
DGCA raps airports over safety violations, gives deadline of 7 days
The aviation regulator conducted intensive night and early morning surveillance operations at Delhi and Mumbai airports and found what officials described as systemic lapses across aircraft maintenance, ground operations, and airport infrastructure.
The enhanced oversight, initiated through an order issued on June 19 — a week after the Air India crash that killed at least 271 people — represents one of the most comprehensive safety assessments of India's aviation ecosystem in recent years, with similar reviews planned for Hyderabad and Kolkata airports.
The surveillance revealed critical maintenance-related violations that directly impact flight safety, the regulator said in a statement. 'At places, AMEs [aircraft maintenance engineers] were not attending to snag rectification; defect reports generated by the aircraft system were not found recorded in the technical logbook; several life vests were not properly secured beneath their designated seats,' the DGCA revealed.
AMEs are part of companies that run engineering and maintenance services. Typically, they are part of the airline but can at times include contractors. The regulator did not identify any specific provider.
During aircraft maintenance, inspectors discovered that prescribed work orders were not being followed. More seriously, unserviceable thrust reverser systems and Flap Slat Levers — devices used by pilots to extend or retract wing flaps and slats during takeoff and landing — were not properly locked as required by safety protocols.
The DGCA found that aircraft maintenance engineers were not taking mandatory safety precautions as specified in Aircraft Maintenance Manuals, creating potential hazards during maintenance operations.
Ground handling operations showed significant deficiencies, with essential equipment including baggage trolleys found in unserviceable condition. Line maintenance facilities were operating without proper tool control procedures, violating established safety protocols.
At the airport infrastructure level, inspectors found faded runway centreline markings that could impair pilot navigation during critical takeoff and landing phases. Rapid exit taxiway green centre lights were not functioning unidirectionally, potentially creating confusion for pilots during ground operations.
Concerningly, obstruction limitation data around airports had not been updated for three years despite numerous new constructions in airport vicinity areas. No surveys had been conducted to assess whether these structures posed collision risks to aircraft.
Multiple vehicles operating in sensitive ramp areas lacked mandatory speed governors. The DGCA immediately withdrew these vehicles by cancelling their Airport Vehicle Permits and suspended the drivers' Airside Driving Permits.
The checks also uncovered problems with pilot training infrastructure. Inspectors found a flight simulator that did not match its corresponding aircraft configuration, with software that had not been updated to current versions — issues that could compromise the effectiveness of pilot training programs.
In a separate incident reported on Monday, an Air India aircraft scheduled to operate the Delhi-Kochi flight was grounded Saturday due to a missing Cabin Defect Log, taking off only after a new log was issued. The Cabin Defect Log is used to record technical issues found in aircraft cabin areas and is essential for regulatory compliance, allowing engineers to identify and address problems before takeoff.
The safety violations had immediate operational consequences. One domestic flight was held up due to worn tires and was cleared for departure only after required maintenance was completed, demonstrating the direct impact of the surveillance on flight operations.
A DGCA official confirmed that 'findings during the surveillance revealed multiple cases where reported defects re-appeared many times on aircraft, indicating ineffective monitoring and inadequate rectification action on repeated defects.'
'All the findings observed during the surveillance have been communicated to the operators for taking necessary corrective actions within seven days,' the DGCA stated, indicating the urgency with which the regulator is treating the identified violations.
The safety review follows other recent enforcement actions, including the DGCA's order directing Air India to remove three officials from crew scheduling duties over 'systemic failures' in operational processes, though those violations were unrelated to the Ahmedabad crash.
Officials confirmed that the review process will continue, with similar comprehensive assessments planned for other major airports including Hyderabad and Kolkata as part of the ongoing safety evaluation ordered on June 19.

