
Your Patients May Lose Health Insurance: Here's How to Help
The projected consequences are dire: Hundreds of thousands of people will forgo preventive screenings, and an estimated 2 million patients will lose their primary healthcare providers.
US patients change primary care providers for a variety of reasons, such as a job change, but the magnitude of the loss of regular providers due to the federal budget cuts may be unprecedented.
Even patients who express little faith in the medical establishment tend to trust their primary care providers, so losing access to them risks worsening their distrust of the system. Research has linked lack of access to primary care providers to delays in seeking care and foregoing of preventative services, worsening of chronic conditions, and missed diagnoses.
'It's incredibly disruptive,' said Danielle Ofri, MD, PhD, a primary care internist at Bellevue Hospital, New York City. 'It's soul-crushing for a physician to tell a patient they can't take care of them for reasons…they have no control over.'
Yet even in these challenging situations, physicians can guide their patients, including their most vulnerable, through the healthcare options that may be left for them.
Talk to Patients About Insurance
The first thing to do when dealing with potential loss of insurance is to initiate conversations with patients about what may happen, said Sarah C. Nosal, MD, FAAFP, a family physician and the president-elect of the American Academy of Family Physicians.
'The medical community needs to be really bringing the conversation forward,' said Nosal.
Such conversations can be awkward for physicians and patients alike. And some doctors may feel like they are steering toward a politically charged topic that may make them uncomfortable.
'But a patient losing vital medical coverage is not political,' said Nosal. 'This is health, and we need to really encourage all physicians…to open up this conversation irrespective of politics.'
'Take some time to empathize with the patient as to how traumatic it is for them,' said Ofri.
Indeed, it's important to realize that patients who risk losing insurance are already under a significant amount of stress, said Ada Stewart, MD, a family physician in Columbia, South Carolina, and past president of the American Academy of Family Physicians.
Patients may be unable to keep up even with the treatments that they can access, said Stewart, who is no stranger to seeing patients lose coverage; her state didn't expand Medicaid. Her experience taught her never to blame the patient if they didn't keep up with their healthcare needs, she said, and to welcome back a patient who wasn't able to see her for a time and whose health may have worsened.
Maximize Coverage Before It Lapses
If patients losing coverage are acutely ill, doctors should treat them pro bono at least on a temporary basis even in office settings, said Ofri, until they can secure care elsewhere.
Indeed, the American Medical Association's code of ethics states that doctors have a fiduciary responsibility to help ensure care continuity for their patients. (Doctors in emergency rooms are legally required to stabilize patients regardless of their ability to pay.)
However, 'asking physicians to treat all these patients pro bono is not a solution, it's a Band-Aid for a flawed system,' Ofri said.
When patients don't have acute or urgent needs, the priority is accessing as much care as possible before losing insurance. 'While people still have coverage, making sure that they're up-to-date on all of their preventative healthcare measures is one of the biggest things because it's such a big expense,' said Nosal.
Doctors should advise that their patients schedule mammograms, colonoscopies, pap smears, and basic blood work as applicable, as well as any appropriate screenings for their age and existing conditions. 'Also, make sure their vaccines are up-to-date,' said Stewart.
For patients with chronic conditions, doctors should recommend that they fill ongoing prescriptions before losing coverage, getting the longest course their insurance will authorize.
They also should encourage patients to research whether any pharmacies, including online, offer discounts on their medications paid out of pocket, so they have a plan on where to get them once they are uninsured.
Another important aspect is to make sure the patients have an accurate understanding of any conditions they may have, as well as a good record of their treatments, test results, and diagnoses. While electronic records are becoming better and more easily accessible, printing them out for the patient is best, especially because patients may not have access to a phone or computer, said Stewart.
'Don't send patients through the medical records Olympics trying to get copies of their records. Just give them copies,' said Ofri, who suggests patients keep printed copies of their medication list, test results, and after-visit summaries from their previous five visits.
