
How Canadian hospitals are trying to break the cycle of readmissions for homeless patients
When the vehicle arrives, Aleman unpacks a walker from the trunk and greets the woman with a warm familiarity. She slows her pace to match her client's as they make their way into the clinic and approach the front desk.
'Hello, we have a dentist appointment,' Aleman tells the receptionist while helping the woman rummage through her purse to find a health card.
They sit side-by-side on plastic chairs in a near-empty waiting room, leaning towards each other to complete paperwork on a clipboard.
'Do you have an emergency contact you want to put down?' Aleman, a homeless outreach counsellor at St. Michael's Hospital, asks, pen in hand.
'No, just you people,' her client quips, and they both laugh.
Aleman works at the Navigator Program, which helps steer unhoused people through a health-care system that can be especially challenging for patients without a fixed address or means to advocate for themselves. The program was established to break the cycle of hospital readmission and improve health outcomes.
Aleman's client said she was admitted to St. Mike's with COVID-19 in late December. When it was time for her discharge on Christmas Day, she was homeless for the first time in her life. She said a family member she was living with had kicked her out of the house.
The navigators took over her case and found her a shelter bed.
'I hate to even think about it,' she said of what would have happened if Navigator hadn't intervened and she walked out into the bitter cold with nowhere to go.
In late January, Aleman helped her secure an apartment in a Toronto Community Housing senior's building.
'Everything I've been through...they've been there,' said the woman, whom the hospital asked not to be named to respect the privacy of patient health information.
Once their hospital treatment ends, patients without housing are discharged back onto the streets, often without a phone, family doctor, health card or shelter. Many return to hospital with worsened health conditions and the cycle continues.
Navigator started at Toronto's St. Michael's Hospital in 2019, and expanded to Vancouver's St. Paul's Hospital in 2023, collectively serving more than 1,000 patients since then.
One of the first clients from the Vancouver hospital was a woman who landed in the emergency department about 26 times in two months.
'Then I started working with her,' said Alex MacKinnon, an outreach navigator at St. Paul's.
MacKinnon said she helped the patient find housing and saw concrete change, despite the ongoing management of chronic illnesses.
'Having checked back in on her ED rates, they went down significantly,' she said.
Dr. Stephen Hwang created the program at St. Mike's after examining readmission rates among homeless patients admitted to internal medicine between November 2017 and 2018. He found that 27 per cent of 129 patients returned within 90 days. Roughly a third of participants were readmitted for an identical diagnosis as their initial admission.
It's like having a 'super advocate,' said Hwang.
A randomized controlled trial is underway to establish whether the program cuts down on return hospital visits. The trial, funded by a Canadian Institutes of Health Research foundation grant, involves 656 people experiencing homelessness.
Hwang said there are already striking indicators that the approach works, such as the fact 67 per cent of Navigator patients who needed a family doctor obtained one through the program.
Navigator has three outreach workers at St. Mike's and one at St. Paul's, including Aleman in Toronto and MacKinnon in Vancouver. On average, they each see 15 patients at a time for about 90 days. In Toronto it's funded by St. Michael's Hospital Foundation while the Vancouver pilot is funded by Staples Canada.
Navigators bring patients coffee, toothbrushes and fresh socks when they're admitted to hospital.
These small acts helps build strong relationships between outreach counsellors and their clients, said Dr. Anita Palepu, an internal medicine physician at St. Paul's.
'It's not just a physical poverty. In some cases, it's also deep social poverty and isolation,' said Palepu.
The hope is that after three months patients stand on more stable ground with a roof over their head, a family doctor and government benefits. Though the Navigator office door always stays open.
One morning in late May, Fred Ellerington, another Toronto navigator, was alerted that one of his patients was in the emergency department. Right away, one of his colleagues left their shared office on the general internal medicine floor to check on them.
'We're embedded,' Ellerington said of the immediate access hospital-based navigators have to patients and their electronic medical records.
Ellerington said the patient had cleared standard health tests but his colleague could tell something was off because the patient's behaviour was odd compared to a visit a week earlier. Ellerington said they encouraged a closer look, suspecting a neurological problem
'They just don't have that relationship with the patient,' Ellerington said.
While the program's primary goal is improving health outcomes, Ellerington said cutting return visits can also save money and hospital beds.
Hwang said he is working to launch the program at the University of Montreal Hospital Centre later this year and wants to see it in more hospitals across the country.
Long-term, he said he wants policies and programs to address the root causes of homelessness, including making housing and mental health supports more affordable.
'But in the meantime, it's just really important that we serve the people who are right before us in our midst, who have urgent, severe health problems, and who end up literally on the doorstep of the hospital.'

