Robotic-assisted surgery steadily transforming gynaecological care: Study

Bengaluru: A study released to coincide with Women's Health Month, is shedding light on how robotic-assisted surgery (RAS) is steadily transforming gynaecological care across the country. May month is observed as Women's Health Month worldwide, to focus on raising awareness about women's health issues and encouraging women to prioritise their physical, mental, and emotional well-being.
'Women's Health Month is a time to reflect on how surgical technology innovations like the da Vinci robotic technology are enhancing healthcare outcomes for women,' said Dr Rooma Sinha, Honorary Professor and Chief Gynaecologist at Apollo Health City, Hyderabad, one of the surgeons who oversaw the study.
The multi-institutional study, 'Trends in Gynaecological Robotic Surgery in India: A Real-World Scenario', was conducted across five tertiary care hospitals -- Apollo Health City (Hyderabad), PGIMER (Chandigarh), Manipal Hospital (Bengaluru), Narayana Health (Bengaluru), and Tata Memorial Hospital (Mumbai). It analysed the data of surgeries performed using the da Vinci technology over a decade.
The study documents a significant rise in the use of robotic surgery for both benign and malignant conditions and is significant for its implications on access, recovery, and surgical outcomes for women. 'Robotic-assisted surgery is increasingly being used to manage benign gynaecological conditions such as fibroids and endometriosis, offering enhanced precision and improved outcomes across a wide range of cases,' said Dr Sinha, who is also the Founder-President of Association of Gynecological Robotic Surgeons.
According to her, over 70 per cent of robotic gynaecological procedures took place in the final five years, indicating a strong upward trajectory. 'We have seen that with robotic procedures, women often spend less time in the hospital and experience fewer complications related to blood loss,' said Dr Vanita Jain, Head of Obstetrics and Gynaecology at PGIMER, Chandigarh, who was also part of the study.
She noted that it is not just about using new technology. 'It is about making recovery more manageable, especially for women who are balancing responsibilities at home and work,' added Dr Jain.
The study found that fibroids were the most common benign indication for robotic surgery, followed by endometriosis, adenomyosis, vault prolapse, and ovarian cysts. These conditions, said the doctors, often require careful dissection and suturing, particularly when fertility preservation is important. 'Myomectomy, or the removal of fibroids while preserving the uterus, especially calls for such precision,' explained Dr Subhas C Saha, Professor, Department of Obstetrics and Gynaecology, PGIMER, Chandigarh, another key facilitator of the study.
'Advanced robotic systems like the da Vinci platform offer us better visualisation and manoeuvrability, which can make a real difference in these challenging cases,' he added. While more public and private insurers are beginning to include robotic-assisted surgery (RAS) in their coverage, the study highlights that there is still room to improve affordability and access.
To optimise value and accessibility, surgeons are refining techniques -- using fewer robotic arms and incorporating multifunctional instruments -- to reduce costs while still delivering high-quality care and better patient outcomes.
'Given the clear benefits demonstrated in studies like this, there is a strong case for insurance payers to further expand coverage for robotic-assisted surgery. Broader, streamlined reimbursement support can help ensure more women across India have access to advanced surgical options,'

New Delhi, Jul 31 (PTI) Semiconductor projects approved by the government till date will produce over 24 billion chips per annum and there are more projects in the pipeline as well, a senior IT ministry official said on Thursday. Speaking at an event organised by Germany-based applied research organisation Fraunhofer-Gesellschaft, Amitesh Sinha, Additional Secretary in Ministry of Electronics and IT and India Semiconductor Mission CEO, said that the government has approved six projects comprising a wafer fabrication plant by Tata Electronics and five packaging units. 'Tata fab is going to produce 50,000 wafers per month. The other 5 packaging units are going to produce 24 billion chips per annum. There are many more proposals under appraisal. So in near future you will see lot of approvals coming up," he said Sinha said that India is going to be a long-term player in the semiconductor space 'We are here for a long-term journey. Semiconductor is not a business for a few years. We assure all of you that the policies will be continued and support for development of the entire ecosystem will be provided," he said. The government has put in place a Rs 76,000-crore scheme to boost the semiconductor ecosystem in India. A significant amount of the fund has been committed to the approved projects. 