Medications can add to heat related illnesses
Extreme heatwaves can trigger heat exhaustion and heat stroke, but health experts say some people on specific medications can be more prone to these types of risks.

For the second half of the 19th century and the early part of the 20th, pneumonia was a leading cause of death that killed hundreds of thousands of people. Sir William Osler – usually considered the father of modern medicine – described pneumonia at the time as "the most fatal of all acute diseases" because it killed about one in four of the people it infected during the Civil War. Thanks to advancements in modern medicine, though, pneumonia in the U.S. is not as fatal as it once was. Still, some 41,000 people in the country die from the disease every year – and more than 1.4 million people still visit an emergency room because of it, according to the U.S. Centers for Disease Control and Prevention.
Here's what pneumonia is, what causes it and how the disease is usually treated once it has been diagnosed.
What is pneumonia?
Pneumonia is a serious respiratory infection that exists inside of the lungs, says Dr. Jillian Diuguid-Gerber, an internal medicine specialist at NYC Health + Hospitals/Woodhull. She explains that it's what happens when the tiny air sacs inside of the lungs (called alveoli) fill with fluid or pus.
This can lead to clogged airways or inflammation that can make it difficult to breathe. Telltale shortness of breath, rapid breathing or other breathing difficulties are the most worrisome symptoms of pneumonia, as they "can be a sign of low oxygen levels," says Diuguid-Gerber. But breathing difficulties are not the only symptom of the disease.
Other symptoms include chest pain, fatigue, loss of appetite, sweating "and fever and cough are especially common," says Dr. David Levine, pediatrician and the Chief of Ambulatory Pediatrics at the Atlantic Health System's Goryeb Children's Hospital in New Jersey.
Symptoms of pneumonia can be concerning in anyone but are especially problematic in children as their lungs are still developing; they have smaller airways than adults. "Pneumonia can also be very worrisome in people with weakened immune systems and with existing lung problems," adds Diuguid-Gerber.

What causes pneumonia?
Pneumonia is caused by bacteria, viruses or fungi, "but usually when people are thinking of pneumonia, they are thinking of the kind caused by bacteria," says Diuguid-Gerber. She explains that the most common bacteria to cause the disease is called Streptococcus pneumoniae, but other bacteria can also cause it. These include Haemophilus influenzae, Mycoplasma pneumoniae and Staphylococcus aureus.
Beyond bacteria, "viruses are also common causes of pneumonia," says Diuguid-Gerber. These include any virus that can affect the respiratory system, including influenza (flu) viruses, the adenovirus (similar to the common cold), RSV, human metapneumovirus and the coronavirus (COVID-19).
While bacteria and viruses are behind most pneumonia cases, Levine says that fungi can also cause pneumonia. Pneumocystis jirovecii and Histoplasma capsulatum are the types of fungi most commonly responsible.
The cause of the disease also determines whether pneumonia is contagious or not. The viral forms of it that are most contagious are transmitted "through respiratory droplets being released into the air when a person coughs, sneezes or even talks," says Levine.

How is pneumonia treated?
Regardless of what has caused the disease to develop, symptoms of pneumonia are often targeted through everyday remedies like getting plenty of rest, staying hydrated with plenty of fluids using a humidifier to soothe irritated airways and taking over-the-counter pain relievers like ibuprofen and acetaminophen to treat discomfort. Hospitalization may be needed in order to provide oxygen support or intravenous hydration.
Treating the cause of the disease can often be done with nothing more than "at-home care," says Levine, "but some cases of the disease require antibiotics." Antibiotics only treat bacterial forms of pneumonia, however, and antiviral medications are used to treat viral forms for the disease, Diuguid-Gerber explains. Antifungal medications are usually needed to treat fungal pneumonia.
The discomfort of pneumonia and associated treatment can often be altogether avoided through vaccination. "Getting the pneumonia vaccine, flu vaccine or COVID vaccine could all help prevent pneumonia," says Diuguid-Gerber. Indeed, pneumonia vaccines have been shown to be between 45% and 75% effective in preventing the disease, depending on what type of pneumonia you're dealing with.
And even if you're among the people who get pneumonia despite having gotten the vaccine, Levine adds, "vaccines can prevent many of the worst outcomes of the disease."

Are July 4th Fireworks Bad For the Environment?
