
Greater Price Transparency Will Improve Affordability
Inefficiencies plague our current healthcare system. Politicians are quick to blame these problems on the market and subsequently advocate for ever greater government control. But government programs, which are already major players in the healthcare market, provide lousy insurance for patients and undermine the viability of doctors and hospitals. Expanding the government's role will only worsen these problems.
Rather than expand the government's role, reforms need to address the price opacity problem that is obstructing the ability of healthcare professionals to deliver higher quality care to patients at lower costs. The price transparency problem afflicting hospitals exemplifies the problem.
As the Center for Medicine in the Public Interest (CMPI) documents, hospitals account
for nearly one-third of the nation's total healthcare expenditures. While attention is often placed on pharmaceutical companies and insurance providers, hospital systems, especially nonprofit institutions, are the primary driver of healthcare cost inflation and systemic inefficiency.
To substantiate this claim, CMPI documents that hospital prices have increased by more than 250% over the last 25 years, which was double the rate of overall medical care inflation. Clearly, if hospital costs are one-third of spending and those costs are rising faster than the overall healthcare sector, then hospital costs are driving healthcare inflation. But it gets even worse.
Hospital costs are also growing twice as fast as the median household's income. Consequently, hospital services are becoming increasingly less affordable for the average family. And while insurers and government payers are supposed to cover these costs, a larger share is being shifted to payments through large deductibles and coinsurance obligations.
Addressing the problem of price opacity will help control the growth in expenditures at hospitals because 'a significant portion of this spending arises from redundant services, excessive administrative overhead, and inefficiencies embedded in payment models.' Establishing a competitive market with clear and transparent prices would impose discipline on hospital prices, make it difficult to inflate overhead costs, and rein in expenditure growth.
It is important to recognize that it matters how transparency reforms are implemented. Previous efforts to mandate transparency have fallen short due to lack of follow-through. In its Seventh Semi-Annual Hospital Transparency Report, PatientRightsAdvocate.org estimated 'that only 21.1% of the 2,000 hospitals are in compliance with all of the price transparency rule requirements.'
Consequently, patients and insurers still cannot easily compare prices and the ability of competition to improve quality and reduce costs is still hampered. The Trump Administration should address this problem by enforcing the existing regulations and ensuring that patients and payers have access to clear and upfront pricing data.
Greater price transparency would also help mitigate the cost pressures created by the rampant abuse of the 340B program by many hospitals. The 340B program was created to subsidize hospitals and clinics serving low-income populations to help these institutions provide more care for vulnerable populations at lower cost.
To achieve this goal, 340B allows qualified hospitals to buy steeply discounted drugs and then resell these medicines to Medicare or privately insured patients at full price and pocket the difference. While the program offers hospitals large profit opportunities, the costs have become a burden that inflates costs for other patients and insurers. As a New York Times investigation noted,
prescription drug spending for state employees [in North Carolina] jumped almost 50 percent from 2018 to 2022. A report in May from the state treasurer's office found that 340B was partly to blame: Hospitals that participated in the program billed the state health plan far more than hospitals that did not — almost 85 percent more for certain cancer drugs. In one example, hospitals bought a drug commonly used to treat melanoma for an average of $8,000 but billed the state $21,512.
Due to the extreme profitability of the program, the program has experienced rapid growth. In 2009, the discounted value of 340B purchases was $4 billion. By 2023, these purchases grew to $66 billion – a sixteen fold increase!
Yet, as I document in JAMA Network, abuse of this well intentioned program has turned it into a profit center for large health systems that do not pass the savings on to patients and tend to provide less charity care than the average hospital. Worsening the impact from the program, it has grown so large that the prices for other patients are increasing to help cover 340B's costs.
With over one-half of nonprofit hospitals participating in the program, not to mention major pharmacy chains, the program's adverse impacts are materially harming the broader healthcare system. Coupling price transparency reforms with policies that address the abuses by contract pharmacies and ensure that 340B hospitals serve their intended purpose can rein in the 340B program materially improve drug affordability.
