Actor Michael Madsen was cremated this week following his death earlier his month, about which more details are now emerging. A number of conditions, including cardiomyopathy — which makes it more difficult for the heart to pump blood throughout the body — along with coronary artery disease and chronic alcoholism all contributed to the cardiac arrest that ultimately killed the 67-year-old 'Kill Bill' star, according to a death certificate issued by Los Angeles County Medical Examiner and obtained by TMZ on Thursday. Thromboembolic disease, when vessels or arteries are blocked by blood clots, was also identified as a significant contributing factor. The death certificate shows that Madsen, who'd reportedly gotten sober in recent months, died less than two months after undergoing coronary angiography, which uses X-rays to look at the heart's blood vessels, per Mayo Clinic. Madsen's manager Ron Smith previously confirmed to NBCLA that Madsen died at his Malibu, California, home on July 3 from cardiac arrest. Days later, Madsen's cardiologist told the outlet that heart disease and alcoholism would be listed as contributing factors to his heart failure. Sister and fellow actor, Oscar nominee Virginia Madsen, remembered her big brother as 'half legend, half lullaby' — 'etched in contradiction, tempered by love that left its mark.' Madsen is survived by four children, including his son Christian, who is also an actor. He was predeceased by one of his sons, Hudson, who died by suicide in 2022.

The document also revealed Michael had undergone a coronary angiography, a medical imaging procedure, just weeks before he passed away. The Die Another Day actor friend and assistant Dougie Smith previously admitted Michael's loved ones were shocked by his passing because he was in good health. He told TMZ that Michael - who had been sober for months - had had a routine hospital visit just days before to get his shoulder examined due to complications from a fall from a horse two years ago as he had been considering surgery. He also underwent a PET scan after having a minor brain bleed when he fell but got the all-clear from doctors. He added: "He looked like he had been in the best health he had been in all year!" Michael suffered heartbreak in 2022 when his son Hudson took his own life at the age of 26. The star is survived by his five other children, Jessica who he had with Dana Mechling, Christian and Max, whose mother is Jeannine Bisignano and Luke and Kalvin, whose mom is the actor's wife DeAnna Morgan. Michael was married to Georganne LaPiere, half-sister of Cher from 1984 to 1988. He went on to marry Jeannine from 1991 to 1995 before tying the knot with DeAnna in 1996 and welcoming Luke, Kalvin and Hudson together. The relationship had its ups and downs, and his death comes just 11 months after his domestic violence case - where he was accused of shoving DeAnna - was dismissed due to 'insufficient evidence'.

