
How vaccines are developed
What are the processes that a vaccine has to go through before it can be distributed to the public?
And how were we able to develop a range of vaccines against Covid-19, a new virus, in a relatively short time, compared to a typical vaccine?
Since the first vaccine was created in 1796, vaccines have been very successful at protecting people against infectious diseases.
We have come a long way since then, equipped with more advanced technologies and methods, as well as extensive testing and regulations to ensure the creation of vaccines that are safer and more effective.
Vaccine development starts in the lab and goes through several stages before it can be introduced to the population.
Identifying the pathogen
To create a vaccine against a particular disease, the pathogen (i.e. bacteria, virus, fungi or parasite) that is causing the disease has to be identified first.
Then the pathogen will be studied in a laboratory to better understand its infection mechanism.
A suitable antigen (a part of the pathogen that induces an immune response) will be selected to develop the vaccine.
The antigen may be a small component of the pathogen, like a surface protein, or the whole organism in a weakened or inactivated form.
Testing the vaccine
Before a vaccine can be tested on people, it is first tested on lab-grown cells and animals to assess its safety and efficacy in preventing disease.
This is done to evaluate the vaccine's ability to induce an immune response and to figure out the safe and appropriate amount to start testing on people.
Researchers may make adjustments to the vaccine at this stage to improve its safety and efficacy.
After the vaccine has been proven to trigger a sufficient immune response and received the necessary approval, it can then be tested on humans in three phases of clinical trials.
A small group of 20 to 100 young, healthy adults are recruited to receive the trial vaccine.
The aim is to observe the vaccine's immune response in humans, determine the right dosage, assess its safety and identify any side effects.
Typically, it takes at least one year to complete Phase I.
The next phase involves several hundred participants, including those with similar characteristics (e.g. age, sex, health status) as the intended recipients of the vaccine.
It aims to obtain more data on the safety and immunogenicity (ability to trigger an immune response) of the vaccine in people from more diverse backgrounds.
Phase II usually takes a longer time to complete than Phase I, and not all trial vaccines will pass this stage to proceed to the next phase.
The final phase of clinical trials involves thousands of people and is usually conducted at multiple sites around the world to ensure that the findings apply to different populations.
One of the main goals of this phase is to detect any rare side effects of the vaccine – this is why more participants are required.
At this phase, researchers confirm the safety and efficacy of the vaccine to support mass distribution to the public.
Phase III may take a few years to complete.
Approval, manufacturing and distribution
Following a positive result from the clinical trials, the vaccine will then be reviewed through a series of steps by relevant local and international regulatory bodies to ensure compliance with quality, safety and efficacy criteria before it can be approved and licensed for public use.
The whole vaccine development process starting from research until approval typically takes around 10-15 years.
In Malaysia, the authority responsible for approving any vaccine or medication is the National Pharmaceutical Regulatory Agency (NPRA).
Once the vaccine has been approved, the vaccine manufacturer can begin to make larger batches of vaccine for distribution to the public.
Vaccines are usually bottled in glass vials and then carefully packaged for safe cold storage and transport.
It is important to store and transport vaccines at the right temperature and with the proper procedure to safeguard the integrity of the product.
Even after the vaccine has been approved for the general population, regulatory bodies will continue to monitor its safety and effectiveness.
Every country has their own system to detect and report adverse events linked to vaccination.
Another part of the monitoring process is the Phase IV study – an ongoing study to evaluate the effect of the vaccine as it is used in the wider population over a longer period of time.
This data will help scientists and policymakers finetune the policies of vaccine use and optimise the impact of the vaccine.
Covid-19 vaccine development
As seen during the pandemic, Covid-19 vaccine development was accelerated due to the urgent need for a safe and effective vaccine to control the disease.
Several factors contributed to the successful effort: Prior research on the SARS-CoV-1 and MERS-CoV vaccines helped reduce the time spent on investigating the target antigen in SARS-CoV-2, the pathogen responsible for Covid-19.
Researchers sped up their processes by combining and overlapping different phases of vaccine clinical trials, e.g. Phase II may have begun even before Phase I ended, or Phase II and III may have been combined. Nevertheless, the scientific designs and safety standards of the trials were maintained and not compromised.
International collaborative efforts between different nations, international health organisations and vaccine makers, who contributed financially, politically and strategically to enable the accelerated vaccine development pathway.
In Malaysia, NPRA granted conditional approval to various Covid-19 vaccines when they were first rolled out.
The evaluation period was shortened from the standard 245 days to less than a month to speed up the distribution of the vaccines to the public.