Markets only work when transparent prices reflect the desires of consumers and the costs of producers. While the third-party payer system creates complications, this principle holds for healthcare markets. Regulatory reforms that promote widespread price transparency offers Congress and the Trump Administration an opportunity to meaningfully bend the healthcare cost curve and improve the quality-of-care patients receive.

There are research-backed, proven systemic reasons that women are struggling more in the workplace, according to leadership expert Meghan French Dunbar. She has now written the book about it—what those reasons are, and what can be done about it. In This Isn't Working: How Working Women Can Overcome Stress, Guilt and Overload to Find True Success—which is out August 5—French Dunbar argues that, by improving workplaces for women, it will not only boost performance for them but also everyone; that burnout isn't a badge of honor anymore; and that, right from the book's dedication page, women 'no longer have to abide by the status quo but can, instead, redefine it.' It's a problem that many women find themselves in: by any measure, French Dunbar was successful, but behind the scenes, she was struggling. Though she was 'doing everything right,' she didn't feel right. The pressure was too much; she had burned out. She resigned as CEO of the organization she founded on February 1, 2020. Her last day was March 13—and then COVID-19 struck. 'I was such a high achiever and had been so conditioned to 'This is what success is, and that's what worth is,'' she tells me over Zoom. 'And COVID just stripped me of that.' She adds, 'I mean, COVID was the worst of times for many of us, and there was also a lot of beauty in the forced pause in a lot of people's lives.' At the time, she had a 9-month-old son and was about to launch an in-person events company. Like so many women, French Dunbar knew busyness intimately. 'The traditional success track was just forced to be put on hold, and I had to stop, and it was a very long process, but I had this moment where I realized that I had no identity or sense of self-worth in the absence of professional accomplishment,' she says. This isn't working. She realized 'I don't want to completely destroy myself in the name of success and work nights and weekends and sacrifice all my relationships,' she says. And that's where the origin of her new book begins. 'The process of writing the book was just such a gift, because I got to interview extraordinary women who are also thriving,' French Dunbar says. 'And so many of them talked about setting boundaries and not tying every sense of worth to your achievement and all of the elements that just got reinforced over and over and over again.' French Dunbar writes in the book that there are a disproportionate amount of women experiencing burnout and stress. 'Working women's well-being is declining faster than men's, regardless of whether they have children,' she notes in the book. An estimated 43 percent of women report feeling burned out, compared to 31 percent of men at the same level. Women's symptoms of depression significantly increase as their authority increases, she further points out, while symptoms of depression in men tend to decrease. On Zoom, she further accentuates the statistic that 75 percent of high-performing women receive negative feedback, as opposed to 2 percent of men. This isn't working. 'I desperately wanted to understand why working women were disproportionately struggling and if there was a better way,' French Dunbar writes in the book. She wanted to replace the traditional leadership playbook with a new leadership playbook. 'The way that you show up at work every single day impacts everybody around you,' she tells me. 'And this is why you need to build healthy, thriving organizations so that everyone is healthy and thriving and that people aren't making each other miserable as a result of coming to work.' It was comforting for French Dunbar to learn that it was the system, not her, that was deficient: 'For me, it felt so validating because, of course, we're women and we're always blaming ourselves and 'I'm not good enough,'' she tells me. When it comes to women, 'we're struggling,' she adds. 'We're struggling in a system that harms the majority of the people, the majority of the labor force—nearly 4 billion people worldwide. Most people are dealing with chronic stress, burnout, some sort of mental or physical health issue as a result of work-related stress in that system. Women, then, are contending with all of these additional barriers, and that is why we have increased risks of chronic stress, anxiety, burnout, all of the other issues. When I understood that, I was like, 'Oh, that means that when we succeed in this system, we're actually extraordinary.'' 'When you look at the data of the odds that you're going to succeed within the system, there's a reason that you're feeling the way you're feeling if you're stressed and overwhelmed and all the things,' French Dunbar says to a very relieved me. 