Interferon-Driven Genes Signal Methotrexate Response in JIA
TOPLINE: Higher baseline expression of interferon (IFN)-driven genes was tied to a better response to methotrexate in patients with juvenile idiopathic arthritis. METHODOLOGY: Analysis of the link between blood biomarkers and response to methotrexate in 97 children (median age at baseline, 8.5 years; 62.9% women) with nonsystemic juvenile idiopathic arthritis (JIA). RNA sequencing was used to understand gene expression on CD4 + , CD8 + , CD14 + , and CD19 + cells and total peripheral blood mononuclear cells at baseline and at 6 months post-treatment with methotrexate. , CD8 , CD14 , and CD19 cells and total peripheral blood mononuclear cells at baseline and at 6 months post-treatment with methotrexate. The link between response to methotrexate therapy and gene activity in specific cell types was tested using the limma-voom, gene set enrichment analysis, and a new 51-gene score. Results were validated in 73 children with JIA, and pretreatment gene expression data were used to compare results with 240 adult patients with rheumatoid arthritis (RA). TAKEAWAY: Gene enrichment in the IFN-alpha (type I) and IFN-gamma (type II) response pathways was linked with treatment response in many cell types isolated from children with nonsystemic JIA. Higher baseline expression of both type I and type II IFN-driven genes was associated with a better response to methotrexate at 6 months post-treatment in JIA patients but not in adult RA patients. The 51-gene score, calculated from the expression levels of 51 specific IFN-response genes, was significantly higher in patients who responded to methotrexate than in those who did not (P = .00556). IN PRACTICE: 'It is possible that MTX [methotrexate] treatment is more effective in a distinct immunophenotype present across many International League of Associations for Rheumatology subtypes, where IFN-driven processes are dominant early in the disease,' the authors wrote. 'Our study provides proof of principle that carefully designed analyses can yield hope for a more precision-based approach to treatment in the future for children and families living with arthritis,' they added. SOURCE: This study was led by Melissa Kartawinata of University College London Great Ormond Street Institute of Child Health in London and Wei-Yu Lin of the University of Cambridge in Cambridge, both in England. It was published online on May 20, 2025, in Annals of the Rheumatic Diseases. LIMITATIONS: This study may have been underpowered to identify pathways with small but biologically significant influence on treatment response. Whole-blood RNA samples were unavailable to test the association between 51-IFN gene score and treatment response. Additionally, treatment response can fluctuate over time, and this study only analyzed outcomes at 6 months. DISCLOSURES: This study received support from the Medical Research Council, Versus Arthritis, Great Ormond Street Hospital Children's Charity, and Olivia's Vision. Additional support was provided by multiple organizations, including the Wellcome Trust and other sources. Several authors reported receiving funds and contributions in kind from various pharmaceutical companies. This article was created using several editorial tools, including AI, as part of the process. Why young adults and even children can get arthritis too
When you hear the term "arthritis," you normally think of old age. It's widely thought to be a disorder that strikes seniors as a result of years of use and abuse on the joints. But as an orthopedic physician, what I experience is a far different reality. Arthritis does and can strike young adults—and even kids. Sadly, because of this widespread misunderstanding, a lot of young patients remain undiagnosed for years, and the disease worsens, causing permanent damage. Knowing Arthritis in the Young Arthritis is not one disease, but rather a collection of more than 100 different diseases that lead to joint pain, stiffness, and inflammation. Although osteoarthritis is the most closely linked to aging, autoimmune forms of arthritis like rheumatoid arthritis (RA), psoriatic arthritis, and juvenile idiopathic arthritis (JIA) may strike children, teens, and young adults in their 20s or 30s. In autoimmune arthritis, the immune system of the body mistakenly targets its own healthy tissues—particularly the joints—leading to inflammation and pain. Gradually, this damages the cartilage, deforms the joint, and restricts movement if not treated. Why Early Diagnosis Matters One of the most important reasons why we stress awareness among youth is that early identification is imperative. In contrast to osteoarthritis, which slowly develops with age, autoimmune arthritis may suddenly start and rapidly advance. by Taboola by Taboola Sponsored Links Sponsored Links Promoted Links Promoted Links You May Like Giao dịch CFD với công nghệ và tốc độ tốt hơn IC Markets Đăng ký Undo The joint lining (synovium) gets inflamed and causes synovial hypertrophy and cartilage damage. Cartilage once destroyed does not regrow, and the damage is usually irreversible. Identifying these conditions early enables us to initiate treatment that can suppress the abnormal response of the immune system. In most instances, this can avert the necessity for joint replacement later on in life. Signs to Look Out For Parents and young adults need to be aware of the warning signs, which are: Chronic joint pain or swelling Morning stiffness longer than 30 minutes Fatigue or low-grade fever Decreased range of motion in joints Limping or inability to do daily activities These should never be brushed aside as "growing pains" or overuse, particularly if they recur. Treatment and Prognosis Owing to advances in medicine, autoimmune arthritis can be treated well with disease-modifying drugs, physical therapy, and lifestyle adjustments. The aim is to treat inflammation, stop symptoms, and avoid damage to the joint. In children with juvenile idiopathic arthritis, early treatment is often followed by remission for the long term. In young adults, successful treatment allows them to have active, productive lives without undue restriction. Arthritis is not only a condition of old age. Children and young adults do get it—and with long-term consequences if it is not diagnosed and treated early. As a rheumatologist, I appealing to parents, teachers, and even pediatricians to be vigilant and take early action in recognizing the warning signs. The sooner we intervene, the more we can safeguard young joints and guarantee an arthritis-free life.