Graduate overcomes life-changing injuries to fulfil dream of becoming a doctor
A mature student has graduated from university as a doctor a decade after a motorbike accident as a teenager left him with catastrophic injuries. Paul Edwards was 17 and studying for his A-levels when he was knocked off his motorbike, suffering multiple fractures. He had to put his studies on hold and delayed his university ambitions for a decade while he recovered. Fast-forward 16 years and, despite living with debilitating pain, Mr Edwards, now aged 32, has graduated in front of his proud parents Charlie and Denise, girlfriend Aesha, close friends and tutors. It was not just his injuries he needed to overcome, as Mr Edwards grew up in Lawrence Weston where only around 15% of students at his secondary achieved five GCSEs. Encouraged by his parents and supportive teachers, he set his sights on studying medicine at the University of Bristol. Prior to his accident he was accepted on to the now obsolete Government-run gifted and talented programme, which sought to help talented students from disadvantaged backgrounds. But in October 2009 a car overran a junction and Mr Edwards collided with the car at 30mph. He suffered catastrophic injuries, including two fractured thighs, multiple fractures to his neck and back, a lacerated liver and head trauma. Mr Edwards spent years in recovery, undergoing multiple surgeries and battling constant pain. In 2014, he had what he calls his 'sink or swim' moment, and with the help of his GP weaned himself off opioid medication. 'I remember my GP saying: 'You can either stay on these drugs and be stuck here, or come off them and find out what life might still offer you',' he said. He enrolled in an access to higher education course and completed the equivalent of three A-levels in nine months. In 2017, Mr Edwards was offered a place to study neuroscience at Bristol, but switched to medicine during the Covid-19 pandemic. 'My sister is a paramedic, and my GP and consultants were so amazingly supportive during my recovery from my accident. I realised I wanted to help. I wanted to give back,' he said. He studied while managing with 'endless and debilitating' pain as a consequence of his injuries. 'I use over-the counter painkillers to treat the pain and I do lots of activity including endurance running and triathlons,' he said. 'I know I'll be in pain anyway, but I would rather be active and in pain, than sedentary and still in pain.' He has recently completed his elective period of study in anaesthetics and will soon begin his foundation doctor training at an NHS hospital on the Isle of Wight. 'I owe the university a lot regarding my success,' Mr Edwards said. 'I have been incredibly well supported, and I really appreciate everything the staff have done for me. 'They really help you achieve the absolute best you can be. I count myself lucky to have met so many wonderful people along the way, who are now absolute friends for life. 'Who knows, maybe the accident was part of a bigger plan – directing me to a path of becoming a doctor> 'I want to let people know that anything is possible if you are determined enough. It's never too late, and Bristol is a great place to be.'

Does My Covid Vaccine Still Work? I Asked A GP If It's Time For A Booster
Recently, the Covid strain XFG – nicknamed 'Stratus'– became the dominant variant in the UK. Before then, the 'Nimbus' strain was rapidly spreading. In both cases, the World Health Organisation (WHO) said that while they were 'variants under monitoring', the 'Currently approved COVID-19 vaccines are expected to remain effective' against the mutations. They can help to prevent extreme symptoms and reduce the risk of people getting seriously ill, the WHO explained, even though both Stratus and Nimbus strains have mutated from the original virus. But does that mean we should all be getting more Covid vaccines? Who needs to get them more often, given that their effects 'wane'? HuffPost UK asked GP, Dr Suzanne Wylie, a medical adviser for IQdoctor, about who may need yearly vaccines. 'As with the flu vaccine, COVID vaccinations are now being tailored to the most prevalent strains circulating at the time,' Dr Wylie said. 'Currently, the general advice is that most people do not need regular COVID boosters unless they fall into higher-risk categories, such as those over 75, individuals with underlying health conditions, or those who are immunocompromised.' This is different to 2024 when booster vaccines were given to everyone over 65. The Joint Committee on Vaccination and Immunisation (JCVI) said in a report that they made these changes because population-level immunity has improved, thanks to exposure to the virus and/or vaccines (this combination is called 'hybrid immunity'). As a result, they said, they chose to focus on people most at risk, including 'the oldest adults and individuals who are immunosuppressed.' For healthy younger adults, Dr Wylie added, 'routine vaccination is not usually necessary more than once a year, if at all, unless advised otherwise during periods of high transmission or in response to a new variant of concern.' The NHS say they will contact you if you're eligible for a vaccine. This may be the case if you're over 75, are immunocompromised, and/or live in a care home for adults. Dr Wylie says that 'Vaccines, including updated COVID boosters, are designed to provide protection against the most common or concerning strains each season.' 'While no vaccine offers complete immunity,' she added, 'they significantly reduce the risk of severe illness, hospitalisation, and complications.' But newer variants, like the Stratus strain, can 'sometimes partially evade prior immunity, which is why updated vaccines may be offered if there's evidence of increased severity or spread.' The UK Health Security Agency (UKHSA) and the Joint Committee on Vaccination and Immunisation (JCVI) monitor Covid risk levels and will help determine if a wider rollout becomes necessary. 'For now,' Dr Wylie told HuffPost UK, 'for most younger, healthy individuals, annual or less frequent vaccination is generally sufficient.' Behind the Wheel and Under the Influence: What Driving Data Reveals About America's Cannabis Shift
