
PGI medical technicians receive arrears after legal battle spanning decades
The finance ministry has approved the payment, correcting a significant pay anomaly that plagued their cadre for decades.
The PGI administration has issued office orders for restructuring of cadres of technical staff (Lab/X-rays/RT) with retrospective effect from March 1, 1992, in compliance with a 2013 judgment by the Central Administrative Tribunal (CAT). The decision follows years of persistent efforts by the PGI Medical Technologists Association, headed by Ashwani Munjal.
This addresses a major anomaly. Previously, even after two promotions, medical technologists found themselves stuck in the same pay grade as their entry-level positions. "For instance, a BSc medical technology recruit starting at grade pay Rs 4,200 (pay level 6) would often retire in grade pay Rs 4,600 (pay level 7), with promotions to levels 1 and 2 offering no change in grade pay, and levels 3 and 4 also remaining within a single grade pay of Rs 4,600.
This rendered promotions largely meaningless in terms of financial benefit," said Munjal.
The journey to this resolution was fraught with legal challenges. "The corrected six-tier cadre restructuring, based on the Dr Prabhakar Committee/cadre restructuring report, will now allow technical staff to progress from grade pay Rs 4,200 up to Rs 7,600 as chief technical officer. This change is expected to benefit around 1,000 serving and 200 retired medical technicians," said Munjal.
The financial implications of this decision are estimated at around Rs 100 crore, including the impact on retirement benefits for those who are serving and retired. The next step involves initiating the process of conducting departmental promotion committee (DPC) meetings to formalise promotions under the new structure.

'A portion of the grant will also cover operational costs,' it said. The initiative is expected to benefit over 8,000 individuals annually. Costly outpatient services in corporate hospitals leave patients high and dry
Hyderabad: Inflatedcosts of outpatient services offered by private hospitals are leaving patients high and dry, including those who may have already spent a fortune as in-patient in the same hospital. Upon discharge, many in-patients are referred to in-house doctors who offer outpatient services during specified hours within the hospital premises. A portion of the high fees of such doctors goes to the hospital. This explains why they charge hefty sums ranging from Rs 1,000 to Rs 2,500 per visit. For other patients too, the same amount is charged by these in-house doctors, be they surgeons or physicians. Such squeezing takes place in corporate hospitals across the city and elsewhere due to the state medical authorities' laxity in regulating this aberration. Due to the lack of regulatory oversight, managements of corporate hospitals are raking in the moolah unethically as if they are not accountable to anybody. There was a time when outpatient services were economical than hospitalization, but now, due to now the rising charges, outpatient expenses are becoming a cause for concern for the common people. The hospitals are charging at least Rs 1,000 for normal OP services and amounts ranging from Rs 1,500 to Rs 3,000 for specialized consultations like cardiology, neurology, nephrology etc., depending on the size and 'brand value'of the hospital. Insurance cover is normally for in-patient services. However, the charges for outpatient services are draining their financial resources. Aside from high registration charges, patients are forced to pay indiscriminate and excessive charges for a battery of diagnostic tests prescribed by doctors. The woes of the patients do not end at registration and consultations but continue while they move out of the hospitals in the form of prescription of medicines. That apart, the doctors in such hospitals prescribe medicines that are only available in their in-house pharmacies, which are costly compared to the prices in outside medical shops. It may be mentioned here that the National Medical Commission (NMC) has issued guidelines asking doctors to prescribe drugs using generic names, avoid unnecessary medications and avoid irrational fixed dose combinations. The State Medical Councils have the authority to take disciplinary action against doctors who violate these regulations. The Supreme Court, while hearing a PIL plea, had suggested the states to introduce a policy so that hospitals do not compel patients to buy essential medical items from in-house pharmacies at inflated rates. Judge Justice Surya Kant highlighted concerns over patients being left with no choice but to purchase medicines, implants and medical devices at exorbitant prices from hospital-linked pharmacies. The Court noted that regulations in private healthcare remain inconsistent, leading to potential exploitation. The petitioner had argued that the practice leads to blackmailing, depriving patients of their rights to have fair pricing. K Shashikala (name changed), who wanted to see a senior endocrinologist for thyroid was asked to pay Rs 900 (Including 100 for registration) for walk-in consultation or Rs 1,000 for registration through phone. During their next visit to show their diagnostic report, they were asked to come to a corporate hospital if it was very urgent as the doctor provides the services there. The patient postponed the visit for two-three days to avoid paying the registration fee in the corporate hospital. When asked, a senior official from the Health Department, who did not want to be identified said that while the doctors have the right to prescribe the combinations as per the needs of the patients, the patients cannot be forced to take medicines from the in-house pharmacies. The official further said that the pricing of the outpatient consultations also depended on the facilities the hospital provided and the modern equipment they possessed.

Pet insurance , though still a niche product in India, can help bridge this gap, offering both financial support and peace of mind during emergencies. Take the case of Gurugram-based Meenakshi Goel, and Peanut and Butter, her two Indie (mixed breed native to India) dogs. After a visit to the vet for one of them left her with a `7,000 bill, she decided to get insurance for both from Bajaj Allianz General Insurance . 'I haven't filed a claim yet, but I sleep better knowing that if something happens, I won't have to choose between treatment and affordability,' she concern is widely shared. A USGI survey of 235 pet owners found that 90% considered veterinary costs a major burden, while 74% said it caused financial stress. Serious conditions like fractures, chronic illness, or cancer can easily rack up bills ranging from Rs.10,000 to over Rs.1 lakh—expense that could hit hard without a financial signs of need for insurance, awareness remains low. Only 26% of the surveyed pet parents had heard of pet insurance, and a mere 10% had purchased it. 'Most of the clients who visit our clinic are unaware that such a product exists, even though it can be extremely useful and necessary,' says Dr B.K. Chaudhary, a veterinarian at Dog and Cat Clinic in added that low agent interest is also to blame, explaining, 'The ticket size and premium are small, so agents don't find it attractive from a commission standpoint.'Even if people are aware, misconceptions often keep them from purchasing a policy. Chennai-based A. Thirumaal, a doctor who is a parent to two cats, explored pet insurance, but ultimately decided against it. 'There are too many exclusions, and most plans are designed with dogs in mind. A cat specific product would be more relevant for me,' he main concern is that his cats often wander outdoors and have been seriously injured in fights with stray dogs, an issue that has already cost him two pets. 'I couldn't find any insurance that covers such incidents,' Dr Thirumaal adds. However, his assumption is misplaced. Most insurers offer 'accidental injury cover' that includes such scenarios. A deeper understanding of policy inclusions and exclusions could help pet owners like Dr Thirumaal clear their misunderstandings about what's not belief that pet insurance comes with too many exclusions is largely unfounded. Some of the pet insurance plans (see table) available in the market cover most of the commonly occurring illnesses like fractures, cancer, or terminal diseases. 'Diseases that are typically excluded from coverage are those rarely found in cats or dogs, except for gastrointestinal issues, which may be covered with extra premium,' says Dr also maintain transparency. Figuring out which one suits your pet's needs can often be confusing. 'It's advisable to consult a vet to determine which options are best suited for you,' adds Dr Chaudhary. A pet's breed, age, and gender can significantly influence its health needs—and consequently, the insurance premium.
