
NICE Backs Leadless Pacemakers for Some Heart Patients
In draft guidance, NICE said there was insufficient evidence to support their use for dual-chamber pacing outside of research settings.
Benefits Over Traditional Pacemakers
The recommendation is based on an evidence review involving over one million patients across multiple studies. These included a randomised controlled trial, four systematic reviews with meta-analyses, five registry studies, four prospective studies, and two retrospective studies.
Leadless pacemakers were found to be effective at detecting abnormal heart rhythms, restoring normal pacing, and improving quality of life. Unlike conventional transvenous pacemakers (TVPs), they do not require a chest incision, reducing the risk of infection and other complications, the regulator noted.
'The evidence demonstrates that leadless pacemakers can effectively regulate heart rhythm while reducing the risk of complications associated with traditional pacemakers, particularly infections and lead-related issues,' said Dr Anastasia Chalkidou, HealthTech programme director at NICE.
How Leadless Pacemakers Work
Bradyarrhythmias, which affect around 1 in 1000 people are typically treated with TVPs. These devices consist of a subcutaneous pacemaker box and leads that connect to the heart.
For single-chamber leadless pacemakers, the proximal end is attached to a deflectable delivery catheter system. It is usually inserted percutaneously through the femoral or jugular vein using an introducer sheath. It is then moved into the right atrium, through the tricuspid valve, and into the right ventricle.
Dual-chamber systems, which are still under evaluation, involve placing two devices — one in the right atrium and another in the right ventricle — during a single procedure.
NICE highlighted that TVPs are known to carry risks, including lead failure and generator-related complications.
Leadless pacemaker implantation may be preferable for patients with a history of device infection, endocarditis, immunosuppression, limited vascular access, or high infection risk.
Call for Further Research
While the draft guidance supports leadless pacemakers for single-chamber pacing, NICE said that more evidence is needed before endorsing their wider use. This includes research on patient selection — such as age, comorbidities, and the underlying cause of bradyarrhythmia — as well as data on implantation sites and clinical outcomes. Long-term durability of leadless devices also remains an area for investigation.
'The incidence of bradyarrhythmias increases with age, making this guidance particularly relevant to our ageing population,' said Professor Tom Clutton-Brock, chair of NICE's interventional procedures advisory committee.
The draft guidance is open for public consultation until 23 July 2025. The advisory committee will meet again on 11 September 2025 to review feedback and finalise recommendations.

ORION CORPORATION PRESS RELEASE 21 JULY 2025 at 10:30 EEST Darolutamide receives EU approval in third indication for patients with advanced prostate cancer European Commission granted approval of darolutamide in combination with androgen deprivation therapy (ADT) for patients with metastatic hormone-sensitive prostate cancer (mHSPC). Approval is based on positive results from the pivotal Phase III ARANOTE trial. This broadened indication allows doctors to use darolutamide plus ADT, with or without chemotherapy (docetaxel), enabling them to better tailor treatment plans to meet the needs of patients with mHSPC. Orion's collaboration partner Bayer announced today that the European Commission has granted marketing authorization in the European Union (EU) for darolutamide, an oral androgen receptor inhibitor (ARi), plus androgen deprivation therapy (ADT) for the treatment of patients with metastatic hormone-sensitive prostate cancer (mHSPC). The approval is based on positive results from the pivotal Phase III ARANOTE trial, which showed that darolutamide plus ADT significantly reduced the risk of radiological progression or death by 46% compared to placebo plus ADT (HR 0.54; 95% CI 0.41–0.71; P<0.0001) in patients with mHSPC. Darolutamide, under the brand name Nubeqa®, is already approved in over 85 countries for use with ADT and docetaxel in mHSPC, and with ADT alone in non-metastatic castration-resistant prostate cancer (nmCRPC) in patients who are at high risk of developing metastatic disease. Darolutamide is developed jointly by Orion and Bayer. Prostate cancer is the second most common cancer and the fifth most common cause of cancer death in men worldwide. In 2022, an estimated 1.5 million men were diagnosed with prostate cancer, and about 397,000 died from the disease worldwide.1 In Europe, there were almost 474,000 estimated new cases of prostate cancer in 2022 with approximately 115,000 deaths. 2 Prostate cancer diagnoses are projected to increase to 2.9 million by 2040.3 About the ARANOTE trial The ARANOTE trial is a randomized, double-blind, placebo-controlled Phase III study designed to assess the efficacy and safety of darolutamide plus ADT in patients with mHSPC. 669 patients were randomized 2:1 to receive 600mg of darolutamide twice daily or matching placebo in addition to ADT. The primary endpoint of this study is radiological progression free survival (rPFS), measured as time from randomization to date of first documented radiological progressive disease or death due to any cause, whichever occurs first. Secondary endpoints include overall survival (time to death from any cause), time to first castration resistant event, time to initiation of subsequent anti-cancer therapy, time to prostate-specific antigen (PSA) progression, PSA undetectable rates, time to pain progression, and safety assessments. Results from the Phase III ARANOTE trial presented at ESMO 2024 and published in The Journal of Clinical Oncology showed that darolutamide plus ADT significantly reduced the risk of radiological progression or death by 46% compared to placebo plus ADT (HR 0.54; 95% CI 0.41–0.71; P<0.0001), in