Ozempic-Like Drugs Could Treat Chronic Migraines, Trial Finds

Medications sold under brand names like Ozempic, Wegovy, and Saxenda have become famous for their weight-loss benefits, but that only scratches the surface of what these injections are potentially capable of.
Originally designed to treat type 2 diabetes, GLP-1 agonists have also shown unforeseen benefits to heart, brain, liver, and kidney health – and now, it seems, the head, alleviating migraines in a new study.
In a pilot trial of 31 patients with high BMI and frequent or chronic migraines, participants who received a daily injection of the GLP-1 agonist liraglutide experienced significantly fewer painful headaches. After 12 weeks, the number of days with a migraine each month decreased from a mean of 19.8 days to just 10.7 days – a reduction of nearly half.

Weight loss, age, sex, and the use of other medications at the same time did not significantly alter the results.
"Our findings show that liraglutide may be effective in the treatment of unresponsive high-frequency or chronic migraine in patients with obesity, and that this effect is independent from weight loss," conclude the authors, who hail from the University of Naples in Italy.
"This suggests… that the mechanisms driving liraglutide's effectiveness in migraine prevention may operate independently of the significant metabolic effects… "
Further studies with larger cohorts and a control group are needed to verify that hypothesis. But GLP-1 agonists, like liraglutide and possibly even its longer-lasting relative semaglutide, may prove to be a promising route for future migraine treatments.
Migraines impact an estimated 14 to 15 percent of the global population, and yet the few medications we have on hand do not work for everybody.
"A substantial number of patients still face an unmet need, especially when preventive drugs prove ineffective," writes a team of scientists led by neurologist Simone Braca.
Participants in the current pilot trial had migraines unresponsive to other treatments, meaning liraglutide worked where other drugs had not.
GLP-1 agonists slow appetite and help regulate blood sugar by mimicking a natural hormone in the body, called glucagon-like peptide-1, which is released after eating. That's why these drugs are so effective when it comes to managing type 2 diabetes and weight gain.
But GLP-1 receptors exist throughout the body, in many different tissues and organs. The fact that liraglutide and similar medications have widespread effects outside of the pancreas makes logical sense. Still, scientists are trying to figure out what those effects are, and whether they help or harm.
In recent years, studies have shown liraglutide and other GLP-1 agonists can greatly reduce intracranial pressure in the brain – a speculative trigger for migraines.
In animal models, these drugs have also suppressed migraines with great effectiveness.
The current pilot trial is only small and it did not investigate the mechanisms behind liraglutide's migraine relief, nor did it directly measure intracranial pressure among participants.
Nevertheless, Braca and her colleagues suspect that reduced pressure in the skull is playing a role in migraine reduction. In recent animal studies, GLP-1 agonists reduced fluid in the central nervous system, thereby lowering intracranial pressure.
"These findings provide a foundation for larger-scale trials aimed at further investigating the role of GLP-1R agonists in migraine management," Braca and her team conclude.
The study was published in Headache.