Fireworks are equal parts lovely and ugly—lovely because, well, they're fireworks, and who doesn't enjoy a good pyrotechnic sky show? The ugly part comes mostly after, in the haze of toxic gases and particles the blasts produce, along with the rain of paper, cardboard, and metal debris, and the lingering trauma to pets and wildlife, who take a lot less pleasure from the bangs and flashes than we do. According to estimates by the American Pyrotechnics Association (APA), the average July 4th in the U.S. will see about 14,000 public fireworks displays. In 2021, the last year for which the APA provides figures, Americans bought and launched more than 428 million lbs. of fireworks. That's an awful lot of ordnance—and it can produce an awful lot of problems. One of the biggest environmental dangers of fireworks displays is the dispersal of fine particles of sulfur dioxide, nitrogen oxide, and more, measuring just 2.5 microns—or 2.5 millionths of a meter. These so-called PM2.5 particles can become deeply embedded in the lungs and even enter the bloodstream, contributing to decreased lung function, increased respiratory distress, aggravated asthma and COPD, irregular heartbeat, heart attacks, and premature death in people with preexisting heart or lung disease, according to the Environmental Protection Agency (EPA). One 2015 study in the journal Atmospheric Environment found that levels of PM2.5 particles rose 42% at 315 firework sites surveyed nationwide during July 4th celebrations. The increases peaked between the hours of 9:00 and 10:00 p.m. and didn't dissipate until noon on July 5. Spikes in some lung conditions may occur from a single fireworks show. "Fireworks massively reduce air quality with their explosions of particulate matter," says Bill Bateman, associate professor in the school of molecular and life sciences at Australia's Curtin University. "This can have a pretty immediate effect on our breathing and our health. In many places around the world there are reported peaks in respiratory problems after fireworks." 'What level of exposure you get depends on your activity levels—for instance, whether you're outdoors—and, if you're indoors, how leaky your house is,' said Jun Wu, a professor of environmental and occupational health at the University of California, Irvine's program in public health, in a 2024 conversation with TIME. 'If you have to go outside, wearing a [N95] mask would be good.' Particles aren't all fireworks produce. According to a 2024 study in Applied Geochemistry, they also release, carbon dioxide, and carbon monoxide—which can cause lung damage—along with several metals, including aluminum, manganese, and cadmium. Some illegal fireworks may even contain lead, warns the American Lung Association (ALA). It is not generally possible to determine if fireworks contain lead since it won't be listed on the ingredients. The Consumer Products Safety Commission recommends buying only known, reputable brands. Airborne contaminants will ultimately settle out—but that can be a problem because they may wind up in water and soil and on plants. Perchlorates, a common ingredient in fireworks, flares, fertilizer, and solid rocket fuel, can pollute lakes, rivers, and reservoirs, endangering wildlife and entering the drinking water supply. In humans, perchlorates affect the function of the thyroid gland, which can alter metabolism. In infants and babies still in the womb, perchlorates may interfere with normal growth and central nervous system development. Soil, too, collects more than trace amounts of perchlorates, along with metals. By themselves, the perchlorates found in fireworks may not be enough to cause harm, but they add to the overall load of the chemical in the environment, which can ultimately be dangerous. Contamination that winds up in water and soil tends to linger there—perhaps longer than expected. From 1999 to 2009, fireworks displays were set off on Independence Days at Mt. Rushmore in South Dakota. From 2011 to 2015 researchers from the United States Geological Survey (USGS) returned and sampled water from 106 sites and soil from 11 sites in that vicinity. All of them showed perchlorate contamination and fireworks debris—especially on the northeast side of the park, where the pyrotechnics went off. 'The lack of alternative perchlorate sources in the area, such as a military site or agricultural land with applied fertilizers, and the presence of firework debris, suggest that past fireworks are the perchlorate source,' said one USGS scientist in a statement. Animals suffer too, both from the same pollutants that threaten humans and from the noise and light of fireworks displays. One 2023 paper in Pacific Conservation Biology reported that nearly 75% of companion animals studied—from horses to dogs to smaller mammals—showed fear responses to fireworks. Some horses broke paddock fences and sustained injury trying to escape the flashes and bangs. Fireworks displays can interfere with the breeding season of birds, with one study the 2023 paper cites showing that fireworks festivals in Valencia, Spain, in April and May of 2015 reduced the breeding success of house sparrows. When the pyrotechnics were canceled in 2020 due to the COVID-19 pandemic, the sparrow populations bounced back. Independence Day celebrations in California similarly diminished populations of Brandt's Cormorants. "Disturbing roosting migratory birds at night can result in mass deaths," says Bateman. "Disturbing breeding birds can result in colony collapses. Seals and sea lions leave [affected] areas and expend energy swimming away during fireworks." And then there's the problem of litter. Every exploding firework rocket will eventually tumble and flutter down to the ground in a cloud of debris. "There are local peaks in microplastics and other plastic waste after fireworks," says Bateman. "Once in the environment they are there for good effectively." There are alternatives to fireworks. Both drones and laser shows can produce visual spectacles that have no environmental footprint apart from the energy used to operate them, and health and environmental groups are increasingly calling for towns, cities, and individuals to make the switch. The Fourth of July celebrates the founding of the nation. What better way to honor that nation than to keep it peaceful and clean?

