JOHANNESBURG, Feb 18 (Reuters) - South African lab technician Nozipho Mlotshwa was waiting for the test results for a potential HIV vaccine, which has eluded scientists for decades, when the order came from USAID to stop work.
The first round of vaccines she and her colleagues made in Johannesburg had produced an immune response in rabbits, which was promising but not conclusive - so they tweaked the formula and sent off four new versions for pre-clinical tests.
"This was very exciting. We were getting quite good results," Mlotshwa, 32, told Reuters in the lab in the Antiviral Gene Therapy Research Unit at the city's University of the Witwatersrand.
Now the animal blood samples containing their results are sitting untouched in a freezer.
A trial of an earlier, separate vaccine candidate, which was about to be tested on humans in South Africa as well as Kenya and Uganda, is also on ice.
Both trials are among the casualties of U.S. President Donald Trump's decision to dismantle the United States Agency for International Development (USAID).
They are part of a wider South African-led HIV vaccine development scheme known as BRILLIANT and funded entirely by a $45 million grant from USAID. It is unclear if or when the project could resume. The U.S. State Department did not immediately respond to a request for comment.
"It feels like you're building something and you could really make a huge difference," Nigel Garrett, Chief Scientific Officer at the Desmond Tutu Health Foundation, a partner in the project, said.
"And then it's wiped away."
The project is one of many research efforts worldwide to be hit by Trump's actions since taking office last month. Others include halting efforts to protect food crops from pests and diseases and blocking publication of a paper on the mpox outbreak.
'HOLY GRAIL'
HIV's ability to mutate quickly has confounded efforts to create a vaccine ever since it was first identified in 1983. The researchers in Johannesburg are using the mRNA technology that created some COVID-19 vaccines.
Several other mRNA-based HIV vaccine candidates worldwide have reached clinical trials. BRILLIANT is unique in being Africa-led, aiming to develop capacity for producing vaccines in Africa.
For the past year the Johannesburg team had been working with genetic sequences from two South African patients who have HIV but whose bodies produce a rare type of antibody that neutralizes the virus. They are trying to simulate that immune response.
"We were gaining momentum," said Patrick Arbuthnot, director of the research unit, adding: "an HIV vaccine is the holy grail of the field".
Trump in January ordered a 90-day pause in all foreign development assistance pending assessment of its consistency with his "America First" foreign policy.
Separately, he has targeted South Africa with an executive order to cut all funding to the country, citing disapproval of its land reform policy and its genocide case against U.S. ally Israel.
The U.S. foreign aid freeze has affected programmes across the globe, stranding shipments of life-saving medical supplies, including HIV drugs, and leaving disaster response teams unable to deploy. Waivers for "life-saving humanitarian assistance" have been hampered.
'GOOD FOR THE WORLD'
Because South Africa has the world's largest population of people living with HIV, at more than 8 million, it is a hub for research on the virus.
"Most of the landmark and groundbreaking studies have been conducted in this country. But these have been good for the whole world," said Ntobeko Ntusi, CEO of the South African Medical Research Council, which is spearheading the HIV vaccine search.
Ntusi said he did not expect funding for projects like BRILLIANT to resume, given the executive order on aid to South Africa. The council gets about a third of its funding from U.S. federal sources, for research that is mostly on HIV and tuberculosis but covers other areas including maternal and infant mortality and antimicrobial resistance, he said.
Garrett said the shot that was ready for testing on humans was a mix of two vaccine substances developed in the United States and the Netherlands which have shown promise but never been tested together.
They are now sitting in storage.
"We had a huge opportunity, good funding. It's difficult for other funders to fill that gap," he said.

It suggests that there could be an increasing number of people in need of help who may not meet the tougher criteria. 'We know the difference that the proper equipment can make to the lives of people with mobility issues, and if the government truly wants people to be able to get into work, then cutting eligibility for support is the wrong way to go about it. 'Ultimately, the numbers could show that the U.K. is on the verge of a perfect storm where we see more people unable to remain productive, because they needed help at the time when it became impossible to get.'

