Moderna's combo Covid and flu mRNA shot outperforms current vaccines in large trial

Moderna's combined Covid and flu shot outperformed the existing standalone vaccines for both viruses, according to the results of a phase 3 clinical trial published Wednesday in the Journal of the American Medical Association.
The vaccine uses the same messenger RNA technology as Moderna's approved Covid vaccine. (There are no approved mRNA-based flu shots.)
The trial, funded by Moderna, included more than 8,000 adults ages 50 and older who were enrolled in October and November 2023. For people ages 50 to 64, the new vaccine was compared to Moderna's Covid vaccine and the flu shot Fluarix. For people 65 and older, it was compared to the Covid vaccine and a different flu shot, called Fluzone, which is a stronger dose typically given to older adults. The trial participants either got the existing shots, or the new combination vaccine plus a placebo shot. (This way, both groups got two injections.)
The main goal of the trial was to see whether the combination vaccine generated more antibodies in blood samples than the current shots against multiple strains of influenza and the XBB.1.5 Covid variant. This so-called serological testing is a common way for researchers to gauge how well a vaccine shot might work and how long the protection might last.
The trial found that antibody levels were higher for Covid and all but one influenza strain in people who got the combination vaccine, compared to those who got the existing shots in both groups.
Dr. Buddy Creech, a pediatric infectious disease physician at Vanderbilt University Medical Center in Nashville, Tennessee, said the antibody levels generated by the new vaccine were about 20% to 40% higher.
Dr. Jesse Goodman, a professor of medicine and infectious disease at Georgetown University and former chief scientist at the Food and Drug Administration, said the trial results are a good sign that the combination shot will provide protection against both Covid and the flu.
'We know that antibody responses generally correlates with a degree of protection,' Goodman said. 'This is very promising and it suggests this vaccine should have similar or higher levels of protection to traditional flu vaccines.'
Side effects such as fever and chills were more common in people who got the new vaccine. Creech attributed that to how the body is responding to the vaccine.
'Immunologically, what's happening in each cell is they're acting as if they've been infected with both viruses, and so your immune system is going to respond and make the type of inflammation that you would if you had flu or Covid or both simultaneously,' he said.
A combination shot would make it easier for people to get vaccinated against both Covid and the flu, especially since both shots usually come out in the fall. A new shot that provides better protection is needed, experts say, because the existing vaccines don't do a good job at stopping infections.
The data is expected to be used to support Moderna's application to the FDA for approval. Last week, Moderna said it doesn't expect regulatory approval for its combination shot until 2026, after the agency said it required late-stage data showing the shot's efficacy against the flu.
It's unclear whether the agency's new rule that all new vaccines will need to undergo placebo-controlled clinical trials to gain approval will apply to the combination shot. The FDA already delayed Novavax's Covid vaccine, requiring an additional clinical trial, and may impose the requirement for Pfizer's and Moderna's updated Covid vaccines for the fall.
A Moderna spokesperson declined to comment. An HHS spokesperson did not immediately respond to a request for comment.
Dr. Peter Hotez, dean of the National School of Tropical Medicine at Baylor College of Medicine and co-director of the Texas Children's Hospital Center for Vaccine Development, noted that mRNA vaccines are a particular target among anti-vaccine activists, and several states have introduced legislation that would ban them.
'One of the things I'm concerned about, at least for the United States, is all the anti-vaccine sentiment, and this specific targeting of the mRNA vaccine platform by anti-vaccine activists,' he said.
Hotez said Moderna's combination shot would be 'a potentially very useful advance,' as it would only require people to take one shot for two respiratory virsuses.
'If this works, you could also use it for adding RSV or as well as other pathogens,' he said.
Goodman, of Georgetown University, said an mRNA-based shot that also targets the flu could help the U.S. respond more quickly if there were a pandemic caused by influenza. The current flu shots primarily rely on growing strains the of the virus in eggs, which is slower to make compared to the mRNA vaccines.

