Researchers race to cure 'Newfoundland curse,' which can kill without warning
The force was enough to knock him backward into his shed, he said.
"It felt like my chest was just blown apart," Rideout said in a telephone interview from his home in Newtown, a small fishing community along Newfoundland's northeast coast.
Rideout has arrhythmogenic right ventricular cardiomyopathy, or ARVC, a heart condition caused by a small mutation in his genetic code that leads to sudden, irregular heartbeats that can kill him without much warning.
The disease affects clusters of families in certain parts of Newfoundland, and it has come to be known as the "Newfoundland curse." The defibrillator implant that threw Rideout into his shed two decades ago is the best known way to ward it off.
But Jessica Esseltine, a biomedical scientist at Memorial University, is hoping to change that by using heart tissue grown in a petri dish to stop the disease before an implant is needed.
"We're trying to figure out how this single gene change, this seemingly very small spelling mistake, can cause such huge changes in a heart cell," the assistant professor said in a recent interview. "The hope is, at some point, we will be able to offer a cure."
Genes are sections of DNA, which is made up of combinations of four different molecules. ARVC is caused by a single swap of those molecules in one gene — a "spelling mistake," as Esseltine called it.
Kathleen Hodgkinson was part of a team of geneticists who, in 2008, first identified the gene at the root of the problem and then developed a method to test for it. Hodgkinson has found the gene in about 28 Newfoundland families, including the Rideouts, as well as in families in northern Germany, Denmark, Norway and Russia. All those families have genetic ties to the Newfoundland families, she said.
"When I first started, families were so frightened," the Memorial University professor said in a recent interview. They told her stories about young, healthy relatives going to the hospital with heart palpitations, testing normally, and then dying — seemingly for no reason.
"Being able to test people and put in the (defibrillator implant) has changed things enormously … but it doesn't stop the disease in its tracks. It doesn't cure it," she said.
Rideout has to go to the hospital, often by ambulance, every time his defibrillator goes off, which is expensive. He also loses his license for six months each time it goes off, as a safety precaution. And the disease is wearing down his heart.
He got the gene from his mother, who passed it down to all three of her sons, said Norma Rideout, Terry's wife. The condition killed Fred Rideout, Terry's brother, when he was 45 years old. Norma was driving him to the hospital when he died, she said.
"It's always in the back of your mind," she said. "I'm just glad our kids don't have it."
Esseltine said sometimes the condition can degrade a patient's heart so dramatically they require a heart transplant.
She takes blood or skin samples from ARVC patients and turns them into stem cells, which she grows as heart cells. Then, she uses a gene editing technique to repair the "spelling mistake" in the faulty gene in the heart cells.
"We're doing all of that in a petri dish right now," she said. "But, of course, the hope would be one day that we could either treat the patient's heart with stem cells from their own skin, or we can use genetic engineering … to repair the mutation in their hearts."
Working with families like the Rideouts brings a meaning and fulfilment to her work she hadn't experienced before, Esseltine said.
"It makes me want to do better, to move the research forward," she said. "Because I see the real lives that it could possibly change if I do a good job."
Terry Rideout, now 55, said Esseltine's work may have started too late to help his condition.
"But for the younger generation," he said, "it's going to be life-changing for them."

Cancer is a 'family affair,' says researcher calling for more support
A cancer researcher and advocate wants the Newfoundland and Labrador government to offer more support for cancer patients and their families. Sevtap Savas, a professor at Memorial University, recently co-authored an article on ways Atlantic Canada's governments can improve cancer care, through her work with Atlantic Cancer Consortium Patient Advisory Committee. From that study, she said health-care systems can do more to treat cancer patients as individuals by looking at their unique circumstances, which is called precision medicine. "It's the approach in medicine where we want to prevent diseases, conditions or treat them more precisely so that, you know, the outcomes will be better, treatment success will be better," she told CBC News. Out of their research, Savas said they heard from family members of cancer patients about the associated costs of a cancer diagnosis. "Treatment is just one part of [the] cancer journey, and it's just focusing on one person who is patient. But there are people around patients, right," she said. "Cancer is actually a family affair." She wants people to be able to tap into more resources that can help with financial, emotional, social and other supports. While there are supports available, like peer support services, Savas said many people don't know about them, which is why she wants the provincial government to promote the services. In December, the Canadian Cancer Society released a report estimating cancer costs an average patient approximately $33,000 across their lifetime — including out-of-pocket expenses and lost income during treatment and recovery. Savas says that cost is another burden on an already struggling family, "because our health-care system does not cover everything." She pointed to the cost of hospital parking or child care. People interviewed as part of their research highlighted the costs not covered by the public health-care system, she added, like how drug coverage can vary by province. "What we need — to get affordable cancer care drugs," Savas said, adding she thinks it should be an initiative taken on by both the provincial and the federal governments. Savas said participants in the research want to see affordability measures brought in by government. "They want everyone to access this, regardless of their geographic location, whether these people are living in urban areas or rural areas or Newfoundland and Labrador or in other provinces," she said. Pitcher this: MUN herbarium home to thousands of N.L. botanicals
