Almost €100m spent on companies that use HSE facilities and staff to cut waiting lists after hours

Close to €100 million has been spent in just over two-and-a-half years on engaging external companies that use
HSE
-owned facilities and equipment after normal working hours – in many cases employing existing health service staff – to reduce public waiting lists.

, published on Tuesday night, reveals 83 serving or former health staff are acting as directors in 148 companies providing what are known as insourcing and outsourcing arrangements to reduce waiting lists.
It says 23 of these company directors are serving HSE personnel.
About 500,000 instances of care were provided from the beginning of 2023 to the first quarter of this year. .

, who drew up the report, is to tell an Oireachtas committee on Wednesday that the figures suggest that 'in waiting-list management we have developed an over-reliance on insourcing to supplement core activity'.
'Insourcing by its nature carries risks and having assessed these I have agreed with the Minister and the Department [of Health] the need to take a series of steps which firstly reduce those risks and increase safeguards and secondly which removes our dependency on insourcing,' he is expected to say.
He is to say that the HSE has carried out a review covering a 27-month period, taking in 2023/2024 and the first quarter of this year, to identify the scale of the dependency the health service has on insourcing and on outsourcing.
Mr Gloster will define insourcing as the practice of engaging external companies or third-party providers to deliver services often outside of normal working hours using HSE-owned facilities and equipment.
'In many cases, these providers may employ or subcontract staff who are already directly employed by the HSE, effectively re-engaging internal staff through a separate commercial arrangement, typically at premium rates,' he is expected to say.
'It is not the use of standard overtime within employment contract arrangements of existing staff, which is a different form of insourcing.'

Following a number of controversies over insourcing arrangements, in April the Minister asked the HSE to examine on a national basis the scale and dependency of the health service on the practice.
Last September an HSE internal audit revealed that two companies which received more than €1.5 million between them in contracts awarded by

without a competitive procurement process were owned or part-owned by employees at the facility.
In May an internal report drawn up within a hospital run by Children's Health Ireland said there were 'serious concerns about the prudent and beneficial management' of National Treatment Purchase Fund (NTPF) funding in relation to a series of clinics run by one consultant.
Mr Gloster is expected to tell the Oireachtas Committee on Health that overall €1.1 billion was spent over the period between insourcing and outsourcing – which involves buying care from the private sector. Much work in this area is carried out by the NTPF.
He will say about €830 million was spent between insourcing and outsourcing over the period in acute settings, with the bulk being spent on outsourcing arrangements.
'Within acute settings €739 million of the spend is outsourcing and this ranges from private hospital care to private ambulances to laboratory products. That is to say not all of the outsourcing is for waiting-list management,' he is to say
'Within acute settings between €71 million and €91 million appears to be insourcing.'
The NTPF, which will receive about €230 million in exchequer funding this year, is expected to say about 30 per cent of its focus is on insourcing.
It is to say it only makes payments in relation to specific named patients who have been on a public waiting list and then only on receipt of detailed invoices from public hospitals confirming that care is completed and the patient discharged.
The NTPF is also expected to say payments are only made to hospitals and never to individual consultants or hospital staff members.