I think it's fair to say that since 2019 it's been a difficult time in the medical profession. Before Covid hit, we were already having reports of mass burnouts, there was already low staffing, people already leaving for Australia. Covid then obviously had a significant impact on everyone, not just doctors – nurses, healthcare workers, and patients. Most of all, after that, we've then had the issues of the pay erosion since 2008, and low staffing, as people know that they can get better terms, conditions and pay in other countries. So in terms of my day-to-day life, I am seeing the impact an NHS, which hasn't kept its staff and hasn't supported its staff, is having an impact on patients every day. I can make a big difference, but I'm in a system that hasn't supported its doctors or nurses or healthcare workers for years, and now it's falling apart around my ears, so my day-to-day life is disheartening, I think would be the nicest way to put it. If I see somebody and I know that they have an issue that might be solved, I have to tell them they're going to have to live with it for 18 months because we haven't trained enough people to even get the waiting lists down. This month, I'm paying £462 for the GMC [General Medical Council], which is the regulator, my exams cost £500 a pop. Some people's exams cost way more, just to stay in training and keep going. And there have been times where it's been difficult to afford it. The F1s [first year doctors] are the ones who are in the most financial issue, because they've just finished medical school, where they will have accrued up to or and above £100,000 in debt. They are then working the first month and they get paid right at the end of it, so they are in dire straits financially at that point, and for that first year, they're only paid £18.62 an hour. I know F1s who are having to take out loans in order to cover their rent. I'm here because the government had the possibility of negotiating. They had the chance to make my life better, and they just keep kicking the can down the road, and at some point everyone needs to stand up and say: 'No, you were elected to make our lives better. You were elected to make everyone's lives better in this country.' And all that's happened is a stagnation. I feel now that the only way to get them to listen is to strike.' More than 300 hospitals across the US are at risk of closure under deep healthcare cuts in President Trump's Big Beautiful Bill. Some 338 facilities in rural areas could be forced to close, convert, or slash services, according to the Cecil G. Sheps Center for Health Services Research. Democratic senators highlighted the findings in a letter to the President last month, urging him to reconsider the legislation. Congress, however, passed the bill earlier this month despite Republican rebels fighting back against its steep cuts to Medicaid among other issues. The act slashes over $1 trillion in healthcare spending over the next 10 years, mostly from Medicaid, which provides health coverage for millions of low-income Americans. 'A majority of adults living in rural areas are concerned that these health care cuts will "negatively impact hospitals, nursing homes, and other health care providers in [their] community,"' the senators' letter read. 'They are right to be worried, as these cuts will have devastating consequences for health outcomes and costs, jobs, and the economic success of rural communities.' The data found that 46 states had at least one rural hospital at risk, with Kentucky and Louisiana seeing the highest numbers, at 35 and 33, respectively. A rural hospital is labeled 'at risk' when it meets one or both of two criteria. It needs to either be in the top 10 percent Medicaid payer mix of rural hospitals across the country, or have experienced three consecutive years of negative total margin. 'In Louisiana, 32 rural hospitals — or a majority of rural hospitals in the state — are serving a high concentration of Medicaid patients,' the letter, signed by U.S. Democratic Senators Edward J. Markey, Ron Wyden, Jeffrey A. Merkley, and Charles E. Schumer. 'In Alaska and Kentucky, 44 percent of the rural hospitals with available Medicaid payer mix data serve high concentrations of Medicaid patients. 'In West Virginia, nearly a quarter of rural hospitals are serving high concentrations of Medicaid patients, and 15 percent of rural hospitals are at the highest relative risk of financial distress, per the financial distress index.' The letter pointed out that rural hospitals are already struggling. In 2023, there were 50 fewer rural hospitals than in 2017 — and a lack of healthcare access in rural America is contributing to worse health outcomes for residents. 'Enacting these drastic health care cuts that will kick millions of people off their health insurance coverage, rural hospitals will not get paid for the services they are required by law to provide to patients,' the authors wrote. Alongside Medicaid cuts, the bill will also cut from Affordable Care Act insurance plans and eliminate insurance coverage for 11.8 million people over the next decade, according to the nonpartisan Congressional Budget Office. In response to bipartisan criticism of the impact of the bill on rural hospitals, the Senate added $50 billion in funding for a new rural health fund. But critics say this will not be enough, as it is about a third of the estimated loss of federal Medicaid funding in rural areas. In response to the claim that the bill will close rural hospitals, the White House said: 'Rural hospitals comprise just 7 percent of all hospital spending on Medicaid, illustrating that they have not benefited from the massive increase in waste, fraud, and abuse under the Biden administration. 'By strengthening Medicaid, we are making more resources available for vulnerable populations and safety net providers, like rural hospitals. We are expanding rural hospital protection, providing targeted funds for rural care, and giving states flexibility to support local providers.' Following the signing of the bill into law, Senator Markey said rural hospitals are now sorting through 'difficult questions.' 'Will they have to lay off staff? Will they need to stop providing labor and delivery care or mental health care? Will they need to close their doors entirely? 'One hospital in Nebraska already announced it would close because of these cuts. That's a preview of coming atrocities,' he said. People would have to travel further for chemotherapy sessions and will miss their annual appointments, he added, saying that the consequences could be 'deadly' for communities. It comes as various private hospital chains have filed for bankruptcy in the last several years, with vital local services closing as a result. Steward Health Care, a major hospital system backed by private equity, filed for bankruptcy in May 2024. The chain is now suing its 'greedy' former CEO Ralph de la Torre, claiming he plundered the company of hundreds of millions of dollars that directly led to its collapse. Prospect Medical Holdings also devastated communities by closing several hospitals after filing for bankruptcy earlier this year.
