
Bearded vultures: Back from the brink
Bearded vultures have a wing span up to 2.85 meters (9.3 feet) allowing them to hunt over 700 kilometers (435 miles) in a day. The feathers on their neck, head and torso are naturally white, but they dye them orange by covering themselves in the iron oxide-rich mud found in the mountains and highlands where they live. Bearded vultures are scavengers, and up to 85% of their diet is bone. Historically, they were called 'ossifrage,' derived from the Latin for 'bone breaker.' They mostly swallow bones whole, their strong stomach acid breaking them down, but if a bone is too big, they will drop it from height onto a rock to break it and expose the nutrient rich marrow inside. While Alpine farmers no longer blame the vultures for missing sheep, or children, the birds are still threatened. Accidental poisoning through eating animal carcasses containing drugs, pathogens or steroids, collisions with power lines and wind turbines and habitat degradation have reduced the global population — which spans from western Spain to China — by as much as 29% in the last three generations. Exacerbating this problem is their slow breeding rate. A breeding pair will only lay one or two eggs a year, and even if both hatch, the stronger chick will kill its weaker sibling. Initial attempts by conservationists to reintroduce the bearded vulture involved capturing birds in Afghanistan and releasing them in the Alps, but the project failed due to the difficulty in capturing and transporting the birds. However, in 1986, three birds that had been raised in captivity at a center in Austria were released successfully in the country's mountains, leading to a flurry of further releases across the Alps. The young birds are put in artificial nests on cliffs, enabling them to acclimatize to the new environment, and after 20 to 30 days they take their first flight. Young bearded vultures are known for traveling vast distances. In 2020, one bird flew 1,200 kilometers (745 miles) from Haute-Savoie in the French Alps to the Peak District in the north of England. Another conservation project in the Maloti-Drakensberg mountains of Lesotho and South Africa is working to save the last bearded vultures in the southern hemisphere. The Bearded Vulture Recovery Program did not have a captive breeding program when it started, so instead, when a vulture lays two eggs, the team takes the second egg from the nest, which would be killed by its sibling anyway, and raises the chicks in captivity before releasing them. They are now raising 27 birds in captivity and aim to reach 150 breeding pairs in the wild. With these global conservation efforts, there is hope that bearded vultures will continue to soar across mountains all over the world. 13 Cancers in One Blood Test — but 75% False Alarms
A prospective cohort study led by Yang Shao, PhD, president and CEO of Geneseeq Technology Inc. and professor at Nanjing Medical University, Nanjing, published in Nature Medicine , on a blood test capable of simultaneously detecting 13 types of cancer. The test demonstrated high sensitivity and specificity and was able to identify early-stage cancers that often go unnoticed during routine screenings. Traditional cancer screening methods are often invasive, expensive, and time-consuming, which can reduce patient adherence. In addition, several cancers — such as pancreatic cancer — are typically asymptomatic in their early stages and progress rapidly, with no established screening protocols currently available. This prompted the development of less invasive approaches, such as multi-cancer early detection (MCED) blood tests that can detect a cancer signal from circulating cell-free DNA. These simple blood tests analyze plasma cell-free DNA using genetic and fragmentomic-based features from whole genome sequencing to simultaneously detect multiple cancer types. Although promising, current MCED tests still have relatively low sensitivity, typically less than 60%. Experts, including those from the American Cancer Society, cautioned that widespread use could create a false sense of reassurance and potentially deter patients from following up with standard screenings. Researchers have developed an MCED blood test that detects 13 cancers: breast, cervical, colorectal, endometrial, esophageal, gastric, liver, lung, ovarian, pancreatic, prostate, biliary tract, and lymphoma. These cancers account for 66.6% of all new cases and 74% of cancer-related deaths worldwide. The test uses two main classifiers: the detection-of-cancer classifier, tasked with confirming the presence of cancer, and the tissue-of-origin