Letters to the Editor: Vaccines save millions of lives. Don't let Robert F. Kennedy Jr. diminish them
To the editor: For Robert F. Kennedy Jr. to be our secretary of Health and Human Services is not only having the fox guard the chicken coop — the fox is invited right into the chicken coop. His unfitness for the job and danger to the health of our citizens was beautifully laid out by columnist Robin Abcarian in her article on the critical role of vaccines and the dangers we face because he belittles their validity and importance ('RFK Jr. is dismantling trust in vaccines, the crown jewel of American public health,' June 29). During my pediatric residency decades ago, some of the 'old-timers' occasionally brought in patients who had diseases that we rarely saw anymore because of vaccines, including measles. One night, a toddler came into the emergency room and we admitted her immediately to the intensive care unit because we could see she had a grave infection. She died the next day. The infection she had, Haemophilus influenzae type B, is now part of the vaccination schedule. That vaccine alone is estimated to have saved 2.85 million lives from 1989 to 2024. There is little that prevents infant mortality like vaccines. Even though the insurance industry has followed Centers for Disease Control and Prevention recommendations for vaccines, paying for the ones recommended by the CDC Advisory Committee for Immunization Practices, I recommend and hope they will pivot to following the recommendations of professional medical organizations such as the American Academy of Pediatrics. Now, as Abcarian discusses, we have a Health secretary who tells Americans to eat healthy while he feasts on junk science that erroneously claims that COVID vaccines, along with other vaccines, are harmful. We should all be concerned about our nation's transition from Operation Warp Speed to what I call 'Operation Warped Mind' — a foolish opposition to vaccines — and the risks that this poses to our health. And we should all question why the president nominated an unqualified person to be the Health secretary and why all Republican senators except Mitch McConnell voted to confirm him. It's notable that McConnell, a survivor of childhood polio, explained his position by saying, 'I will not condone the relitigation of proven cures, and neither will millions of Americans who credit their survival and quality of life to scientific miracles.' Countries around the world use its guidelines and recommendations for diagnosing and treating COPD. The GOLD system previously classified COPD from stages 1 to 4 solely based on your lung capacity measured by a spirometry test. But the newest GOLD guidelines combine your spirometry results with subjective measures of your symptom severity to determine your level of risk based on their ABCD rubric. A spirometer test involves exhaling into a machine called a spirometer. The machine measures how hard and quickly you can exhale. Your doctor examines two numbers in your results: your force vital capacity (FVC) and your force expiratory volume in one second (FEV1). Your FVC is the total amount of air you can breathe out and FEV1 is the amount you breathe out in one second. A ratio of your FEV1/FVC needs to be less than 0.7 for a diagnosis of COPD. Doctors grade your spirometry results from grade 1 (least severe) to grade 4 (most severe). In the old system, these grades correlate with stage 1 to stage 4 COPD. Doctors determine the severity of your symptoms using either the British Medical Research Council (mMRC) questionnaire or the COPD Assessment Test (CAT). Stage 1: Early Stage 1 COPD is considered mild. At this stage, you may not be aware that you have anything wrong with your lung function. A doctor assigns you with grade 1 COPD if your FEV1 is between 80 and 100% of your predicted value. Symptoms If you score grade 1 on your spirometry test, you may not have any noticeable symptoms. If you do have symptoms, you may develop a cough and increased mucus production. You may mistake the early stages of COPD for the flu. Treatment With stage 1 COPD, your doctor may recommend a bronchodilator medication to open up the airways in your lungs. These medications are usually taken through an inhaler or nebulizer. Your doctor may also recommend getting flu, COVID-19, and pneumonia vaccines to prevent illnesses that may worsen your respiratory symptoms. Changing lifestyle habits that led to the development of COPD can potentially help slow the progression of your COPD. Most people with COPD have a history of smoking. Quitting smoking and avoiding secondhand smoke are two of the most important things you can do to improve your outlook with COPD. Stage 2: Mild Your COPD is considered to be stage 2 when your FEV1 drops to 50 to 79% of your predicted value. Symptoms