Sindh's largest mental health hospital struggles

The proper functioning of Sindh's largest mental health care facility, which even serves patients from South Punjab and Balochistan, remains handicapped due to shortage of doctors and staff. Some other funding woes of Sir Cowasjee Jehangir Institute of Psychiatry and Behavioral Sciences, aka Giddu hospital in Hyderabad, also surfaced during the recent visit of the Sindh Human Rights Commission's Chairman Iqbal Ahmed Detho.
Although the Sindh government has earmarked Rs651 million for the hospital, including Rs377 component for salaries and allowances of employees, the 500-beded hospital is confronted with severe staff shortage. "There are 12 operational wards but we are coping with a crisis of not being able to run the facility 24/7," a senior officer, who requested anonymity, told The Express Tribune.
According to him, there are six posts of psychiatrists but five of them are vacant. Likewise, the sanctioned strength of the lower staff is 225 but 92 of these posts are also vacant. On the day of the visit on June 26 some 313 out of the 500 beds were occupied, according to Medical Superintendent Dr Nisar Ahmed Soho.
The recently built ward number five with its 200 beds is expected to see its inauguration in August. "Even if it is inaugurated, how can we run it with the staff scarcity," the officer said.
The budget 2025-26 has set aside slightly over Rs90 million for the institute's medicines and Rs78 million for patients' meals. But both the allocations, according to the hospital's officers, fall short of their actual expenditures.
"Last year we received around Rs65 million for medicines and about Rs30 million from that amount was spent on epilepsy medicines alone," the source claimed, pointing out that the health condition technically fell under the domain of neurology and not mental health services. "Yet we have to provide services to thousands of epilepsy patients because it has been the norm for decades."
The provincial government provides Rs270 per meal per patient with an annual allocation of Rs78 million for the upcoming fiscal. However, the institute needs Rs147.825 million for a three-time meal per year in view of the 500 beds, notwithstanding the addition of 200 more beds.
The substance abuse in the society is increasing alarmingly but the institute's 50-bed narcotics rehab ward is also not fully functional, reportedly. A doctor, who also wanted to remain anonymous, claimed that in view of the cases requiring admission for rehab, the ward's capacity is too low.
The infrastructure repair and maintenance works have also become a bane for the administration. The facility is dependent on the Provincial Buildings Department for such works. In March this year, the MS wrote separate letters to the department for repair works in washrooms and to fix leakage in the water tank.
"... this office is unable to carry out any work without funds. When the funds are available the work will be carried out," the department's executive engineer replied to the MS, repeating the same excuse in both his correspondences.
Referring to these issues, the SHRC's Chairman underscored that institutional coordination is indispensable "to mitigate vulnerabilities faced by individuals with mental illnesses". Detho suggested that a committee should be formed to address the persistent gaps.

Civil Hospital to have high-tech modular Ots

The Civil Hospital Karachi is set to receive three state-of-the-art modular operation theatres, designed to meet international infection-control standards. The project, estimated to cost up to Rs450 million, is expected to be functional within two months. Speaking to The Express Tribune, Medical Superintendent Dr?Khalid Bukhari said that the modular OT plan was under consideration for two years. It has officially been approved in the 2025-26 fiscal year. Credit for the scheme goes to Sindh Health Minister Dr?Azra Fazal Pechuho and Health Secretary Dr?Rehan Baloch. Under the provincial initiative, 15 modular OTs will be built across Sindh, with three at Civil Hospital Karachi. Dr?Bukhari noted that while private hospitals in Karachi already have modular OTs, this is going to be the first time that such facilities are installed in a government hospital in the city. He explained that these theatres provide a sterile environment during surgery with HEPA filters, ensuring continuous clean air for both the surgeon as well as the patient. Key features of the modular OTs include airtight doors, dedicated air-handling units, antibacterial flooring, and the use of stainless steel or antimicrobial materials to facilitate cleaning and prevent microbial growth. Temperature, airflow, lighting, and gas systems are all fully controlled, with shadowless lights and automatic doors aiding surgical precision. Compared to traditional OTs, modular versions drastically cut down the risk of infection. Currently, the hospital has 14 conventional operating theatres, where surgeons handle complex cancer and other long procedures lasting six to eight hours. In this context, modular OTs have become essential. Dr?Bukhari said that approval for the three theatres has been secured and a request for four more will be submitted soon. He mentioned that all equipment including tables, head lights, and related hardware will be imported. "These modern OTs will be a boon not only for medical staff but also for patients," he added.

NICVD boss dismisses corruption charges

Professor Tahir Saghir, Executive Director of the National Institute of Cardiovascular Diseases (NICVD), has denied allegations of financial irregularities, claiming that the institute operates with full transparency and a commitment to public service. Speaking to The Express Tribune, Prof. Saghir said that the NICVD provides free treatment to over 2,000 patients daily, including 35-40 emergency angioplasties, alongside 1,000-1,200 emergency cases and 800-1,000 OPD visits. "This is a high-volume centre, and our teams work day and night to serve patients," he added. About corruption charges, Prof Saghir clarified that the NICVD undergoes both government and private audits annually. While audit objections are routine, he said that all queries are responded to with detailed explanations. He highlighted procurement reforms, saying that previously only 20-30% of medical supplies were acquired through tenders, whereas now over 90% of purchases are processed via the Sindh Public Procurement Regulatory Authority (SPPRA) digital system, minimising human intervention. "This data is also shared with international bodies, including the IMF," he added. Refuting a human rights report alleging Rs40 billion in corruption, Prof Saghir pointed out that the NICVD's total annual budget is only Rs9 billion. "Where did the extra Rs31 billion come from? Yet, surgeries and medicines continued uninterrupted," he questioned, terming the accusations baseless and malicious. He cited Sindh government recognition of the institute's performance, which led to an increased budget after NICVD saved Rs1 billion last year, followed by an additional Rs2 billion allocation. Prof Saghir provided an example of emergency decision-making, explaining that when the main A/C unit in the emergency ward failed, the administration hired temporary cooling systems for Rs200,000-250,000 to ensure patient comfort, bypassing lengthy tender procedures.
