
How Massive Medicaid Cuts Will Harm People's Health
On July 3 the House of Representatives will vote on the Trump administration's new budget bill, which incorporates massive cuts to Medicaid, the state-federal health insurance program that serves more than 70 million low-income people. The bill, which passed the Senate on July 1, would cut $930 billion from Medicaid, Medicare and Affordable Care Act funding combined over 10 years, with more than 11 million people losing coverage by 2034. Experts have calculated that, taken together, the cuts will lead to more than 51,000 additional deaths per year by decreasing people's access to health care.
Experts say the evidence shows that gutting Medicaid will have dramatic effects on health far beyond people enrolled today—some of whom may not even realize they use Medicaid because the program goes by different names in different states. Even those with private insurance will be affected.

'When you slash $1 trillion from the health care system, you can't expect it not to have far-reaching harms,' says Megan Cole Brahim, a health policy researcher at Boston University. 'It will really affect everyone, including people who aren't enrolled in Medicaid.'
The bill includes two main Medicaid-related provisions. One increases the requirements people must meet to qualify for and remain on Medicaid: This would drive down the total number of people who receive benefits, Cole Brahim says, leaving more people without coverage. The second provision reduces the amount of money the federal government sends to the states to fund Medicaid coverage. This will cause great variability in how different states handle the cuts, she notes, because each state will have authority to make its own choices about whether to try to scrounge up the funding from other sources to close the gap and maintain Medicaid access.
Medicaid's Overall Health Impacts
Cuts to Medicaid at the scale proposed in the House and Senate bills are unprecedented, Cole Brahim says, but scientists still have plenty of data to work from to predict the effects of such a massive cut. Researchers have tracked differences in health outcomes in states where Medicaid was expanded, particularly after the passage of the 2010 Affordable Care Act. That legislation gave states the option of extending Medicaid coverage to more people, up to those with an income of 138 percent of the federal poverty level, with federal support. So far, 40 states and Washington, D.C., have opted for expansion—and researchers have monitored health outcomes over time in those states compared with states that did not.
'Medicaid expansion was really a natural experiment,' says Brian Lee, a transplant hepatologist at the University of Southern California, who was co-author of a 2022 study in the Lancet that evaluated death rates in conjunction with Medicaid expansion across the U.S. When Medicaid coverage was offered to more people, overall death rates fell by nearly 12 per 100,000 adults per year on average. Where states were home to more women or more Black people, the decline in death rate was larger.
And Lee notes that the 2022 findings are just the bird's-eye view of the way Medicaid access shapes people's health. For example, studies have found that more people get diagnosed with chronic conditions such as diabetes and cardiovascular disease in Medicaid expansion states. Earlier diagnosis permits earlier treatment, which, logically, would reduce mortality rates over the course of decades.
'Medicaid expansion, in relative terms, is pretty new,' Lee says; the earliest states began implementing the program in 2014. 'A lot of people think that, actually, the best benefits are to come' —or at least, they thought that they were to come.
Medicaid for Specific Needs
The new policy changes won't just affect the people who lose access to Medicaid in the short term. 'This isn't a small program for a fraction of our population,' Cole Brahim says. 'It covers about one in five people, and the majority of people in the United States will have had Medicaid at some point in their life.'
One key area in which Medicaid is crucial is older adult health services and other long-term support, including both home-based care and residential facilities, says Jasmine Travers Altizer, a researcher at New York University who studies aging. Two out of three people in the U.S. will require some form of long-term health services at some point in their life, she says.
Even after people reach the age of 65, Medicare—a companion federally run insurance program for older adults —only covers nursing home and daily home health care services for 100 days, she notes. Many people don't have independent long-term care coverage, which can cost thousands of dollars a year in premiums, leaving them with no real alternative to Medicaid. And when people can't access proper medical care, they often need to rely on relatives for support, including full-time care—which comes with its own economic consequences for families and society at large.
Our youngest populations also depend on Medicaid, Cole Brahim says. More than two in five births are paid for by the service —a proportion that rises to more than half for Black and Hispanic births.
And although proponents of the Trump administration's Medicaid cuts say that these changes wouldn't affect pregnant people and kids, Cole Brahim notes that they would absolutely prevent some people from having coverage before pregnancy. 'Making sure people are connected to care before they become pregnant is really critical for maximizing health outcomes, both for the mom and the baby,' she says.
Medicaid Cuts Lead to Provider Shortages
All three experts emphasize a universal risk to Medicaid cuts: the reduction of health care facilities and personnel. Federal Medicaid funds are used to directly cover care of individuals on Medicaid, but this money indirectly keeps practitioners' and hospitals' doors open.
