Former Ryanair exec eyes up €300m in savings for HSE

A former senior Ryanair executive called for an 'immediate prohibition' on any new management consultancy contracts for the HSE.
Michael Cawley, an ex-deputy chief executive of the airline who now serves on the HSE board, said €70million had been spent on the contracts last year and a 'detailed report on the benefits, if any' of that spending should be provided.

Targets included the 'over-prescribing [and] excess supply' of some medications and properties that were lying idle.
He also said all postal correspondence should be eliminated and that patients, suppliers and others should be contacted via email or text.
Mr Cawley said cutting costs would take people 'out of their comfort zone' but that savings could be made while delivering better services to patients. He wrote: 'The reallocation of resources needs to become an everyday, core activity of the HSE. Pic: Gareth Chaney Collins
'This can only happen if there is full… commitment from management.'
The email said no item of expenditure should be considered too small and that all staff should be motivated to look at where savings could be made.
'It is important that we are prepared to accept the risk of making mistakes. I have found that efficiencies arise in the most unexpected areas while often, where we planned and anticipated savings, none emerged,' he wrote.
'Therefore, we must attack all costs and most importantly not be discouraged by failure to convert.'
Mr Cawley said management would need 'to be firm in its resolve' to face down the inevitable opposition. In a series of suggestions – a number of which have been withheld under FOI laws – he said training expenditure should be reduced to cover only that which was required by regulatory bodies.
He said that in January, the HSE had 83 properties that were identified for sale or disposal out of more than 4,000 around the country. 'There is no reason why they can't be disposed of in the current year,' Mr Cawley wrote.
He said 'very high' computer maintenance charges needed to be tackled, and these were likely caused by old software. The email added: 'IT should do an analysis by end of April to establish how we can reduce these costs, if necessary, by upgrading equipment and by discarding many of the surplus [or] superfluous licences we are paying for.'
Mr Cawley also said a proposal from a company to use public hospital equipment and operating theatres during evenings and weekends should be closely examined.
He said this could lead to a reduction in waiting times without the need for investment in extra capacity, especially where patients did not need a bed afterwards.
Asked about the email, which was sent in late January, an HSE spokesman said: 'The welcome input from the board member was one of many engagements by the CEO as he sought inputs on areas to maximise savings across the health service.'

The analysis for the Department of Health said there may be no family doctors to take up practice when current GPs retire in these areas. It highlights the pressure on GP surgeries particularly in areas where the population has increased and aged including Cavan, Meath, Kildare, Louth, Westmeath and Wicklow. There were 3,262 clinically active GPs in 2022. The practice most at risk of not having a replacement GP are mostly single-handed surgeries with one doctor. These are mostly found in Mayo, west Donegal, Limerick, Tipperary, Wexford, Leitrim and Galway. The report calls for better succession planning and more incentives for GPs in areas under most pressure. It said that capacity constraints can be managed by 2030 through a greater flow of doctors coming from training. At that point 4,000 graduates and doctors recruited from abroad are due to come on stream as over 1,000 doctors are set to retire. 'Intake to the GP training scheme has increased by 86pc since 2015. Accounting for attrition of graduates and the possibly higher productivity of older GPs, by 2030 around 2.2 GP national graduates will have on-boarded relative to each GP who becomes inactive.' Inadequate succession planning for retirement may be a risk to the stable supply of GP services in some areas, with single GP practices making up half of the risk group. ADVERTISEMENT However, the HSE appears to be effective in identifying and supporting panels at risk. Still, there remain a few locations where vacancies persist – these arise equally between rural areas and non-city urban areas, it said. There is some indication that GPs could be deferring retirement in capacity-constrained areas, said the report. Young GMS contract-holders often set up in areas that neighbour those with capacity constraints. As such, access to a GP may be feasible in many areas that are identified as having low capacity, but may indicate that people living near the boundaries of the geographic areas travel into neighbouring areas for care, it pointed out. Just over one in five GPs are in single-GP practices and a further one in five GPs operate from two-person practices. In areas where single GP practices are common but the population is ageing, general practice nurses and midwives (GPNM) numbers are relatively high, which may be offsetting high service usage. There are strong health service management, health outcome and economic rationales for general practices structured around group practices. Coverage of major schemes such as the modernisation scheme and the under 6 contract is high. However, uptake of contracts related to the maternity and infant scheme, childhood immunisation, and cancer screening are low in these areas suggesting that continuity of supply in terms of GP workforce may affect access to healthcare. Meath and central Cork have relatively low uptake of the GMS contract despite being areas with relatively higher cardholding populations There are areas where average panel size remains high regardless of assumed contract-sharing: in North Donegal, East Mayo, and South Wexford, there are a high amount of public patients and relatively low number of GPs.

