What is bird flu, and should you be worried about it?
In January, the first person in the US died from bird flu. In February, two more people were hospitalized, and officials detected two new spillovers into cows, indicating the virus is here to stay among livestock and farm workers. The price of eggs has also skyrocketed as bird flu moves through egg-laying chickens.
'The past couple of weeks, it's all been new plot twists in the H5N1 story,' said Meghan Davis, an associate professor of environmental health at the Johns Hopkins Bloomberg School of Public Health.
People who work closely with wild and domesticated animals should take precautions, such as washing their hands, wearing a face mask while handling sick or dead poultry and cleaning their litter, and monitoring symptoms after contact with animals.

But most people should feel 'alert, not alarmed', Davis said.
'The overall risk to the average person remains low,' said Boghuma Titanji, an infectious diseases physician and assistant professor of medicine at Emory University.
'But have an awareness, because that could potentially change – because the outbreak is not quite contained.'
Bird flu is influenza that usually spreads in birds – but it can also be transmitted to other animals and people.
There are several types of bird flu, but right now the H5N1 strain is a highly pathogenic avian influenza spreading widely in wild and domestic species, and spilling over into people.
Bird flu usually causes respiratory symptoms and sometimes stomach upset.
Patients usually have typical flu symptoms: cough, fever, fatigue, body aches and headaches. Some patients have runny or stuffy noses and sore throats, and some people experience diarrhea, nausea or vomiting.
More severe illness includes difficulty breathing or shortness of breath, seizures and what the US Centers for Disease Control and Prevention (CDC) calls 'altered consciousness' – all signs that a patient should go to the hospital.
Very red or irritated eyes (known as conjunctivitis or 'pink eye') have also been a common symptom in this outbreak, and are not a typical flu symptom.
Since 2003, there have been more than 950 reported cases of H5N1 globally, and 49% of patients have died.
In the US outbreak, the mortality rate has been much lower so far. There have been 70 confirmed cases, and several more probable cases, in the past year. One person has died, and three others have been hospitalized.
It's possible the mortality rate is lower because the cases detected in this outbreak have been less severe. But the mortality rate is still significantly high in cases that are bad enough to need hospital care, said Davis.
In countries with poor access to healthcare, the mortality rate may be higher because sick patients aren't able to get care that could save their lives, she said.
'The virus can be really nasty for some people,' said Megan Ranney, an emergency physician and dean of the Yale School of Public Health.
We don't know yet what puts someone at risk of getting really sick, Ranney said.
Flu viruses mutate and evolve quickly. If bird flu begins spreading between humans, there could be changes in how it sickens people, she added. 'There's really no way for me to say, if it becomes a pandemic, that we're going to have the same case fatality ratio that we're seeing right now.'
Bird flu spreads through contact with animals and unpasteurized animal products. That means the people who are most at risk are those who care for chickens and dairy cattle, those who come into contact with wild birds and animals, and anyone drinking raw milk or eating undercooked meat.
There have also been two US cases, in California and Missouri, where patients got sick with H5N1 with no known contact with animals or raw food.
There has been no documented transmission between humans.
However, details about the outbreak have been scarce in the past month especially, starting with a communications blackout at health agencies, so it's not clear if new patterns of transmission have emerged.
'We're missing opportunities to learn more about this virus and the risk that it poses to people and our economy,' said Jennifer Nuzzo, professor of epidemiology and director of the Pandemic Center at Brown University School of Public Health. 'But also, we're missing the opportunity to possibly discover something that would make us worry less about the virus.'
Flu antivirals like Tamiflu seem to work against bird flu by reducing the severity of illness and possibly keeping patients from transmitting it to others.
But Tamiflu needs to be given soon after illness starts. And scientists recently identified a strain that could demonstrate some resistance to antivirals.
'We need more therapeutics,' said Nuzzo.
Yes, there are bird flu vaccines for people and for animals. But we don't use them yet.
Some of the shots for people have been licensed for use in the US, while others are still awaiting approval. The US government has stockpiled 4.8m doses of H5N1 vaccines.
But none of them are available to the public. The reasons why are complicated: there have been few hospitalizations and deaths in the US, there isn't evidence of person-to-person transmission, and officials worry about hesitation over a new vaccine, Nirav Shah, principal deputy director of the CDC, has told the Guardian.
Experts say the vaccine should be made available to farm workers, veterinarians and other people at risk. 'This is a recurring occupational hazard,' Nuzzo said.
There are also vaccines for poultry, and scientists are working on a cow vaccine as well. But none of these have been given to animals yet.
