Monsoon results in an increase in subsoil ground water, which can be contaminated by sewage leaks and this contaminated water can be transported to the wells and sometimes to households through the pipe water supply. Hepatitis E virus and hepatitis A virus are endemic in India and are excreted through human excreta. It can contaminate drinking water as well as food processed by such water. These viruses are transmitted to humans orally through contaminated water and food causing acute hepatitis manifesting as jaundice due to liver dysfunction. They can be prevented by drinking boiled and then cooled water or after treatment by due process of chlorination. Avoid eating foods from street vendors and prepared in unhygienic conditions.
Q. As a leading hepatologist, what trends do you notice in liver disease prevalence over the last decade in India, particularly among the younger population?
The prevalence of alcohol induced liver damage and fatty liver disease are rapidly increasing in India. The two problems constitute more than three-fourth of overt clinically damaged liver presenting in various stages of liver dysfunction. More than half of the hospital beds in a gastroenterology unit or even many medical units, comprise of advanced and not infrequently life threatening complications of these two etiology-induced liver damage. Though hepatitis B and hepatitis C associated chronic liver disease is in decreasing trend, it continues to be a problem. These conditions can lead to chronic liver disease like cirrhosis and liver cancer which often compromise quality and quantity of human life. All of them can be prevented by awareness, HBV vaccination, avoiding alcohol consumption and foods which cause obesity (sweets, processed foods, aerated drinks, saturated and trans-fats). Younger people are suffering from these diseases due to increased alcohol consumption, lack of regular physical exercise to reduce cardio-metabolic risk factors like obesity, diabetes, dyslipidaemia and hypertension etc.
Q. NAFLD is now being called the silent epidemic. What factors are contributing to its rapid rise in urban and rural India?
Sedentary lifestyle, consumption of junk foods, rapid rise in prevalence of type-2 diabetes mellitus, along with even moderate alcohol consumption (which often accompanies snacking causing additional unwanted calories consumption) are causing more and more fatty liver disease as well as liver damage in people. It is often due to both fat and alcohol which is medically known as Met-ALD (metabolic dysfunction - met with alcoholic liver disease).
Q. How does NAFLD progress into more serious conditions like non-alcoholic steatohepatitis (NASH), cirrhosis, or even liver cancer and what early warning signs should people watch for?
A. About 70% to 80% of diabetics and about 30% to 50% of overweight and obese people (BMI > 23 to 25 with abdominal obesity) may have increased fat in the liver which can progress to more advance liver damage like NASH, now renamed as MASH - Metabolic dysfunction associated steatohepatitis. It indicates liver inflammation and damage often leading to fibrosis. At least, 10% to 25% of people, depending upon the presence of various other factors like dyslipidaemia, diabetes, and even infrequent alcohol consumption, lack of physical activity and consumption of high-risk foods may progress to serious liver damage in the next 10 years. Such people are also more prone to heart attacks, strokes and cancers.
Q. What are some under-discussed lifestyle factors, beyond alcohol and obesity that are damaging liver health these days?
Prolonged sitting, more time spent on screen watching, less sleep, more consumption of carbohydrates and sweets, fried foods, frequent consumption of painkillers and many unwanted complementary alternative medicines even un-indicated hepatotoxic antibiotics may be associated with liver damage.
Q. How do comorbidities like diabetes and hypertension worsen liver function or disease outcomes?
Diabetes and hypertension are manifestations of metabolic dysfunction of the body due to lifestyle changes and often are associated with fatty liver, cardiac dysfunction, renal complications and many other abnormalities. Their association with pre-existing fatty liver are well known synergistic factors for progression of liver disease.
Q. Given the rise in poly-pharmacy, especially in middle-aged individuals, how concerning is drug-induced liver injury (DILI)?
DILI is another problem compounding the liver disease. Drugs for tuberculosis, anti-cancer drugs, complementary alternative medicines, some antibiotics, painkillers and many other drugs can cause liver damage. Therefore, close monitoring of liver functions is needed during such medications.
Q. Is there a role for routine liver function screening even in asymptomatic individuals, especially among those with sedentary lifestyles or obesity?
Definitely. All risk factors, if present in any individual, without symptoms should be evaluated for liver, cardiac and renal dysfunction. Occurrence of hypertension, increase in uric acid, compromised bone density and presence of polycystic ovaries in females should also be looked into. Even fat infiltration to muscles can be there causing increased fatigue and relative muscle loss. Fat in the liver easily detected by an abdominal ultrasound indicates a generalised metabolic dysfunction of the whole body. The human body and its organs evolutionarily have substantial reserves and therefore metabolic dysfunction associated organ damage can be asymptomatic. Therefore, individuals with risk factors like obesity, diabetes, hypertension and alcohol abuse should be screened for presence of abnormalities in various organs. If diagnosed early, all of these abnormalities are completely reversible.
Q. Is there a connection between immunity, gut health, and liver function that people need to be more aware of, particularly during seasonal changes like the monsoon?