The steep cuts to federal funding will ultimately mean doctors and hospitals have more trouble making ends meet. As facilities begin to close, people in affected communities—regardless of their insurance provider—will face longer wait times for appointments and longer travel times to facilities that are still in operation. People in more rural communities, which are already underserved, may lose care access entirely, even while remaining insured.
Cole Brahim is particularly worried that obstetric and pediatric care will see more closures, noting that these departments are often less profitable because insurance providers already reimburse these services at lower rates, she says.
Travers Altizer is also concerned about the cuts' effects on nursing homes, which are in crisis as well. In one recent survey of nursing home providers, 27 percent said they would have to close their facilities if Medicaid cuts occurred. Even more—58 percent—said they would need to reduce current staffing; 44 percent they would slow new hires. For Travers Altizer, those possibilities mark a return to the early days of COVID, when staffing shortages left some nursing home residents unable to get out of bed or otherwise meet basic needs.
She also notes that people whose lose Medicaid coverage because of cuts will still need to seek care; they'll just do it in different ways—ways that are ultimately more expensive overall. Rather than primary care visits, people will lean on emergency rooms. Without skilled medical support, people will rely on friends and family. Without federal money, people will continue turning to online fundraising platforms such as GoFundMe at ever increasing rates.
'There's this big idea that we need to cut Medicaid [because] we need to save money, and Medicaid is this big federal government program,' Travers Altizer says. 'But taking away this support won't save money; it's going to shift costs.'

The passage of the 'Big Beautiful Bill' on Thursday was a moment when President Donald Trump's hold on the Republican Party was tested — and he won. GOP holdouts objected to the trillions of dollars the bill is projected to add to the debt and the cuts to Medicaid and other safety net programs. But in the end, Trump was able to browbeat Republicans into supporting his costly plans to fund long-term tax cuts for the wealthy, more deportations of immigrants and boosting defense spending. Trump's victory further strengthens his power in Washington and clears the way for him to take even more aggressive actions on immigration, trade policy and depart from American tradition to hammer the chair of the Federal Reserve Jerome Powell into lowering interest rates, a move that could give the U.S. economy a sugar high of cheap cash. Trump's control over the GOP 'is as close to total as any President has ever had over his own political party,' says Whit Ayres, a long-time Republican pollster. 'It gives him the support of his own legislative caucus for almost anything he wants to do.' Trump plans to sign the bill into law on Friday at 5 p.m. as B-2 bombers jet over the White House during a Fourth of July celebration for military families. 'We'll be signing with those beautiful planes flying right over our heads,' Trump told reporters traveling with him on Air Force One on Thursday evening. In order to get the bill passed, some Republicans broke their promises not to cut Medicaid, other Republicans reneged on pledges to keep down the national debt. In the House, only two Republicans—Rep. Thomas Massie of Kentucky and Rep. Brian Fitzpatrick of Pennsylvania—ended up voting against Trump's bill. The bill narrowly passed the House 218-214 with all Democrats voting against it. Massie wrote on X that he voted 'no' because 'it will significantly increase U.S. budget deficits in the near term, negatively impacting all Americans through sustained inflation and high interest rates.' The bill barely made it out of the Senate earlier in the week. Republican Senators Susan Collins of Maine, Rand Paul of Kentucky and Thom Tillis of North Carolina voted against Trump's bill forcing Vice President J.D. Vance to cast a tie-breaking vote. Tillis said the Medicaid cuts would be devastating to his state, and, after bucking Trump, said he wouldn't run for reelection. Republican Senator Josh Hawley of Missouri had argued against the cuts to Medicaid but ended up voting 'yes,' and Senator Ron Johnson of Wisconsin, a Republican, had criticized the bill for not cutting enough to offset adding trillions to the debt, but ended up voting for the bill. Now Trump is faced with the daunting task of selling the unpopular bill to the public. Only 29% of voters said they support the bill, according to a Quinnipiac University poll published on June 26, while 55% percent of voters said that they oppose the bill and 16% were unsure. The cuts to social safety net programs and the increase in the deficit could lead to political blowback that helps Democrats take back control of the House in the 2026 mid-term elections. The bill could be a political risk for Trump, says Ayres, if 'a whole lot of Trump supporters lose their health care and get angry about it.'

