No issues with AI-171 aircraft or engine; probe far from over: Air India CEO

A couple of days after India's Aircraft Accident Investigation Bureau released its preliminary report on last month's Air India flight crash, the airline's chief executive told employees to avoid drawing premature conclusions as the investigation was far from over.
In an internal note accessed by Mint, CEO Campbell Wilson highlighted that AAIB's report had not found any issues with the engines or the Boeing 787 aircraft.
'The Preliminary Report found no mechanical or maintenance issues with the aircraft or engines, and that all mandatory maintenance tasks had been completed. There was no issue with the quality of fuel and no abnormality with the take-off roll,' Wilson said in the note.
He added that the pilots had cleared their mandatory pre-flight breathalyser test and the report contained no observations pertaining to their medical status.
Wilson also said every Boeing 787 aircraft operating in Air India's fleet was checked within days of the 12 June crash and found fit for service. 'We continue to perform all necessary checks, as we will any new ones that authorities may suggest,' Wilson said in the internal note to Air India employees.
On 13 June, the Directorate General of Civil Aviation (DGCA) ordered Air India to carry out additional maintenance actions on its fleet of Boeing 787-8/9 aircraft equipped with Genx engines with immediate effect.
The civil aviation regulator also ordered Air India to conduct a one-time check of various aircraft systems before the departure of any flight from India from the midnight of 15June.
Wilson highlighted that AAIB's preliminary report provided both greater clarity and opened additional questions.
'The release of the Preliminary Report marked the point at which we, along with the world, began receiving additional details about what took place,' Air India's CEO said, asking employees to refrain from speculation until the final report is out.
AAIB released itspreliminary report exactly a month after the AI-171 Boeing Dreamliner aircraft crashed in Ahmedabad on 12 June, killing 260 people, including all but one of the 242 on board. In the report, AAIB highlighted that both engines were shut mid-air within seconds after take off.
The aircraft started to lose altitude before crossing the airport's perimeter wall.
'The aircraft achieved the maximum recorded airspeed of 180 Knots and immediately thereafter, the Engine 1 and Engine 2 fuel cutoff switches transitioned from RUN to CUTOFF position one after another with a time gap of 01 second,' read the preliminary report.
CCTV footage from the airport showed the aircraft's ram air turbine, typically used only in an emergency, getting deployed during the climb immediately after lift-off.
'In the cockpit voice recording, one of the pilots is heard asking the other why did he cutoff. The other pilot responded that he did not do so,' AAIB said in its report, adding that the pilots tried to restart the engines.
'The Engine 1 fuel cutoff switch transitioned from CUTOFF to RUN at about 08:08:52 UTC. Engine 1's core deceleration stopped, reversed and started to progress to recovery,' said AAIB. 'Engine 2 was able to relight but could not arrest core speed deceleration and re-introduced fuel repeatedly to increase core speed acceleration and recovery.'
AAIB found out that the throttle control module in the aircraft was replaced in 2019 and 2023. 'However, the reason for the replacement was not linked to the fuel control switch. There has been no defect reported pertaining to the fuel control switch since 2023 on VT-ANB,' it said in its preliminary report.VT-ANB refers to the crashed aircraft's registration.
Fuel samples taken from the bowsers and tanks used to refuel the aircraft were tested at DGCA's laboratory and found satisfactory. All applicable airworthiness directives and alert service bulletins were complied with as well. The aircraft's left engine was installed on 1 May 2025 and the right engine on 26 March 2025.
'At this stage of investigation, there are no recommended actions to B787-8 and/or GE GEnx-1B engine operators and manufacturers,' AAIB said in its preliminary report.

Tesla is using its artificial intelligence and manufacturing capabilities to build its Optimus humanoid robots. Version three of 'Optimus is the right design to go to volume production," said Musk. Tesla plans to make a few hundred of those by the end of 2025. Originally, Tesla planned to have a few thousand, but the new design slowed things down a little. Tesla still plans to ramp production higher in 2026. Tesla is betting big on AI, using it for robots and to train its self-driving cars. Robots are 'probably the world's biggest product…There's a market for 20 billion robots," said Musk. 'Hypothetically, if Tesla was making one billion of these a year…maybe on the order of $30,000, I'm just guessing here, that's $30 trillion in revenue." It's an incredible prediction. 'Long way to go between here and making one billion robots a year," added Musk. There isn't a significant market for humanoid robots yet. Nvidia CEO Jensen Huang has said that robots can become the world's largest market. (Housing, consumer electronics, and cars are three of the largest today.) The world spends 'what $50 trillion on human labor a year now?" says Futurum chief market strategist Shay Boloor. Useful robots are 'pretty disruptive if you can conquer it." Musk, for his part, doesn't fear the labor disruption, believing it will end up creating an age of abundance with hard labor essentially eliminated. 'I've never seen any technology advance as fast as AI," said Musk, describing it as a supersonic tsunami. It's all pie in the sky for now, but quite a vision of the future. Investors should pay attention to events like the 'Takeover." It's almost like a Wall Street event for the EV maker. Tesla is more of a retail stock than the megacap companies. Smaller retail investors hold more than 40% of the shares available for trading, according to Bloomberg. The average for the rest of the Magnificent Seven is closer to 25%. Whether the interview will move Tesla's stock on Monday is anyone's guess. Shares dipped 8% on Thursday after Tesla reported second-quarter earnings on Wednesday evening. New Delhi: Air India has asked its pilots to ensure rules are followed during their mandatory annual test, officials told HT on Sunday. The internal email, sent by the director training late night on Saturday, is a notice to all pilots stating that the airline is upgrading the CAE system. CAE, a Canadian simulator company, is a pilot training management platform used by airlines. The notice read, '..This step is being introduced as an additional mitigation measure, while our planning teams continue to monitor the process closely as well…' 'Pilots have been asked to double-check their assigned trainer for Annual Line Route check (ALC) against their previous ALC record,' one of the officials aware of the development said. ALC is a yearly flying test for pilots where a senior trainer flies with them during a regular flight to see how they perform. The test is done on an actual flight. A comment from Air India was awaited till the time of going to press. A second official aware of the matter however, said that the software upgrade triggered the email to the pilots. 'Civil Aviation Rules (CAR) mandate the pilots to perform these annual tests with different trainers every year. Since the software is being upgraded, automated safeguards may not be fully functional and so manual verification has been requested from the pilots.' 'This is not related to the recent notices,' he insisted. This is part of its strategy to ensure norms are followed during such planned training - non compliance have triggered slew of notices from the aviation safety regulator, another official said. To be sure, India's aviation regulator on Wednesday issued four show cause notices to Air India for multiple violations of operational procedures and regulations on training cabin crew and managing their duty hours. The government, in its first meeting with the senior management of Air India including the Chief Executive Officer (CEO) Campbell Wilson received a detailed explanation of the airline's efforts towards maintaining safety, reviewed recent incidents, discussed fleet maintenance, and asked the airline to reassess its operations within a month. The move came amid heightened concerns over passenger safety and the operational standards being followed by both Air India and its low-cost subsidiary. 'The government, last week, ordered Air India to maintain the highest level of safety standards, after which it will review the airline's operations after a month,' another official said. These efforts are being made to ensure compliance and many more will follow. 'The airline is now taking more efforts,' he added.