As the legalization of recreational cannabis spreads across the country, a new concern is emerging on America's highways. According to a study published by The Texas Law Dog, states where cannabis is legal for recreational use are reporting a concerning rise in car crashes and road fatalities. These trends suggest that the push for cannabis freedom may be unintentionally fueling a public safety crisis. Legalization on the Rise, So Are Crashes In 2011, recreational marijuana was illegal nationwide. As of 2025, 25 states allow adult-use cannabis, and the results on roadways are becoming harder to ignore. The Texas Law Dog's report found that the widespread legalization of marijuana is linked to a 6% increase in motor vehicle crashes and a 4% increase in fatal crashes nationwide. Individual state data provides even starker illustrations. In Oregon, crash fatalities have increased by 22% since legalization. Alaska and Colorado have seen fatal crash spikes of 20% and 16%, respectively, while California reports a 14% rise. These increases aren't isolated they're part of a growing national trend. Cannabis and Driving: A Deadly Mix As marijuana becomes more accessible, its use is also becoming more casual. By 2019, nearly one in five Americans aged 12 or older reported using cannabis up from just 11% in 2002. More troubling is that a significant portion of users are taking to the roads while impaired. The Texas Law Dog analysis revealed that 85% of cannabis users admit to driving the same day they consume the drug, with over half doing so within an hour. These findings are critical, given marijuana's well-documented impact on reaction time, attention, and coordination. Cannabis is also emerging as a dominant factor in fatal crashes. During the COVID-19 pandemic, cannabis involvement in fatal accidents surpassed alcohol for the first time accounting for 33% of deaths compared to alcohol's 29%. The Challenge of Poly-Substance Use The threat of cannabis-impaired driving becomes more complex when other substances are involved. The study highlights that pre-pandemic, 18% of drivers involved in fatal crashes had multiple drugs in their systems. Post-pandemic, that figure climbed to 25%. This trend poses challenges not just for enforcement, but also for toxicology and crash analysis. Determining whether cannabis was the primary impairment in such cases becomes increasingly difficult when alcohol or other drugs are also present. A Tale of Two Policies: Medical vs. Recreational Interestingly, not all cannabis laws have had the same effect on public safety. States with legalized recreational cannabis saw a 4–6% increase in crash rates. Meanwhile, states with only medical cannabis programs experienced a reduction in traffic fatalities between 8% and 11%. This discrepancy may stem from how cannabis is distributed and consumed in these states. Recreational laws tend to widen access without the same medical oversight, potentially leading to more frequent and unregulated use by the general public. A False Sense of Security Behind the Wheel One of the most revealing aspects of The Texas Law Dog's findings is the disconnect between perceived and actual risk. While 70% of users said driving within an hour of cannabis use was 'highly dangerous,' and 93% believed their friends or family would disapprove, many still drive high. Why the contradiction? Nearly 30% of users believe law enforcement wouldn't be able to detect their impairment. With limited roadside tools to measure THC levels and no national threshold for cannabis DUI, many drivers feel they can get away with it. The Legal Patchwork Fuels Public Confusion Differences in cannabis laws from state to state only add to the problem. In Alaska, recreational users can carry up to one ounce, but medical users may possess up to 2.5 ounces every two weeks. In California, medical patients are allowed eight ounces. In some states, like Idaho and Nebraska, all cannabis possession is still illegal. This legal inconsistency creates a minefield of confusion for both law enforcement and drivers, especially those crossing state lines. Uniform standards on THC limits and impaired driving penalties could go a long way in closing these gaps. Solutions That Balance Freedom and Safety Legalizing cannabis doesn't have to mean compromising traffic safety. The Texas Law Dog study calls for a smarter approach to impaired driving prevention. Among the proposed strategies: Development of reliable roadside THC testing Standardized DUI laws across state lines Expanded public education on cannabis impairment Stronger restrictions on driving post-use Increased funding for drug-impaired driving research States that legalize cannabis should do so with a parallel commitment to traffic safety. That includes investing in law enforcement training, refining legislation, and ensuring that public messaging keeps up with the pace of policy reform. Conclusion: Freedom Without Responsibility Isn't Freedom at All The movement toward cannabis legalization shows no signs of slowing, but the data shows a clear cost: more crashes, more deaths, and more uncertainty on the road. The Texas Law Dog's study reveals a growing risk that demands a balanced, evidence-based response. As lawmakers navigate the future of drug policy, ensuring public safety must remain part of the equation.