Nursing home crisis looms as one in five will be over 65 by 2040

One in five people in Ireland will be aged 65 or older in just 15 years, creating a looming increased demand for nursing- home beds and home supports, even if more citizens enjoy healthy ageing, according to a report published today. An analysis by the ESRI said long-term residential care beds and home-support hours provided to the older population will need to increase by at least 60pc by 2040. By then, 21pc of people will be aged 65 or older and the population aged 85 and over will more than double. It said that in 2022, there were an estimated combined 33,324 short-stay and long-stay beds in nursing homes, including those used temporarily after hospital discharge and rehabilitation. Around seven in eight people are in for long-stay care, mostly supported by the Fair Deal scheme. Short-stay bed requirements are projected to grow from 3,745 in 2022, to between 6,430 and 7,265 by 2040. Long-stay bed demands could need to rise from 29,579 in 2022 to between 47,590 and 53,270 by 2040. And home-support hours may need to rise from 28.7 million annually in 2022 to between 44.9 million and 54.9 million by 2040. Healthy ageing could reduce the extent of the need for nursing-home beds, along with expanding home-support services. However, the substantial impact of increases in the number of older people in Ireland will offset much of the potential easing of the situation that having more people living in good health in older years will bring. Senior research officer at the ESRI and lead author of the report, Dr Brendan Walsh, said: 'Ireland has experienced tremendous improvements in life expectancy in recent decades. This means there is, and will be, a much larger population at older ages who require long-term care services to support them at home, or within residential facilities.' ADVERTISEMENT Health Minister Jennifer Carroll MacNeill said the research will 'help us plan better'. She said the Programme for Government pledges to build more public nursing-home beds, create a home care scheme to help people stay in their homes longer, and increase home-care hours. 'We are already making progress in increasing both residential care capacity and home-support hours for our older population,' she said. 'This is shown by the €4m allocated in Budget 2025 to staff and opening of 615 new community beds. 'The Department of Health and the HSE are also working on a new Long-Term Residential Care Additional Capacity Plan, to be published in 2025. 'This capacity review, commissioned by the Department of Health and the HSE, shows our commitment to planning based on evidence. 'With the Hippocrates Projection Model and our ongoing work with the ESRI, we can adapt our plans to new data and policies as they come up.' Junior minister for older people Kieran O'Donnell said that the ESRI research would be 'invaluable' for capacity planning for residential care and home support. 'It is evident that significant action will be required by Government in order to ensure that the appropriate care services are available for our older population and to deliver on Programme for Government, Sláintecare and Project Ireland 2040 commitments,' Mr O'Donnell said.

THE NUMBER OF long-term residential care beds and home supports for older people will need to increase by at least 60% by 2040, according to research. The Economic and Social Research Institute (ESRI) found the number of people aged 85 and over, who use a substantial amount of long-term residential care and home support, is projected to more than double in the coming years. The ESRI published a report today into long-term residential care (LTRC), research which was funded by the Department of Health. The research found that by 2040, one in five people in Ireland will be aged over 65. In 2022, there was an estimated combined 33,324 short-stay and long-stay beds in LTRC homes. Short-stay bed requirements are projected to grow from 3,745 beds in 2022 to between 6,430 and 7,265 beds by 2040, growth of 72% to 94%. Long-stay bed requirements are projected to grow from 29,579 beds in 2022 to 47,590-53,270 by 2040, growth of 61%-80%. Almost 29 million home support hours were provided to the older population in 2022, with three-quarters of these hours provided through the HSE's home support service and a quarter privately purchased. Requested home support hours are projected to grow from 28.7 million annually in 2022 to between 44.9 million and 54.9 million annually by 2040, growth of 57% to 91%. The ESRI said although healthy ageing effects may reduce future requirements, the substantial impact of increases in the older population will offset much of any potential moderations. Our findings provide policymakers with an important evidence base to help develop these plans and policies.' Health Minister Jennifer Carroll MacNeill said the research will 'help us plan better'. She said the Programme for Government pledges to build more public nursing home beds, create a homecare scheme to help people stay in their homes longer, and increase home care hours.