classifier, responsible for pinpointing the primary site of malignancy by analyzing and integrating feature frameworks, including copy number variations, fragment size coverage, fragment size distribution, nucleosome footprint, and fragment-based methylation. To develop the test, researchers analyzed 6553 blood samples, 3076 from patients with cancer and 3477 from healthy individuals, divided into a training dataset of 4807 samples and an internal validation dataset of 1746 samples. Independent validation was performed using a prospectively enrolled cohort of 1465 participants in an age-matched fashion, comprising 732 patients with cancer and 733 non-cancer individuals between April and November 2021. In the third ongoing phase, 3724 asymptomatic adults aged 45-75 years in the Jinling cohort underwent both complete physical examinations and the MCED test in June 2023. Positive Results In independent validation, the MCED test showed an overall sensitivity of 87.4% and specificity of 97.8%. The sensitivity was particularly high for certain cancer types, such as 100% for liver and biliary tract cancer, 94.5% for lung cancer, and 82.3% for colorectal cancer. Even cancers that are difficult to diagnose early, such as pancreatic and ovarian cancers, showed a sensitivity of 76.9% for pancreatic cancer and 90.5% for ovarian cancer. Breast cancer had the lowest sensitivity at 63.8%. The test was effective in detecting early-stage disease, with a sensitivity of 79.3% for stage I and 86.9% for stage II cancer. This increased to 92.4% for stage III and 97.1% for stage IV. When considering the top two tissue origin predictions, the accuracy increased to 90.7% for the internal set and 91.7% for the independent set. It performed best in identifying cancers of the colon-rectum, lungs, and liver but was less accurate for pancreatic and stomach cancers, correctly identifying the origin in 50% or fewer cases. In a prospective screening cohort of asymptomatic individuals, the MCED test identified 23 of 43 cancer cases within 1 year, with an overall sensitivity of 53.5%. When limited to the 13 cancers that the test was designed to detect, the sensitivity increased to 62.1%. Most of these cases (93%) were early-stage cancers (stage 0, I, or II). The specificity remained high at 98.1%, with a positive predictive value (PPV) of 25% and a negative predictive value of 99.4%. Notably, 8 of the 23 positive patients who received a positive MCED result had their cancers undetected through physical examination, and 4 had cancers for which there is currently no recommended screening, highlighting the potential of the MCED test to effectively detect cancers that would otherwise have gone undetected. 'Our study demonstrated high sensitivity, highlighting our classifier's ability to detect cancer cases, even in populations with lower disease prevalence. This underscores the capacity of our classifier to effectively detect incident cancer cases under real-world screening conditions, facilitated by comprehensive physical examinations,' the authors concluded. These findings suggest that the MCED test could be a valuable complement to existing screening methods, particularly for cancers without routine early detection tools. The ability to detect early-stage pancreatic and ovarian cancers is particularly promising. However, broader validation across diverse populations, cost-effectiveness analyses, and studies on the psychological impact of screening outcomes are critical before widespread clinical implementation. One key limitation was that the PPV achieved was 25% for the MCED test, which was lower than the 38% reported in the PATHFINDER study published in 2023. 'The PATHFINDER trial enrolled participants with a higher cancer prevalence and utilized the MCED test results to trigger diagnostic workup, systematically investigating participants with positive test results, thus inherently enriching their cohort for cancer diagnoses within the workup pathway. Conversely, the Jinling study adopted a standardized comprehensive physical examination for all participants as the primary screening modality, independent of MCED test outcomes and within the context of lower cancer prevalence,' the researchers noted. However, the absolute number of false positives in the Jinling study was low (20 of 3724 participants, or 0.54%). False-positive results can lead to unnecessary anxiety, further invasive diagnostic procedures, and potentially inappropriate treatment for patients. The researchers emphasized the need to improve the sensitivity of the MCED test for very early-stage cancers while reducing false positives.

