to cut waiting lists. It follows a review of the practice by the chief executive of the Health Service Executive, Bernard Gloster, who has recommended that it be phased out by the end of the year.
What appeared to be a novel solution to reducing waiting times in the wake of the Covid epidemic has come to be seen as a costly problem. Under the initiative, introduced by then health minister Stephen Donnelly, private companies were encouraged to use spare capacity – in terms of trained staff and equipment – at public hospitals to provide services to patients on public waiting lists.
The underutilisation that the initiative capitalised on was due in part to working hours and rostering limitations at public hospitals. The services were provided on a commercial basis and followed the precedent set by the outsourcing by the HSE of the treatment of public patients to private hospitals.
The initiative succeeded in its primary objective of reducing waiting times, but its cost has proved problematic. Over ¤100 million was paid out in the 27 months to March to third party outsourcing companies, many of which are run by or employ existing HSE staff.

Concerns were raised at the outset about the possibility of conflicts of interest and the creation of perverse incentives whereby doctors and staff involved with third-party insourcing businesses stood to benefit from long public waiting lists. Some of these concerns appear to have been borne out, with a number of controversies prompting the review by Gloster.
The Minister's dilemma is that while there may be political pressure to wind down the third-party insourcing initiative, there is even greater pressure to address waiting lists which are being driven ever upwards by demographic trends. The insourcing initiative is generally accepted to have made a material difference to waiting times.
The HSE has agreed to new rostering arrangements for staff and hospital consultants which should see equipment and facilities used more efficiently, particularly over weekends and in the evening. However, this is unlikely to be sufficient to plug the gap created by abandoning third party insourcing. The existing reliance on private hospitals will most likely be exacerbated.
The decision facing the minister is not a simple accounting exercise in which costs and benefits can be traded off. The HSE has clearly lost confidence in the initiative, and it is increasingly becoming a political liability for the Government. It may well have run out of the road, though phasing it out without adding to waiting list pressures will not be easy.

Public opinion is now toxic, warns former minister for health
People in Northern Ireland must wait 11 times longer than people in the Republic for urgent operations, yet the HSE gets no credit, a former minister for health has said. Stephen Donnelly said waiting lists in the Republic have fallen by 60 per cent over the last three years, yet few in the public know anything about it, or want to know anything about it. 'It is improving rapidly. They're doing incredible work. It's not all the way there, but it's moving really quickly,' the former minister told the Patrick MacGill Summer School in Glenties, Co Donegal. 'It's hard to find another country in Europe that has achieved anything like that. Emergency department pressures are falling. Women's healthcare is being transformed. But it's moving in the right direction so quickly that we are on track to have achieved universal healthcare within the next five years.' Despite the improvements, the public narrative about the Republic's health system is 'a black hole', said Mr Donnelly, who served as served as health minister from June 2020 to January 2025. 'Contrary to the national view on this, we have a public health service that is improving at an extraordinary rate,' he said. The negativity surrounding public attitudes towards the health service is typical of the attitudes towards nearly every other element of Irish public life, which is increasingly corrosive and destructive, said Mr Donnelly, who lost the seat he held for Fianna Fáil in Wicklow at the last general election. 'If we are to be serious about navigating what is an increasingly fractured and turbulent world, we're going to have to have a different conversation about who we are. We need to balance the conversation,' he told the summer school. Of course we should, we do and we must always do that, but we've got to start bringing some balance.' Six months after he stepped down as minister for health, Mr Donnelly said: 'What strikes me is that the political debate has become harsher and harsher. You would be forgiven if you were watching a lot of TV for believing we live in a failed state. 'And we really, really don't. Social media has just gone completely off the reservation. Fourteen years ago when I was first in politics, it was fairly all right. People used it to put out their ideas and have chats. Now, it's just poison. 'There's lies and misinformation and hatred and racism and poison, and ... but it is being consumed all the time,' said Mr Donnelly, who was a Social Democrat TD before he joined Fianna Fáil. Social media would have us believe that Ireland is in 'a state of perpetual crisis', he said. 'That the doctors and the nurses are all leaving. The teachers are all leaving. The young people are all leaving, or the Government is uncaring, incompetent and corrupt.' 'You meet them, and they just say, 'What is wrong with you people?'' Ireland does face some 'immense, some deadly serious challenges', but so does every other country, he said. 'The level of negativity that we see here is not normal. It does feel like there is something particular going on in Ireland at the moment.' Ireland is 'incredibly well placed' to become a world leader in the use of artificial intelligence in healthcare, he said. 'There's a wave of innovation coming. Some of it's already here.'