Covid-19 boosters are still currently available at selected healthcare facilities for those in high-risk groups.

Climate ‘green swan' events: Financial sector braces for shocks
India's financial system faces a growing threat from green swan events—climate-driven crises marked by unpredictable, cascading impacts that could destabilise markets and worsen inequality. Unlike rare black swan events, green swans are inevitable due to the climate crisis, combining physical risks (floods, droughts) and transition risks (policy shifts, tech disruptions). With over $70 billion in climate-related losses since 2019 and 2,000+ fatalities, India ranks among the most vulnerable nations globally. Traditional risk assessment models that extrapolate from historical trends are insufficient for fully appreciating the systemic risks posed by the climate crisis. They involve non-linear, irreversible environmental shifts—like melting glaciers, draughts, or collapsing ecosystems—that trigger financial chaos. The Reserve Bank of India (RBI) warns these events could recur with rising intensity, overwhelming traditional insurance and hedging tools. For India, where 45% of the workforce relies on climate-sensitive agriculture, the stakes are existential. The interconnected nature of these risks means that localized climate disasters can cascade through the entire financial system, affecting institutions and markets far beyond the initially impacted areas. Also, unlike health shocks, climate disks are here to stay. While health crises like Covid-19 caused acute, time-bound disruptions, green swans pose perpetual threats with irreversible tipping points. For example, coastal flooding could permanently displace millions, unlike pandemic recovery cycles. Traditional risk assessment models that extrapolate from historical trends are insufficient for fully appreciating the systemic risks posed by the climate crisis. The RBI is rolling out phased guidelines to address climate risks, including green deposit frameworks, climate disclosure rules, and stress-testing protocols. Specifically, in the context of financial inclusion, banks and microfinance institutions (MFIs) face dual pressures where cyclones and floods destroy collateral (homes, livestock), spike loan defaults, and disrupt operations and policy transition risks where sudden policy changes (e.g., coal phaseouts) could strand assets in carbon-intensive sectors. Microfinance, a $70 billion industry serving 60 million low-income borrowers, is especially exposed. Climate disasters threaten 30-40% of MFI portfolios tied to agriculture and livestock. Women, who form 95% of microfinance clients, face heightened risks of income loss and displacement. Having made some strides in financial inclusion, green swans could reverse those gains. MFIs report repayment rates dropping to 60% post-disasters. Given that these events could reshape finance, particularly for marginalised communities, it would be useful to seek lessons from Bangladesh's experience with similar climate-related challenges in financial inclusion. Bangladesh, which shares many climate vulnerabilities with India, has implemented several innovative approaches to address the financial implications of green swan events. A comparable context is Bangladesh's 1998 floods, where the country's financial sector had to deal with smallholder farmers and microfinance borrowers losing livestock and crops, their primary loan security. Through Green Refinancing, Bangladesh Bank provides low-cost loans for solar energy and waste management, supporting 20 million people through its Solar Home System programme. Grameen Bank's experience with green swan events is pioneering. It transitioned its group-based lending and emergency savings accounts to buffer climate shocks. During floods, repayment flexibility and asset diversification (e.g., shifting from crops to poultry) helped sustain its high recovery rates. A key lesson was that post-disaster loan waivers eroded repayment discipline in Bangladesh. Instead, staggered repayments and climate-resilient livelihood training proved more effective. One of the aspects that the Indian financial sector must prioritize are climate stress tests, which mandate banks to simulate monsoon failures or renewable energy transitions. Green bonds for financial inclusion can channelise funds into affordable flood-resistant housing or drought-tolerant crops. Green swan events represent a profound challenge for India's financial system, particularly in terms of maintaining inclusive financial services for vulnerable populations in the face of increasing climate risks. For India's 800 million people living on under $6 daily, balancing growth with climate resilience isn't optional—it's survival. Bangladesh's mix of regulatory innovation and community-centric finance provides a blueprint, but scaling solutions demands urgent public-private collaboration. As India continues to develop its approach to green swan events, coordination between financial regulators, government agencies, financial institutions, and civil society will be essential to ensure that vulnerable populations are not left behind in the transition to a more climate-resilient financial system. Seeking out diversity moving from Brazil