How Scold-and-Fold Became the GOP's Calling Card
At 6:50 PM on Wednesday, conservative hardliner Rep. Keith Self (R-Texas) tweeted that the Senate's version of the Republican megabill was 'morally and fiscally bankrupt.' At 3:20 AM Thursday morning, Self nevertheless voted to advance that very bill. Rep. Chip Roy (R-Texas) likewise slammed the Senate amendments as a 'travesty' that undermined 'the good things' in the House bill. He, too, joined Self in finally supporting the legislation in the wee hours Thursday morning. Both held out longer than their moderate colleague Rep. David Valadao (R-Calif.), who said in a post on X last week that he could not vote for any Senate changes to the Medicaid provisions in the House version of the bill. These reversals may be surprising, but they were not remarkable. It was par for the course for congressional Republicans who, in recent years, have shown a proclivity for taking bold, theatrical stands before meekly capitulating in the face of political pressure — particularly from President Donald Trump. In just the past six months, hardline conservatives have expressed concerns about backing a continuing resolution, two different budget resolutions and the initial House version of the Republican megabill before caving in to pressure from Trump. In May, conservatives originally threatened to withhold their vote for the 'One Big Beautiful Bill' because it provided a tax deduction targeted at residents of blue states. The head of the House Freedom Caucus, Rep. Andy Harris (R-Md.), even insisted it might take weeks to appease hard-right concerns on May 21. It passed early in the morning the next day after hard right conservatives yielded overnight. It was no different this week when they loudly and repeatedly expressed horror and dismay at the version of the 'One Big Beautiful Bill' passed by the Senate. There was outrage that it increased the deficit. Complaints that it didn't do enough to roll back efforts to encourage clean energy were abundant. Conservatives wanted changes to the bill to address these concerns. The problem is that it would have required the bill to be sent back to the Senate, either through legislative 'ping pong' or through the traditional expedient of a conference committee. That would have been time consuming — and prevented Republicans from passing the legislation by Trump's self-imposed July 4 deadline. Republicans had until the end of the year to do the most urgent part of the legislation, the extension of the 2017 tax cuts, but the decision was made to treat Trump's deadline as a firm time limit in the same way Congress might handle the looming debt crisis or the expiration of government funding. On Wednesday morning, Rep. Tim Burchett (R-Tenn.), who railed against the imposition of 'arbitrary deadlines,' told POLITICO those kinds of deadlines led to 'panicking' and it wasn't going to work. Needless to say, Burchett eventually voted for the bill, too. The truth is that there really wasn't any other option but to stick to an arbitrary deadline, because it's what Trump wanted. Steve Scalise, the number two House Republican, bluntly explained that extending the deadline to make last-minute alterations wasn't going to happen because Trump didn't want it. 'It became clear from the president's meeting at the White House to further conversations later, that there's going to be no more amendments to the bill,' he said. Republican Rep. Troy Nehls (R-Texas) analogized the situation to a restive football team that occasionally forgets who is calling the plays and needs a reminder. 'You may be a good receiver, you may be an all star running back, but the quarterback is still Donald J. Trump.' When it was all over, the holdouts justified the Big Fold in predictable fashion, claiming to have won valuable concessions even if none of those would be enacted into law. Burchett, one of the holdouts who eventually capitulated, said that he had 'a very good conversation with President Trump and Vice President Vance.' That discussion gave him confidence that the administration would 'go after fraud and get rid of a lot of the nonsense in our energy policies.' Harris, another holdout, told reporters, 'we came to significant agreements with the administration overnight on executive actions that will make America great again.' Rep. Lloyd Smucker (R-Pa.), who long expressed concern about the deficit implications of the legislation — which by a nonpartisan Congressional Budget Office estimate will add at least $3.3 trillion to the national debt over the next decade — the process represented a combination of policy negotiation and airing of grievances. 'It was a little bit of hearing people out,' said the ardent fiscal hawk. But there were also policy issues as 'everyone became convinced it just didn't make sense to send it back to the Senate.' A more jaundiced and skeptical view was taken by Rep. William Timmons (R-S.C.), who described the dramatic scold-and-fold as a venting exercise by the usual suspects who always 'cry wolf.' 'There's just a couple people that really want to talk a lot about things that are important to them, and they use these pressure points as opportunities to force people to listen to them,' said the four-term incumbent. 'I don't think you accomplish much, but we got it done.' In the final hours before the House narrowly passed the sweeping legislation, despite all the apocalyptic rhetoric about the bill's drawback, all was forgotten and forgiven. The group of conservative holdouts, including Self and Roy, gathered in the center aisle of the House at about 3:15 in the morning. The dozen hardliners, who did not want to support the Senate bill as written, joined together for a smiling group picture just before they all cast their votes to help make that text the law of land. The photographer, using a cell phone camera, was Speaker Mike Johnson himself. 'He just pulled out his camera and we took the picture,' said Rep. Bob Onder (R-Mo.), one of the hardliners who yielded. 'It was awesome.' The Republican holdouts had ended their fight by getting together to strike a pose. It wasn't difficult. After all, they had been posing all week.

Prospects for peace across the Middle East are now the brightest they've been in over a century. Meanwhile, the Supreme Court backed up the president on multiple key fronts, including his deportation powers — and most crucially by making it clear that activist lower-court federal judges can't routinely impose national injunctions on the executive branch.
