
Study Finds Link Between IL-6 and Diabetic Eye Disease
The prospective, single-center study, conducted at the Vanderbilt Eye Institute in Nashville, Tennessee, included 328 eyes of 164 people with diabetes, of whom 23 did not have diabetic retinopathy, 118 had moderate nonproliferative retinopathy, and 23 had proliferative retinopathy, a more severe form of the disease. Eyes without retinopathy had lower levels of IL-6 collected in the aqueous fluid than those with the condition.
A Potential Biomarker
'IL-6 is potentially a biomarker of diabetic eye disease severity and it's a novel target for therapy,' said Stephen J. Kim, MD, chief of the Retina Division at Vanderbilt Eye Institute. 'We can start focusing on inhibiting IL-6; that's the breakthrough, that we now have an inflammatory cytokine that we can use to follow disease, that correlates with disease severity, and we can now inhibit it.'

Kim and his colleagues published the results of the study, called INSPIRE, on June 27 in Ophthalmology Retina .
The study found that the median level of IL-6 was 5.4 pg/mL (2.99-877) in eyes with no retinopathy, 9.25 pg/mL (5.35-22.35) in eyes with moderate nonproliferative retinopathy, and 15.71 pg/mL (9.24-45.58) in eyes with proliferative disease ( P < .001).
The study also found no difference in median A1c between the three groups ( P = .03). 'There was no correlation between IL-6 and A1c in our study, and that's not surprising because A1c does not correlate very well with risk of progression in diabetic eye disease,' Kim said.
Previous studies implicating IL-6 in diabetic eye disease were small, retrospective studies in which the role of IL-6 was a secondary finding. INSPIRE claims to be the largest analysis of aqueous IL-6 in diabetic eyes.
'The value of this study is that this was a prospective, rigorously assessed, meticulous study,' he said.
A limitation of the study is that it makes two assumptions, Kim said: Aqueous levels of IL-6 are commensurate with levels produced in retina, acting as a surrogate marker; and the study's cross-sectional nature. 'We're not showing a cause and effect that IL-6 levels caused diabetic retinopathy or diabetic macular edema,' Kim said. 'We're just showing a very strong relationships between the two that's suggestive of cause and effect.'
INSPIRE is a 3-year study, and the opportunity exists to demonstrate cause and effect, Kim said.
The finding is 'very practical' because four IL-6 inhibitors are commercially available in the US: tocilizumab (Actemra), indicated for rheumatoid arthritis (RA) and other diseases; sarilumab (Kevzara) for RA; siltuximab (Sylvant) for HIV; and satralizumab (Enspryng) for neuromyelitis optical spectrum disorder. 'This can translate very rapidly,' Kim said.
'This is a potential new treatment to a huge unmet need, and IL-6 may also be a biomarker that we can use to follow not only someone's disease severity but to predict how they will do in 12 months or 24 months,' Kim added.
'Pioneering' Step
Thomas W. Gardner, MD, MS, a professor of ophthalmology and internal medicine at the Kellogg Eye Center at the University of Michigan in Ann Arbor, Michigan, said other studies have shown elevated inflammatory markers in the vitreous or aqueous fluid in both human and experimental models. 'What has been lacking is a prospective, longitudinal study using human samples and correlating those findings with the disease state,' he said. 'This study is important in being pioneering in that step.'

Genentech, a subsidiary of Roche, is developing an IL-6 inhibitor, vamikibart, for ophthalmic use, he noted.
While obtaining aqueous fluid from the eye can be 'easy and safe,' Gardner said, handling and processing the samples is fraught with uncertainty. 'How individual ophthalmologists would have those samples analyzed is something that has to be established,' he said. 'But that's a minor issue.'
Gardner called INSPIRE 'a big first step. It isn't necessarily an answer, but it is important for the proof of principal underlying being able to faithfully analyze a molecule such as IL-6 and to incentivize more studies of this type.'