The certificate from the L.A. County Medical Examiner confirms Madsen's body has since been cremated, per TMZ. Coronary artery disease is a condition that causes the arteries that supply blood to the heart to become narrowed or blocked - restricting blood flow to the heart muscle and leading to oxygen deprivation and damage. This disease is the most common type of heart disease and affects one in 20 Americans above the age of 20. It usually develops due to a buildup of plaque (fatty deposits and cholesterol) on the inner walls of the coronary arteries and can cause mild discomfort to severe pressure or squeezing in the chest, which may radiate to the arms, neck, jaw, or back. Cardiomyopathy is a disease of the heart muscle that affects its size, shape and structure. It affects the heart's ability to pump blood around the body efficiently and can also impact the way electrical signals make the organ beat. Earlier this month Madsen's cardiologist revealed his official cause of death to be heart failure with heart disease and alcoholism named as contributing factors, per NBC LA. The Los Angeles Sheriff's Department considers the case closed with no foul play indicated. The death is listed as from natural causes. Madsen's lawyer said the actor - known for his roles in Quentin Tarantino's Reservoir Dogs and Kill Bill - was battling an alcohol addiction before he tragically died. Hours after his death, obtained photos of Madsen's heartbroken family — including his estranged wife DeAnna — gathering outside his house. A man who looked to be Madsen's son Max, 31, consoled DeAnna while standing in the home's driveway as more family members arrived. Madsen's eldest son Christian, 35, and his youngest son Luke, 19, also appeared to be at the scene. The house was leased out by another person and several thousand dollars were owed in back rent, has learned. His arrest came just a month after his son, US Army Sgt. Hudson Madsen shot himself dead on the Hawaiian island of Oahu, where he was stationed with the 25th Infantry Division. Hudson was an Afghan war vet. Hudson's death came a week after his wife Carlie underwent surgery to remove a tumor from her breast. 'I am in shock as my son, whom I just spoke with a few days ago, said he was happy - my last text from him was 'I love you dad,'' he told the Los Angeles Times. 'I didn't see any signs of depression. It's so tragic and sad. I'm just trying to make sense of everything and understand what happened.' Madsen said his son's marriage was 'going strong' and he had recently completed his first tour in the U.S. Army. 'He had typical life challenges that people have with finances, but he wanted a family. He was looking towards his future, so its mind-blowing. I just can't grasp what happened.' Madsen has requested a full military investigation into his son's death. The actor thinks 'the officers and rank and file were shaming' Hudson for wanting therapy. He believes this stopped him from seeking help for mental health issues he had been keeping to himself. In 2019, the actor was axed from a $100,000 role in movie Confessions of a Serial Killer after crashing his Land Rover into a pole and subsequently being arrested for DUI. He was later sentenced to four days in jail. In 2012 he was also arrested for DUI after driving erratically but struck a plea deal that involved attending AA meetings in lieu of more severe punishment. However, after Madsen failed to attend his court ordered Alcoholics Anonymous meetings his probation was revoked. That same year he was arrested at his Malibu home after allegedly getting into a physical fight with his son after catching him smoking marijuana. The actor was being held in jail on $100,000 bail after being charged with child endangerment with cruelty to a child: a felony charge, which means it is considered a crime of high seriousness. He did not face any charges after his arrest for suspected child endangerment.

Taylor Swift's father, Scott Swift, is recovering after undergoing a heart procedure. The 73-year-old had quintuple bypass surgery just over a month ago and is recovering well, Fox News Digital has confirmed. The procedure was not a result of a heart attack. "He's doing fantastic!" a representative for Taylor said. Scott's wife of 37 years, Andrea, and their children, Taylor and Austin, have all been by his side during his recovery. "Coronary artery bypass surgery is a procedure that detours (or bypasses) blood around a blocked section of one or more coronary arteries," according to Harvard Medical. Surgery is typically conducted if several of the coronary arteries are blocked and the blockages become widespread. "The most common symptom of coronary artery disease is a type of chest pain called angina. This pain is usually described as a squeezing, pressing, or burning pain in the center of the chest or just below the center of the rib cage. The pain may spread to the arms (especially the left), abdomen, lower jaw, or neck." This isn't the first health scare that has hit the Swift family. In 2020, Taylor revealed Andrea had been diagnosed with a brain tumor while undergoing treatment for cancer, which returned for the second time while the Grammy winner was filming her Netflix documentary "Miss Americana." In an interview with Variety, Taylor opened up about what the diagnosis means for her mother, her family and her music career. "Everyone loves their mom; everyone's got an important mom," she said. "But for me, she's really the guiding force. Almost every decision I make, I talk to her about it first. So obviously it was a really big deal to ever speak about her illness. "She was going through chemo, and that's a hard enough thing for a person to go through," she continued. "While she was going through treatment, they found a brain tumor." "And the symptoms of what a person goes through when they have a brain tumor is nothing like what we've ever been through with her cancer before. So it's just been a really hard time for us as a family," she explained at the time. Andrea was first diagnosed with cancer in 2015 and then went into remission. Taylor revealed in Elle magazine in 2019 that her mom was battling the disease again. "I've had to learn how to handle serious illness in my family. Both of my parents have had cancer, and my mom is now fighting her battle with it again," she wrote in a personal essay for the publication. "It's taught me that there are real problems and then there's everything else," Swift described. "My mom's cancer is a real problem. I used to be so anxious about daily ups and downs. I give all of my worry, stress, and prayers to real problems now."
