New drug could halt Alzheimer's
Trials suggest the treatment, called trontinemab, could be the most powerful weapon yet against dementia.
Research will now examine whether the drug should be given to those without any symptoms in order to prevent the disease.
Results presented at the Alzheimer's Association International Conference in Toronto found the 'game-changing' treatment can clear the devastating plaques associated with Alzheimer's far more rapidly than any current licensed drug.
Nine in 10 patients prescribed trontinemab experienced amyloid clearance within 28 weeks, meaning visible markers of disease had vanished.
Experts hope these changes will be matched by improvements in memory and functioning, with an 18-month trial of 1,600 patients now under way.
Separate research will examine whether the drug could be given to people without any signs of dementia, in the same way that statins are used to ward off heart disease.
On Monday, experts said the findings were 'very promising', suggesting that the drug was much more powerful than existing medications, with far fewer side effects.
Around one million people in the UK suffer from dementia, with Alzheimer's disease the most common form.
Powerful precise effects at low doses
Trontinemab is one of a class of new medicines aimed at clearing amyloid plaques. The new findings from phase two trials suggest that, in less than seven months, it has outperformed the ability of existing drugs to clear plaques in 18 months.
Experts hope that if given early enough, the drug could save some patients from developing symptoms at all. It has been engineered to more easily cross the blood-brain barrier, meaning it can ensure powerful precise effects at low doses.
The lack of side effects means it could be offered to large populations. This also means it could be offered at a far lower price than current medications, which require intense monitoring, including regular scans.
Experts believe it could become the first Alzheimer's drug to be funded by the NHS.
Last year, medicines regulators in the UK gave the green light to the first two treatments found to slow progression of Alzheimer's disease.
Both lecanemab and donanemab use monoclonal antibodies to clear amyloid plaques in Alzheimer's disease. In trials they have been found to slow decline by 27 per cent and 35 per cent respectively, over 18 months. However, the risk of brain swelling and brain bleeds means they need intense monitoring.
The new drug appears to have a far better safety profile. The phase two trial of 149 patients found less than five per cent of cases suffering amyloid-related imaging abnormalities, and all cases quickly resolved.
It also requires less frequent infusions, with mass trials to examine the impact of giving the drug to patients once a month for six months, then every three months.
Scientists have been intrigued by the promise of trontinemab because of the way it has been designed to be transported across the blood brain-barrier, which normally prevents chemicals in the bloodstream from entering the brain.
'Game-changing'
Prof Sir John Hardy, the chairman of molecular biology of neurological disease at University College London's Institute of Neurology said the advance could be 'game-changing'.
He told The Telegraph: 'This is absolutely great news. It sucks the plaque out of the brain really quickly, much faster than we have seen with lecanemab or donanemab.'
The scientist, who was the first to identify the role of amyloid in the disease, said the drug's safety profile was a 'massive improvement' on the current drugs on the market, raising hopes it could be used both to prevent disease and stop it in its tracks.
He said: 'There is no doubt this could be game-changing. We hope that if we can use these drugs to people early, we can halt the progression of disease, even before people have symptoms. Now we need to see the size of the clinical effect.'
Neither of the current drugs have been funded by the NHS as a result of their high costs, much of which stems from the need for intense monitoring, including regular scans.
Prof Hardy said: 'These results show it is much faster and safer than previous drugs, which means less monitoring. That brings down the cost significantly, it means fewer MRI scans, so that would surely mean it would get Nice [National Institute of Health and Care Excellence] approval.'
'Very promising evidence'
Prof Jonathan Schott, the chief medical officer at Alzheimer's Research UK said: 'We urgently need a range of treatments for Alzheimer's that are effective and safe for the people affected by this devastating disease.
'Evidence presented at the Alzheimer's Association conference in Toronto on trontinemab is very promising, showing that the drug can effectively and rapidly clear amyloid from the brain, seemingly with very few side effects.
'We now need to see whether these early stage results carry through to later stage clinical trials, which are planned to start later this year, including in the UK. These trials will show whether the drug is not only safe, but impacts on memory, thinking and quality of life.'