Efgartigimod Use Extends to Myositis, Sjögren Disease
BARCELONA, Spain — Efgartigimod could have a role to play in the treatment of both myositis and Sjögren disease, according to data from two 'proof-of-concept' phase 2 studies reported at the European Alliance of Associations for Rheumatology (EULAR) 2025 Annual Meeting. In the ALKIVIA study, involving 89 adults with active idiopathic inflammatory myopathy (IMM), significantly greater clinical improvement in muscle strength and physical function — as measured by the 2016 American College of Rheumatology (ACR)/EULAR Myositis Response Criteria's Total Improvement Score (TIS) — was achieved with efgartigimod than with placebo. The least-squares mean TIS at week 24 was 50.45 in the efgartigimod arm vs 35.65 in the placebo arm (P = .0004). There was also a significant improvement in mean TIS over time. In the RHO study, a significantly greater proportion of the 23 adults with primary Sjögren disease treated with efgartigimod achieved the trial's primary endpoint — a clinical response in at least three of five items using the Composite of Relevant Endpoints for Sjögren's Syndrome (CRESS) measure — than did the 11 adults given a placebo. At week 24, 45.5% of efgartigimod-treated individuals vs 11.1% of the placebo-treated participants met this primary endpoint of the trial. Addressing Unmet Needs IMM and Sjögren disease are two rheumatic and musculoskeletal diseases lacking effective treatments; targeting Immunoglobulin G (IgG) autoantibodies with efgartigimod was of interest, said both of the presenting investigators for the two studies at the meeting. Hector Chinoy, MD Hector Chinoy, MD, a consultant rheumatologist for Salford Royal Hospital and professor of rheumatology and neuromuscular disease at the University of Manchester, England, said during his presentation of the ALKIVIA study findings that, with regard to IMM, 'We use too [many] steroids. We need a steroid-sparing profile, one providing a sustained response for both muscle and extra muscular involvement.' Isabelle Peene, MD, of Ghent University Hospital and the VIB Center for Inflammation Research, both in Ghent, Belgium, said in her presentation of the RHO study data: 'The most prominent autoantibodies in Sjögren's are, of course, those against Rho52, Rho60, and SSB. As there is a high unmet need in Sjögren's, we wondered whether the mode of action of efgartigimod could alter this disease course.' Blocking Antibody Recycling The data from the two studies looked 'very promising,' Elizabeth Price, MBBCh, PhD, a consultant rheumatologist who works at Great Western Hospitals NHS Foundation Trust, Swindon, England, told Medscape Medical News. Price, who has a specialist interest in Sjögren disease, said, 'Efgartigimod is one of a new class of monoclonal antibodies targeting the [IgG1] neonatal Fc receptor [FcRn]. This receptor is involved in binding circulating immunoglobulins and transporting them across membranes (including the placenta).' The drug, which had already gained regulatory approval in the US and some other countries for use in myasthenia gravis and chronic inflammatory demyelinating polyneuropathy, worked by reducing the level of IgG and other pathogenic autoantibodies. By binding to FcRn, efgartigimod essentially blocked the ability for IgG to be recycled back into the circulation, and thus IgG levels were reduced without having an impact on antibody production in general, it was reported. The ALKIVIA Study ALKIVIA was an ongoing trial, Chinoy explained; a 52-week phase 3 component was continuing from the phase 2 results he presented. The trial recruited people with several subtypes of IMM, including dermatomyositis, immune-mediated necrotizing myositis, polymyositis, or antisynthetase syndrome. Participants were randomly allocated 1:1 to treatment with efgartigimod or placebo in addition to background IMM treatment. Those in the efgartigimod arm received a 1000 mg dose given subcutaneously once a week. Baseline characteristics reflected 'a typical profile of a myositis patient,' Chinoy said, and generally were comparable between the groups. Overall, the mean age was 56.6 years, and the median time since diagnosis was 4.4 years. Just over 75% were women, and 72% were White. Chinoy noted that, in addition to higher TIS in the efgartigimod vs the placebo arm after 24 weeks, significantly more participants taking efgartigimod achieved TIS improvements of ≥ 20 (91.5% vs 73.8%, P = .025), ≥ 40 (78.7% vs 47.6%, P = .0029), and ≥ 60 (34.0% vs 9.5%, P = .0055), signifying mild, moderate, or major clinical improvement, respectively. Clinical improvement occurred more quickly in the efgartigimod than in the placebo