France international Baltimore denied holders United any chance of a comeback when she lashed home at the start of 10 minutes added time to cap off the triumph.
England boss Sarina Wiegman was watching on at Wembley, where Reddit founder Alexis Ohanian, who four days ago announced he had purchased a minority stake in Chelsea's women's side, took in the win alongside wife and 23-time tennis grand slam singles winner Serena Williams.
The contest got off to a cagey start, United with the slight advantage after winning a few set-pieces and forcing Hannah Hampton – battling Mary Earps to be England's first-choice keeper at July's Euros – to palm away Grace Clinton's corner.
Aggie Beever-Jones, who has enjoyed a splendid season with the WSL champions, was able to beat Millie Turner's challenge, but it did take the sting out of her effort, which was comfortably saved by Phallon Tullis-Joyce.
Chelsea then dealt with a cross at the other end of the pitch, but it was only cleared as far as the unmarked Dominique Janssen, who missed a big chance to hand United the lead when she fired just over the crossbar.
Mayra Ramirez made easy work of the United defence, skipping freely around a collection of red shirts and forcing Tullis-Joyce to react quickly before Lucy Bronze could take advantage and prod home.
The American keeper denied Beever-Jones again, this time diving to stop an effort that curled dangerously close to her right post.
But the Blues soon took the lead after Erin Cuthbert was brought down by Celin Bizet, who caught the Scotland international with a boot to the midriff.

Baltimore picked out the bottom left with her spot-kick, and Tullis-Joyce dove the wrong way.
United substitute Ella Toone, who came on for Turner after the restart, got herself involved quickly, stinging Hampton's palms.
Bompastor made her first change shortly after the hour, replacing Ramirez with Macario, and United narrowly escaped conceding a second when Gabby George nearly turned a cross into her own net.
VAR silenced United's protests for a penalty, arguing Hampton had unjustly impeded Elisabeth Terland, before Beever-Jones missed a golden chance to double the Blues' advantage, skying an effort after she was teed up by Macario's backheel.
United States international Macario then added her own name to the scoresheet, rising highest amongst a sea of red shirts to meet Baltimore's free-kick and powering her header past Tullis-Joyce.
But the Blues were not finished scoring, and Baltimore bagged her brace, lashing home moments after 10 minutes of added time were announced.

Bruno Fernandes has received a new offer to leave Manchester United after rejecting a lucrative move to Al Hilal earlier in the summer. Al Hilal offered the Old Trafford captain a deal worth £200m over three years back in June but saw their advances rebuffed. United fans were mostly relieved to keep hold of their talisman, although some suggested it would be wise to cash in given Al Hilal were offering over £100m. Club legend Paul Scholes was even of the opinion that Fernandes should be sold given the numbers on the table. A few months after rejecting Al Hilal, Fernandes has received a new transfer approach from fellow Saudi Pro League side Al Nassr. Reports in Portugal said Al Nassr were even optimistic of signing Fernandes given their head coach Jorge Jesus previously managed the midfielder at Sporting and the fact they have Cristiano Ronaldo on their books. However, the Daily Mail say Fernandes is 'unlikely to reverse' his decision to stay at Manchester United and is fully focused on the upcoming Premier League season. Transfer expert Fabrizio Romano also says Fernandes is 'fully involved' in Manchester United's project and wants to help bring the glory days back to Old Trafford. United endured one of their worst-ever campaigns last season, finishing just 15th in the Premier League and losing a drab Europa League final to Tottenham. Fernandes, who has made 290 appearances for Manchester United, is currently with his teammates in the United States ahead of their pre-season friendly against Everton on Sunday night. Arsenal have made 'initial contact' with Rennes to discuss a move for highly-rated centre-back Jeremy Jacquet, according to reports. Following a subdued start to the summer transfer window, Arsenal have bolstered Mikel Arteta's squad with six key signings. The likes of Viktor Gyokeres, Martin Zubimendi and Noni Madueke have all arrived at the Emirates Stadium to strengthen a squad that has finished second in the Premier League for the past three seasons. Despite the heavy investment so far this summer, Arsenal are interested in further deals before the summer transfer deadline, most notably looking to sign Eberechi Eze from Crystal Palace. Arsenal have also expressed interest in signing highly-rated defender Jacquet and made contact with French club Rennes over a deal according to Fabrizio Romano. It is claimed the Gunners have made 'initial contact' with Rennes, who finished 12th in Ligue 1 last season, to declare their interest in Jacquet. Arsenal will look to bring Jacquet to north London this month if current defenders move on, but if not they have already indicated they will move for the 20-year-old next summer. Arteta has his eyes on defensive reinforcements as his 2023 signing Jakub Kiwior continues to be linked with a move away from the club. It was reported over the weekend that 'several clubs' were keen to sign Kiwior, who made 30 appearances for the Gunners last season. While the Poland international has struggled for starts since his £17.6m move from Spezia, Arteta would prefer to keep him as he is viewed as an important back-up player. Kiwior has been heavily linked with a return to Italy and there is also a chance Oleksandr Zinchenko will leave Arsenal this summer. While only making 12 league appearances for Rennes, towering centre-back Jacquet is rated as one of the most promising defenders in France. He has represented his country throughout the youth levels and helped France's U19 side reach the final of the Euros last year by being named in the Team of the Tournament. Jacquet spent the first-half of last season on loan at Ligue 2 side Clermont before being recalled by Rennes in January. In a statement at the end of last season, Rennes said: 'Recalled from Clermont in the winter window, where he had been on loan since January 2024, Jeremy Jacquet has established himself with confidence in the Rennes defence and raised his game to the level of Ligue 1. 'Playing in a three-man or four-man defence, or even as a full-back, Jeremy has put in a number of convincing performances for SRFC, demonstrating his ability to adapt on the pitch. 'On Monday, Jeremy Jacquet signed a new contract with Stade Rennais F.C. that will run until 2029. With this extension, the club is reaffirming its confidence in a player who carries the Red and Black DNA.' Arnaud Pouille, chief executive of Rennes, added: 'We're delighted to see Jeremy extend his contract. It's a strong signal for the club, which validates a project built with its academy. 'Jeremy is a very intelligent, talented young man who is well surrounded by people. He will continue to progress with the Rouge et Noir. I'd also like to thank his family, with whom we have an excellent relationship.' Loic Desire, sporting director of the French club, said: 'Jeremy perfectly embodies the club's desire to build its project around Rennes' training. 'He was present at the Gambardella final this weekend, proof of his attachment to the Rouge et Noir academy. We hope that he will go very far and continue to progress with Stade Rennais F.C.' Jacquet, meanwhile, said he was 'very happy' to extend his contract at Rennes and determined to improve next season.