He said it was 'exciting' that the drug would now be tested in some people without symptoms under the phase three trials.
Levi Garraway, Roche's chief medical officer said: 'Combining new treatment avenues with advanced diagnostics may enable earlier and potentially more effective intervention.
'With plans for phase three trials in both early symptomatic and preclinical Alzheimer's disease, we are advancing science with the goal of delaying – and ultimately preventing – progression of this devastating condition.'

"My work stress headache was actually rare brain disease which left me paralysed"
A 'fit and healthy' woman was left paralysed after headaches she thought were 'work stress' turned out to be signs of an ultra-rare brain disease. Alicja Faryniarz, 44, said she knew something 'wasn't right' but her symptoms were constantly dismissed by doctors as migraines or sinus problems. The accountant began suffering from severe headaches during her 20s which led to fainting episodes and extreme fatigue. However each time she went to her GP, she says they "brushed it aside" and prescribed her with painkillers despite her suffering "intense pain". It wasn't until September last year that her brain disease was picked up when she suffered a stroke while playing a tennis match. Alicja collapsed on the court and was taken to hospital where she was diagnosed with brain arteriovenous malformation (AVM). AVM affects less than 1 per cent of Brits and is caused by tangled blood vessels that create irregular connections between arteries and veins in the brain. They are usually congenital, but they can form later in life, and people with the disease rarely show symptoms, according to the NHS. Alicja had a three inch AVM which ruptured and paralysed the right side of her body and also left her in a coma for three days. She is now warning of the "hidden" dangers of the rare condition which she had no idea she was born with.

Misinterpreting Words and Gestures in Medicine
This transcript has been edited for clarity. Hello. I'm David Kerr, professor of cancer medicine at University of Oxford. Today I'd like to talk about words and gestures, and how we can communicate unconsciously with patients in ways where we can end up being misunderstood. In the clinic on Friday, one of our patients, somebody we looked after for quite a long time, had evidence of oligometastatic recurrent colorectal cancer. By that, I mean the tumor had come back in three small spots within the peritoneal cavity. She's had multiple chemotherapies and we've done hepatic resections. We looked after her for 4.5 years with her metastatic disease and she's remained remarkably well, fully fit and active, and has been taking care of her young family and so on. What we thought that we would do for her treatment would be to offer, with my specialist colleagues in Oxford, stereotactic body radiation therapy (SBRT). This is very highly focused radiotherapy — spot welding, if you like — that can be delivered, with fantastic accuracy, safely to small areas of disease. We had arranged for her to see one of my senior colleagues in Oxford. The way that our NHS communicates with our patients is: A letter goes out saying, 'You have an appointment to see Professor Kerr and Dr Muirhead,' a fantastic radiotherapy colleague. Of course, our literate, clever patient looked up who Dr Muirhead was, and she saw the word "palliative" splashed across all the work that Rebecca does. Immediately, because that word was used, she assumed that she'd come to the end of the road, that there was nothing effective that we could offer for her treatment. Quite the opposite was the case, but the use of the word "palliative," to her mind, meant palliative care, end of care, and imminent death. For those of us who were somehow forced to do Latin to get into medical school, it comes from palliare, the Latin word for "to cloak" or "to hide." This was a word that was just utterly, completely misinterpreted, and the 2 weeks between receiving a letter and meeting me in a clinic had been the worst 2 weeks of the patient's life over the past 4.5 years. You can imagine the tears of joy when we explained exactly what it was we were going to do. It reminded me of another time, when I was a very young doctor in Glasgow, a senior registrar in my mid-twenties. I had looked after a young woman about the same age as me with metastatic ovarian cancer, along with my boss, Stan Kaye, another fantastic oncologist. We looked after her well and continually, but the disease caught up with her eventually. We were very active at that stage, one of the first centers in Europe to be involved in phase 1 trials with the National Cancer Institute. I wanted to explain that we could offer her a position