New cholesterol drug lowers LDL when statins aren't enough, study finds

A new medication that combines an already approved drug with a new unapproved one has been shown to cut the level of LDL, or 'bad' cholestero l, when statins aren't helping enough.
In the Phase 3 trial, Cleveland Clinic researchers found that the combination of the new drug, obicetrapib, with an established medication, ezetimibe, reduced low-density lipoprotein (LDL) cholesterol levels by 48.6% after about three months' use — producing more effective results than either drug alone. Ezetimibe is a cholesterol-lowering drug that is often prescribed with statins to reduce LDL even more.
The research was presented Wednesday during a late-breaking science session at the annual meeting of the European Atherosclerosis Society in Glasgow, Scotland, and simultaneously published in The Lancet.
In the multicenter clinical trial, the lead researcher, Dr. Ashish Sarraju, a preventive cardiologist at the Cleveland Clinic, and his colleagues enrolled 407 patients with a median age of 68 with LDL cholesterol levels greater than 70 mg/dL even though they had taken medication to lower it.
The participants were randomly assigned to four groups: a group for a pill that combined obicetrapib with ezetimibe, a group for each of the drugs separately and a placebo group. All participants continued on the medications they were taking before they started the trial, along with the medications being studied.
The reason: Some people have to take a number of prescriptions to get LDL down to desired levels.
'We need to give patients and their doctors all the options we can to try to get LDL under control if they are at risk for, or already have, cardiovascular disease,' Sarraju said. 'In higher-risk patients, you want to get LDL down as quickly as possible and keep it there as long as possible.'
High-risk patients either had had strokes or heart attacks or were likely to in the future.
For that reason, the researchers enrolled patients in the trial who, despite already being on statins or even high-intensity statins, still had LDL levels that were too high.
The hope is that lowering LDL levels will reduce the risk of adverse cardiovascular events such as strokes and heart attacks. According to the American Heart Association, the optimal total cholesterol level for an adult is about 150 mg/dL, with LDL levels at or below 100 mg/dL ('dL' is short for 'deciliter,' or a tenth of a liter). For high-risk patients, Sarraju recommends an LDL no higher than 70 mg/dL.
The trial was funded by the maker of obcetrapib, Netherlands-based NewAmsterdam Pharma. It expects to have conversations with the Food and Drug Administration about approval for the new combo drug 'over the course of the year,' a spokesperson said.
A multitude of modifiable factors can result in high LDL, such as a diet high in saturated fats, processed foods and fried foods; being overweight; smoking; and older age.
Dr. Robert Rosenson, director of lipids and metabolism for the Mount Sinai Health System in New York City, said other drugs in the same class have failed to reduce heart attacks or stroke, 'but I am cautiously hopeful.'
The drugmaker is currently running an additional trial to determine if the combo drug not only lowers cholesterol but also protects against adverse heart events.
While lifestyle changes can help bring down LDL, levels remain stubbornly high for some people. Only 20% of patients at high risk of heart disease are able to manage their LDL, said Dr. Corey Bradley, a cardiologist at the Columbia University Vagelos College of Physicians and Surgeons.
Heart disease is the leading cause of death for adults in the United States.
'High LDL is one of the leading risk factors for heart disease, and we have such a poor handle on controlling that risk,' Bradley said. 'Many people have such a high LDL they will require multiple agents to control it.'