'A lot of rare earth materials and permanent magnet recycling, all those are the areas where you can see synergy with Fraunhofer. Coming back to semiconductors, we are seeing that India has approved some proposals and is going to approve many others," Sinha said. He asked German semiconductor companies to support fabrication activities in India. Sinha said that there are a lot of collaboration opportunities with Germany as India is looking to enter in the space of high technology research. 'Now we are also going to develop the supply chain ecosystem in India. In the supply chain, a lot of technologies will be required for the high grade purity of chemical gases and materials. Fraunhofer's research is very strong in materials. In 2D materials or graphene also, there is a lot of scope and few of our R&D organizations are doing research in 2D and graphene technologies also. So there also I see scope," he said. 2D materials have the potential to produce over 10 times smaller chips than silicon-based chips being developed at present. Sinha said India is a trusted player in the global supply chain and has policies which are very transparent. 'The honourable prime minister of India has already announced that we would like to be a trusted player in the global supply chain. Whatever industry we are setting here, whether it is semiconductor or artificial intelligence or quantum computing, it is for the better use for the entire world," Sinha said. Could your gut help clear out toxic 'forever chemicals'? Scientists are beginning to explore the possibility that certain gut bacteria might help remove per- and polyfluoroalkyl substances (PFAS) - colloquially known as forever chemicals - a class of chemicals linked to cancer and other health harms from the body. A study published in Nature Microbiology found that certain gut bacteria were able to absorb PFAS, allowing mice with these microbes in their gut to excrete the chemicals through faeces. The findings add to growing interest in how gut microbes could influence the body's handling of environmental pollutants. What are PFAS and how do they accumulate in the body? PFAS are a large class of synthetic chemicals often called 'forever chemicals' due to their strong carbon-fluorine bonds, which make them highly resistant to degradation in the body or environment. 'These chemicals have been used since the 1940s across everyday products, from nonstick cookware and food packaging to firefighting foams,' said Uday Nagesh Shivangi, senior consultant and clinical lead in medical gastroenterology at Narayana Health, Bengaluru. These chemicals primarily enter the body through ingestion of tainted water or contaminated food like fish, meat, or eggs. Inhalation of contaminated dust and skin contact with PFAS containing items are also possible, though less prominent. Once inside, PFAS bind to serum proteins like albumin and accumulate in organs like the liver and kidneys, as well as in the bile staying in the body for years, explained C. Kolandasamy, senior consultant, department of surgical gastroenterology, Rela Hospital, Chennai. The process of enterohepatic circulation in which substances excreted in bile re-enter the intestines and are reabsorbed into the bloodstream can further prolong their half-life in the body. . 'Some of these chemicals, like PFOS or PFOA, can remain with half-lives of over 8 years,' Dr. Shivangi explained. Can gut bacteria help eliminate PFAS? In the recent study , researchers observed that mice carrying specific types of gut bacteria were able to excrete PFAS more effectively through faeces, compared to germ-free mice. 'This research gives us early clues about how the gut microbiome might influence PFAS elimination,' Dr. Kolandasamy said. 'Certain bacteria appear to bind to PFAS molecules, preventing their absorption and helping excrete them via faeces.' The mechanism is not one of chemical breakdown, but rather of sequestration, where microbes like Lactobacillus or Bacteroides bind to PFAS in the gut and trap them for elimination. Some microbial metabolites, such as bile acid conjugates, may also play a role in promoting excretion. 'Not all microbes do this,' cautioned Kayalavizhi Jayaraman, consultant gastroenterologist, Prashanth Hospitals, Chennai, 'Animal studies have shown that strains like Bacteroides can accumulate PFAS within them. While it's tempting to think of these microbes as filters, much more research is needed before recommending them for PFAS clearance.' The idea is scientifically compelling but still at a preclinical stage. 'So far, the most convincing data comes from lab and animal studies,' said Dr. Kolandasamy. 'Mice with a diverse microbiome show reduced PFAS absorption compared to germ-free mice. However, no human clinical trials have conclusively demonstrated that boosting specific gut bacteria reduces PFAS levels.' Dr. Jayaraman added that human evidence is mostly observational at this point. 'We do see associations between PFAS exposure and gut dysbiosis -- meaning changes in microbial diversity, but studies on actively changing the human microbiome to eliminate PFAS are still lacking.' Role of diet and gut health While there is no specific diet proven to eliminate PFAS, certain dietary strategies could support the gut's natural filtering functions. 'Dietary fibre has shown potential,' said Dr. Kolandasamy. 