Waiting for closure on AI 171 crash, India is still flying blind on aviation safety
There's mystery surrounding the black box as well, particularly regarding the analysis of the data captured in its recorders. As this data would be critical in revealing the cause of the crash, the location of where the black box has been opened and analysed is a talking point. A week after the crash, the civil aviation ministry had said that the Aircraft Accident Investigation Bureau (AAIB) — under the civil aviation ministry —would decide on where the data would be decoded. When reports started surfacing about the black box being sent to the United States, officials clarified that the data was actually being analysed in India. Was there a change of mind — and if so, why? Conspiracy theories on social media and elsewhere on what caused the crash have complicated matters in the absence of any official indication so far, despite the black box being found without much damage soon after the accident. According to reports, there were two black box units on the aircraft that crashed — one was recovered on June 13 and another on June 16. Numerous 'preliminary reports'' are doing the rounds, highlighting a range of reasons for one of the biggest air disasters. The blame has been fixed on everything from sabotage to faulty seat configurations, pilot error to engine malfunctioning in these reports, which are being flagged as artificial intelligence-generated fake content. Within a week or so, the preliminary report on the crash of Air India flight 171 is expected to be out, but it may or may not be conclusive. Until there's closure on the June 12 crash of the Boeing 787 — which went down seconds after takeoff from Ahmedabad airport, killing 241 people on board and 34 on the ground — millions who saw the plane burst into a ball of fire in repeated television replays may struggle to get their confidence back in flying. The truth about what triggered the fatal crash of the Dreamliner should not only help bring the guilty to book, but also outline the future course of action in aviation safety. At this point, the truth is elusive. As a recent report in Business Standard pointed out, a key recommendation after the Kozhikode air crash in August 2020 —in which Air India Express aircraft from Dubai overshot the runway during landing in heavy rain, killing 21 people—was that India must develop its own laboratory to analyse flight data and cockpit voice recorders (black box). In April 2025, about two months before the Ahmedabad crash and five years after the Kozhikode accident, a black box laboratory (as part of the AAIB) was inaugurated. The confusion around where the black box would be decoded ended up putting a question mark on the efficiency of the black box lab, which came into being some 13 years after the AAIB was set up in New Delhi. People in the aviation community claim they have a fair idea of what caused the crash last month. While the preliminary report has to come out within a month of the accident, which is July 12 in this case, many experts believe there's no reason to wait that long if the black box data has already been analysed. That brings us to the significance of communication during a crisis. In the initial days after the crash, Air India was perceived as non-communicative. N Chandrasekaran, chairman of Tata Sons and Air India, admitted as much in a media interview and said the airline was doing a course correction. Soon after, Air India Chief Executive and Managing Director Campbell Wilson reached out to passengers through mailers explaining the reasons for flight cancellations, and assuaging their fears with 'sorry'' and 'thank you'' notes. Chandrasekaran is reportedly doing a lot of heavy-lifting himself after the crash, a lot of which is on the communication front, both internal and external. But it's not about Air India alone. The many instances of plane diversions, emergency landings and narrow escapes in Indian aviation over the past month are almost unprecedented. Whatever the reasons — faulty rostering, fearful pilots or technical glitches — it's time for the regulator, Directorate General of Civil Aviation (DGCA), to prioritise air safety over everything else. Rather than taking knee-jerk decisions after a major accident, safety of fliers and a smooth aviation system in India should be its top agenda. Equally, making effective communication a part of the drill in times of crisis may not be a bad idea. As experts have observed, the aviation sector is growing by leaps and bounds. Domestic air passenger traffic grew from around 67 million in 2014 to 161 million in 2024, according to data from the regulator. Against this backdrop, trained and experienced human resource — including pilots and engineers —is not keeping pace with the requirement. While refraining from commenting on the Air crash, aviation experts have been raising the bigger and critical issue of regulatory oversight, which needs to improve significantly. The latest air crash, followed by a series of aviation incidents, may be a wake-up call for India to have an autonomous body like the UK Civil Aviation Authority created by an Act of Parliament. SpiceJet Flight Experiences Loose Window Frame Mid-Air, Airline Says No Safety Risk
SpiceJet flight number SG 1080 from Goa to Pune witnessed a minor incident on Tuesday when a window frame came loose mid-air. The airline said the incident involved the upper frame of a window on the aircraft, which became dislodged during flight, and had no impact on passenger safety. A spokesperson for SpiceJet said "A cosmetic window frame on one of SpiceJet's Q400 aircraft became loose during flight and was found dislodged. It is important to note that this was a non-structural trim component, fitted onto the window for the purpose of shade, and did not compromise the safety or integrity of the aircraft in any way.' 'Cabin pressurisation remained normal throughout the flight, and there was no impact on passenger safety," ANI reported quoting a SpiceJet spokesperson. Despite the visual disruption, cabin pressurisation remained normal, and there was no impact on passenger safety, the spokesperson said. The airline emphasised that the Q400 aircraft is equipped with multiple layers of window panes, including a robust, pressure-bearing outer pane that ensures continued safety even if cosmetic fittings are affected, it added. Passengers on board were not at risk at any point, and the airline has reiterated its commitment to maintaining the highest safety standards. The frame was fixed upon landing at the next station, in accordance with standard maintenance procedures, airline added in its statement. This incident comes weeks after the Ahmedabad plane crash occurred, in which 260 people were killed, including former Gujarat Chief Minister Vijay Rupani. Only one person miraculously survived the crash. The bodies of 260 people who died in the Air India-171 plane crash have been handed over to their family members, Medical Superintendent of Civil Hospital. The Aircraft Accident Investigation Bureau (AAIB) is leading the investigation into the crash and working intensively on analysing data retrieved from the black boxes of Air India Flight AI-171, in their lab in New Delhi.