Three sharks fitted with sensors are swimming in warm Atlantic Ocean waters to collect critical hurricane data, a contrast to the National Oceanic and Atmospheric Administration's usual flying Hurricane Hunters in the skies. 'The ocean is so huge, so enormous, that it's just inaccessible to anything, for the most part,' Aaron Carlisle, a University of Delaware marine ecologist leading the effort, told The Washington Post. 'But by instrumenting the animals that live out there,' he said, 'you can basically turn them into these ocean sensors that are constantly collecting data.' The sharks are collecting information on water conductivity and temperature. Record sea surface temperatures have fueled particularly large and strong hurricanes in recent years. The temperatures are the result of a warming planet and human-caused climate change. It's unclear whether the sharks will ever get close to the cyclones. But, by monitoring the temperature, the scientists can better understand what the U.S. is in for each hurricane season, including where the hurricanes will go and if they're supercharged. Sharks, a keystone species and an apex predator, have a unique access to data that has been hard to get. Weather satellites are unable to see past the ocean's surface and the robotic gliders that scientists send to the continental shelf are effective but slow and expensive to maintain. The tags on the sharks have the ability to collect that data more efficiently. Two mako sharks are tagged to measure temperature, depth, and conductivity. A white shark has a satellite tag to help evaluate if the species could be a good candidate for similar tagging in the future. They may also test hammerheads and whale sharks. 'Sharks are faster than [robotic] gliders. They can stay out for longer periods of time,' Caroline Wiernicki, a shark ecologist and Ph.D student working with Carlisle, told The Post. 'So the hope is that we can have these sharks go out and work in concert' with existing monitors, she said. The research is being led by Carlisle and fellow University of Delaware professor Matt Oliver. They are working with the NOAA's Mid-Atlantic Regional Association Coastal Ocean Observing System – the regional arm of the agency's Integrated Ocean Observing System program. In the future, the plan is to tag dozens of sharks a year and feed that data into hurricane computer models. So far, Carlisle told The Post that one of the two sharks has relayed temperature data back to them, but the other has been swimming in water too shallow for the sensor to turn on. The researchers said that they chose the makos because they often return to the surface, allowing the tags to send the data to satellites for the scientists to retrieve. Able to reach swimming speeds of over 40 miles per hour, shortfin makos are the fastest sharks in the ocean. Following a review, NOAA said in 2022 that it would not list them as a threatened or endangered species. They are listed as endangered by the International Union for Conservation of Nature. Right now, one in three species of sharks and rays are threatened with extinction. Overfishing has driven global shark and ray numbers down by more than 70 percent since the 1970s, according to the International Fund for Animal Welfare. The charity notes that humans kill around 190 sharks per minute and 100 million sharks each year in commercial fisheries. The researchers explained to USA Today that they did not expect the sensors to have much harmful impact on their test subjects and that they undergo a thorough permitting and review process. As the ADA turns 35, groups fighting for disability rights could see their federal dollars slashed
TOPEKA, Kan. — Nancy Jensen believes she'd still be living in an abusive group home if it wasn't shut down in 2004 with the help of the Disability Rights Center of Kansas, which for decades has received federal money to look out for Americans with disabilities. But the flow of funding under the Trump administration is now in question, disability rights groups nationwide say, dampening their mood as Saturday marks the 35th anniversary of the landmark Americans with Disabilities Act. Federal dollars pay for much of their work, including helping people who seek government-funded services and lawsuits now pushing Iowa and Texas toward better community services. Documents outlining President Donald Trump's budget proposals show they would zero out funds earmarked for three grants to disability rights centers and slash funding for a fourth. Congress' first discussion of them, by the Senate Appropriations Committee, is set for Thursday, but the centers fear losing more than 60% of their federal dollars. The threat of cuts comes as the groups expect more demand for help after Republicans' tax and budget law complicated Medicaid health coverage with a new work-reporting requirement. There's also the sting of the timing: this year is the 50th anniversary of another federal law that created the network of state groups to protect people with disabilities, and Trump's proposals represent the largest potential cuts in that half-century, advocates said. The groups are authorized to make unannounced visits to group homes and interview residents alone. 'You're going to have lots of people with disabilities lost,' said Jensen, now president of Colorado's advisory council for federal funding of efforts to protect people with mental illnesses. She worries people with disabilities will have 'no backstop' for fighting housing discrimination or seeking services at school or accommodations at work. The potential budget savings are a shaving of copper from each federal tax penny. The groups receive not quite $180 million a year — versus $1.8 trillion in discretionary spending. The president's Office of Management and Budget didn't respond to an email seeking a response to the disability rights groups' criticism. But in budget documents, the administration argued its proposals would give states needed flexibility. The U.S. Department of Education said earmarking funds for disability rights centers created an unnecessary administrative burden for states. Trump's top budget adviser, Russell Vought, told senators in a letter that a review of 2025 spending showed too much went to 'niche' groups outside government. 'We also considered, for each program, whether the governmental service provided could be provided better by State or local governments (if provided at all),' Vought wrote. Disability rights advocates doubt that state protection and advocacy groups — known as P&As — would see any dollar not specifically earmarked for them. They sue states, so the advocates don't want states deciding whether their work gets funded. The 1975 federal law setting up P&As declared them independent of the states, and newer laws reinforced that. 'We do need an independent system that can hold them and other wrongdoers accountable,' said Rocky Nichols, the Kansas center's executive director. Nichols' center has helped Matthew Hull for years with getting the state to cover services, and Hull hopes to find a job. He uses a wheelchair; a Medicaid-provided nurse helps him run errands. 'I need to be able to do that so I can keep my strength up,' he said, adding that activity preserves his health. Medicaid applicants often had a difficult time working through its rules even before the tax and budget law's recent changes, said Sean Jackson, Disability Rights Texas' executive director. With fewer dollars, he said, 'As cases are coming into us, we're going to have to take less cases.' The Texas group receives money from a legal aid foundation and other sources, but federal funds still are 68% of its dollars. The Kansas center and Disability Rights Iowa rely entirely on federal funds. 'For the majority it would probably be 85% or higher,' said Marlene Sallo, executive director of the National Disability Rights Network, which represents P&As. The Trump administration's proposals suggest it wants to shut down P&As, said Steven Schwartz, who founded the Center for Public Representation, a Massachusetts-based organization that works with them on lawsuits. Federal funding meant a call in 2009 to Disability Rights Iowa launched an immediate investigation of a program employing men with developmental disabilities in a turkey processing plant. Authorities said they lived in a dangerous, bug-infested bunkhouse and were financially exploited. Without the dollars, executive director Catherine Johnson said, 'That's maybe not something we could have done.' The Kansas center's private interview in 2004 with one of Jensen's fellow residents eventually led to long federal prison sentences for the couple operating the Kaufman House, a home for people with mental illnesses about 25 miles (40 kilometers) north of Wichita. And it wasn't until Disability Rights Iowa filed a federal lawsuit in 2023 that the state agreed to draft a plan to provide community services for children with severe mental and behavioral needs. For 15 years, Schwartz's group and Disability Rights Texas have pursued a federal lawsuit alleging Texas warehouses several thousand people with intellectual and developmental disabilities in nursing homes without adequate services. Texas put at least three men in homes after they'd worked in the Iowa turkey plant. Last month, a federal judge ordered work to start on a plan to end the 'severe and ongoing' problems.