During stage 2, your symptoms worsen from stage 1. Your coughing and mucus production may become more severe, and you may experience shortness of breath when walking or exercising. Typically, it's in this stage that people realize that something is wrong and seek medical attention from a doctor. Treatment You may be given bronchodilator medication to increase airflow to your lungs. Your doctor may recommend pulmonary rehabilitation, a program designed to increase your awareness about your condition. It's usually offered as a group class where you learn how to manage your condition better. If you have a flare-up of symptoms, you may need to take steroids or oxygen. Stage 3: Severe By the time you reach stage 3, COPD is considered severe, and your forced expiratory volume is between 30 to 50 percent of your predicted value. You may have trouble catching your breath doing household chores and may not be able to leave your house. Symptoms By stage 3, you may get more frequent flare-ups, and your shortness of breath and coughing typically get worse. You will likely find you get tired easier than before. Other potential symptoms can include: Treatment The treatment options for stage 3 COPD are similar to stage 2. You're more likely to need oxygen therapy at this stage. Stage 4: Very severe Stage 4 is considered very severe. Your forced expiratory volume is less than 30% of your normal value, and your blood oxygen levels are also low. You're at risk of developing COPD complications like heart or lung failure. Symptoms By stage 4, you'll likely have frequent flare-ups that can be potentially fatal. You may have trouble breathing even when you're resting. Treatment Treatment options during stage 4 are similar to the previous stages. Your doctor may also recommend lung surgery to improve breathing. Surgical options include: lung transplant lung volume reduction surgery bullectomy Is there an end stage to COPD? A grade 4 score on your spirometry test is the highest grade you can receive. This category is also sometimes referred to as end stage COPD. For many people at this stage, quality of life is usually fairly low, and symptom flare-ups can be fatal. What is the life expectancy of someone with COPD? Having COPD lowers your life expectancy. However, the amount that it affects your life expectancy depends on many factors, such as how far your COPD has progressed and whether you quit smoking or change the other lifestyle habits that lead to COPD. An older 2009 study found that COPD led to a small reduction in life span for people who have never smoked, but a larger life span decrease for people who currently smoke or smoked in the past. According to the researchers, a 65-year-old man who smokes would lose 3.5 years from their life expectancy for smoking, plus the following values for having COPD: Stage 1: 0.3 years Stage 2: 2.2 years Stage 3 or 4: 5.8 years For former smokers, the reductions are: Stage 1: 1.4 years Stage 2, 3, or 4: 5.6 years For somebody who never smoked, the reductions are: Stage 2: 0.7 years Stage 3 or 4: 1.3 years In a 2017 study, researchers followed a group of long-term smokers with COPD for 5 years. They found that participants who had died before a 5-year follow-up had a longer average smoking time and a lower prevalence of quitting smoking. The researchers concluded that smoking time may be related to COPD mortality rate, and quitting smoking has the largest potential to influence COPD outlook. A 2020 study of 532 people with COPD at a hospital in Taiwan suggests that loss of life years can vary based on a person's GOLD stage: GOLD stage 1: no reduction GOLD stage 2: 6.2 years GOLD stage 3: 9.3 years GOLD stage 4: 9.4 years This study did not distinguish by smoking history. Can you prevent COPD from progressing? COPD can't be reversed, and it's currently not possible to stop the progression of your COPD completely. You can help slow the progression of COPD as much as possible by working with your doctor and following a proper treatment program. Living with COPD No matter how far your COPD has progressed, it's important to change the lifestyle habits that caused your COPD in the first place. The most important thing you can do is quit smoking. If you don't quit, you'll continue to damage your lungs, and your COPD will progress quicker than it would otherwise. Eating a nutrient-rich eating plan filled with plenty of vegetables, healthy fats, protein, and unprocessed foods may help you manage your symptoms and maintain a healthy weight. Eating foods that cause bloating, like apricots or peaches, may worsen breathing problems in some people. Regular exercise may help decrease symptoms of COPD by strengthening your respiratory muscles and improving your cardiovascular health. Your doctor may be able to suggest exercise that is safe for you.
