
Why It's Difficult For People With Chronic Pain To Gain Their Doctor's Trust
Many people with chronic pain find themselves interacting with clinicians who do not seem to trust them. Why is it hard to establish a trusting relationship with your doctor? And what can you do about it?
I have been on both sides of the exam table – as a physician caring for people with chronic pain and as a patient slowly coming to realize he is struggling with chronic pain. I would like to share a story I hope will help you understand why it can be difficult to gain the trust of your physicians.
It was the early 90s, and I was splitting time between two primary care clinics. One Tuesday, I took care of a man in his mid-30s who had previously sustained a serious back injury, with X-rays showing significant damage to multiple lumbar vertebrae. He told me that he experienced a flare of his lower back that morning after picking something up. The pain was now so bad that I had to help him get on the exam table. After assuring myself he had not suffered any nerve damage, I prescribed him a modest supply of oxycodone. (Remember it was the 90s—that's what we did!)
Two days later, I was working in the other clinic and saw the same man in the waiting room, scheduled to see one of my colleagues. I told the colleague that I was familiar with the patient he was about to see and asked my colleague to reach out to me after he finished examining him. The patient told my colleague that he had tweaked his back 'just that morning.' My colleague was inclined to prescribe him narcotics, but I told him about the Tuesday appointment, and we got to digging around for more information. After a few calls, we discovered that this patient was notorious for rotating across clinics seeking narcotics.
In retrospect, I can see all kinds of things I did wrong that week. I gave out narcotics too freely to a patient I barely knew. When I discovered his deception, I did not call an addiction expert to see if we could help this man overcome his need for narcotics. Instead, my colleague and I confronted him about his deceptive behavior and sent him home without further treatment.
But I am not telling you this story to reflect on my mistakes. Instead, I want to share my feelings.
I felt betrayed. I was trying to help this man, earnestly assessing his situation and prescribing a treatment I thought would alleviate his pain. I felt embarrassed that he had so easily fooled me. The dude knew all the right things to say, all the right 'pain postures' to pose in to convince me to prescribe the medications he desired. And I felt angry. I tried to help him and he showed his gratitude by lying to me.
Flooded with emotions, I lost the ability to feel compassion for whatever issues were causing this man to spend his time in medical waiting rooms seeking narcotics.
More importantly, I became more suspicious of subsequent patients who came to clinic complaining of pain. How do I know they are telling me the truth? Their x-rays look horrendous but how do I know whether they are feeling pain? That wince they display when I raise their leg in the air – how do I know that's not an act?
I am telling you this story because someday you may tell a physician about your pain and sense that the doctor does not believe you. Maybe that physician, like me, has been burned.
We physicians should not allow a handful of deceitful patients to undermine our trust in the rest of the people we care for.
We patients, on the other hand, should recognize that some physicians view us with suspicion not because of anything we said or did, but because of other patients they have encountered, patients who left them feeling angry and embarrassed. We should hope that our physicians greet us with trusting attitudes. But we should not be surprised if building such trust takes time.