In a room full of tall metal cabinets, Julissa Roncal carefully flicks through stacks of manila folders. They're filled with pages of expertly pressed and dried plant specimens. "See the little berries there? How do you dry a fleshy fruit and put it on a paper, completely dry?" she said, pulling out a page with a flat, dry branch with berries on it. They're perfectly preserved and dated 1966. "It does take a lot of time, effort and art to put those fleshy fruits on a dried specimen that will last for decades." Roncal is the curator of the Memorial University Agnes Marion Ayre Herbarium. The collection has about 100,000 specimens tucked in a small office on Mount Scio Road. "They are basically cataloged or distributed following a particular order that represents the classification," she explained. "Each specimen is labelled with the year it was collected, the person who collected it, and where it was found." She said about 80 to 90 per cent of the specimens are from Newfoundland and Labrador, and the rest are from other parts of the world. The collection, Roncal said, is not only important for research and history, but because it belongs to the people of the province. "This is a specimen of the Newfoundland provincial flower," she said, pulling out a page with a dried pitcher plant adhered to it, titled Carbonear, 1945. For decades, researchers and botanists have dried, pressed, and preserved specimens from three main categories: algae, mosses and vascular plants. The collection is used to train biology students at MUN, who learn about databases, manipulating large data sets, and get experience in botany and biology in systematics, taxonomy and species distribution. "We can extract DNA, for example, from these specimens for genomics research," Roncal said. And depending on the research, the collection can also aid in climatological research. "We can also track whether these species have changed their flowering times or their physiologies throughout time," she said. "For example, we can detect whether a particular species is flowering sooner or later and correlate it with climate change, so that information can be observed or rescued from the information that is hosted or housed here." Roncal said there are about sixteen thousand specimens digitized and accessible to the public. Work at the herbarium started years before Roncal's tenure as curator. She describes the herbarium's namesake — Agnes Marion Ayre — as an influential figure. "She was a suffragist, she fought for women's rights, for voting. So she's definitely an inspiration beyond botany for all of us," she said. Ayre was an amateur botanist, and collected and preserved some of the specimens in the collection. Roncal said those contributions were the starting point. "So it is the result of decades of botanical exploration and accumulation of these specimens," she said. But Ayre didn't just catalogue plant species she found, she also painted them with watercolours while in the field. There are about two thousand of Ayre's paintings between the Centre for Newfoundland Studies and the herbarium. It's a process that Roncal said added more detail than a typical press would. "Painting allows you to devote time to deep, thorough observation of the plant that you have in front of you," Roncal said, looking at a painting from the 1920s. "So that's why the combination of both is ideal to really get to know what species and identify with confidence what you're looking at," she said. "And putting a name to what you're looking at." Scientists gather in St. John's to advocate for greater effort in fight against avian influenza
The world's top experts on avian influenza are in St. John's this week, working toward a global response to the growing amount of the disease being found around the planet. Andrew Lang, a microbiologist at Memorial University and one the co-chairs of this year's International Symposium on Avian Influenza, said the disease continues to spread around the world at a rapid pace. Cases of illness from the D1.1 variant of avian influenza were discovered in Newfoundland and Labrador this spring, the same variant that infected a person in British Columbia and killed another in the United States. "Unfortunately this virus has just not gone away and doesn't seem like it's going to go away," Lang said on Monday. "So we just have to deal with that, and all the new coming changes, when this virus evolves very quickly." More than 400 people will attend the conference this week both in-person and online. St. John's is the first Canadian city to ever host the symposium. Michelle Wille, a post doctoral scientist studying viruses in wild birds at the University of Melbourne, said part of the goal of the conference is to highlight the need for greater resources in fighting the virus. The avian flu panzootic — the animal equivalent of a pandemic — started around the same time as the COVID-19 pandemic, she said. It meant people prioritized COVID-19 over the avian influenza. "COVID has provided us with lots of really useful tools, techniques, updated platforms to analyze and share data. And so we're really leveraging all of those advanced technologies to help us track avian influenza," Wille said. "What we're dealing with, the problem is massive. But we are getting very little resources to deal with that globally." Andy Ramey, part of the United States Geological Survey Alaska Science Centre, said the true scale of disease remains unknown. Birds across each continent — with the notable exception in Australia — have been infected, and antibodies have even been found in animals like foxes and bears. Ramey is pitching the need for a unified approach to fighting the virus at the conference. "One health approach is key here, because we're all connected. There's … just so many ways that health in one sector is not limited within that sector," he said. "The scale of number of infections that we thought we maybe knew about is actually probably order, or orders, of magnitude bigger than what we thought."