You're probably reading this at the start of your weekend. But now imagine that weekend had started a day earlier... and did so every week. Sound good? That could be the new reality many companies, and employees, face in the future, after the four-day business week took another step towards implementation in the UK. This year, 17 businesses of different types have been trialling the move, organised by the 4 Day Week Foundation. Almost 1,000 different employees were involved, all retaining their normal salary - and after the culmination of that study, all 17 companies have opted to keep going, following an earlier trial which saw 5,000 people adopt the scheme permanently. Not all are doing straight four-day weeks. Some were trialling nine-day fortnights. And there are clearly some industries which would face challenges to alter schedules and regulations. But the initial results for those taking part appeared positive, with nearly two-thirds (62 per cent) of employees saying they noted a 'reduction in how often they experienced burnout'. Firms who took part in the trial have cited customer satisfaction and employee performance as reasons for continuing, with one expecting mass adoption across the next decade. Vicky Walker, chief people officer at Westfield Health, told The Independent that less work has shown to mean more output. 'Studies have shown that a shorter work week can lead to enhanced efficiency and focus among employees. People may be more motivated to complete their tasks and increase productivity. There's now a growing recognition for flexibility and work-life balance and this trial is a step in the right direction,' Ms Walker said. 'These flexible benefits can also help attract new talent and improve employee retention rates, which for some businesses, is a key business target.' There is, however, much more to consider for employers and the wider economy, before we see it adopted on a larger scale. 'Ultimately, what matters for the economy is hours worked multiplied by productivity per hour. If people are simply working their weekly hours in four days rather than five, then there should be little economic impact. If they are working harder - fewer hours for the same output - then we could see an uptick in productivity, but that wouldn't necessarily translate into growth because of fewer hours,' Thomas Pugh, chief economist at RSM UK, told The Independent. This is essentially the French model of the system, Mr Pugh explained - but success in it will naturally lead to businesses asking one pertinent question: 'If you can be as productive in four days as in five, then why not be as productive over the full five days?' There also remain questions over the potential for staggered days off - 'Do people want Wednesdays off, for example?' - and of course whether a firm's opening hours would still match up to those of their clients. It's not a 'solution to the UK's economic problems', Mr Pugh added. Andrew Timpson, a tax partner at RSM, pointed out there would be implications if reduced hours meant reduced pay - and not just for the Treasury. 'If base pay is reduced, there could be an impact on salary sacrifice and pensions,' he said, 'plus if employees are earning less then the tax take will reduce. Some employees may also find themselves slipping under thresholds which will change the personal allowance, or other aspects such as child benefit.' The discussion outside of finance has also taken centre stage. A study by BHN Extras recently concluded over a third of UK workers (34 per cent) said they would accept a pay cut for a shorter commute and only 23 per cent enjoyed their commute. The survey suggested both the cost and the sentiment was stronger in London, but would knocking a day off the commute into work be a big draw for all? It won't be one for everybody, nor every business. Implementation costs and rescheduling alone may make it prohibitive, while there's no doubt that some management may not appreciate the thought of it, let alone the reality. But just like working from home, hot-desking and zero-hours contracts before it, the four-day week looks set to play some part in the future of UK business - and like all those others, divide plenty of opinion on the way. For Ethan Noblesala, 32, a diagnosis of prediabetes and a blunt prediction from his doctor in November, 2023, motivated him to improve his health. "My doctor told me, 'If you don't change your lifestyle, you're going down a route that's leading to more surgeries, more operations and more medication,'" Noblesala tells TODAY. He says he was at his healthiest during college, but as he built his career as an entrepreneur, he neglected his health and focused on building his businesses. 'As I started that corporate lifestyle, I was slowly gaining weight,' he says. To foster business growth and connections, he was spending a lot of time at happy hours. He was lucky if he slept four hours a night. Uber Eats was the cornerstone of his diet. Exercise or even spending any time outside was practically non-existent. And his habits only got worse during COVID. His weight climbed to 235 pounds, which was a strain on his 5-foot, 6-inch frame. 'Even though I was running a successful business with my co-founder, I decided to take a leave to really focus on what was most important to me — my health,' he says. That attention to his health worked. He lost 85 pounds, and he says he feels 10 years younger. Here's how he did it. Noblesala has a background in finance, so he's motivated by numbers. He honed in on them to make changes, tracking his protein, fiber and calories. He discovered that when he ate mainly protein and fiber-dense meals, he felt satiated even when he was only eating 1,500 calories a day: 'Your body needs nutrition, but I never felt like I was starving myself or restricting myself.' He built on small goals and habit changes: Drinking a glass of water every day: 'I was 85 pounds heavier. I wasn't going to go out and run five miles, or try any of the crazy trends I see online. This was something I could do.' Shopping for groceries and checking the nutrition labels for protein, fiber and calories instead of eating out — and overeating — five times a week. Having a go-to, high-fiber, low-calorie meal that he liked to eat: Shirataki noodles with chicken breast in a low-fat buffalo sauce. Finding healthier swaps for foods he likes, such as Costco chicken nuggets instead of McDonald's: 'I found it exciting to make these new discoveries. It's something I love to do.' Noblesala grew up playing sports and loved how they kept him active while also being a way to connect with others. He'd often ask his friends if they wanted to play basketball, tennis or something else together. 'Through tennis, I discovered pickleball. That was the hook for me in terms of living a healthier and active lifestyle. I've become a pickleball fanatic,' he says. In December 2023, he joined Life Time. 'One thing I appreciate there is the community. I get to play pickleball, my favorite sport. Some people say going to the gym is a chore, but I'm excited to go." "There are endorphins you get from exercising, from being around others and from having that community," he says. "In the past, going to the gym was a solo grind, and I had to force myself to go. Now, it's not a chore. It's part of my lifestyle." Having friends at the gym and at pickleball builds accountability for him, too. 'My friends will say, 'Ethan, I'll see you tomorrow.' And, of course, then I go after work. They're expecting me,' he says. He has also joined some running clubs, such as Pitch & Run, and now he's working toward an ambitious goal — running the New York City marathon in November. He earned the opportunity to participate through his alma mater, Chaminade High School in Long Island. 'I'm nervous for the marathon because I know it's a daunting mental task. I'm focusing on the interval training and the strength training,' he says. Noblesala recognizes now how his lack of sleep was making his health worse: 'Your body needs to recover. I'm averaging six and a half to seven and a half hours of sleep a night now, and I definitely feel it when I don't get seven hours of sleep.' He's also careful about balancing his workouts with rest. He finds fitness data so valuable that he wears an Apple watch, a Whoop tracker and an Oura ring. After a day where he ran seven miles and played two hours of pickleball, he saw that his Whoop told him to take it lighter the next day. 'You can make these informed decisions with technology. It was harder to do this 10 years ago,' he says. Now that Noblesala is 85 pounds lighter and his life is centered around healthy living, he has no intention of returning to his old habits.
