In six of India's deadliest air disasters, investigators pinpointed human error

New Delhi: India has witnessed several major air disasters over the years, many of which have resulted in significant loss of life and triggered important safety reviews. Most investigations into these accidents have pointed to a combination of human error, non-compliance with procedures, and in some cases, systemic failings in aviation oversight.
On August 7, 2020, Air India Express Flight 1344 from Dubai to Kozhikode skidded off the runway and plunged into a valley while attempting to land in heavy rain. The Boeing 737-800 aircraft was carrying over 180 passengers and crew, of whom 21 lost their lives. According to the final report by the Aircraft Accident Investigation Bureau (AAIB), the crash occurred due to non-adherence to standard operating procedures by the pilots, particularly a delayed decision to initiate a go-around. The report also pointed to underlying systemic failures as a contributing factor.
Another major tragedy occurred on May 22, 2010, when Air India Express Flight 812 overshot the runway while landing at Mangalore. The aircraft broke apart after falling into a gorge, killing 158 people. The Directorate General of Civil Aviation (DGCA) concluded that the captain ignored repeated warnings from the first officer to initiate a go-around and continued with an unstabilised approach.
On July 17, 2000, Alliance Air Flight 7412 crashed during landing in Patna. The Boeing 737-200 aircraft stalled on approach and crashed into a residential area, killing over 60 people. Investigators found that the pilots lost control of the aircraft due to a failure to recover from an impending stall and poor execution of go-around procedures.
Indian Airlines Flight 605 crashed on February 14, 1990, during its approach to Bangalore. The Airbus A320 hit a golf course short of the runway, killing 92 people. The DGCA attributed the crash to pilot error, including an incorrect descent path and a failure to recognize and correct the situation in time.
On April 26, 1993, Indian Airlines Flight 491 crashed shortly after takeoff from Aurangabad. The Boeing 737-200 collided with a truck and power lines, resulting in 55 fatalities. The investigation found that the captain misjudged the takeoff and that inadequate airport security and infrastructure played a role in the crash.
India's worst aviation disaster, however, remains the Charkhi Dadri mid-air collision on November 12, 1996, in which 349 people were killed. A Saudi Arabian Boeing 747 and a Kazakhstan Airlines Ilyushin Il-76 collided mid-air near Charkhi Dadri in Haryana. Investigators determined that the Kazakh crew failed to maintain the correct altitude and that poor English communication contributed to the accident. This incident led to sweeping changes in Indian airspace, including the mandatory use of airborne collision avoidance systems (ACAS).

Rupani had visited the Western India Institute of Aeronautics at Indus University in Ahmedabad, where he examined various aircraft models. "My interaction with the students of the aviation department was filled with energy and enthusiasm. I extend my best wishes for Indian students to shine on the global stage," he added. The former chief minister's last rites were performed with full state honors at Ramnathpara crematorium in Rajkot on June 16. A funeral procession carried Rupani's body through Rajkot before reaching the cremation ground, where he was accorded a 21-gun salute by the state police.

What began as a routine evening flight from Pudong to Narita became for some passengers a near death experience. 'I thought I was going to die,' wrote one shaken passenger on social media, describing the dramatic drop in altitude and the moment oxygen masks fell from the ceiling. 'My body is still here, but my soul hasn't caught up. My legs are still shaking. When you face life or death, everything else feels trivial.' According to flight tracking data and witness reports, the Boeing 737-800 plunged more than 10,000 metres (10km) to just 3,000 metres (3km) in about 20 minutes. Although no injuries were reported, passengers recounted moments of chaos, fear, and a chilling silence as the aircraft descended. According to The Standard news agency, a woman named Wang recalled a chilling silence inside the cabin after the oxygen masks dropped. She said she genuinely feared for her life, and seeing flight attendants nearly in tears only deepened the panic. Another passenger told the outlet she had written a goodbye message to her husband as the plane was landing rapidly. Japan's transport ministry confirmed that the flight crew declared an emergency after an alert indicated cabin pressure irregularities which is an issue that while rare can be fatal if left unaddressed. Quick action by the pilots and communication with air traffic controllers helped ensure the aircraft landed safely at 8:50 pm local time. Yet for those onboard, the nightmare didn't end with touchdown. Passengers were kept inside the aircraft for over an hour, reportedly without clear communication. Amid growing complaints, Spring airlines Japan agreed to offer 15,000 yen in compensation to each passenger but only after individuals filed claims themselves. The incident again throws a spotlight on the Boeing 737-800, a model involved in numerous aviation disasters, including the 2022 crash of China Eastern Airlines flight MU5735 and the Jeju air tragedy in South Korea that killed 179 in 2024. While Monday's emergency ended without casualties, it has stirred unease among travelers already wary of the aircraft's history. For now, questions linger—about airline communication, aircraft maintenance, and passenger treatment—but for many who lived through the terrifying plunge, gratitude trumps outrage. 'We're alive. That's all that matters…But I'll never forget the feeling of that fall. It changes you,' one of the passengers said. Spring Airlines' Chinese parent company was quick to distance itself, noting the incident involved Spring Airlines Japan, a joint venture with JAL. Japan Airlines, which took over majority control of the budget carrier in 2021, has not publicly commented beyond confirming the safe landing.
