Regulator begins phased inspections of health insurers; issues linked to claims settlement and compliance with the new Health Insurance Master Circular

As part of routine inspections to review the implementation of regulatory norms, the Insurance Regulatory and Development Authority of India (Irdai) initiated inspections at several health insurance companies and flagged concerns with select firms regarding the implementation of certain norms under the new Health Insurance Master Circular, people aware of the development said.
According to a CNBC-TV18 report, eight general insurance companies—New India Assurance, ICICI Lombard General Insurance, HDFC ERGO General Insurance, Tata AIG General Insurance, Star Health and Allied Insurance, Niva Bupa Health Insurance, Care Health Insurance, and ManipalCigna Health Insurance—are under the scanner for violations related to claims settlement and the implementation of the circular.
The report stated that the violations noted by the regulator involve improper implementation of the Health Insurance Master Circular, which could lead to enforcement actions.
'In order to review the implementation of health prescriptions, Irdai had initiated inspection of all insurers in a phase-wise manner as part of its regulatory supervision and oversight. Accordingly, regulatory inspections were initiated for eight general and health insurance companies as part of the initial phase. The inspection report was shared with the insurance companies. The observations mainly related to industry-wide challenges in implementing the new prescriptions,' an ICICI Lombard spokesperson said in response to a Business Standard query.
'The company has submitted its response to the inspection report, in keeping with our endeavour and spirit of upholding the regulatory prescriptions. At ICICI Lombard, and as an industry leader, we follow high standards of regulatory compliance and corporate governance in all our operations,' the spokesperson added.
Queries sent to the remaining insurance companies remained unanswered at the time of going to press.

As insurance is a 'push product,' it demands a market-specific strategy. Here are a few ideas that will help the mission succeed. Insure India has been in the works for nearly three years. Its Insure India programme rightly lays emphasis on the insurance industry's three pillars: customers, providers and distributors. The processes it outlines aim to deliver the right products to the right customers, while enabling regulatory redesign, promoting innovation and ensuring a robust grievance redressal mechanism. The Insurance Regulatory and Development Authority of India (IRDAI) has embraced a laudable and ambitious goal of spreading the canopy of insurance cover over every insurable head, including risks to property. Its Insure India programme rightly lays emphasis on the insurance industry's three pillars: customers, providers and distributors. The processes it outlines aim to deliver the right products to the right customers, while enabling regulatory redesign, promoting innovation and ensuring a robust grievance redressal mechanism. The sector's regulator aims to achieve its objective of complete coverage by 2047 through the insurance trinity of Bima Sugam, Vistar and Vahak. Bima Sugam is a one-stop digital marketplace for the seamless sale and purchase of insurance. Bima Vistar is a comprehensive policy incorporating life, health, property and accident cover, with defined benefits for each category of risk to ensure quick settlement of claims without validation by third-parties (like surveyors). Bima Vahak is a proposed women-centric network of distributors at the Gram Sabha level. The role of a 'bima vahak' or distributor will be to educate and convince women in particular about the benefits of comprehensive insurance. Insurance continues to be a push product. Notwithstanding increasing awareness of financial products, the significance of human intervention has not diminished. Even pure digital insurance companies have had to hire 'feet on the street' to sign up customers. This is an abstract product—a promise that is redeemable on the occurrence of a specified event (or 'moment of truth') and so it requires careful marketing. Insurance penetration in India is low. Opening the industry to private participation does not seem to have made much of a difference. In 2006, total premiums as a proportion of GDP reached 4.8% (life 4.1% and non-life 0.6%), but in 2021, life policies were down to 3.2% and non-life up marginally to 1%. Since then, the life segment has declined to 2.8% of GDP and non-life has stayed flat at 1%. By this measure of penetration, coverage has shrunk. It would, however, be a good idea to pause and think if a special 'push product' like insurance can be promoted like in any other business. India's avowed objective of financial inclusion began former prime minister Manmohan Singh, but it did not take off until the ministry of finance and Reserve Bank of India (RBI) under Prime Minister Narendra Modi's regime launched the revised Banking Correspondence (BC) model for the opening of bank accounts. Over 1 million BCs across the country have helped widen out people's access to banking, direct benefit transfers and credit. This model has already demonstrated its viability in servicing millions under the Pradhan Mantri Suraksha Bima Yojana (PMSBY) and Pradhan Mantri Jeevan Jyoti Bima Yojana. Leveraging this human network and the public trust created can be of great help. As for bima vahaks, a commission of ₹ 10-15 per policy under the proposed model will be inadequate to secure the desired results, as this compensation is too low for a push product that requires specialized salesmanship. Even under the PMSBY, a premium of ₹ 436 yields ₹ 32 as commission. The National Pension System (NPS) was a non-starter until the intermediation fee was enhanced. For deeper penetration of services, we need a 'low margin, high volume' model. Asking BCs to take on NPS and insurance sign-ups will allow a viable cross-selling proposition that keeps distribution costs low. Allowing corporate bima vahaks can help reach out and achieve volumes too. Also Read: Heath insurance in India ought to cover preventive care as well Further, having one insurer per state runs the risk of monopolistic dominance, reduced service quality and even low penetration. The Pradhan Mantri Jan Dhan Yojana's 'sub-service area' model uses multiple service