'You should get the seasonal flu vaccine to protect you from seasonal flu,' Davis said. Another reason is that in rare cases when someone catches H5N1, they might catch the seasonal flu at the same time – and the two variants could mix together in a process called reassortment.
This could cause H5N1 to become more easily transmissible or cause more severe disease, suggested Davis. 'Brand-new viruses that can transmit easily are the ones that tend to cause epidemics or pandemics.'
Fewer cases of seasonal flu takes pressure off the healthcare system during flu season, and makes it easier for doctors and health departments to identify rare infections like H5N1.
Any hopes that the current outbreak would simply pass through 'have been dashed', Nuzzo said.
'We have to be looking ahead to what would happen if this virus began spreading more easily between people,' Nuzzo has said. 'If it could spread easily between people, we would be in a pandemic, and it would be around the globe in a matter of weeks.'
Experts expect another flu pandemic at some point. It might not be H5N1 – it might be a combination of H5N1 and a different flu variant, or a different type altogether.

It's important to let your doctor know why you think you might have bird flu – for instance, if you work closely with animals or consume raw milk, meat or eggs.
'It will help the health authorities to get things in the queue much faster for further testing,' Davis said.
Raw milk can contain enormous quantities of virus. While no cases have been reported yet in humans from drinking raw milk, it has sickened and killed mammals such as cats – so officials strongly recommend against it for people, as well. Pasteurization (boiling milk for a certain amount of time) kills the virus and makes milk safe to drink. Eggs and meat should be cooked fully to kill all pathogens, including bird flu, according to the US Food and Drug Administration (FDA).
Davis recommended 'good hygiene practices writ large in the kitchen,' such as washing your hands after handling raw eggs or meat.
It's illegal for sick animals to enter the food chain, but US officials have detected bird flu in meat from dairy cows intended for slaughter. It's important to cook meat to an internal temperature of at least 145F/63C.
'There are many, many, many different pathogens that you can get exposed to from these sources,' Davis said, such as campylobacter, cryptosporidium, E coli, listeria and salmonella.

Healthcare Access, Geography Predict HIV in Black Women
Limited healthcare access and sociostructural factors were stronger predictors of HIV than behavioral risk factors among Black women in the southern United States, based on modeling data from more than 300,000 women in or near Atlanta, Georgia. The findings were presented in a late-breaker study at the International AIDS Society Conference on HIV Science. Nonbehavioral factors such as structural and social determinants of health can complicate HIV risk assessment, wrote Meredith Lora, MD, of Emory University, Atlanta, and colleagues. Previous risk prediction models have underperformed in Black women, whose HIV risk is approximately 20 times that of White women, the researchers wrote. To better assess HIV risk in Black women, the researchers used machine learning to create a model that analyzed electronic medical records data from all women treated at a single center in Atlanta between 2012 and 2022. The study population included 333,263 women; 617 (0.19%) had incident HIV diagnoses and 89% of those with HIV identified as Black individuals. The model included features with a prevalence of 5% or higher in the electronic medical records data and included novel structural determinant features and healthcare utilization features. Overall, the top predictors of HIV included younger age, Black race, residing in high HIV incidence zip codes, and at least one change in phone number or address prior to HIV diagnosis. In addition, women who underwent more HIV screening tests were more likely to be diagnosed with HIV, and women with HIV were more likely than those without HIV to seek care in an emergency department (ED) vs primary care or women's healthcare settings. Although more women with HIV tested positive for sexually transmitted infectious compared to those without HIV in the 2 years before diagnosis, 'seeking sexual health was more important to the model than STI positivity,' the researchers wrote. The model achieved a test area under the receiver operating characteristic curve (AUROC) of 0.90 and area under a precision-recall curve (AUPRC) of 0.14. The appearance of frequent HIV testing as a predictor of risk suggests the presence of undisclosed risk factors that may merit further investigation, the researchers noted in their abstract. EHR models designed for racially diverse female populations may identify more candidates for pre-exposure prophylaxis (PrEP), but that is not enough, the researchers emphasized. 'Evaluating how these models are implemented to support PrEP uptake and behavior change is critical for real-world impact,' they concluded. Assistance in Risk Assessment The current study is important given the traditional and historical difficulty of women in the United States in predicting their own risk of HIV, said Monica Gandhi, MD, director of the University of California San Francisco Bay Area Center for AIDS Research, and a professor of medicine at UCSF, in an interview. Gandhi cited a recent study from the CDC showing that more than 2.2 million individuals in the US need PrEP, but only 336K have received prescriptions. 