A teenager who drowned at a South Wales beach in a post-exam 'coming of age ceremony' might not have died if a lifeguard had been present, a coroner has said.
David Ejimofor, 15, died after getting into trouble in the water off Aberavon Beach, Port Talbot.
His family said he jumped into the sea with friends at high tide in what was said to be a well-known local end-of-school-year tradition.
'Vibrant and promising' David was rushed to the Princess of Wales Hospital in Bridgend, but he could not be saved.
In a prevention of future deaths report, the assistant coroner for Swansea and Neath Port Talbot said the teenager drowned in June 2023 after leaping from the breakwater.
Edward Ramsay outlined how no lifeguard was stationed around the pier to stop the dangerous activity, as had been the case in the past.
The coroner said he had received no explanation why the measure had been stopped before the tragic incident, and warned future deaths could occur unless safety measures are improved.
'The breakwater should not have been used for that purpose but was known to have been used for that purpose by local children and teenagers, especially in the spring and summer months when the weather was good and the tides were high,' Mr Ramsay said.
'In the past, lifeguards had been stationed at or around the pier, at these times, to deter this activity.
'No lifeguard was present at the time that David jumped.
'Had there been one, it is possible that David would not have jumped and therefore would not have drowned.
'I was not given, in evidence, a satisfactory or cogent explanation as to why that measure had been removed prior to David's death, nor why that measure continues to be absent today,' the coroner continued.
After his death, the teenager's family set up a petition addressed to Neath Port Talbot Council demanding they improve the safeguarding at the beach.
But the coroner said he had not been shown 'any evidence that other deterrence measures put in place since David's death (including clearer signage and a limited-height barrier) are otherwise working effectively to reduce the risk.'
In a tribute, David's family said he was 'a vibrant and promising young boy' and a beloved 'son, brother, cousin, nephew and friend'.
They also described him as 'well-behaved and God-fearing' with an 'unwavering passion for sports and health'.
'He was always striving to be the best version of himself. He has an infectious smile, caring nature, and boundless enthusiasm. He touched the lives of many.'
'David's dreams of a bright future were cut short, but his memory will forever live on in the hearts of those who knew and loved him.
'As described by locals, this is a 'yearly coming-of-age ceremony' practised after GCSEs and A-levels that the entire community is aware of,' his family wrote as part of the petition.
'If everyone, including schools, are aware of this practice, surely we would expect the council to have better safeguarding measures in place, especially at this time of year.
'David Ejimofor was not the first to suffer such a fate at the hands of this pier.
'We do not want David to be another statistic.
'We want his name to serve as an end of this unfortunate trail.
'It is for this reason that we seek change and reform by improving safeguarding measures at Aberavon Beach.'
The coroner's report has been sent to Neath Port Talbot Council, Associated British Ports and the Royal National Lifeboat Institution, who have 56 days to respond.

The grieving mother of a teenager whose body was pulled from a river during the weekend heatwave last night said her 'heart was broken'. Emilis Vanagas, 15, was one of five people to lose their life as temperatures soared above 30C and thousands flocked to beauty spots across Scotland. The youngster's body was pulled from Glasgow 's River Kelvin after emergency services were called to the spot, near Maryhill Road, at about 6.40pm on Saturday. A major search and rescue operation was launched, but the schoolboy's body was found in the waterway around five hours later. Tributes have now been paid to the youngster who had only just celebrated his birthday. Mother Daiva Daivute paid her respects to the son she lost on social media, and said: 'My heart is broken, my dear son. 'Always remembered, never forgotten. Forever 15.' The devastated parent, who said it was 'hard to live without him' and that she would do 'anything to have him back in our lives' added: 'I miss him so much. He always cared about everyone around him - he always was nice with everyone. 'We are all going to keep him deep in our hearts and his will never be forgotten.' Aunt Patrycja Majewska said the youngster had just celebrated his 15th birthday and had the 'whole of his life ahead of him'. She said: 'His parents tragically got the news on Saturday that they found Emilis in the river close to their house. 'Unfortunately he was pronounced dead at the scene. 'His parents are devastated after this tragic loss.' Many others paid their respects to the well-liked teen, telling of their devastation. And G20 Youth Project, a local community group, also paid tribute. A spokesman said: 'Our community is mourning the loss of a 15-year-old boy whose life was taken far too soon. 'As you can imagine, no family is ever prepared for something like this.' The circumstances behind how the teenager, known as Emz, ended up in the water remain unclear. A Police Scotland spokesman said inquiries into the death were ongoing but that it was not being treated as suspicious. He added: 'Around 6.40pm on July 12 a search and rescue operation was launched following a report of a 15-year-old male in the River Kelvin, near Maryhill Road, Glasgow. 'Emergency services attended and around 11.30pm the body of a male was recovered from the water.' In a bid to raise money for Emilis's funeral, Ms Majewska has set up an online fundraising page. As of last night it has raised more than £2,900 as hundreds of people gave money to the cause. Four others died in Scotland at the weekend as temperatures shot up to their hottest in more than two years. A 50-year-old man died on Sunday after being pulled from Loch Lomond the day before, while a 63-year-old woman was pronounced dead after falling at the Campsie Glen waterfall. Meanwhile a motorcyclist was killed in a six-vehicle pile-up on the main route to the Highlands. The 71-year-old biker was pronounced dead after the crash on the A82 northbound, between Renton and Balloch, on Saturday. Another motorcyclist, aged 65, lost his life the following day after a crash with a car on the A85 near Lochewe.

Two British tourists have reportedly died on the Algarve after jumping into their holiday apartment pool after a night out. The alarm was raised around 4.30am this morning at a hotel-apartment in the party resort of Albufeira where two British men died last month. Emergency responders sent to the scene made unsuccessful attempts to revive the two men. Police are said to be working on the theory the pair, aged 27 and 29, got into difficulties because they had been drinking and didn't know how to swim. Portugal's Policia Judiciaria have been asked by colleagues at the GNR police force to investigate the circumstances surrounding the men's deaths but they are not being treated as suspicious at this stage. Local reports emerging from Portugal suggest the two men who died this morning were both British and were holidaying with friends. Portuguese police could not be reached this afternoon for an official statement. MailOnline has approached the Foreign Office for comment.

TWO British tourists have drowned after they jumped into their apartment swimming pool after a night out. The 27-year-old and a 29-year-old tragically died while on holiday to Albufeira in Portugal. 1 Emergency services rushed round to the popular party resort at around 4.30am on Tuesday morning. Both men were pronounced dead at the scene. They were both friends from the UK in their late 20s, according to local media. Police are now said to be working on the theory the pair got into difficulties because they had been drinking and didn't know how to swim.