providers, which creates healthy competition, enhances outreach and assures better accountability. It has fared well and needs to be followed. The country has over 1.3 billion bank account holders with know-your-customer norms met and verified through Aadhaar, PAN or other RBI-approved identities. This should be used, rather than repeating KYC verification. Whereas a single fixed premium limit has merit, its validity is questionable, given the variation in risk profiles. The pricing of a policy must correspond to the risk covered. Else, Peter would be financing Paul. If limits are necessary, it may be better to use a range of risk-based premium brackets, with bima vahaks trained to use risk-profile guidelines. While retaining simplicity, it will bring down cross-subsidization. The need to enhance insurance coverage is urgent. It calls for a massive awareness campaign with innovative approaches. Media advertising alone may not yield the desired results. Roadshows with well-aimed short films and engaging NGOs, self-help groups, schools and colleges would help. Since a window for tweaking India's approach is still open, the IRDAI should consider these suggestions on merit. Well begun is half done, as goes the saying. In the aftermath of the tragic Air India crash in Ahmedabad on June 12, insurance companies have begun processing a flurry of claims spanning multiple categories — including travel insurance, personal accident, life insurance, and even Kisan Credit Card-linked covers. The ill-fated Boeing 787 Dreamliner, bound for London, crashed shortly after takeoff, killing 260 people — 241 onboard and 19 on the ground. The incident, India's deadliest aviation accident in over a decade, has prompted the Insurance Regulatory and Development Authority of India (IRDAI) to issue special directives to fast-track claim settlements. ICICI Lombard General Insurance reported receiving claims under travel and personal accident policies. These include accidental death, trip cancellations, baggage loss, and hotel booking cancellations. 'We are working on a priority basis to provide seamless assistance and ensure timely settlements. Our commitment is to support our policyholders and their families during this difficult time,' said Priya Deshmukh, Head – Health Products, Operations & Services, ICICI Lombard. Bajaj Allianz also confirmed receiving three personal accident claims and one marine cargo insurance claim, indicating that goods in transit may have been affected. Personal accident covers are emerging as a significant category, second only to life insurance claims. Quick Settlements, Group Policies Activated New India Assurance has already settled seven individual personal accident claims and five under group policies. A notable case involved a group personal accident policy for 4,000 residents of Mehsana district, where the claim was processed and settled within a week of the crash. One Kisan Credit Card-related claim worth ₹50,000 was also honored. Axis Max Life Insurance, under its 'InstaClaim' process, settled one claim within three hours of receipt. 'This reflects our focus on empathetic action when it matters most,' said Manu Lavanya, COO, Max Life. However, most insurers have refrained from disclosing the exact number or details of claims, citing the ongoing process of receiving formal requests from the families of the deceased. In one notable case, a group personal accident policy covering 4,000 people in Mehsana district resulted in a claim settlement within a week of the crash, a representative of the insurer said. IRDAI Steps In to Ease Process Recognizing the scale of the tragedy, the IRDAI issued a circular on June 14 instructing insurers to fast-track payouts. The regulator has waived formal requirements like FIRs and postmortem reports and directed companies to submit weekly updates starting June 16. Insurers were also asked to appoint nodal officers to assist affected families. What's Next for Claimants? Insurance company representatives said that many bereaved families are still coping with their loss and may approach insurers in the coming weeks. Given the exceptional circumstances, companies have promised to show flexibility and prioritize compassionate engagement. The incident has triggered claims under various insurance categories like travel, personal accident, and life insurance, including those from Kisan Credit Card holders. 'We have received claims primarily under travel insurance and personal accident insurance products. These include accidental death, trip cancellations, loss of baggage and hotel cancellations.' "At ICICI Lombard, we are working on a priority basis to provide seamless assistance and ensure timely settlement. We remain committed to supporting our policyholders and their families during this difficult time," Priya Deshmukh, Head Health Products, Operations & Services, ICICI Lombard, said in response to a query by PTI. On June 12, the London-bound Boeing 787 Dreamliner began to lose thrust almost immediately after taking off from the Ahmedabad airport and ploughed into a medical college hostel in Meghaninagar area, killing all but one of the 242 onboard and another 19 on the ground in the deadliest aviation accident in a decade. Bajaj Allianz General Insurance said it has received claims related to personal accidents and marine cargo. "We have received three claims related to travel personal accidents coverage and one under marine cargo insurance," Bajaj Allianz General Insurance said in response to a PTI query. Marine cargo insurance typically covers goods in transit via road, rail, air, sea, or courier. Personal accident claims have emerged as a significant category, second only to life insurance. In a circular dated June 14, the IRDAI asked all insurance companies to submit weekly updates on claims from June 16 onwards and further directed them to appoint nodal officers, waive formalities such as FIRs and postmortem reports, and expedite disbursals under life and personal accident policies. Company representatives said they have been expediting the claims received from the families. They said several kin are still grappling with the loss and would eventually approach them if there are any claims. Axis Max Life Insurance COO Manu Lavanya said that under the company's InstaClaim process, "we were able to settle a claim for one of the victims of the tragedy within just three hours, which reflects our focus on empathetic action when it matters most."