'And you're absolutely incredible that you've been succeeding in this system, because everything is ripping against you.' French Dunbar had been in what she calls 'the hamster wheel of achievement addiction' for a very long time, and the walls eventually crumbled down around her. 'I was so burned out and so miserable that I had to walk away from my own company that I loved,' she says. 'And that is a very common story that I hear, is pushing yourself so far until people are hospitalized or having breakdowns, having panic attacks and then going to work the next day being like, 'That's totally normal. Everyone around me is having panic attacks. It's totally fine.'' Spoiler alert: it's not totally fine. The book teaches readers a new way forward, one that, when everyone—including and especially women—in the workplace feels better, it actually improves the company's performance. This is good not just for employees' quality of life, but 'it makes better business sense,' French Dunbar says. One of many examples of a mindset shift from the book? It's one that French Dunbar says she's 'having the hardest time practicing what I preach'—the 'relentless pursuit of achievement and tying your self-worth to that and getting so busy that you don't even know who you are anymore,' she says. (Yep. Been there.) 'We are so busy that we don't even have the time to check in on how we are actually doing.' The reframe? A tool from Stanford psychologist Carol Dweck, who delineates the difference between performance goals and learning goals, with an emphasis on the latter. 'A performance goal is like, 'I want an A in French,' and a learning goal is 'I want to learn French,'' French Dunbar says. 'And so, with something like a book or your career, not everything has to be about your next title or the salary or The New York Times bestseller list. It is like, 'I'm doing this for the experience of gaining knowledge, having experiences and enriching who I am as a human,' instead of, 'I will only consider this to be a successful thing if it hits these certain milestones.'' Releasing the relentless pursuit of achievement 'has been a big one for me,' she says, and learning that 'you matter and you are worthy just as you are'—regardless of achievement. The first step, she tells me, is admitting there's a problem and that the current work culture that society has isn't okay. Check. The book takes readers through what to do next. 'Your stress, your overwhelm, your exhaustion, your burnout—that is not on you,' French Dunbar says. 'That's not a you problem. It's a cultural problem of the way that we're doing business.' But, she adds, 'It can be done in such a better way. There are extraordinary humans who have already done this and have already written the blueprint.' That blueprint makes up the pages of This Isn't Working. Above all else, Dunbar says? When readers close the book, no matter how burned out or exhausted or overwhelmed they are, she wants this to be made clear: 'There is hope,' she says.

Supernus strengthens neuropsychiatric portfolio with Sage Therapeutics
Supernus Pharmaceuticals has acquired Sage Therapeutics, strengthening its neuropsychiatry product portfolio. In June 2025, Supernus and Sage entered a definitive agreement for the acquisition, with Supernus offering $8.50 per share in cash (or an aggregate of $561m), plus a non-tradeable contingent value right of up to $3.50 per share, totalling $12 per share in cash, or $795m. The acquisition follows Sage's rejection of a $469m takeover offer from Biogen in January 2025, which was deemed significantly undervalued by Sage's board of directors. The acquisition brings Sage's US Food and Drug Administration (FDA)-approved Zurzuvae (zuranolone) capsules and a central nervous system discovery platform under Supernus' portfolio. Zurzuvae is intended for treating postpartum depression. The company expects to realise cost synergies of up to $200m annually and accretive financial results in 2026 due to the acquisition. Moelis & Company served as the exclusive financial advisor to Supernus, while Goldman Sachs advised Sage. Legal counsel for Supernus was provided by Saul Ewing, and Kirkland & Ellis represented Sage in the legal aspects of the transaction. Supernus Pharmaceuticals CEO and president Jack Khattar stated: 'Sage is an ideal fit in our corporate development strategy, adding a significant fourth growth product to our portfolio and further diversifying our sources of future revenue. Rare condition made mom feel like she was "walking on a hot bed of Legos"