Nosal said she routinely does that for her patients, printing out after-visit summaries, medication lists, and any results of tests such as ECGs. She encourages her patients to keep a physical file with all their medical records, should they be unable to access digital ones.
Help Patients Find Resources
Doctors and clinics also can help their patients by guiding them toward resources available to uninsured individuals, such as federally qualified health centers.
Medical practices can 'put together a list of all the free services or the services that use a sliding scale and at least provide some direction,' said Ofri, adding this effort shouldn't fall on individual doctors but on organizations such as state medical associations.
Some government health systems, like New York City's, are preparing resources to offer to Medicaid patients at a risk of losing coverage.
Clinicians can help their patients verify whether they qualify for aid and are up-to-date on the necessary paperwork. This often means assisting patients with some bureaucratic tasks too, such as guiding them through filling out applications.
While necessary, all this support comes at a cost. '[It] takes people away from caring for the individual, and so it can be a hindrance for the overall care and for the practice in itself,' said Stewart, adding that it is yet another way in which Medicaid cuts end up affecting quality of care. 'It really takes a toll on our offices,' she said.

Sixteen states sue White House over healthcare access for transgender youth
Sixteen states are suing the Trump administration to defend transgender youth healthcare access, which has rapidly eroded across the US due to threats from the federal government. The Democratic attorneys general of California, New York, Massachusetts, Illinois and Connecticut are leading the lawsuit, announced on Friday, which challenges the president's efforts to eradicate vital medical treatments for trans youth. The complaint targets one of the president's first executive orders issued in January, which described puberty blockers and hormone therapy as 'chemical and surgical mutilation', called for federal funds to be withheld from hospitals that provide the treatments and suggested the US Department of Justice could investigate doctors. Those gender-affirming treatments, which are accessed by a small fraction of youth in the US, have for years been the standard of care endorsed by major US medical associations. Under intensifying threats that hospitals could lose federal funding, and growing fears that providers could be criminally prosecuted, a number of major institutions have abruptly ended gender-affirming care for trans youth. The crackdown has sent families scrambling for alternatives, including in blue states long considered sanctuaries for LGBTQ+ rights, where clinics and lawmakers had previously assured youth they would be shielded from Donald Trump's agenda. Last month, Children's Hospital Los Angeles, one of the nation's largest and most prominent institutions to serve trans kids, shuttered its gender-affirming care center for youth after three decades, citing funding threats from across the federal government. Other institutions that have recently pulled back gender-affirming care services for trans youth include Phoenix Children's hospital in Arizona, Stanford Medicine, Denver Health, the University of Chicago, the University of Pennsylvania and Children's National hospital in Washington DC. Some have ceased surgeries for patients under age 19, which are rare, while others have also ended hormone therapy and puberty blockers. The justice department in July also announced that it had sent subpoenas to more than 20 doctors and clinics that provide gender-affirming care to youth, a move that sent shock waves among providers and raised alarms that patients' information could be shared with the federal government. The suit notes the administration has already launched criminal investigations and explicitly threatened providers in California, Colorado and Massachusetts and that officials have 'demanded extensive data, including patient medical records'. The blue states are also challenging a June memo from Brett Shumate, assistant US attorney general, which directed the justice department's civil division to 'use all available resources to prioritize investigations of doctors, hospitals, pharmaceutical companies and other appropriate entities' that provide gender-affirming care. Republican lawmakers in more than 25 states have moved to restrict trans youth healthcare in recent years. But the treatments remain legal in other parts of the country, and states such as California have anti-discrimination laws that explicitly protect the services. The plaintiffs argue that Trump's actions should be declared unlawful, alleging his order exceeds federal authority. California's attorney general, Rob Bonta, also challenged Trump's definition of children as people under the age of 19, affecting 18-year-old adults' access to care, saying the administration's directives force hospitals to violate state laws. Bonta said denying this care had been shown to worsen mental health outcomes, including increased rates of depression, anxiety and suicidal ideation. 