arm, with minimal improvement seen at a respective 30 vs 72 days (P = .002) and moderate improvement after 113 days vs not estimable (P = .029). As for safety, similar percentages of efgartigimod-treated (87%) and placebo-allocated (88%) participants experienced at least one adverse event. Most adverse events occurring with efgartigimod were related to its mode of administration, with injection site bruising, redness, pain, or other reactions recorded. There were fewer grade 3 or higher adverse events in the efgartigimod arm than in the placebo arm (14.9% vs 28.6%). Two deaths occurred with efgartigimod but none with placebo. Neither death was attributed to the drug; one involved a road traffic accident and the other was a case of preexisting liver cirrhosis that had 'only really come to light during the course of the study,' said Chinoy. Questioning the Data Ronald van Vollenhoven, MD, PhD, of Amsterdam UMC and Amsterdam Rheumatology and Immunology Center ARC, Netherlands, said after Chinoy's presentation that 'this approach of lowering total IgG is very interesting.' He questioned, however, whether the drug could actually be targeting pathogenic antibodies vs endogenous immunoglobulins, 'which obviously serve a useful purpose.' Richard Alan Furie, MD, of Northwell Health in New York, New York, said he thought problems with placebo response rates had been seen in lupus, and he asked, 'What do you think the reasons are for high placebo response rates in myositis, and what is being done about it?' Chinoy responded: 'Patients were allowed to be on background treatment; they could be on up to 20 mg of prednisolone at inclusion into the study.' The baseline data showed that 82.0% had been treated with systemic corticosteroids, and more than three-quarters had received disease-modifying antirheumatic drugs. This background treatment, along with some geographical variation that was noted in the TIS responses, was probably behind the high placebo response rate, Chinoy said. 'I think for future trials, we may be looking for a more stringent endpoint, for primary endpoints,' he added. The RHO Study The design of the RHO study was slightly different. It was a phase 2 study that was not designed to run directly into a phase 3 trial; randomization to efgartigimod or placebo was 2:1 rather than 1:1; and efgartigimod was given once weekly at a 10 mg/kg intravenous dose rather than subcutaneously. Participants enrolled in the study had to meet the following criteria: a diagnosis of Sjögren disease as per ACR/EULAR 2016 criteria within the past 7 years, an EULAR Sjögren's Syndrome Disease Activity Index (ESSDAI) ≥ 5, positive test for anti-Ro/SS-A antibodies, and residual salivary flow. Three of the 34 people who were randomized did not meet these eligibility criteria, however, and were not included in the efficacy analysis, Peene reported. Although baseline demographic data were fairly typical for Sjögren disease — almost all participants were female — Peene acknowledged after questioning that there were some differences between the groups. For example, the median age was 49 years in the efgartigimod group and 58 years in the placebo group, and the median clinical ESSDAI scores were 13 vs 18. 'It's a very small study,' Peene said. 'We just wanted to have [an] idea of, is it working? And what's the estimate of the treatment effect? But I agree that this is a little bit skewed.' Alongside CRESS, another relatively new outcome measure was used, the Sjögren's Tool for Assessing Response (STAR). Results showed that 54.5% of efgartigimod-treated vs 33.3% of placebo-allocated participants had a score of score ≥ 5, which was the threshold needed to be a responder. No relevant changes in the EULAR Sjögren's Syndrome Patient Reported Index score were observed. But there were improvements in clinical ESSDAI total scores (-7.0 vs -4.0), which will be the primary endpoint in the follow-up phase 3 trial called UNITY. Efgartigimod was also shown to reduce mean levels of both IgG (by 58%) and anti-Ro52 autoantibodies (by 64%) vs baseline to week 4 onward. By comparison, IgG levels were more or less unchanged in the placebo arm, and anti-Ro52 autoantibodies had increased by just under 40% from baseline. There were also reductions in rheumatoid factor and complement component 1q seen with efgartigimod but not placebo. At least one adverse event occurred in 87% of participants taking efgartigimod and 64% of those given placebo. No grade 3 or higher or fatal adverse events were observed. One adverse event in the efgartigimod arm led to discontinuation, but this was not related to the study drug, Peene said. The most common adverse events — which occurred