reports from operators of Model 737 airplanes that the fuel control switches were installed with the locking feature disengaged. The airworthiness concern was not considered an unsafe condition that would warrant an airworthiness directive by the FAA," it said.
AAIB also said the fuel control switch design, including the locking feature, is similar on various Boeing airplane models including part number 4TL837-3D, which is fitted in B787-8 aircraft VT-ANB.
'As per the information from Air India, the suggested inspections were not carried out as the SAIB was advisory and not mandatory. The scrutiny of maintenance records revealed that the throttle control module was replaced on VT-ANB in 2019 and 2023."
'However, the reason for the replacement was not linked to the fuel control switch. There has been no defect reported pertaining to the fuel control switch since 2023 on VT-ANB," the report stated.
After being in cut-off mode, the fuel control switches of the aircraft's two engines were turned on later, but the London-bound aircraft could not get enough thrust and altitude before it crashed into a building in Ahmedabad, killing 260 people.

Air India finds 'no issues' with Boeing 787 fuel control switch locks
Air India on Wednesday night informed its pilots that precautionary inspections of the fuel control switch locking system on all its Boeing 787 aircraft had been completed with no issues found, adding that the throttle control modules on all these planes had already been replaced earlier as per Boeing's maintenance schedule. Two days after the release of a preliminary probe into the AI171 crash, India's aviation regulator, the Directorate General of Civil Aviation (DGCA), on July 14 directed all airlines operating Boeing 737 and 787 aircraft to inspect the locking mechanism of fuel control switches and submit their findings by July 21. The preliminary report of India's Aircraft Accident Investigation Bureau (AAIB) found that on the night of the June 12 crash, both fuel control switches on Air India flight AI171 transitioned from RUN to CUTOFF just seconds after take-off, leading to a dual engine failure and subsequent tragedy. The report did not explain the reasons for this switch movement. In its internal message, Air India said: 'Over the weekend, our Engineering team initiated precautionary inspections on the locking mechanism of Fuel Control Switch (FCS) on all our Boeing 787 aircraft. The inspections have been completed and no issues were found.' It added: 'All our Boeing 787-8 aircraft have also undergone Throttle Control Module (TCM) replacement as per the Boeing maintenance schedule. The FCS is part of this module.' The fuel control switch, located on the aircraft's centre console, controls the fuel supply to the engines. Its locking mechanism is meant to prevent accidental movement from the 'RUN' to 'CUTOFF' position. A disengagement of this lock could result in inadvertent engine shutdown. The airline also advised pilots to remain vigilant: 'Please continue to remain vigilant and report any defect in the Technical Log, as per the existing reporting process. The Coruson tool is also available, should there be any concerns identified during operations.' Business Standard had earlier reported that several Indian airlines had started inspecting this component after international attention turned to its design. Singapore Airlines and Etihad Airways began voluntary checks on their Boeing 787 fleets earlier this week, while South Korea's transport ministry said it would soon mandate similar inspections across Boeing aircraft in the country. According to the Indian carriers impacted by the fuel switch advisory include Air India (which has 57 Boeing jets), Air India Express (75), SpiceJet (29), IndiGo (10 leased Boeing aircraft), and Akasa Air (30). Of these, only Air India and IndiGo operate the Boeing 777, which uses a different fuel control switch and is not subject to inspection. The locking feature on the switch was the subject of a US Federal Aviation Administration (FAA) advisory in 2018 (SAIB NM-18-33), which warned that the switch could be installed in ways that disable the lock. Although not declared an 'unsafe condition' warranting a mandatory airworthiness directive, the FAA had recommended that airlines inspect the switches and replace any with disabled locking mechanisms. Honeywell is the manufacturer of these switches. In recent days, regulators and airlines have taken fresh interest in the switch's design and reliability, prompting precautionary inspections worldwide. The FAA advisory had highlighted the potential for 'unintentional fuel shutoff' if the locking mechanism was not engaged.

