Two life-saving medications that no one is using
Rapidly, varenicline (then sold under its brand names, Champix and Chantix) became unavailable. This was a disaster for public health. Research from University College London estimated that varenicline being unavailable resulted in about 1,890 more avoidable deaths each year because fewer people were successfully quitting smoking.
But there was hope. Cytisine (also known as cytisinicline), a naturally occurring plant-based product that had been used for decades in Eastern Europe, and more recently to great effect elsewhere in the world, was licensed in the UK and made available from January 2024.
Even so, there was an extended period when neither were available to people trying to quit smoking in the UK (and in other countries, too). But in the UK at least, things were looking up.
Based on a limited but growing body of evidence, cytisine probably works as well as varenicline at helping people quit smoking, and it may be better tolerated with fewer side effects.
It may also appeal to more smokers who may want to use a natural product rather than a drug designed in a lab. So, with varenicline withdrawn and a similarly effective treatment available, we should have seen lives saved as people who would have taken varenicline were encouraged to try cytisine instead.
Why isn't anyone prescribing it?
This didn't happen. Cytisine – despite now being licensed and available in the UK – is still shockingly underused.
Since January 2024, only 0.2% of people trying to quit smoking have used it (the same proportion that used it in 2018, when it wasn't even officially available in the UK). Official NHS data from people accessing stop-smoking services in England confirm that only 0.7% were prescribed cytisine in 2024.
So why is this? High-profile trials continue to show cytisine's effectiveness for quitting smoking (and even for quitting vaping).
Maybe cytisine's relatively complex dosing schedule puts people off. Cytisine starts with six pills a day (one every two hours) and gradually tapers off over a few weeks: more confusing and less convenient than one-a-day varenicline.
Another possibility is that the public's attention has shifted. With so much focus in recent years on vaping as a smoking cessation aid, prescription drugs for smoking cessation may have fallen off the radar.
It could also be that GPs are reluctant to prescribe cytisine because of its cost and the assumption that local authorities should pay for it, not primary care. While it was once hoped that due to its low-cost availability in Eastern Europe, it would become the 'aspirin of smoking cessation drugs', the licensed product in the UK is now as or more expensive than other drugs.
But the simplest explanation is probably the most accurate: not enough people know about cytisine. People who smoke, GPs, pharmacists and even stop-smoking services may not know it's an option. And if no one is talking about it, no one is prescribing it. And even if they do know about it, there may be a lack of confidence in using or prescribing it because it is a new drug.
That's a problem. The UK government has made the shift from treating illness to preventing it a central part of its health strategy. Smoking remains the leading cause of preventable death in the country and the world. If we're serious about prevention, then effective smoking cessation support must be top of the agenda.
Now, varenicline is available again (without its brand names and reformulated to remove the impurities). This is welcome news, but only 1.1% of past-year smokers reported using varenicline. That's only a quarter of the number from before its withdrawal.
This raises an important question: should we return to prescribing varenicline by default, or is it time to consider cytisine as a first-line treatment? Researchers are continuing to learn more about cytisine, but as the evidence in favour of cytisine grows, maybe it needs a PR campaign for both prescribers and people who smoke.
None of this is to say that cytisine is a miracle cure, or that it will work for everyone. But that's true of every way to help people quit smoking. Quitting smoking is hard, and people trying to quit need more options, not fewer, and those options need to be visible and accessible.

NHS maintained 93% of planned care during resident doctors' strikes
The NHS maintained care for an estimated 10,000 more patients during the latest doctors' strike compared with last year's industrial action. Early data shows that 93% of planned operations, tests and procedures went ahead during the five-day walkout across England. In previous walkouts, the majority of non-urgent care was postponed. Fewer than a third of resident doctors, previously known as junior doctors, chose to join the strike, which ended on Wednesday morning. The number was down by 7.5% (1,243) on the previous round of industrial action in July last year, according to the early analysis of management information collected by NHS England. NHS chief executive Sir James Mackey noted that care was still disrupted for thousands of people. He said: 'While this is really good news for the vast majority of patients whose treatment went ahead, we should still acknowledge that there were thousands whose care was disrupted. 'NHS staff will continue to work hard to ensure patients can get their rescheduled care as soon as possible, but for those patients – and for all our staff who had to work extra shifts or with different responsibilities – a repeat of this action will be unacceptable. 'So, I would urge the Resident Doctors Committee to get back to the negotiating table and work with us and the Government on the meaningful improvements we can make to resident doctors' working lives.' Health Secretary Wes Streeting said: 'I want to again thank staff from across the NHS who have put in an incredible shift to make sure that the NHS was open to as many patients as possible over the five days of unnecessary strike action.' Mr Streeting said earlier this week that he 'never left' the negotiating table, and that he is willing to meet with the resident doctors committee of the British Medical Association (BMA) to resume talks in their ongoing dispute over pay and working conditions. But he also warned that 'it should be clear to the BMA by now that it will lose a war with this Government'. NHS Providers, which represents hospital trusts, said the walkout took a toll and that trusts are concerned about potential wider industrial action in the health service. 'Even though trusts cancelled fewer appointments than before, the strike still took its toll on patients and an already stretched NHS,' Saffron Cordery, NHS Providers deputy chief executive, said. She said NHS trust leaders want to focus on improving services instead of dealing with strikes, which she said have set back progress in cutting waiting lists. She added: 'This dispute can't drag on. The union says resident doctors want this to be their last strike. With talks due to resume, let's hope so. Bringing disruptive strikes – where the only people being punished are patients – to an end must be a priority. 'We're concerned, as trusts worked hard to minimise disruption and to keep patients safe during the resident doctors' strike, by the threat of wider industrial action in the NHS.'

