Forum: Providing medical concierge services may not address patients' needs
A recent Opinion piece on healthcare costs (Some practical ways to rein in rising healthcare costs, premiums, May 21) mentioned how insurers can provide medical concierge services to direct patients to specialist doctors, based on their Integrated Shield Plans.
As a general practitioner (GP) since 1985, I have always referred my patients to specialists within their budget who I personally know have the skill sets that are relevant to their specific medical issues. For example, I would refer a young female patient with an enlarged thyroid gland to a surgeon I know is specially noted for neat, fine scars.
A young man likely to need prostate surgery for possible cancer would be sent to a urologist I know is exceptionally careful about conserving urinary continence and erectile function.
And I would send a patient with recurring bladder cancer to a hospital with a large multidisciplinary urology department – because I foresee that the patient will eventually need care available in an integrated group.
All this care in matching patients' needs with surgeons' skill sets can be wiped out by medical concierges referring patients to other specialists – with choices based on 'lower charges'.
Of course, it can be said that all general surgeons can do thyroid surgery competently, or all urologists can do a prostatectomy competently upon completing their specialist training.
But while all surgeons and all hospitals are competent, some are exceptional, and this matters in more complex cases. That is why many GPs try to match an individual patient's medical needs to doctors with specific skill sets.
In contrast, a medical concierge could simply locate another specialist who could be charging less and shunt the patient there, indifferent that the patient could have had a better outcome with the original specialist.

Malaysian injured while climbing Indonesia's Mount Rinjani

According to Malaysian newspaper Sinar Harian, the victim sustained serious injuries, including a fractured hip and head wounds, after falling approximately 200m before reaching the bridge to the popular site. Mr Yarman, head of the Mount Rinjani National Park Centre, confirmed the incident, which occurred around 2.20pm local time, and stated that a rescue operation was launched immediately upon receiving initial information. 'Initial information indicates the victim was still able to walk slowly despite injuries to the leg and a head impact. Our team was dispatched to the location to provide emergency treatment and conduct the rescue operation,' he told the media on June 28. He stated that the rescue procedure was carried out according to established safety standards, including providing initial treatment at the location before the victim was transferred to the nearest control post, such as the Sembalun Post. If further treatment is required, the victim will be referred to health facilities like Bhayangkara Mataram Hospital or the Community Health Centre in the Sembalun area. Mr Yarman also advised all climbers to be more cautious and not to underestimate the challenges when climbing Mount Rinjani. 'Rinjani's topography is very challenging. Climbers need to be careful, not push themselves if tired, and rest or seek help immediately if they encounter difficulties,' he stressed. The decree did not mention electronic cigarettes. Violaters of the ban will face a fine of €135. The Health, Labour and Welfare Ministry spearheaded this initiative, which leverages the appeal of these iconic figures to encourage people, especially children, to develop better sleep habits. The characters are expected to participate in various public awareness events, adding a fun, recognisable element to the important message that everyone needs to get enough sleep. The ministry recommends nine to 12 hours of sleep for elementary students, and eight to 10 hours for junior and high school students, to maintain good health.
