Death of Premier Inn receptionist who suffered horror fall at Scots hotel to be probed at FAI
A Fatal Accident Inquiry (FAI) is set to be held into the tragic death of a hotel receptionist who fell through a panel on a walkway roof at hotel in Dundee.
Garry Anderson suffered horror injuries after an incident at the Premier Inn Hotel, Kingsway on August 22, 2023. The 62-year-old sadly died two days later on August 24.
The circumstances surrounding Garry's death are set to come under the microscope. The Crown Office and Procurator Fiscal Service announced today that it has lodged a first notice to begin the court process for a discretionary FAI.
The purpose of a Fatal Accident Inquiry (FAI) includes determining the cause of death; the circumstances in which the deaths occurred, and to establish what, if any, reasonable precautions could have been taken, and could be implemented in the future, to minimise the risk of future deaths in similar circumstances.

Procurator Fiscal Andy Shanks, who leads on death investigations for COPFS, said: 'The death of Garry Anderson occurred while in the course of his employment and as such a Fatal Accident Inquiry is mandatory.
'The lodging of the First Notice enables FAI proceedings to commence under the direction of the Sheriff.
'Mr Anderson's family will continue to be kept informed of significant developments as court proceedings progress.'

A GoFundMe was created by a friend and student of Raul's mother, who described the little boy as 'a bright light - joyful, kind, full of curiosity and love.' 'There are no words to describe the depth of this tragedy,' the GoFundMe tribute states. 'His goal was to be a soccer goalie and could not wait to make those dreams come true,' the description continues. 'Their presence brought warmth to everyone around them. Now, their family is left not only grieving a heartbreaking loss but also facing overwhelming emotional and financial burdens.' The fundraiser has reached $7,393 of its $20,000 goal. A candlelight vigil is being held on July 21 in Raul's memory. The accident remains under investigation. 'Raul was more than just a player on the field he was a part of our family,' Ennis Soccer Association said in a July 14 Facebook post. 'A teammate, a brother, a friend. His spirit, kindness, and smile touched everyone who had the privilege of knowing him.'

Two children and two adults are still in hospital after the horrific school bus crash that killed a 10-year-old boy. A 70-seater a coach carrying Minehead Middle School pupils on Thursday crashed off the road, flipped onto its roof and slid 20ft down a steep bank. A total of 21 people were injured in the crash as it drove year 5 pupils back from an end of term trip to the zoo. Four children and three adults remain were in hospital - the two remaining youngsters were taken to Bristol Royal Hospital for Children by air ambulance following the accident. A police spokesperson said the investigation into the crash was ongoing. One boy, Jayden, who was on the bus and suffered a suspected broken collarbone in the crash, visited Minehead Middle School to lay flowers at the scene earlier today with his mother Kim. He said: 'I just remember climbing up the hill to get back on the road and that's it. On his injuries, Jayden said: 'I'm alright, it just hurts.' Emergency services, including five helicopters, rushed to the scene on the A396 in Somerset to free passengers who had become trapped inside the bus. 1 killed and 2 missing after a group is swept over an Oregon waterfall
One person was killed and rescuers continued searching on Sunday for two others after a group of people were swept over an Oregon waterfall, authorities said. The group went over Dillon Falls on the Deschutes River about 10 miles from the city of Bend, Oregon, on Saturday afternoon, the Deschutes County Sheriff's Office said. Three people were rescued from the river and taken by ambulance to a hospital in Bend. One person died at the scene, the sheriff's office said. Search efforts for the missing went into Saturday evening and resumed Sunday morning, said Sgt. Josh Barker with the sheriff's office. Further details — including the victims' identities and how the group got swept over the falls — were not immediately released. Dillon Falls, which is within Deschutes National Forest, features a large drop followed by a series of rapids that surge through a narrow gorge of lava rock.