'Women may not be able to adjudicate their own risk of HIV, as that risk depends on the risk in their male sexual partners,' said Gandhi, who was not involved in the current study or the CDC study. 'Machine learning using electronic medical records [EMR] can make risk prediction more accurate by reporting both health and social factors,' she noted. The current study identified predictors of HIV risk using extracted EMR data, that the healthcare system may be able to mitigate, she said. 'I was surprised by the fact that seeking sexual health was a stronger predictor of HIV than STD positivity, which indicates that women actually are more aware of their risk for HIV in the US than suggested in previous studies that usually involved smaller sample sizes, Gandhi told Medscape Medical News . 'I was also surprised that women with frequent changes in address or phone number had higher risk,' she said. This suggests mobility as a risk factor, which has been identified more commonly in sub-Saharan Africa, she noted. However, Gandhi was not surprised by the preference for ED care or the increased HIV risk among Black women vs other races, both of which have implications for HIV prevention, she said. The preference for sexual health care in the ED setting in the study population suggests that the ED is an important setting in which to perform STD testing and start PrEP or set up an individual with HIV prevention services, said Gandhi. 'Designated PrEP services for women in historically Black neighborhoods are indicated, and women who are mobile and change their address or phone number frequently should be counseled on HIV prevention modalities, specifically PrEP,' she added. Looking ahead, qualitative research is needed to ask a subset of women with HIV who presented to the ED for sexual health services why they chose the ED; this could help inform how to re-engage women in care, Gandhi said. Additional research on women's knowledge of PrEP and whether women with HIV were offered PrEP in the past also would help identify more opportunities for HIV prevention, she said.

Why COVID Spikes in the Summer, and How to Stay Safe
With cases climbing again, you might be wondering why a "winter virus" keeps returning in the heat, and what you should be doing right now to protect yourself. "A couple of months ago, experts were uncertain about a summer wave," said Jodie Guest, PhD, an epidemiologist at Emory University in Atlanta who has tracked COVID since the start of the pandemic. "The low levels of respiratory illness in spring 2025 and stable variant landscape suggested a quiet summer." But three key indicators now have COVID watchdogs sounding the alarm: a new virus strain, increasing wastewater levels, and emergency room visit upticks in parts of the U.S. The CDC now lists the "COVID-19 epidemic trend" as growing or likely growing for more than half of U.S. states. The strongest indicators were seen in Arkansas, Hawaii, Illinois, Iowa, Kentucky, North Carolina, Ohio, Pennsylvania, Texas, and Virginia. Here's what to know about summer COVID, and how to protect yourself – especially if you have a summer vacation or travel coming up. I thought COVID was a big risk during flu season. Did something change? No, COVID has always had two waves, about six months apart – one in the summer and the other at the height of winter "respiratory season," when flu and respiratory syncytial virus (RSV) also rage. The winter wave typically peaks sometime between "December and February, coinciding with colder weather and increased indoor gatherings," said Guest, senior vice chair of the Department of Epidemiology at Emory's Rollins School of Public Health. "Summer waves have occurred sporadically, often driven by new variants or waning immunity." Here's when COVID peaked the past three summers: 2022: Around Aug. 1 2023: Around Sept. 30 2024: Around Aug. 31 This timing has prompted some experts to theorize that summer waves may be linked to people spending more time indoors with air conditioning during the height of summer heat. Why are there two COVID waves, but influenza only spikes once per year? Some scientists point to rapid mutations and waning immunity from past infection or vaccination. "A big chunk of people will get sick in a wave, and they'll have pretty good immunity to that particular variant, and that immunity is enough to sort of stop that wave in its tracks," said Emily Landon, MD, an infectious disease specialist at UChicago Medicine in Illinois. The virus then needs to change enough to evade existing immunity before another cycle starts back up. A second theory suggests there are three distinct groups of people – a summer group, a winter group, and a group that gets infected twice a year. The idea is based on the idea that people vary in how long they're immune after infection, vaccination, or both. Scientists still need more data to know for sure, "but the bottom line is we're certainly seeing a big increase in cases in the summer and a big increase of cases in the winter," Landon said. Is this surge caused by the new variant? Yes, partly. NB.1.8.1 – or Nimbus – has been on the rise in Asia recently, and it now accounts for as many as 43% of new cases in the U.S., up from 24% at the beginning of June, according to CDC data. It's the variant known for a " razor blade" sore throat symptom, although it doesn't appear to cause more severe illness than other versions of the virus, Landon said. This geographic pattern has become pretty set: A rising variant in Asia or Europe typically foreshadows a rise in the U.S., said Sabrina A. Assoumou, MD, MPH, an infectious disease doctor and professor at Boston