Victim identified in fatal four-vehicle crash in Siloam Springs
The report said around 11:30 a.m., a 2010 Chevrolet Traverse entered onto Highway 412 west and failed to yield to oncoming traffic. An 18-wheeler swerved into the left lane to avoid the Traverse but struck the vehicle's left side.
The 18-wheeler then struck a 2019 Chevrolet Equinox head-on and left the roadway, coming to a rest on the north side of the road.

A fourth vehicle was mentioned in the report, however, no details were included on its involvement in the crash.
The driver of the Traverse, identified as Peggy Nelson, 84, of Westville, Okla., was killed in the crash, according to the report. Three other people were injured in the crash.
Conditions were clear and dry at the time of the crash, according to the report.