in one of the phase 1 trials that we were doing, but of course, as a prelude to that, I had to explain that we had no more conventional treatment to offer. I come from the west of Scotland. We're not a very naturally tactile group of male human beings, but I leaned over, held her hand, and explained the situation, thinking really nothing of that small gesture. Later in her illness, as she did inch closer to death, we continued to look after her and to palliate — that word again. She said that the single scariest thing that she remembers of all the journey that we'd been over for all those years was when I held her hand, because she thought it must be very, very bad indeed for that young doctor to reach out, to hold her hand, and just to be gentle and kind. Words and gestures need to be chosen carefully. But, clearly unconsciously, whether aware of it or not, we're sending subliminal messages out to our patients. We may not be able to control them, but it's interesting how patients can interpret and misinterpret what we see and sometimes do. RSV Jab Shields Older Adults from Hospitalisation
The UK Health Security Agency (UKHSA) has reported that the respiratory syncytial virus (RSV) vaccine is around 82% effective in preventing hospitalisation among adults aged 75-79. The data come from a study run in partnership with Nottingham University Hospitals and other NHS trusts. UKHSA called the results 'strong protection'. The study used data from a hospital-based sentinel surveillance system covering 14 hospitals in England. It included adults admitted for acute respiratory infections between 1 October 2024 and 31 March 2025. Of 1006 adults hospitalised with acute respiratory infections (ARI), 173 (17.2%) had RSV infection. Vaccine Efficacy by Clinical Subgroup Data showed vaccine efficacy against hospitalisation of: 82.3% for any RSV-associated ARI 86.7% for severe cases needing oxygen supplementation 88.6% for lower respiratory tract infection, including pneumonia 77.4% for exacerbations of chronic lung disease 78.8% for exacerbations linked to chronic heart disease, lung disease, or frailty 72.8% for adults with immunosuppression Maternal RSV Vaccination Also Effective The UKHSA also reviewed data from the maternal RSV vaccination programme, introduced alongside the older adults programme in September 2024. A separate study, published in The Lancet Child & Adolescent Health , found the maternal RSV vaccine reduced infant hospitalisation risk by 72% when given more than 14 days before birth. Both programmes use Pfizer's bivalent pre-F vaccine, Abrysvo). The programme for older adults offers the vaccine to those turning 75, as well as a one-off catch-up campaign for all adults aged 75-79 years. The maternal programme offers the vaccine from 28 weeks of pregnancy. Uptake Rates Vary Across Groups By 30 June, uptake among eligible older adults had risen to 62.9%, up from 60.3% in March. Among 36,657 women who gave birth in March, 54.7% had received the RSV vaccine during pregnancy. Coverage in the maternal programme varied by ethnic group. Uptake Uptake ranged from 73.3% among women of Chinese ethnicity to 26.4% among Black and Black British Caribbean women. RSV Seasonality and Surveillance Findings RSV activity typically starts in October. Bronchiolitis, the main clinical presentation in infants, is primarily caused by risk of hospitalisation with RSV infection is highest among babies born in late summer or autumn. The UKHSA's first annual RSV report reviewed the 2024-2025 season. It found: RSV activity began around mid-October across all UK nations It peaked between 18 November and 8 December (weeks 47 to 49) It declined to baseline levels by late February (weeks 7-8 of 2025) Primary care surveillance included around 300 GP practices in England, which swab patients presenting with ARI. Peak RSV positivity was: 53.1% in children under 5 during week 46 (11-17 November) 18.5% in adults aged 75 and over during week 49 (2-8 December) Hospital emergency department data also showed that bronchiolitis in infants under 1 year peaked in late November. MHRA Warns of Guillain-Barré Syndrome Risk In July, the Medicines and Healthcare products Regulatory Agency (MHRA) issued a drug safety alert on RSV vaccines. Clinicians are advised to monitor for symptoms of Guillain-Barré syndrome (GBS) following vaccination. GBS is an acute demyelinating disease affecting the peripheral nervous system. Early symptoms include tingling, numbness, or pins and needles in the feet and hands, followed by muscle weakness and difficulty moving joints. In some cases, it can progress to breathing difficulties, drooping facial muscles, problems with swallowing or speech, and double vision. Although rare, GBS can be fatal.