Beneath the surface of Michelle Silva and Evandro Santos's success story is a tale of hard work and overcoming familial separation. The married couple are originally from the town of Minas Gerais in Brazil where they were married and had two children. Mr Santos and Ms Silva came to New Zealand at the beginning of 2020 under a work visa, leaving their children with family members in Brazil to see what life was like in this country. Ms Silva, who was trained as a materials engineer, had a galvanizing job in Christchurch and not long after the move Mr Santos, a civil engineer, got a job as a tiler in Wānaka. The couple were then not only separated from their children but from each other too. 'This is the immigrant life," Ms Silva said. 'Sometimes we don't have a choice, we don't have other options." It was diversity that drew them to uproot their comfortable life in Brazil. Ms Silva wanted her children to experience a spectrum of cultures and learn different languages. 'I decided I want to go to a country where my kids, they can learn English and they can learn about another country,'' Ms Silva said. She was confident about the idea, but Mr Santos needed some convincing. He said their life in Minas Gerais, unlike other parts of Brazil, was very safe. 'For me, I feel homesick a lot because our life here it's harder. "You need to work more than we worked in Brazil," Mr Santos said, explaining that New Zealand's higher cost of living had meant more financial stress at times. Once Ms Silva found a job in Wānaka the two were reunited, but their son and daughter remained oceans away. Due to the Covid-19 lockdowns, it took the couple two years to bring their their family back together. Their daughter, Ana Ribeiro, is now 9 years old and their son, Lucas Souza, is 16, both of whom have been able to immerse themselves in learning a second language and meeting people from all over the world. 'To be honest, they love to live here," Ms Silva said. 'Ana has friends from Canada, from London and she has friends from Qatar. And she learned gymnastics here and how to swim." Seeing their children benefit from the move was the first success, next was building a business from nothing. Mr Santos worked for a company in Wānaka to start with but quickly felt he wanted to do more. 'Evandro never wanted to work for someone. 'He always wanted to work for himself," Ms Silva said. Three years ago, Mr Santos became self-employed and after one year the couple set up their business, VIP Tiling. Mr Santos tiled and Ms Silva managed the business and the pair were to growing their enterprise, slowly expanding their team. In addition to building a business they also built a life in Wānaka with a supportive Brazilian community by their side. "What I love about New Zealand, it is like communities are open," Ms Silva said. 'We go to the New Life Church and we have a big community there. "But at the same time, we support the Calvary International Church as well, because it's a Brazilian church." Although the couple experienced periods of homesickness from time to time, they said they could not imagine living anywhere else and felt fulfilled with the life they had worked hard to set up in Wānaka.

After delay in US, dentist travels to Pune for surgery to fix stuck anti-reflux device
Jeffrey Lyle Mason, a US based dentist who had been experiencing sleep disturbances was sure the issue was related to some stomach ailment. He underwent a procedure to prevent the stomach acid from flowing backward but did not get much relief. It seemed like forever to get a revision surgery in the United States and the dentist decided to research medical tourism. According to Dr Mason, the procedure in the US did not provide relief and the magnetic device had slipped into the chest and was closely stuck to the oesophagus. Removing it without any injury to the vital structures and nerves of the stomach was crucial to preserve the anti-reflux functioning. However the revision surgery — a fundoplication, a surgical procedure used to treat gastroesophageal reflux disease (GERD) was taking a while. 'I hopped on Deepseek that guided me about Mexico, Costa Rica and India,' Dr Mason recalled and further research on some of the best surgeons in India led him to Pune. He told The Indian Express that his search led him to Pune's Dr Shashank Shah and team and he soon got an appointment for a surgery. 'A few days before I was set to fly to India, I started having severe pain in the abdomen area and was grateful to have got the appointment as I needed that surgery,' Dr Mason said. The revision surgery went off well and Dr Mason said that he was grateful for the vital communication from Dr Shah's team. 'They were very accommodating,' he added. When contacted, Dr Shashank Shah, noted laparoscopic bariatric surgeon said that Dr Mason had been operated upon at Las Vegas last year. 'He was operated with a magnetic anti-reflux device. Unfortunately, the device slipped into his chest leading to discomfort and hence decided to connect with gastrointestinal surgeons across the world. He did his own research and we are happy that Dr Mason selected Pune. The surgery was undertaken on May 16 this year. The magnetic device was removed and the hiatus hernia operated upon.. He recovered well and returned on May 20 to the United States,' Dr Shah said. At the LaparoObeso centre in Pune that performs complex and advanced surgeries, Dr Shah said that they had seen almost one patient every month from countries like Malaysia, Dubai, Australia, Abu Dhabi, UK and others. 'Medical tourism is on the rise post Covid -19 and at our centre patients have been largely seeking treatment for complex hernia and bariatric surgeries,' Dr Shah added.