more often in the efgartigimod than in the placebo arm — were headache (17% vs 9%), nasopharyngitis (17% vs 9%), influenza (13% vs 0%), and urinary tract infection (13% vs 9%). Promising Results, but More Data Required Both the RHO and ALKIVIA studies were picked by Helga Lechner-Radner as part of her clinical highlights of the meeting. During the EULAR 2025 Highlights session, Lechner-Radner, a senior physician at the Medical University of Vienna, Austria, said: 'We need more data; the phase 3 [trials] of this compound [are] currently underway and we really look forward to [their results].' The ALKIVIA and RHO studies were funded by argenx; study authors included employees of the company. Chinoy reported relationships with AstraZeneca, Pfizer, PTC Therapeutics, and UCB. Zai Lab Announces Positive Topline Phase 3 Results for Bemarituzumab in Fibroblast Growth Factor Receptor 2b (FGFR2b) Positive First-Line Gastric Cancer
At an Interim Analysis, Bemarituzumab Plus Chemotherapy Significantly Improved Overall Survival in People With FGFR2b Overexpression Compared to Chemotherapy Alone SHANGHAI & CAMBRIDGE, Mass., June 30, 2025--(BUSINESS WIRE)--Zai Lab Limited (NASDAQ: ZLAB; HKEX: 9688) today announced the Phase 3 FORTITUDE-101 clinical trial evaluating first-line bemarituzumab plus chemotherapy (mFOLFOX6) met its primary endpoint of overall survival (OS) at a pre-specified interim analysis. Bemarituzumab plus chemotherapy demonstrated a statistically significant and clinically meaningful improvement in OS as compared to placebo plus chemotherapy in people living with unresectable locally advanced or metastatic gastric or gastroesophageal junction (G/GEJ) cancer with FGFR2b overexpression and who are non-HER2 positive. FGFR2b overexpression was defined as 2+/3+ staining in ≥10% of tumor cells by centrally performed immunohistochemistry (IHC) testing. Gastric cancer is the fifth leading cause of cancer-related death worldwide, with nearly one million new cases and over 650,000 deaths globally each year 1, highlighting a critical unmet medical need. In China, there are over 350,000 new cases each year. The disease is associated with a poor prognosis, particularly in advanced stages where the five-year survival rate is less than 10%. There are currently no approved therapies specifically targeting FGFR2b overexpression in gastric cancer in China. "Bemarituzumab is the first FGFR2b inhibitor to demonstrate a statistically and clinically significant overall survival benefit in a randomized Phase 3 trial for the first-line treatment of FGFR2b-positive gastric cancer," said Dr. Rafael Amado, M.D., President, Head of Global Research and Development at Zai Lab. The most common treatment-emergent adverse events (>25%) in patients treated with bemarituzumab plus chemotherapy were reduced visual acuity, punctate keratitis, anaemia, neutropenia, nausea, corneal epithelium defect and dry eye. While ocular events were consistent with the Phase 2 experience and observed in both arms, they occurred with greater frequency and severity in the Phase 3 bemarituzumab arm. Detailed results from the trial will be shared at a future medical meeting. Zai Lab holds the development and commercialization rights for bemarituzumab for mainland China, Hong Kong, Macau, and Taiwan. Bemarituzumab has been granted Breakthrough Therapy designation by the China Center for Drug Evaluation of the National Medical Products Administration for the treatment of FGFR2b-positive gastric and gastroesophageal junction adenocarcinoma. A Phase 3 study of bemarituzumab plus chemotherapy and nivolumab is also ongoing in patients with first-line gastric cancer, with data readout anticipated in H2 2025. About FGFR2b The FGFR2b protein (also known as fibroblast growth factor receptor 2b) is an emerging biomarker which, when overexpressed, promotes aberrant signaling leading to tumor cell proliferation.2 The FGFR2b protein is overexpressed by G/GEJ tumor cells in approximately 38% of patients with advanced G/GEJ cancer. FGFR2b protein overexpression is defined as 2+/3+ staining intensity on tumor cell membrane, as detected by immunohistochemistry (IHC) testing. The FORTITUDE-101 trial spanned 300 sites across 37 countries, with 547 patients enrolled. The primary outcome measure of the trial is overall survival in patients with FGFR2b ≥10% 2+/3+ tumor cell staining. Key secondary outcome measures include progression-free survival and overall response rate. Candidates were excluded from the trial if they were known to be human epidermal growth factor receptor 2 (HER2) positive. FORTITUDE-101 included more comprehensive ocular-related monitoring than previous studies of bemarituzumab.