'Fiber can bind to bile acids, which also carry PFAS, promoting their excretion. Similarly, probiotics, particularly those containing Lactobacillus strains have shown reduced PFAS absorption in rodent models.' Pilot studies have also explored the use of bile acid sequestrants such as the cholesterol-lowering drug cholestyramine to lower serum PFAS levels. 'These findings suggest gut-focused therapies could be promising in the future,' he added. Dr. Jayaraman emphasised a preventive approach. 'While we don't yet have a diet protocol to clear PFAS, eating wholesome foods, increasing fibre, and limiting processed and packaged food items may support overall gut health and resilience. It is also wise to avoid non-stick cookware and limit known PFAS exposures.' Future research With growing awareness of chemical exposures in everyday life, researchers are exploring whether microbiome-based therapies such as designer probiotics or even faecal microbiota transplantation (FMT) could one day help detoxify environmental chemicals like PFAS. 'There is already interest in engineering bacteria that express PFAS-binding proteins,' said Dr. Kolandasamy. 'But these are still early-stage concepts, and there are regulatory and scientific challenges to overcome.' Dr. Jayaraman added 'While it's exciting to imagine a future where a probiotic capsule could help clear toxins, we need years, if not decades of research, to reach that point. Until then, the focus should remain on minimising exposure and supporting gut health naturally.' This is a field worth watching,' said Dr. Kolandasamy, 'but for now, prevention and awareness remain the best defense.'

Chandigarh: PGI mulling video consultations for follow-up patients to ease OPD rush

To ease the overwhelming patient rush during OPD hours, the Post Graduate Institute of Medical Education and Research (PGIMER) is planning to introduce tele-consultation services for follow-up patients. PGIMER sees a daily OPD footfall of 8,000-10,000 patients, of whom two-thirds visit the hospital for follow-up. (Ravi Kumar/HT) The telemedicine department has circulated a proposal to all departments, seeking their participation. During their OPD visit, if follow-up patients opt for online consultation, they will be given a specific date and time for a video call with the doctor. Each clinic or OPD will need to allocate a separate room for these tele-consultation sessions. If implemented, the initiative will save patients, especially those travelling from far-off places, time, money and effort, allowing them to consult doctors from the comfort of home. Currently, PGIMER sees a daily OPD footfall of 8,000-10,000 patients, of whom two-thirds are follow-ups and one-third are new patients registering for the first time. Managing this rush has been a long-standing challenge for the institute. One initiative in this direction is the Digi Seva system, already implemented in the department of hepatology. It allows follow-up patients to register online, bypassing long queues. Patients using online registration are also given priority, further cutting down wait times. The telemedicine department has been offering tele-consultation to doctors of Haryana, who require expert opinion on various cases, through a memorandum of understanding between the institute and the Haryana government. Since 2021, the department has provided 2.5 lakh consultation to various government hospitals doctors. However, direct doctor-to-patient tele-consultation is currently not offered at any government hospital in the region. If PGIMER manages to roll out this service, it will be the first government institution to do so, said a professor at the hospital, not wishing to be named. A meeting involving stakeholders from various departments was held last month, following which a formal proposal has been circulated. So far, internal medicine, nephrology, neurosurgery, endocrinology, obstetrics and gynaecology, and gastroenterology departments have shown interest. Adequate manpower a challenge While the project is in its initial stage and technical modalities need to be worked out, adequate manpower remains a major challenge in implementing the proposal. Each department will need at least one data entry operator to schedule appointments and an assistant for the doctor during video calls. Departments will either have to spare staff from their existing pool or fresh recruitments will have to be done. At present, the telemedicine department has 25 staffers, of which 23 are working under two health ministry-funded projects. PGIMER itself has provided only two contractual staffers for cleaning and sweeping. Other than tele-consultation, the department also creates educational content, including video lectures and training modules for MBBS students, under the health ministry's learning management system. The department has no sanctioned faculty posts. It has long been demanding the creation of such posts to strengthen its tele-consultation, academic and research work. The proposal for faculty positions was tabled in the institute's 131st Standing Finance Committee meeting last month and sent for review. The head of the telemedicine department declined to comment, citing the proposal's early stage. The next meeting will be held once responses from all departments are in.