Karun Kaushik and Selin Kocalar weren't planning to raise a Series A so soon. Their AI compliance startup, Delve, which announced a $3 million seed round in January, was growing fast and signing customers at a steady clip. Then, inbound interest started rolling in, COO Kocalar told TechCrunch. Delve, which automates regulatory compliance with AI agents, ended up fielding multiple term sheets, eventually closing a $32 million Series A at a $300 million valuation. The round was led by Insight Partners, which took up most of the round, with participation from CISOs at Fortune 500 companies. Insight has been 'amazing to work with, and we felt they were the right long-term partner for us,' said the COO. Delve's new valuation represents a roughly 10x jump from its previous round. Similarly, its customer base has grown from the 100 companies it reported back in January to over 500, many of them fast-growing AI startups like recently minted AI unicorn Lovable, Bland, and Wispr Flow. And while the 2-year-old company, made up of AI researchers from MIT, Stanford, and Berkeley, has seemingly struck gold by using AI to eliminate hundreds of hours of manual processes, its story began much differently. Kaushik and Kocalar met as classmates during their freshman year at MIT. Both had deep interests in AI and health tech. Kaushik had already scaled a COVID diagnostic system to thousands of users during the pandemic. In 2023, they began working on an AI-powered medical scribe to help doctors handle patient documentation. However, handling sensitive healthcare information meant they quickly encountered the costly and time-consuming world of HIPAA compliance. Instead of continuing with the medical scribe, they started building tools to help other companies get HIPAA-compliant faster and more affordably. That pivot got the team into Y Combinator last year and helped them raise their seed round from General Catalyst, FundersClub, Soma Capital, and others. The founders dropped out during their sophomore year in 2023. What started with HIPAA quickly expanded. 'As our customer base grew, they started asking for support with other frameworks: SOC 2, PCI, GDPR, ISO, basically the whole alphabet soup of compliance,' Kocalar narrated. Compliance paperwork can be necessary in everything from launching products to closing enterprise deals. But instead of driving growth, its manual work can become a bottleneck. 'Compliance frameworks are standardized. Businesses aren't,' says CEO Kaushik. 'That mismatch is why traditional software breaks down and teams fall back to duct-taped workflows across email, Slack, and shared drives.' Delve replaces that busywork with AI agents that run in the background (after integrating with customers' tools) like internal team members. These agents collect evidence, write reports, update audit logs, and track configuration changes across fragmented systems, automating compliance workflows in real time. Kocalar says compliance is just the wedge into broader back-office operations. Long-term, the AI startup wants to automate a billion hours of other work — eventually expanding into adjacent areas like cybersecurity, risk, and internal governance. Insight Partners' interest reflects this roadmap. 'Since compliance touches every part of how a business runs, from scaling operations to closing deals to building customer trust, modernizing this function will modernize the entire organization,' said Praveen Akkiraju, managing director at Insight. 'That's what makes Delve's approach so important.' Still, the startup won't be without competition. Several AI companies are emerging with agents to automate business workflows. In addition, larger AI labs like OpenAI are releasing general-purpose agents capable of performing complex tasks. That said, Kocalar says these developments are a validation, not a threat, to Delve's business. She points to the company's domain depth in contrast to more general-purpose agents. 'We're positioning ourselves to improve as AI advances and labs roll out more sophisticated agentic technologies. But what truly sets us apart is the deep, domain-specific knowledge we're building into the platform,' she said. 'Compliance is always shifting as new regulations emerge and existing ones evolve, with companies interpreting them in different ways. That's where Delve stands out.' New data from the Centers for Disease Control and Prevention shows activity of COVID-19 is increasing across a wide swath of the country. As the U.S. approaches the midpoint of summer, the CDC is reporting cases of the virus are growing or likely growing in more than half of all states, including many in the South and Southeast. The center's July 18 report shows at least 26 states plus Washington, D.C., are encountering a summer surge, according to its epidemic trend modeling, using data from emergency department visits. The CDC notes its modeling indicates trends in overall cases, not the actual numbers of current infections. New COVID variant is spreading: What are the symptoms? A familiar summer surge The rate of positive COVID-19 tests is increasing nationally, the CDC said of the last week in data, with emergency department visits for the virus are increasing among children 4 years old and younger. Though there is an uptick in COVID-19 cases in more than two dozen states, the CDC says the overall amount of people seeking care for acute respiratory illnesses is at a very low level. That's accompanied by low levels of seasonal influenza activity and very low levels of RSV activity, the CDC says. Overall, trends in COVID-19 cases continue downward as compared to the last few years, according to long-term data trends in deaths, emergency room visits and positive cases. The Pandemic Literally Aged Our Brains