'It's madness': Kerry teen had to trek through three countries to get her endometriosis diagnosed
'It's madness that we had to leave the country twice to try and get a diagnosis." That is according to the mother of a teenage girl whose endometriosis was not diagnosed for three years by doctors in Cork, Kerry, and even Spain before finally she was told by a clinic in Romania what was causing her to have crippling stomach cramps, vomiting, and back pain. 'Endometriosis is being spoken about a lot recently in Ireland, but that talk needs to be followed with action." said Lorraine Fitzgerald. Kaysey O'Connor is 16 now, but since she was around 13 she was blighted by the symptoms of endometriosis. Endometriosis — 'the missed disease' During that time, she was told she had cysts, kidney stones, and ovulation pain and more after MRI scans, ultrasounds, and X-rays. The condition is estimated to affect one in 10 girls and women globally. It is known as 'the missed disease' as worldwide it can take up to 10 years to diagnose. There is no cure, but symptoms can be eased with treatment. Kaysey has been seen in University Hospital Kerry, Cork University Hospital, Cork University Maternity Hospital, and a Spanish hospital also. However, until she was seen in a Romanian private specialist clinic, she did not have a name for what was wrong with her. Lorraine said: The day we went to Romania my daughter was one of three Irish women that day, just that day, in the clinic. She described Dáil discussions this week on endometriosis care as 'very disheartening'. The HSE could not comment on Kaysey's case directly but a spokesperson said: 'The HSE acknowledges that endometriosis care in Ireland has historically been challenging. For this reason, the management of endometriosis has been established as a key priority for the HSE, with significant progress achieved in recent years.' This includes a network of clinics including in Cork, although the HSE also acknowledged recruitment challenges for these clinics. They said other developments being worked on now include a national awareness campaign for the public and better resources for patients.

Warning labels on alcohol an idea from ‘different time', Minister warned Cabinet colleague
Plans to require health warnings on alcohol products were thought up in a 'very different' time to the current period of global economic uncertainty, Minister for Enterprise Peter Burke privately warned a Cabinet colleague. The Government is expected to delay requirements for alcohol products to carry warnings about the links between alcohol consumption, liver disease and cancer. The mandatory health labelling had been due to be introduced next year, but it is expected will now not come into force until 2029. In a May 15th letter, Mr Burke asked Minister for Health Jennifer Carroll MacNeill to consider pushing back the health labelling plans in light of the 'profound' risk Ireland was facing from the current global economic uncertainty. The danger of US president Donald Trump 's sweeping tariff threats starting a transatlantic trade war with the European Union is causing significant concern inside Government including on its impact on Irish exports of whiskey and other alcohol products. Mr Burke appealed to his Fine Gael colleague to take these new circumstances into account and 'pause' plans for alcohol labelling. 'Recent months have seen significant global economic uncertainty and a rapidly shifting trading landscape – which you will be aware could have profound competitiveness implications for small open economies like Ireland,' he wrote. The fact Mr Burke had asked Ms Carroll MacNeill to delay the introduction of the labelling was previously reported but this is the first time the contents of his letter have been reported. 'The proposed measures will mean increased production and sale costs for Irish producers and importers, and add to the price payable by consumers, at a time when prices are also rising due to a multitude of other factors,' Mr Burke wrote. This would come at the same time companies and producers were already seeing 'very significant disruption' to their supply chains, he said. 'Notwithstanding the overarching health benefits of the proposal, I would ask you to consider pausing the introduction of the proposed new requirements,' the correspondence said. It is expected a decision will be taken at a Cabinet meeting on Tuesday to defer the new rules for several years. In his letter, Mr Burke said Ireland's plans had faced pushback from other EU governments, as it was believed the labelling rules would hamper the movement of trade and goods within the bloc's single market. There had also been intense lobbying from the drinks industry, over the 'likely negative impact on sales and costs,' he said.