Safety notices raised alerts on B787 fuel valves, switches
KOLKATA: A day after the Directorate General of Civil Aviation belatedly ordered checks on the fuel control switches of Boeing aircraft, including B787 and B737, based on a Special Airworthiness Information Bulletin (SAIB) NM-18-33 issued by US regulator Federal Aviation Authority (FAA) on December 17, 2018, another Airworthiness Directive (AD) issued by FAA on fuel shutoff valves on September 19, 2015, has come to light. This AD, which required mandatory compliance, was flagged again by the UK Civil Aviation Authority (CAA) a month later, issuing a safety notice requiring airlines to mandatorily check fuel shutoff valves on Boeing aircraft. It specifically mentioned that while fuel shutoff valves on various Boeing aircraft, including B737, B757, B767, and B777, require inspection, those on B787 needed to be replaced. The AI plane that crashed on June 12 was a B787 Dreamliner. While the report mentions transitioning the fuel switch from ON to CUT OFF and back to ON, a section of pilots says this sequence was required to be carried out by pilots as per the Boeing manual in case fuel flow to the engines ceased. The 2015 AD, the 2019 SAIB, as well as the reflagging of the valve problem by CAA, point to an inherent issue with the throttle control module used in B787 aircraft and not just the fuel switches hinted at in the crash preliminary report. The CAA safety notice (no. SN-2015/005) issued on May 15, 2025, less than a month before the June 12 AI crash, reiterates the 2015 AD issued by FAA. It reads: "...the fuel shutoff valve actuators on the B787 which require replacement, test and/or inspection..." "The Continuing Airworthiness Management Organization (CAMO) for operators of B787 aircraft should immediately make themselves aware of the relevant AD and establish whether it affects any aircraft... The operation check AD must be documented in the Operations Manual along with reference to the AD number," the notice added. Aviation safety experts and airline pilots said the AD is absolutely critical. An airline official said Indian carriers are mandated to follow the ADs issued by DGCA. The CAA AD requires compliance by UK airlines. They acknowledge that the AD issued by FAA in September 2015 needed to be mandatorily acted upon and said the DGCA issued the concerned AD in 2015. AI back then was under govt management. Airline sources said the necessary actions were taken. UK aviation regulator CAA issued safety notice (no. SN-2015/005) on May 15, 2025, requiring valve replacement and checks in B787 aircraft. The safety notice was based on an Airworthiness Directive (AD) issued on September 19, 2015, by the Federal Aviation Administration (FAA), US, addressing a potential unsafe condition affecting fuel shutoff valves installed on various Boeing aircraft. DGCA issued an AD on the fuel control valves in 2015 following the FAA alert. But there was no mention of it in the preliminary crash investigation report issued by the Aircraft Accident Investigation Bureau. Instead, the report mentioned an FAA Special Airworthiness Information Bulletin (SAIB) NM-18-33 dated December 17, 2018, that warned of the potential disengagement of the fuel control switch locking feature. This was not mandatory in nature and hence not acted upon, it stated. On July 14, 2025, DGCA issued a circular ordering checks of fuel control switches on B787 and B737 aircraft.

Manufactured by US defence giant Boeing, the Apache is currently operated by the armed forces of countries like the United States, the UK, Israel, Egypt, and now India. India first acquired 22 Apache helicopters for the Indian Air Force under a 2015 agreement. The six new ones being inducted are for the Indian Army's Aviation Corps. According to Boeing, the Apache is equipped with 'the latest communications, navigation, sensor, and weapon systems." These include an upgraded targeting system, called the Modernised Target Acquisition Designation System, that helps pilots identify and strike targets in both day and night conditions, and even in poor visibility due to rain, dust, or fog. In February 2020, the Indian government approved the procurement of six AH-64E Apaches in a deal worth around $600 million. This was in addition to the earlier IAF purchase. At the time, Boeing stated: 'This order will expand the Indian Army's capability to provide integrated combat aviation support to land forces." When Will The Apache Helicopters Arrive? According to NDTV, the initial batch of three Apache helicopters is scheduled to arrive next week. They will be delivered to the Indian Army at Hindon Air Force Station, where they will undergo a joint inspection by Boeing, US officials, and Indian authorities.