This transcript has been edited for clarity. Welcome to Impact Factor , your weekly dose of commentary on a new medical study. I'm Dr F. Perry Wilson from the Yale School of Medicine. Living through the COVID pandemic aged your brain by about five and a half extra months. This, according to new research. It's a pretty scary headline, but what does 'brain age' even mean, how do you measure it, and what risks does a five-and-a-half month older brain have to face? We're going to use what's left of our brains to dig through the data this week. Stay tuned. The study generating those scary headlines is 'Accelerated Brain Ageing During the COVID-19 Pandemic', from Ali-Reza Mohammadi-Nejad and colleagues, appearing in Nature Communications . But before we get into it, a reminder that SARS-CoV-2, the virus that caused the COVID-19 pandemic, is a weird one, especially when it comes to the brain. It was pretty early in the pandemic when we realized that one of the rather unique symptoms that came with COVID infection, differentiating it from the common cold and flu infections and the like, was anosmia — the loss of smell. It turned out this was due to infection of some epithelial cells in the nose, with inflammation impacting the olfactory bulb of the brain. To be clear, evidence of direct brain infection by SARS-CoV-2 is fairly weak. Neurons don't express high levels of the ACE2 receptor — the viruses' point of entry to other cells. And while some autopsy studies have found viral genetic material in the brain, it has been quite difficult to find live virus in brain cells. But secondary effects of the virus — inflammation and microclotting— do affect the brain. And multiplestudies have suggested that infection can worsen certain cognitive outcomes. There was more to the pandemic than just the virus, though. There was social isolation, higher levels of stress, and loss of work and other cognitively stimulating activities. In other words, there were lots of reasons for people's brains to get a little worse during those few years. That's really what the Nature Communications study is looking at. It's an overall sense of how our brains are doing, and 'brain age' is a useful proxy for that concept. So, what is brain age? It's pretty straightforward. Imagine a carnival barker doing the 'guess your age' thing. You come up, they take a look at you, squint, and call out a number, usually with startling accuracy. The carnival barker is using various clues — features on your face, wrinkles, hairstyle, and so on — to make a guess. We can do the same thing with an MRI machine. We just have to teach a computer what the brains of different ages look like. That's what researchers did here. Using the huge UK biobank cohort, they took brain scans of more than 15,000 healthy people of various ages and trained a machine-learning model to predict age based just on the images. It got pretty accurate, as you can see here. Of course, it's not perfect. Nor would we expect it to be. Some people's brains look younger than their stated age. Call it the Paul Rudd effect. And, of course, some brains look older than they should. The distance between your calendar age and your brain age is the 'brain age gap' (BAG), with higher numbers implying that your brain is older than it should be. This gap seems to stay pretty consistent over time. If your BAG is 5 years right now, it tends to stay around 5 years even a few years later, provided nothing intervenes to more rapidly age your brain. If anyone ever accuses you of having an excessive BAG, you can say 'that's not my bag, baby.' (And that ends the '90s references for this week.) It's the repeated brain scans that make this study interesting. The researchers identified about 1000 participants who had multiple brain scans, on average about 2 years apart. For 432 of those folks, the pandemic happened between the two scans. So, we can look at how much their brains aged in that timeframe and compare that aging to a group who was not exposed to the pandemic. This is the meat of the study. Here are the primary results. Remember, BAG is supposed to stay pretty stable if nothing else has changed. And that's what we see in the control group. Between the two scans, there was, on average, no change in BAG — though the distribution here encompasses changes in both directions. Now we add the pandemic group. What you see is that the distribution has shifted, on average by 5.5 months. That's where we get that headline that started this whole thing. But of course, 5.5 is just the average change in BAG. Different people's brains aged at different speeds. Still, overall, this is a statistically significant difference. You're probably wondering why some brains aged more than others. The 5.5 month result is the effect of the pandemic – not necessarily infection. Out of the 432 individuals with a postpandemic scan, 134 actually had documented COVID-19 before that second scan. Interestingly, the increase in brain age was similar among those infected and those not infected. To be fair, only five of those infected were sick enough to be hospitalized, but it still seems that the bulk of the aging effect of the pandemic may have been due to factors independent of the virus itself; factors that made the pandemic effect worse. The brains of older people and men seemed to age more during the pandemic than those of younger people and women. Poor socioeconomic status and lower education also led to more pandemic brain aging. Still, it's not how you look; it's how you feel. Do older-looking brains on brain scans function worse? The study doesn't offer much support for this. Cognitive testing found no compelling relationship between changes in brain age and changes in various tests of cognitive ability across the entire cohort. There did seem to be some relationship among those infected with COVID-19: They did worse on cognitive tests if their brains had aged more than expected. But I'm often wary of signals like this that appear in a very particular subgroup. Leaving the realm of data and stepping squarely into the land of speculation, I think what we are likely seeing here is that the pandemic was a population-wide neurologic stressor. The isolation, the anxiety, and the lack of cognitive stimulation led our brains to atrophy a bit. It does not seem that this is a direct result of viral infection, though again, we don't have data on people with more severe COVID-19. The big question is, what happens next? Now that we are reintegrated with society, back at work, and using our brains a bit more, will they grow younger again? It remains to be seen. But if the pandemic-induced gray in my hair is any indicator, there may be no going back.