A 103-year-old marathon runner claims he halted the spread of his cancer with a radical diet overhaul. Mike Fremont, from Cincinnati, Ohio, went against doctors' advice and rejected drugs and surgery when he was diagnosed with stage 3 colon cancer at age 69. Doctors at the Cleveland Clinic told him the disease would kill him within three months unless he began chemotherapy and radiotherapy, followed by surgery to remove the main tumor. At that point, the cancer had already spread to his lymph nodes — a stage known as metastasis — when the disease is still potentially curable, but at high risk of becoming terminal. Typically, doctors use drugs to shrink the tumor so it can be surgically removed. But instead of opting for proven treatments, Fremont switched to a strict vegan diet centered on whole foods like sweet and white potatoes (without oil), tofu, leafy greens, legumes, vegetables, fruit, and non-dairy milks. He claims the plant-based lifestyle cured his metastases. Two years later, he underwent surgery to remove the tumor and says he has been cancer-free ever since. But doctors remain fiercely skeptical, warning that even the strictest vegan diet cannot replace chemotherapy or immunotherapy. 'While a healthy diet plays a supportive role in cancer care, doctors argue it must be combined with medical treatments. Nutrition alone cannot eliminate cancer.' Cancer experts previously told the Daily Mail they are seeing a 'concerning' rise of patients using controversial treatments. A 2023 study found 70 percent of cancer patients use alternative medicine, one in three of that share use it as their sole method of treatment and 27 percent hid it from their oncologists. Dr Heber added: 'A nutritious diet should be used along with proper treatments in cancer patients, but diet alone absolutely cannot cure cancer.' Around 75 percent of colorectal cancer patients will be alive five years after their diagnosis with treatment. Without treatment, survival rates drop to just 44 percent after three years, making medical intervention absolutely crucial. A former Yale University wrestler, Fremont had been an avid runner since the 70s when he was grieving the death of his wife. His first of more than a dozen marathons was the 1972 Boston Marathon. At 103, the retired engineer and climate activist still holds multiple world records as the fastest marathoner in the 88, 90, and 91-year-old age categories. That's why it was so shocking when Fremont got the world-upending news in 1991. He said: 'I contracted a cancer which the Cleveland Clinic said would kill me in 3 months unless operated upon. 'I said no, I was going on a diet! 'The tumor had metastasized into my lymphatic system and would have required efforts to treat the metastases with radiation and/or chemotherapy.' Two years later he underwent an operation to remove the tumor from his colon when it began to bleed. After the procedure, doctors checked for cancer spread in 35 places around Fremont's body, he said, and found zero signs of cancer spread to other parts of the body. Fremont believes that his stellar health and advanced age are primarily due to his habits, rather than genetics. His father died of liver cancer, and his mother of a heart attack. He said: 'The results of the diet have been perfect and it has been since that diet that all my world records have come about as well as such distinction in the US.' And he doesn't plan on slowing down. 'My route is 5 miles long, through a scenic park near Cincinnati with mixed surface, lots of hills,' he said. 'The first mile is at my maximum speed, the other four under pressure to run fast. I resolved at age 98 to run 5 miles rather than 10, but to do it at speed.' Fremont has always followed a disciplined fitness plan, saying he runs five miles three times a week and incorporating push-ups and pull-ups. A typical day of eating for Fremont begins with oatmeal, syrup, and blueberries for breakfast. Lunch consists of beans, while dinner includes broccoli florets topped with ketchup. Fremont also lives a low-stress lifestyle, using exercise as a form of emotional release. He also does not drink or smoke. His healthy lifestyle has been shown to prevent cancer and its recurrence. Research has shown that a structured exercise program for stage II and III colon cancer patients reduced recurrence by 28 percent, lowered mortality risk by 37 percent over eight years, and showed benefits with just 1.5 to 2.25 hours of brisk walking weekly. Focusing on whole, unprocessed plant foods increases the amount of beneficial nutrients and compounds taken in that help reduce inflammation, a key factor in cancer development. Additionally, these dietary choices can support healthy weight, cholesterol levels, and blood sugar control, all of which contribute to a lower cancer risk But there is no concrete evidence diet alone can reverse cancer.

Slim Jims — the iconic spicy, dried meat stick— are set for a major recipe overhaul. The company that makes them said it is removing all artificial dyes from its products by 2027 amid pressure from health secretary Robert F Kennedy Jr. Chicago-based Conagra Brands said Birds Eye frozen vegetables, Orville Redenbacher popcorn and Duncan Hines baking products will also be affected by the change. The coloring agents will likely be replaced with natural ingredients such turmeric or beet juice, which could change the look, change and texture of many iconic foods. The announcement follows similar plans from brands like Nestle, Kraft Heinz and General Mills, many of whom have said they also won't launch any new products with artificial dyes. Health officials have long called for the removal of dyes like Red 40 and Yellow 5 over concerns of hyperactivity and conditions like ADHD in children. Some studies have also linked them to cancer in animals and hormonal issues. Conagra said in its annoucement Wednesday it will remove all dyes from its frozen foods by the end of 2025 and stop selling products with artificial dyes in schools by the start of the 2026-2027 school year. By the end of 2027, it will fully discontinue use of synthetic dyes from all of its products. RFK Jr has outlined plans to strip eight petrolium-based dyes from the US food supply during his tenure as the head of the Department of Health and Human Services. Slim Jims don't specifically say on the packaging that they contain artificial dyes, though it's possible other additives like sodium nitrite could be removed. The push comes amid concerns in recent years over health effects of artificial dyes, including animal studies suggesting they can cause hyperactivity, cancer and other health problems. With a recipe change could also come differences in color, texture and taste, though it's unclear how subtle the shifts will be. Conagra said many of its products already use natural alternatives like turmeric and annatto, especially in items like pickles, frozen vegetables and popcorn. However, products like Duncan Hines frosting and Swiss Miss pudding still contain Red 40, Yellow 5 and Blue 1. Tom McCough, executive vice president and chief operating officer of Conagra Brands, said in a statement: 'Conagra Brands is known for innovating delicious, on-trend foods, and our transition away from FD&C colors is just one aspect of our broader strategy to modernize our portfolio to align with consumer preferences. 'Our leading portfolio of frozen brands, including Birds Eye, Healthy Choice, Marie Callender's and more, will be 100% free from FD&C colors by the end of 2025.' Health advocates have long called for the removal of artificial dyes from foods, citing studies indicating they can cause neurobehavioral problems, including hyperactivity and attention issues, in children. The FDA has maintained that the approved dyes are safe and that 'the totality of scientific evidence shows that most children have no adverse effects when consuming foods containing color additives.' The agency currently allows 36 food color additives, including eight synthetic dyes. However, it announced in January that Red 3 — used in candies, cakes and some medications — would be banned in food by 2027 because it caused cancer in laboratory rats. Several states, including California and West Virginia, have passed laws restricting the use of artificial colors in foods. FDA Commissioner Dr Marty Makary announced in April that the agency will phase out the use of eight artificial food dyes in America's food supply within the next two years. The massive shakeup will free children from the 'toxic soup' that makes up much of America's food supply, Dr Makary said. It's unclear if Conagra will reformulate the affected products or discontinue them entirely. The company has also not announced plans to change the recipes globally.