When I speak with people in Dublin I'm shocked by the lack of knowledge of the scale of crisis in Northern Ireland 's health service. 'But you have a free service in the North, so much better than the HSE.' A free service with inadequate capacity can be no service at all. Long waits – often for several years – are standard for health treatment in Northern Ireland. Recently published analysis by the Economic and Social Research Institute concluded that while 12 per 1,000 people on waiting lists in Ireland were there for 18 months or longer, in the North the comparable figure was 86 per 1,000. Sadly, the crisis in healthcare in Northern Ireland is not a unique failure in political delivery . Indeed, health waiting lists cause other problems. Health incapacity is the most common cause of economic inactivity – thousands who want to work are unable to do so because of long waits for surgery and other treatment. Water infrastructure is another crunch point. Lack of sewage and water supply capacity is constraining housing construction and industrial development. Around £300m (€350m) a year is being invested in water infrastructure, compared to the minimum of £500m that NI Water says it needs and the £640m a year that the construction industry argues for. READ MORE [ Stormont is slow, afraid of new thinking and costly, says report Opens in new window ] The North's infrastructure crisis – and yes, it is a crisis – is further illustrated by last week's court judgment blocking the upgrade of the A5 , the most dangerous road on the island which connects Donegal as well as Derry to Dublin. The court found that NI's Department for Infrastructure did not explain how the scheme meets the obligations of Stormont's own climate change legislation. Another serious failing in Northern Ireland is the education and skills system . The North has too few graduates, with around a third of undergraduates studying away because of lack of capacity within the local universities. Nor are there enough adults with high-level vocational skills. This, in turn, reflects a schools system based around academic selection – with many children from lower-income families not making the grade into the best-performing grammar schools. A consequence is that Northern Ireland has lots of teenagers who switch off in school and leave the education system at an early age. Rates of early school leaving are three times higher in the North than in Ireland. One in ten school pupils in the North becomes disengaged as a teenager. These children are more likely to become economically inactive as adults, less likely to gain well-paid employment, more likely to suffer ill health and more likely to gain criminal records, becoming prisoners at high cost to the state. All these problems (and many more) can be argued to be results of a political system that is unable to make difficult choices and allocate resources objectively. The Belfast Agreement achieved peace, but failed to provide an effective system of government. Indeed, Stormont has not even been sitting or operational for 40 per cent of the time since the agreement was signed. The Belfast Agreement never evolved beyond 'conflict by peaceful means'. After working within the Stormont system as a political adviser, I left convinced that it cannot be made to work effectively. I was astonished at the continued sectarianism I perceived from some politicians, 27 years after the Belfast Agreement and the declaration of peace. For many politicians in the North, governing is a zero-sum game – our community loses if your community gains anything. The result is that both communities are held back by the failure to govern for the good of all. And, equally important, Northern Ireland is no longer a society of just two communities. While Catholics today outnumber Protestants, these religious groupings contain a wide range of differences and neither forms a majority. The third section – from other and no religions and arriving from elsewhere – is large and in a sense, underrepresented politically. It is difficult to see how the existing structures of mutual veto by the senior representatives of unionism and republicanism can be remoulded to create a functional system. One wag suggested it is like giving joint government to Jeremy Corbyn and Nigel Farage and expecting it to work. (Or, it might be said, to Conor McGregor and Paul Murphy.) The permanent impasse at Stormont and its inbuilt dysfunctionality causes many to find Irish unity the attractive alternative. But unity is not an easy option. For one thing, there is the cost. Unaffordable says Professor John FitzGerald, given the scale of the subvention (subsidy) from the UK government. Affordable argues Professor John Doyle, not least given the potentially transformative impact of unification. Then there is the timeline, process, destination and lack of preparation. It is perhaps wrong to consider Irish reunification as a potential 'big bang' event. As Professors Seamus McGuinness and Adele Bergin have pointed out, the handover of Hong Kong to China took 13 years. The transition of East Germany is an ongoing process that has so far taken 35 years. What concerns me most is the suggestion that Irish unity should lead to a federalised arrangement in which Stormont continues. When I argued to an Oireachtas committee a couple of years ago that the Northern Ireland Assembly is so dysfunctional that it cannot be retained within a new Irish State, the reaction from some senators seemed like suppressed horror. Why an institution that does not work and apparently cannot work should be retained within a new island-wide constitution is completely beyond me. For many close observers, abolition of Stormont is the single most attractive element of Irish unity. For all their faults, the Irish Government and the Irish State work and are effective. Difficult decisions are taken, with mostly good outcomes. If the Irish Government comprises adults, their equivalents in the North often seem like rowdy teenagers – more focused on arguing and scoring points than on reaching compromise, consensus and the best solutions. It would be understandable if people in Ireland read this and think, 'why do we want them?' Despite the challenges, the emotional desire for Irish unity remains stable across the South's population. The work of the Shared Island Unit has illustrated the challenges involved – it is the practical path to be navigated that remains to be agreed. The number of Saturday outpatient appointments at Children's Health Ireland (CHI) Crumlin has increased by more than 240 per cent over the past four years, new figures show. The hospital group said a 'high proportion' of Saturday clinics are funded by the National Treatment Purchase Fund (NTPF). Concerns have been raised in recent months about overreliance on the fund as well as potential mismanagement of public money. According to data provided to Sinn Féin health spokesman David Cullinane in response to a parliamentary question, there were 419 Saturday outpatient clinics in Crumlin hospital in 2021, rising to 1,426 last year – an increase of 240 per cent. A total of 742 such Saturday appointments were carried out in the first five months of this year. [ NTPF suspends special funding at CHI over controversy Opens in new window ] The figures come following recent controversy at CHI Crumlin after a 2022 internal review, highlighted concerns about the necessity of some weekend clinics for which one consultant had received more than €35,000 in fees. Following the leaking of that report to media, the NTPF temporarily suspended funding for the children's hospital group, while the HSE referred the review to gardaí. Mr Cullinane described the figures as an 'unusually high increase'. 'We have to look at productivity Monday to Friday. We need to look at whether there's a perverse incentive – I'm not saying that's the case here – but this Saturday work has to be done on the basis of contracted hours and not at an increasing cost to the public purse,' he said. In response, a spokesman said additional consultant-led clinics at weekends are 'part of CHI's efforts to improve access and shorten wait times'. 'The NTPF is the main funding source in 2025 for initiatives to increase outpatient activity. A high proportion of Saturday outpatient clinics are NTPF-funded, based on approved initiatives aimed at reducing waiting times,' the spokesman said. In a 33-page report to the Public Accounts Committee, CHI said it had received €8.655 million from the NTPF since 2020 and €28.239 million under the HSE's access to care initiatives to tackle waiting lists. Ahead of CHI appearing before the committee on Thursday, the report submitted to politicians said its board was 'advised that there was engagement with the NTPF by the Children's Health Ireland executive' following the 2022 internal review. However, the report said the NTPF had been engaged in 2021 in relation to compliance with its procedures for waiting list management. As a result of this, the report states the NTPF were 'not specifically engaged with on this specific matter' due to previous audits taking place at the same time as the unpublished report. Concerns have been raised about the reliance of the health system on the NTPF which was set up some 20 years ago to tackle waiting lists through the use of private hospitals or the use of public facilities outside of core working hours – a process called insourcing. Minister for Health Jennifer Carroll MacNeill had requested Bernard Gloster, chief executive of the HSE, to conduct an analysis of the use of NTPF funding. The report is now with the Minister who has called for a move away from paying public staff for out-of-hours clinics. At the Oireachtas health committee on Wednesday, Mr Gloster is expected to say there is now an 'over reliance' on insourcing to supplement core activity. Over a 27-month period, from 2023 until the first quarter of this year, a 'rounded figure' of €100 million has been used on insourcing in acute settings, Mr Gloster will tell politicians.