The human body should be assessed holistically. Organ cross talk is a naturally evolved process. Gut microbiome is an important determinant of our immune competence and is a barrier against environmental infection. They can enhance liver damage in fatty liver disease and can get altered in presence of diabetes, alcohol consumption and various other bowel as well as systemic illnesses. Diet (vegetarian and non-vegetarian) can alter gut microbiome affecting human health in susceptible individuals. However, monsoon per se cannot be blamed solely for gut health. Rather, it is diet and types of diet, alcohol, presence of uncontrolled diabetes, frequent consumption of antibiotics, bowel habits etc which can influence gut microbiome.
Q. What is your take on hepatitis A and B vaccination rates in India? Are we doing enough to prevent these preventable liver diseases?
Hepatitis B virus immunisation by vaccination starting at birth is already a part of the universal immunisation programme of the Government of India and is being implemented in all states. However, I think Hepatitis A virus vaccination should be given to all children starting at the age of around two years, because hepatitis A-virus induced acute hepatitis is on the rise in all states and often is causing severe liver disease in children and young adults.
Q. Do you believe India needs a national liver health programme or policy, similar to its initiatives on diabetes and heart disease and why?
Yes. Fatty liver disease prevalence is around 30% to 40% in the Indian population. Alcohol consumption is on the rise. More and more DILI are being documented. Substance use is on the rise. Liver due to its huge reserve remains often asymptomatic despite having damage due to various etiologies. When clinically liver dysfunction becomes overt, often disease is in an advanced stage and may be difficult to get corrected needing liver transplant.
Q. How can primary care physicians and public health workers be better trained or equipped to detect liver-related diseases early?
Awareness, availability of liver function tests, ultrasound screening of liver and regular training of doctors and health professionals about various tests and screening methods are necessary to encourage identification and screening of high risk persons prone to develop liver disease.
Q. As liver diseases do not discriminate between vegetarians or non-vegetarians and alcoholic or non-alcoholic, what would be your liver care tips for the common man?
I think regular exercise, nil alcohol consumption, less consumption of foods with high glycaemic index, control of diabetes and dyslipidaemia, if present, avoidance of un-indicated medication, avoiding red meat consumption, saturated and trans-fat, aerated drinks and processed food will be helpful to all in not only preventing liver disease but also promoting good health in general. Further, since smoking aggravates all diseases and it should be completely avoided.

From 'mild jaundice' to liver transplants: How Hepatitis A is turning deadly for adults in India
Hepatitis viruses B and C have long been regarded as the more dangerous members of the hepatitis family due to their association with liver cirrhosis and liver cancer. However, hepatologists in Mumbai are now observing a surprising shift in the pattern of infections. While new Hepatitis C cases are dwindling, Hepatitis A, once associated primarily with mild childhood jaundice, is now causing severe illness in adults. As a result, they don't develop immunity, and adult infections can be much more severe.' Hepatitis C: On the Brink of Elimination? Since the introduction of direct-acting antiviral treatment under the National Viral Hepatitis Control Programme in July 2018, doctors report a drastic fall in new Hepatitis C infections. 'New cases are becoming rare, which suggests the national programme has been effective,' Dr Shukla told TOI. According to data from Maharashtra's health department, over 28 lakh people were tested for Hepatitis C in 2024–2025, with 9,476 testing positive. Of these, 2,102 have commenced the curative three-month course. By comparison, only 1,339 Hepatitis C cases were recorded in 2021–2022. 'There are virtually no new infections. We recommend that people who underwent blood transfusions before 2000 get tested, as they can now be treated and cured,' he said. Doctors remain optimistic that Hepatitis C could be eliminated by 2030, in line with World Health Organisation (WHO) targets. Hepatitis B Remains a Major Concern While progress has been made on Hepatitis C, Hepatitis B remains a formidable challenge. WHO has set a goal of reducing hepatitis cases by 95% by 2030, but experts believe that Hepatitis B could take 'a decade or more' to show meaningful decline. 'Hepatitis B is one of the most infectious viruses, with a tendency to reactivate. Current treatment aims to suppress the virus rather than eliminate it,' Dr Shukla explained to TOI. State health data reveals that confirmed Hepatitis B cases rose sharply, from 9,000 in 2021–2022 to over 65,000 in 2024–2025, including 23,000 among pregnant women. Dr Sharan Shivaji Narute from Medicover Hospitals, Pune, highlighted the case of a 53-year-old man who required a liver transplant after battling chronic Hepatitis B. 