patients with mHSPC. Consistent benefits in rPFS were observed across prespecified subgroups, including patients with high-volume (HR 0.60, 95% CI: 0.44-0.80) and low-volume (HR 0.30, 95% CI: 0.15-0.60) mHSPC. The incidence of adverse events in the treatment group with darolutamide plus ADT in the ARANOTE study was comparable to placebo plus ADT. Darolutamide plus ADT was generally well tolerated and showed lower discontinuation rates due to adverse events compared to placebo plus ADT. About darolutamide Darolutamide is an oral ARi with a unique chemical structure that binds with high affinity to the androgen receptor and exhibits a strong antagonistic effect against the androgen receptor inhibiting the receptor function and the growth of prostate cancer cells. Additionally, preclinical models and neuroimaging data in healthy humans, support darolutamide's low potential for blood-brain barrier penetration. Darolutamide (plus ADT or plus ADT and docetaxel) demonstrated a side effect profile, in both mHSPC registrational studies where the incidences of adverse events were roughly similar to the respective comparator arm. Darolutamide is a treatment option for doctors and patients, considering its tolerability and low risk of drug interaction. A robust clinical development program is underway investigating darolutamide across various stages of prostate cancer. The program includes the Phase III ARASTEP trial evaluating darolutamide plus ADT compared to ADT alone in hormone-sensitive high-risk biochemical recurrence (BCR) prostate cancer, who have no evidence of metastatic disease by conventional imaging and a positive PSMA PET/CT at baseline. Furthermore, darolutamide is also being investigated by Bayer in the collaborative Phase III DASL-HiCaP (ANZUP1801) trial led by the Australian and New Zealand Urogenital and Prostate Cancer Trials Group (ANZUP). The study evaluates darolutamide as a treatment for localized prostate cancer in combination with radiotherapy. About metastatic hormone-sensitive prostate cancer At the time of diagnosis, most men have localized prostate cancer, meaning their cancer is confined to the prostate gland and can be treated with curative surgery or radiotherapy. mHSPC is a stage in the disease where the cancer has spread outside of the prostate to other parts of the body. Up to 10% of men will present with mHSPC when first diagnosed.4,5,6 For patients with mHSPC, ADT is the cornerstone of treatment, in combination with chemotherapy docetaxel and/or an androgen receptor inhibitor (ARi). Baseline Viral Load Shapes Immune Response in Women With HIV
TOPLINE: Most women living with HIV achieved an immunologic response after 2 years of initiating antiretroviral therapy. A higher baseline HIV viral load increased the odds of an immunologic response in women living with HIV, but this association diminished among those who acquired HIV through intravenous drug use. METHODOLOGY: Researchers conducted a register-based cohort study to determine the prevalence and factors associated with immunologic response in women living with HIV in Sweden. They analyzed 841 women with viral suppression (mean age, 37 years; baseline CD4 T-cell count < 500 cells per μL) diagnosed with HIV after 2000. The primary outcome was an immunologic response 2 years after initiating antiretroviral therapy among those who achieved sustained viral suppression within 6 months of treatment initiation. TAKEAWAY: Among the women studied, 90% (95% CI, 0.88-0.92) achieved an immunologic response after 2 years of follow-up. Its peak is still a few weeks away, but it began earlier this week — and so did a slew of poorly researched articles advising when to watch it. What almost no one else seems to realize is that this year, the normally prolific Perseids will generally disappoint because the peak rates of 'shooting stars' on Aug. 12-13 will occur in a night sky bleached by a near-full moon. Happily, all is not lost. Here's how and when to watch the Perseid meteor shower in 2025 — including what experts think is the best time to get a glimpse of it, despite the harsh sky conditions. The Problem With The Perseid Meteor Shower In 2025 The Perseids can produce about 75 'shooting stars' per hour on its peak night, but only in a dark sky. This year, under clear, dark skies with a wide-open view, observers might spot far fewer. 'I'd expect a typical person with a clear view of the full sky to be able to see somewhere in the ballpark of five to 10 meteors per hour during the night of August 12-13,' said Dr. Qicheng Zhang, astronomer at Lowell Observatory in Flagstaff, Arizona, in an email. Why You Shouldn't Wait For The Perseid To Peak Although Aug. 12-13 is the peak night, meteors may be more easily visible on nights both before and after, when sky conditions are better. 'A few meteors might be visible in the early morning on the first couple of days [of August]How And Where To See 'Shooting Stars' The advice for meteor showers is simple — get far from light pollution. If you intend to go looking for Perseids this week, or after the peak has passed, seeking out dark skies is essential. Aim for a wide, open space with minimal haze and obstructions to catch as many meteors as possible. However, for the peak night on Aug. 12-13 for the Perseids this year, that's not helpful advice. Even if you get as far from artificial lighting as possible on that peak night, you'll still find yourself under a sky similar to that of a city. Why You Need To Be Patient Seeing a meteor shower successfully means being patient. It also means not looking at a smartphone, whose white light instantly kills night vision. Add the worsening problem of light pollution, and it's easy to see why almost nobody ever sees 'shooting stars' these days. 'As with most annual meteor showers, observing meteors takes a lot of patience, and it may take watching for an hour or more to see a handful of meteors,' said Zhang. Also, any strong and sudden outbursts in meteor activity — which are hard to predict, but possible — may last only a few hours.