When Amber Meade twisted her ankle and broke a bone in her foot, she expected recovery to be easy. She needed surgery to shave down the damaged bone and make sure the nerves and tendons in her foot were where they should be. The procedure seemed to go smoothly, but in the weeks that followed, she began to feel debilitating pain. A second surgery found that a screw placed in her foot had been pressing on the head of one of her nerves. That screw was moved, and a nerve decompression was performed but the pain didn't stop. "It would just turn black and blue, swell up really big, and I just was unable to put any weight on it," 41-year-old Meade said. The pain upended everything. She left her job as a surgical assistant since she could no longer stand for long periods. She was unable to take part in family activities with her two young sons. Even light breezes could trigger the pain. Resting under a blanket was unbearable, making sleep difficult. Meade compared the sensation to "walking on a hot bed of Legos constantly, while getting stabbed with a pencil." Meade spent months seeing different specialists. Finally, after nearly a year, a doctor suggested Meade might have something called chronic regional pain syndrome. It was the first time Meade had heard of it. It would take even longer before she could find a treatment that helped her pain. What is chronic regional pain syndrome? Chronic regional pain syndrome, or CRPS, is a chronic disorder with no cure, said Dr. Rohan Jotwani, an interventional pain specialist and anesthesiologist at NewYork-Presbyterian and Weill Cornell Medicine. Jotwani was not involved in Meade's care. CRPS can occur after surgery, like Meade's, or from other injuries to the nerve. "For some patients, they'll have an injury to a nerve and it will get better with time. For some patients, that injury will actually develop into its own chronic pain disorder," Jotwani said. Diagnosing the condition is difficult, Jotwani said. No test or scan can definitively identify it, and because it is a rare condition, many doctors may not be familiar with it, he added. Physicians have to rely on clinical criteria. Telltale hallmarks for the condition are extreme levels of pain at the source, even when nothing should be causing it - like when a bedsheet causes pain, as Meade experienced. CRPS patients may also experience swelling, changes in temperature and skin color or texture changes. They may even begin to lose function in the affected area. Diagnosing chronic regional pain syndrome is only the start, Jotwani said. Actually treating the condition requires a "multi-pronged approach" that can vary from person to person. Treatment typically starts with physical therapy. Patients may also take medication to decrease nerve signals in the area, so they feel less pain, or try interventional techniques like a nerve block. Meade tried multiple options, but nothing was improving her pain. "Doing regular therapies was not working. I even heard, at 33, that I was getting older and that 'sometimes we just hurt more.' I thought that was comical," Meade said. Treating chronic regional pain syndrome In 2022, long after Meade's pain started, a doctor suggested she see physical therapist Dr. Anita Davis, who specializes in treating the condition and leads the comprehensive pain rehabilitation program at Brooks Rehabilitation in Florida. The pair worked to develop a physical therapy routine that would work for Meade. "Before me, she had all of the traditional exercises ... and those things were just not possible with that much pain in her foot," Davis said. "As healthcare providers, we ask patients to rate the pain from zero to 10. These folks are typically going to be at the top of the scale, even on a good day. When I ask her to do something that's painful on top of her existing pain, it's just crazy to think about doing that." They also worked on relaxation and emotional exercises to cope with the mental load of the constant pain. "Pain management for Amber ... isn't just the physical aspects," Davis explained. "Anything that enhances relaxing the nervous system is valuable." The protocol led to some improvements for Meade, but while CRPS patients can have "symptoms subside for a length of time," it is "a lifelong condition," Davis said. "There's not a cure for it," Davis said. Working towards a less painful future Meade said she still feels pain and discomfort, and is at risk of flare-ups when the weather changes or if she steps the wrong way. In addition to working with Davis, she is taking medication and receiving ketamine infusions every few months. She said the combination of ketamine treatment and physical therapy allows her to "do a lot more than I have been able to." The effects of the infusion usually last around four weeks. Meade uses that period to work on strengthening her body. "I try to make some headway in my physical health before that starts going down, because I know it's going to," Meade said. "The pain is a lot less when my physical health is better." Meade said even without the effects of the infusions, she is dealing with less pain than before her diagnosis and treatment. She can focus on her day-to-day activities. She sleeps better and can walk on the beach, one of her favorite activities. She can't do too much running around with her kids, but she can play catch and help with homework most days. Meade said she hopes to improve her functionality and eventually go back to work and be more active, but for now, she is grateful for what she has gained. "I want to try to get to that point where I have less and less flares, and get up to doing what everybody enjoys doing: Working, coming home, having something to work towards, going on vacations," Meade said.