'At one point, we were draining up to 13 litres of fluid from his body each session. In June 2024, his son donated a portion of his liver. The transplant was a success,' he said. Testing Uptick and Expanded Access Increased testing capacity is reshaping India's hepatitis outlook. Shiva Sriram of Molbio Diagnostics told TOI that improved diagnostics have made a significant impact. Their Truenat machines saw a 150% rise in Hepatitis B tests (from 3,060 to 7,705) and a five-fold jump in Hepatitis C tests (from 475 to 2,475) between 2023–24 and 2024–25 in Maharashtra. Though tests for Hepatitis A and E remain relatively low despite seasonal spikes, especially during monsoon, Sriram anticipates this will change as the Union Government is reportedly planning to deploy Truenat machines to all 40,000 primary health centres in the next two years. Pregnant Women at Increased Risk Doctors warn that pregnant women are especially vulnerable to Hepatitis A and E, which are primarily spread through contaminated food and water. A recent case in Pune underscored the risk. A 32-year-old pregnant woman admitted with vomiting and fever was initially misdiagnosed with morning sickness. Elevated liver enzymes revealed it was Hepatitis A. 'She had a history of eating food from outside. Her eyes and urine were bright yellow. Fortunately, she responded well to intravenous fluids and medication,' said Dr Padma Srivastava, her gynaecologist at Motherhood Hospitals, in a statement to TOI. Dr Nitin Gupte, gynaecologist at Apollo Spectra in Pune, added that 2 to 3 out of every 10 pregnant women in his outpatient department show symptoms of hepatitis. Lifestyle changes, rising obesity, and dietary habits are among the key drivers of what experts are now calling a 'silent epidemic." A Modern Lifestyle Problem At the core of this alarming trend is the modern sedentary lifestyle. Long hours of sitting—whether at desks, in front of screens, or during commutes—combined with a lack of physical activity disrupt normal metabolism and promote fat accumulation in the liver. What's more concerning is that even individuals who appear slim or fall within normal BMI ranges may be at risk due to what's now termed 'lean NAFLD," where visceral fat affects internal organs without visibly increasing body weight. Unhealthy eating habits are another major contributor. 'The easy availability and frequent consumption of processed, high-sugar, and high-fat foods, especially among college students and young professionals, is drastically impacting liver health. Add to that sugary drinks and late-night eating habits, and the liver is constantly under assault," explains Mool Meena, Founder & CEO, Sheopals. What makes fatty liver especially dangerous is its potential to act as a gateway to more serious conditions. NAFLD increases the risk of type 2 diabetes, cardiovascular disease, and liver cancer. In some cases, it may lead to liver failure, requiring transplantation. Prevention Through Lifestyle and Ayurveda The good news is that fatty liver is largely reversible, especially in its early stages. A multi-pronged approach involving a healthy diet, regular exercise, weight management, stress reduction, and routine health checkups can make a significant difference. Nutritionists recommend a balanced diet rich in fruits, vegetables, lean proteins, and whole grains, while avoiding saturated fats and refined sugars. Exercise, including strength training and cardio, for at least 150 minutes a week, is critical. Losing just 5–10% of body weight can substantially reduce liver fat and inflammation. Complementing lifestyle changes, traditional herbs are also gaining recognition for their hepatoprotective properties. Herbs like Kutki (Picrorhiza kurroa), Bhumi Amla (Phyllanthus niruri), Punarnava (Boerhavia diffusa), Bhringraj (Eclipta alba), and Makoi (Solanum nigrum) have been used in Ayurveda to help repair liver cells, detoxify the liver, and protect against chemical damage. 'Scientific interest in these herbs is growing, as they offer a natural adjunct to lifestyle management for fatty liver. "Hepatitis A is emerging as a big enemy, causing liver disease in adults and, in some rare cases, failure," said Dr Akash Shukla, head of gastroenterology in BMC-run KEM Hospital, Parel, and a member of the technical resource group for the National Viral Hepatitis Control Programme. Senior hepatologist Dr Aabha Nagral concurred: "As the general standard of living in India, especially metros such as Mumbai, has improved, people are no longer getting exposed to the hepatitis A virus in their childhood. They don't gain immunity against it and hence could suffer from severe disease if affected in adulthood." Hep C elimination by 2030? Almost six years after the direct-acting antiviral Hepatitis C treatment—a "sureshot cure"—was made available for free through the National Viral Hepatitis Control Programme in July 2018, there are fewer new patients coming in at private hospitals. "New patients coming in with Hepatitis C has dropped, indicating that the national programme has worked well," said Dr Shukla. Data from the state health department shows that 28 lakh people across the state were tested for Hepatitis C in 2024-2025, with 9,476 testing positive. Of these, 8,270 were subjected to further tests, and 2,102 were started on the three-month curative treatment. Incidentally, in 2021-2022, the state had 1,339 Hepatitis C cases. But Hepatitis B, considered to be one of the most infectious viruses, is still a "decade or more" away from any reduction in incidence, said doctors. This is because of its peculiar genetic structure and ability to reactivate; present treatment focuses on checking the multiplication of the virus and preventing liver cirrhosis and cancer. Maharashtra health department data shows the number of confirmed Hepatitis B cases went up from 9,000 in 2021-2022 to 65,000 last year, including 23,000 pregnant women.