Think Beyond Stroke in Sudden Facial Droop
Infectious mononucleosis (IM), also known as the kissing disease or 'mono,' is typically caused by the Epstein-Barr virus (EBV). It is a clinical condition characterised by fever, lymphadenopathy, and pharyngitis. Bell's palsy is a condition that causes sudden, temporary facial paralysis or weakness on one side of the face. It can have various aetiologies, such as congenital, neurologic, infectious, neoplastic, or traumatic. This report describes the case of a woman who was diagnosed with Bell's palsy, a rare neurologic complication of EBV-associated IM. The Patient and Her History An 18-year-old woman with a medical history of low back pain and sinusitis presented to the clinic with complaints of fever, sore throat, and left-sided facial drooping. She reported noticing these symptoms 1 week prior. The neck pain had progressively worsened and radiated to the left ear. The patient also experienced difficulty forming words and was unable to raise her left eyebrow or smile due to facial drooping. She noted no improvement with over-the-counter medications and denied recent travel. Findings and Diagnosis On physical examination, several 1-cm mobile tender anterior cervical lymph nodes were noted. Tonsillitis with white exudates and left ear tympanosclerosis with associated pain were observed. The left corner of the mouth was drooping, and the patient had difficulty making facial expressions and closing her left eye. The spleen was palpable and tender to touch. Neurologic examination showed that all other cranial nerves were grossly intact. The muscle strength was 5/5 in both the upper and lower extremities, and deep tendon reflexes were present throughout. The lungs were clear to auscultation, and pulses were palpable in all extremities. No oedema, cyanosis, or clubbing was observed. The skin was dry, and no rashes were observed. Laboratory tests showed mild leucocytosis. Rapid antigen tests for streptococci, influenza, and COVID were negative. A rapid heterophile antibody test for EBV was positive. The mononucleosis spot test was positive for reactive heterophile antibodies, a hallmark finding of IM. Immunofluorescence assays showed positive immunoglobulin M and immunoglobulin G antibodies against EBV, with the presence of immunoglobulin M antibodies indicating either a recent primary infection or a reactivated infection. On the basis of these findings, a diagnosis of left-sided Bell's palsy due to EBV infection was confirmed. The patient was prescribed prednisone 60 mg for 1 week, with a plan to taper the dose. Tylenol was recommended for pain management as needed. Artificial tears were prescribed to prevent corneal damage and dryness due to the inability to close the left eye. An ear, nose, and throat (ENT) referral was made for the evaluation of left ear tympanosclerosis and associated pain. The patient was counselled to avoid contact sports because of the risk for splenic rupture, a common complication of IM. At the 2-week follow-up, the patient reported symptoms such as runny nose, fatigue, and ongoing ear pain, pending evaluation by an ENT specialist. At that time, there was no evidence of lymphadenopathy, fever, or splenomegaly. The patient's facial paralysis had improved by approximately 70%, and her speech was also markedly improved. The patient was counselled to rest, stay hydrated, and closely monitor for new symptoms. Discussion IM, most often caused by the EBV, is a viral illness that primarily affects teenagers and young adults. It is transmitted through saliva — which is why it is often referred to as the 'kissing disease' — but can also be spread via respiratory droplets, blood, or organ transplants. Symptoms may include sore throat, fever, swollen lymph nodes, and extreme fatigue, as well as swelling of the liver and spleen. A heterophile antibody test is typically sufficient to diagnose IM. Most people recover on their own with rest, fluids, and over-the-counter pain relievers. However, in rare cases, complications such as a ruptured spleen, neurologic issues, or liver inflammation can occur. Neurologic complications, such as cranial nerve palsies, Guillain-Barré syndrome, meningoencephalitis, and mononeuritis multiplex, occur in approximately 1%-5% of all patients with acute EBV infection. In children, neurologic manifestations may be the only clinical sign that can occasionally delay diagnosis. Acute peripheral facial paralysis is the most common acute mononeuropathy and often presents a diagnostic challenge for physicians. Its incidence has been reported as 21.1 per 100,000 per year in children younger than 15 years. Paediatric facial nerve paralysis can be congenital or acquired, but the idiopathic form, known as Bell's palsy, is the most frequent, accounting for 40%-75% of cases. Bell's palsy is diagnosed by the abrupt onset of unilateral facial weakness or complete paralysis of all muscles on one side of the face. It may be accompanied by dry eyes, pain around the ear, an altered sense of taste, hypersensitivity to sounds, or decreased tearing. Recovery time varies, but complete resolution of symptoms usually occurs within 2-3 weeks. There is always a small possibility of permanent functional loss, which can result from structural damage to axons and myelin. Treatment is usually supportive, often involving a combination of steroids and, in some cases, physical therapy. Facial nerve palsy is most often idiopathic and related to microcirculatory changes but may also result from congenital defects, infections, trauma, neoplasms, or systemic illness. Infectious causes account for 5%-30% of cases and are the most common cause in young children. Common pathogens include herpes simplex virus 1, varicella-zoster virus, EBV, Borrelia burgdorferi , HIV, and influenza virus. The causes of facial palsy are classified as follows: Genetic: Hereditary myopathies, gene mutations Syndromic: Craniofacial malformations and brainstem disorders Delivery-related: Prematurity, instrumental delivery, caesarean section Infectious: Viral, bacterial, and fungal agents including chronic or acute otitis media Inflammatory: Autoimmune and vasculitis conditions Neoplastic: Tumours involving the facial nerve or surrounding structures Traumatic: Skull fractures affecting the facial nerve Iatrogenic: Injury during head and neck surgeries Idiopathic: Bell's palsy Timely diagnosis and stroke exclusion are critical to avoid permanent damage. Lyme disease should be ruled out, especially in patients with recent travel to endemic areas and a bull's eye rash.