'Trump and Bondi are not trained medical professionals. They aren't at all qualified to give anyone medical advice. What's worse, they refuse to listen to the actual experts,' he said at a press conference. Citing comments from one parent of a trans child treated at a center that is shutting down, Bonta said: 'This closure will be life and death for trans youth who can no longer get the care they need.' 'What would you do if your child was diagnosed with gender dysphoria? I would fight for my kid every single day,' added William Tong, the Connecticut attorney general. 'We're fighting for … parents who just want to do the best for their kids, as we all do, to help them live their best and truest lives.' Joining the lawsuit are the attorneys general of Delaware, Hawaii, Maine, Maryland, Michigan, Nevada, New Jersey, New Mexico, Rhode Island and Wisconsin, along with the District of Columbia and the Pennsylvania governor. 'On day one, President Trump took decisive action to stop the despicable mutilation and chemical castration of children – which everyday Americans resoundingly support,' said Taylor Rogers, a White House spokesperson, in an email. 'The president has the lawful authority to protect America's vulnerable children through executive action, and the administration looks forward to ultimate victory on this issue.' A justice department spokesperson added in an email: 'As Attorney General Bondi has made clear, this Department of Justice will use every legal and law enforcement tool available to protect innocent children from being mutilated under the guise of 'care.'' The suit comes after the US supreme court upheld Tennessee's ban on trans youth healthcare. 'Hormone therapy truly saves lives,' said Eli, a trans 16-year-old who lost his healthcare in Los Angeles, in a recent Guardian interview. Department of Veterans Affairs looks to end certain abortion services for veterans
The US Department of Veterans Affairs is proposing to end certain abortion services to veterans, rolling back a Biden-era move to expand abortion rights. In a proposed rule filed Friday, the department said that it is seeking to revoke access to abortions and abortion counseling for veterans and the beneficiaries of the Civilian Health and Medical Program of the Department of Veterans Affairs. 'We take this action to ensure that VA provides only needed medical services to our nation's heroes and their families,' the department said in the filing. Under the Biden administration's rule, the department currently provides access to abortions when a pregnant veteran's life or health is at risk if their pregnancy were carried to term, or if the pregnancy was the result of rape or incest — regardless of state laws. The proposed rule would allow abortions in cases where 'a physician certifies that the life of the mother would be endangered if the fetus were carried to term,' which, according to the filing, had been permitted even before the 2022 expansion. The Biden-era rule was part of the administration's efforts to expand abortion access after Republican-led states pushed ahead with restrictions in the wake of the 2022 Supreme Court ruling that eliminated the federal right to an abortion. The VA argued at the time that it was necessary to give veterans access to abortions, saying, 'As abortion bans come into force across the country, veterans in many States are no longer assured access to abortion services in their communities, even when those services are needed.' But on Friday, President Donald Trump's VA slammed the Biden administration's rule, calling it federal overreach. 'The stated reason for (the expansion) was a reaction to a Supreme Court decision, Dobbs v. Jackson Women's Health Organization … that itself was intended to prevent federal overreach and return to States control over the provision of abortion services,' the filing states. 'Yet, the last administration used Dobbs to do the exact opposite of preventing overreach, creating a purported Federal entitlement to abortion for veterans where none had existed before and without regard to State law.' Twenty states have banned or limited access to abortion. States where abortion is limited report higher rates of maternal and infant mortality, as well as greater economic insecurity. The proposed rule will now be open for public comment for 30 days starting Monday. In his first term, Trump made good on campaign promises and appointed Supreme Court justices who helped overturn Roe v. Wade. Since that ruling, Trump has been keen to leave regulations over the issue to state governments. The Trump administration has overall been quiet on the issue of abortion in his second term. However, in June, the US Department of Health and Human Services and the Centers for Medicare & Medicaid Services moved to rescind a 2022 federal guidance to health care providers specifying that people should be able to access an abortion in the event of a medical emergency, even if state laws restrict such procedures.
