Much of the threat today comes from Pakistan, where doctors are facing a terrifying challenge. The typhoid strain spreading there is no longer afraid of our antibiotics. Not even the newest ones. This strain, now called extensively drug-resistant Typhi or XDR Typhi, does not respond to the usual weapons – ampicillin, chloramphenicol, trimethoprim, fluoroquinolones or even cephalosporins. These used to be life-savers. Now they barely scratch the surface.
A team of researchers led by Stanford's Dr. Jason Andrews studied nearly 3,500 bacterial samples collected from Nepal, Bangladesh, India and Pakistan between 2014 and 2019. What they saw was eye-opening. The number of drug-defiant typhoid strains is growing fast. They are replacing treatable ones. And they are not staying local.
The study revealed something more chilling – these mutated bacteria are on the move. Since 1990, scientists have tracked nearly 200 instances of international spread. Southeast Asia. East Africa. Southern Africa. And now, Western countries. Yes, even in the United Kingdom, the United States and Canada, typhoid superbugs are starting to show up.
Every plane ticket. Every border crossing. Every contaminated sip or bite. The bacteria need only one chance.
Globally, typhoid and paratyphoid infect around 13 million people every year. Over 1.3 lakh people lose their lives, most of them children in Asia and Africa. Despite its scale, typhoid rarely makes headlines. But that silence is dangerous. Especially now.
The only treatment left is oral antibiotics. But their power is fading. Over three decades, the bacteria have gradually built resistance. They have learned. Adapted. And now, they are spreading those learnings across borders.
The UKHSA data is just one part of the puzzle. Researchers say there are large blind spots, particularly in regions like sub-Saharan Africa and Oceania. We simply do not have enough bacterial samples from these places. Even in better-monitored countries, the samples usually come from just a handful of sites. That is like trying to map a wildfire by staring at a single tree.
That lack of information means something troubling – we may be underestimating the scale of resistance and how fast it is travelling. The bacteria's genes and the mutations that make it immune are quietly crossing oceans. And we are not watching closely enough.
Dr. Andrews calls this a 'real cause for concern'. He is not exaggerating. The way these strains have spread shows that typhoid control is a global emergency. Surveillance needs to expand. So do new treatment strategies. And vaccines need to reach where the outbreak begins, not where it ends.

The couple, against whom no charges have been brought, were held "in a high-security facility for several months, then in underground cells, without daylight, before being transferred last week" to the intelligence services in Kabul, according to the UN. The experts said Peter Reynolds requires heart medication following a stroke in 2023. Since his detention, he has suffered two eye infections and intermittent tremors in his head and left arm. His wife, who is anemic, is "weak and fragile" and has reported numbness in her feet, the experts said. The couple, who married in Kabul in 1970, had been running education programmes in Afghanistan and held Afghan passports. Taliban officials have refused to detail the reasons for their arrest but a source familiar with the case told AFP in April that the couple were in possession of several non-Islamic books.

Outright bans on youth contact sport are one option. Another is to focus on protective gear. But in rugby, at least, research has yielded surprising results. Padded 'scrum caps" do protect ears and reduce cuts, but neither scrum caps nor helmets can stop a brain from being pushed into the skull, the cause of concussions. Many players are unaware of this, leading to dangerous false confidence and more aggressive play. New types of protective headgear are emerging. In 2022 a British startup called Rezon began to sell a special padded headband for rugby and other sports. Known as Halos, it is composed of nine layers which slide over each other when hit at a non-perpendicular angle. The resulting friction, the company claims, reduces by 61% the transmission of rotational forces to the brain. Rule changes are another solution. Some are hotly contested. Forbidding tackling for minors, for example, may end up increasing injuries when players grow up without having learned the best techniques. The American Academy of Paediatricians, for its part, has declined to endorse a blanket ban. Widespread rules against specific types of impacts including head-first 'spear-tackling", however, have curbed injuries. So have increasingly common policies that limit full contact during practice. Young players can also be grouped differently. Children enter puberty at different ages, which can lead to glaring mismatches in body size and agility. The injury risk to smaller kids is compounded when teams short on players call up older children too. To avoid such risks, young players are increasingly grouped by physical maturity, not age. This 'bio-banding" appears promising, but research remains regrettably thin. Parents who are still concerned should consider two final points. First, any impacts that rough-and-tumble sports may have on mental development could be revealed by periodic cognitive tests. Second, a child prevented from playing rugby or tackle football might well find other dangerous hobbies. A paper published in the Medical Journal of Australia in 2019 reviewed 8,857 children taken to emergency rooms for head injuries and found that recreational activities such as horse-riding, biking and skateboarding caused more serious injuries than contact sports.