University Chobanian & Avedisian School of Medicine. Another regional clue: "We have typically seen that the [U.S.] rise in cases starts in the South," said Assoumou. She noted that some of the highest wastewater levels of SARS-CoV-2 (the virus that causes COVID) are in Florida and Alabama right now. "This is often followed by increases in other parts of the nation such as the Northeast." How can I protect myself from summer COVID? Make sure your vaccination is current. If you got a COVID shot last winter, you're probably good, Landon said. But if you're 65 or older, immunocompromised, or have a health condition that puts you at high risk – the CDC website maintains a list of qualifying conditions – ask your health care provider if you need a booster now. People with upcoming travel should consider a booster, too. Landon, who has rheumatoid arthritis and receives treatment that affects her immune system, just got a booster because she's going on vacation soon. "I want the best protection since I know that we're seeing the beginning of a summer spike," she said. Remember that masks are still an effective tool. If you're traveling, wear a mask like an N-95, KN-95, or KF-94, and make it as snug as you can tolerate. Don't just wear it on the plane; wear it while lining up to board, too. It is OK to slip it down to sip a drink or eat a snack while in flight. "I would absolutely recommend that everyone wear a mask on an airplane," Landon said. "The last thing you want is even any kind of cold, let alone COVID, when you're on vacation." Pack some COVID tests in your travel bag. If you know you have COVID, you can consider starting antiviral medication right away, which can reduce your risk of hospitalization. Maybe bring a Paxlovid prescription too. If you're older or have a condition that puts you at high risk, ask your doctor to prescribe Paxlovid or another antiviral called molnupiravir to bring with you in case you get sick. Ask sick people to stay home. If you're planning a summer party, Landon suggested adding a note to the bottom of the invite that says, "We have some high-risk people coming. If you're not feeling well, we'll take a pass and meet up with you another time when you're feeling better." Some people feel obligated to attend parties even when they aren't feeling well. "Letting them know that they're off the hook if they're sick and that you really don't want them to come if they're sick is a really important way that you can help protect your own health and help other people to do the right thing," Landon said.

Unpasteurized vs. pasteurized: What to know about food safety
A long-practiced food processing method has been up for debate in recent years, with some opting to drink and eat without sterilizing it first. The polarizing process is pasteurization — a heating process that kills the microbes behind common foodborne illnesses. Prominent pushes for 'raw milk' consumption are growing, whether it's Health and Human Services Secretary Robert F. Kennedy Jr. or Gwyneth Paltrow. While still on the 2024 campaign trail for his failed presidential bid, Kennedy said he only drinks unpasteurized milk. In April, he suspended a quality control program for testing the nation's dairy supply. All the while, experts and researchers are advocating for Americans to only consume pasteurized milk. Pasteurized vs unpasteurized: What's the difference? Pasteurization is the process of heating a food product for a certain amount of time to destroy bacteria and viruses present, according to the Centers for Disease Control and Prevention (CDC). Dairy products, eggs, juices, canned food, flour, honey and alcohol, among others, are typically pasteurized. Unpasteurized products don't undergo the heating and germ-killing process. They can include milk, artisanal cheese, juice, meat and more. Benefits, drawbacks of pasteurization By getting rid of harmful microbes, pasteurization can prevent foodborne illnesses like listeriosis, typhoid fever, tuberculosis, diphtheria, Q fever, and brucellosis, according to the Food and Drug Administration (FDA). Pasteurization can also change the nutritional value, flavor and appearance of food, though only minimally, per the National Library of Medicine. 'Pasteurized milk offers the same nutritional benefits without the risks of raw milk consumption. Since the early 1900s, pasteurization has greatly reduced milk-borne illnesses,' the CDC says. Are unpasteurized foods safe? What are the risks? Some believe that so-called 'raw milk' and other unpasteurized products are a healthier option, since they are straight from nature. Some champion positive health returns from drinking unpasteurized milk, including raw milk farmer Cliff McConville: 'I can tell you that I used to get sick like three times a year, like, you know, with colds or flu, and I almost never get sick anymore.' But most medical bodies and independent experts say the benefits of unpasteurized food don't outweigh the potential health problems. The CDC and the FDA have warned that raw milk is unsafe because bacteria in it can cause illness or even death. The bird flu virus can remain infectious in raw milk for over a day at room temperature and more than a week when refrigerated, according to a non-peer-reviewed study from a group of UK scientists. The study, published in medRxiv, examined the stability of the H5N1 avian influenza virus in raw cow and sheep milk, with researchers simulating storage conditions common in dairy settings. 'High viral titres were detected in milk from infected cows, raising concerns about onwards human infections,' the authors wrote.
