18-wheeler driver admits he was asleep at the wheel before I-20 crash that killed 5 in Terrell, arrest affidavit says
The driver of an 18-wheeler that accused of causing a chain reaction crash in Terrell last weekend that killed five people told law enforcement he was asleep at the wheel before the crash happened, according to an arrest affidavit. Alexis Osmani Gonzalez-Companioni, 27, was driving his 18-wheeler when he failed to see stalled traffic around 2:40 p.m. on June 28, striking a Ford F-150 carrying five people, the Texas Department of Public Safety said. The impact pushed Gonzalez-Companioni's semi into the F-150 and another 18-wheeler, causing a jackknife that struck a Jeep Compass, a Ford Mustang and a Honda. A horrific crash scene on I-20 near Terrell The DPS trooper who responded said when he got to the scene of the crash on Saturday, he saw body parts of multiple people scattered around the road, according to the arrest affidavit obtained by CBS News Texas. The trooper also said there was a woman stuck in the Ford F-150 screaming for help and another woman in a Jeep screaming, slumped over. One person was ejected from the F-150. The force from the 18-wheeler crashing into the F-150 caused it to crash into another 18-wheeler, causing the transmission of that 18-wheeler to fall out, the affidavit said. Gonzalez-Companioni admitted to the trooper that he was asleep and woke up to a loud bang, the affidavit said. He was arrested and charged with five counts of manslaughter and one count of aggravated assault with a deadly weapon. One person remains in critical condition after the crash Texas DPS identified the victims in the F-150 as Billy McKellar, 79, Zabar McKellar, 52, Krishaun McKellar, 45, and 16-year-old Kason McKellar. One person in the Jeep, 49-year-old Nicole Gregory, also died at the scene. The fifth injured passenger of the F-150 was identified as 20-year-old Evan McKellar by Texas DPS and family members. As of Tuesday, she remained in critical condition. DPS reported on Monday that the person care-flighted from the F-150 had died, but the family confirmed to CBS News Texas Tuesday that the person was still alive and in critical condition.

Federal investigators eye driver fatigue in deadly 18 wheeler crash on I-20 tied to Florida-based carrier
Federal investigators are examining the role driver fatigue may have played in a deadly I-20 crash near Terrell over the weekend involving a Florida-based truck driver and his employer, Hope Trans LLC. The National Transportation Safety Board, in coordination with the Texas Department of Public Safety, has opened a safety investigation into the crash that killed five people on Interstate 20 in Terrell, Texas. The driver, identified as Alexis Osmani Gonzalez-Companioni, 27, is from Florida and works for Hope Trans LLC, a trucking company with a listed address in the Orlando area, according to USDOT records. Records show the company had a previously listed address of Tacoma, Washington. Federal transportation records show Hope Trans LLC has been in operation since 2021. The company currently operates 65 trucks and employs 80 drivers. It is listed as being in compliance with federal safety regulations. In the past 24 months, Hope Trans LLC has been involved in three crashes resulting in injuries, though none were fatal until this weekend's incident. In terms of driver compliance, based on federal inspection in the past 24 months, the company's record is slightly better than the national average. Five percent of Hope Trans LLC drivers were found "out of service" during a federal inspection, compared to the national average of 6.7%. The Federal Motor Carrier Safety Administration website shows no "acute/critical" violations for hours-of-service compliance or driver fitness. Federal rules limit commercial truck drivers to a 12-hour driving limit after 8 consecutive hours off-duty, as well as a 70-hour weekly limit over 7 consecutive days. As the NTSB continues its investigation, questions remain about what could have been done to prevent the crash—and whether warning signs were missed.

Westbound lanes of I-20 at US-377 in Benbrook are shut down due to a crash. The crash happened around 2:25 a.m. on Monday, according to the Texas Department of Transportation. UPDATE: 4 dead, several injured in multi-vehicle crash backing up I-20 near Kaufman County
UPDATE: According to the Terrell Volunteer Fire Department, four people are dead, two are critically injured and several others have sustained injuries following the seven vehicle crash on I-20 westbound near Kaufman County. The injured passengers were taken to Dallas area hospitals for treatment. Three 18-wheelers and four passenger vehicles were involved and the cause of this crash is unknown at this time. The fire department said that I-20 westbound is still closed and is expected to remain closed for several hours. One eastbound lane has been opened and the Department of Public Safety is investigating the crash. KAUFMAN COUNTY, Texas (KETK) — A multi-vehicle crash in the Kaufman County area is causing lane closures and traffic back up on I-20. Residents urged to conserve water after lightning strikes Nacogdoches water treatment plant According to the Elmo Fire Department, injuries were sustained during a five vehicle crash including two 18-wheelers near Mile Marker 505 on I-20. Elmo Fire Department is encouraging drivers to exit 512 westbound and find a different route. Texas Department of Transportation live cameras show that all westbound lanes are closed at Kaufman County Road 126. KAUFMAN COUNTY, Texas (KETK) — A man is facing five counts of manslaughter and one count of aggravated assault with a deadly weapon following a multi-vehicle crash on I-20 near Kaufman County on Saturday that left five people dead, officials said. The initial impact from Gonzalez-Companioni caused other vehicles to collide into each other, resulting in the deaths of five people. Gonzalez-Companioni was driving westbound on I-20 when he ran into traffic near Hiram Road bridge, according to KXAS. The Terrell Volunteer Fire Department said seven vehicles were involved in the crash including three 18-wheelers and four passenger vehicles. Monthlong road closure set for South Avenue in Tyler Kaufman County Jail records show that Gonzalez-Companioni was booked at 1:22 a.m. Sunday morning on five manslaughter charges and one aggravated assault with a deadly weapon charge with a total bond of $1.5 million. Injured passengers were taken to a local hospital for treatment and their conditions are unknown at this time.