Medicine Shortages Leave Pharmacies at ‘Breaking Point'
Continuing problems with the supply of medicines have become a 'distressing new normal' for community pharmacies and patients, according to a report by Community Pharmacy England (CPE). The organisation said that the supply chain is 'stuck at breaking point' and warned of a growing risk to patient health and increased workload and stress for its members. The 2025 Pharmacy Pressures Survey found that 87% of pharmacy staff now face daily supply issues – up from 67% in 2022. The survey gathered responses from owners of over 4300 pharmacy premises and 1600 team members. The CPE called for urgent action to address shortages. The report follows a warning in March from the Centre for Long-Term Resilience (CLTR), which said the UK is 'perilously vulnerable' to disruptions of critical medical countermeasures. These included shortages of critical antibiotics such as gentamicin. Serious Shortage Protocols Extended A growing number of essential medications are now covered by Serious Shortage Protocols (SSPs). SSPs allow pharmacists to either substitute a prescription with an alternative agent for one month, or to supply a reduced quantity that is in stock without a new GP prescription. In the past year, they have been issued for: Earlier this month, the government extended the current Serious Shortage Protocol (SSP) for Creon, a pancreatic enzyme replacement therapy. The National Pharmacy Association (NPA) warned that some patients were rationing doses, skipping meals, or travelling long distances to access the medicine. Rising Aggression and Workload The Pharmacy Pressures Survey 2024, published in May last year, identified a 'beyond critical' situation, with most pharmacies impacted by supply disruptions. It said patients were forced to play 'pharmacy bingo' in their search for medications. Some were directing their anger towards staff, the CPE warned then. The latest CPE report found that: 96% of pharmacy team members had encountered patient frustration in response to supply issues. 86% of pharmacy owners said that shortages were leading patients to have to visit multiple pharmacies in search of medicines. 79% of team members reported incidents of aggression when medicines were unavailable or delayed. Supply issues have also worsened, with 80% of pharmacy owners encountering daily wholesaler shortages and 26% reporting daily supply chain failures. Most (74%) pharmacy owners reported that their staff were having to spend longer than ever before on medicines procurement, with 39% spending one to two hours daily sourcing alternative medicines. More than 90% of pharmacy staff reported increased workload and stress because of supply problems. Patient Safety at Risk CPE chief executive Janet Morrison said the survey showed that 'as medicine supply issues remain a daily reality across the country, the risk to patient health has become a distressing new normal'. The survey results 'suggest a system that is stuck at breaking point', she said. Olivier Picard, chair of the NPA, which represents over 6000 independent community pharmacies, said that staff were 'at the sharp end of medicines shortages' and often forced to turn patients away despite having safe alternatives in stock. 'It is madness to send someone back to their GP to get a prescription changed,' Picard said. 'It risks a patient either delaying taking vital medication or forgoing it altogether, which poses a clear risk to patient safety.' Picard endorsed the CPE's call for the government to give greater flexibility to pharmacists to use their professional judgment to supply an appropriate alternative medication when the prescribed version is unavailable. Louise Ansari, chief executive of Healthwatch England, also supported giving pharmacists more autonomy. She said allowing substitutions with patient consent — where clinically safe — could ease pressure on patients and services. 'Providing clear guidance to the public on what steps to take if their medication is unavailable is crucial,' she added.