Curating your fitness routine to better match your personality could help you get the most out of your workouts, a new study shows. The paper, published last month in Frontiers in Psychology, shows that certain personality traits may indicate which type of exercises people enjoy and how likely they are to stick with them. "We found that our personality can influence how we engage with exercise, and particularly which forms of exercise we enjoy the most," Flaminia Ronca, one of the study's co-authors and an associate professor at University College London, said in a press release. "It's OK if we don't enjoy a particular session," Ronca said. "We can try something else." Ronca and her fellow researchers recruited 132 adults, and placed them into two groups. One group was told to follow at-home workouts of cycling and strength training, and the other group was asked to follow their usual routines. Participants were given a questionnaire to determine their personality type and stress levels. By the end of the study, a total of 86 participants completed their group's assignment and surveys which asked which workouts they enjoyed the most before and after the experiment began. The paper focused on the Big Five personality traits used in psychology: Here are the workouts that each personality type found the most exciting. People who scored high in extraversion seemed to enjoy high-intensity workouts (HIIT) and team sports. Some exercises that extroverts should try are: Those who got high scores for conscientiousness tended to like goal-oriented exercises. They really enjoyed activities like training for a marathon or joining a run club. Exercises that conscientious people should explore are: People that scored high in neuroticism, or those who often experienced mood swings and anxiety, preferred light, one-on-one workouts. "This group might appreciate being given space for independence and privacy during an exercise program," Ronca said. Anxious people may benefit from having private sessions with a personal trainer or a Pilates instructor, and would likely avoid group classes. People who fall into the group of openness and agreeableness liked fun workouts and looked for variety. Millions on NHS waiting lists yet to have first appointment
Millions of people in England on NHS waiting lists are yet to have their first appointment, new data reveal. Figures from NHS England last month suggest nearly half of those waiting for hospital care have not had their first appointment with a specialist or a diagnostic test since their GP referral. An analysis by health data specialist MBI Health suggests nearly three million out of the 6.23 million patients (48 per cent) waiting for treatment are left in limbo. Experts referred to the issue as a 'frontlog' and warned the crisis 'has gone unchallenged for too long'. MBI's analysis also found that around 70 per cent of referral-to-treatment pathways fall into the category of being 'unseen' since the patient's GP referred them to a specialist. Delays in making a first assessment can lead to late diagnosis, worsening symptoms and pressure on emergency services. The analysis found that ear, nose and throat, trauma and orthopaedics, gastroenterology, ophthalmology and gynaecology and obstetrics departments were consistently the specialist departments with the greatest number of patients not seen for the first time. 'That's a breakdown' As part of the Government's 10-year health plan, the NHS is expected to meet its target of carrying out 92 per cent of routine operations and appointments within 18 weeks by March 2029 – a target that has not been achieved for almost a decade. The latest figures show how challenging that target will be given an estimated one million of the three million unseen patients have already gone more than 18 weeks without receiving any care. 'If accurate, three million people are trapped in an invisible waiting-list crisis, stuck without basic diagnostic tests of first appointments while their conditions worsen,' Rachel Power, the chief executive of the Patients Association, told the Guardian. 'The scale is staggering, as nearly half of all patients on a waiting list haven't been seen by anyone. That's not a healthcare service; that's a breakdown. 'These aren't just statistics. They're people checking their phones daily for hospital calls that never come, unable to plan their lives while their symptoms deteriorate.' Last month it was found people of working age are making up a growing proportion of those on the NHS waiting list for treatment in England. Data tables published for the first time by NHS England also show people in the most deprived parts of the country are more likely to wait more than a year to start hospital treatment than those in the least deprived. The figures, analysed by the PA news agency, showed 56.1 per cent of those on the list at the end of June this year were of working age (defined as age 19 to 64), up from 55.8 per cent a year ago and 55 per cent in June 2022. At the same time, the proportion of people on the waiting list under the age of 19 has fallen, standing at 10.8 per cent in June this year, down from 11.2 per cent a year earlier and 11.9 per cent in June 2022. The proportion who are over 65 has remained broadly unchanged at around 33.1 per cent. People of working age are also more likely to have to wait more than a year to start treatment (three per cent of patients in this age group at the end of June) than those over 65 (two and half per cent). However, the proportion is the same as those under 19 (also three per cent). An NHS spokesman said: 'NHS staff have made significant progress in reducing waiting lists in the last year, which are down by more than 260,000 since June 2024. 'All patients are assessed by clinicians as they are added to waiting lists, and we will continue with our plan to improve waiting times for patients throughout the year.'
