Common Bacteria Could Cause 12 Million Stomach Cancer Cases: What to Know

Researchers say about three-quarters of gastric cancer cases worldwide are caused by the common bacterium Helicobacter pylori.
H. pylori infection doesn't always produce symptoms, so it's important to get screened for any persistent gastric-related pain, such as indigestion and bloating.
You can lower your risk of gastric cancer by eating a healthy diet, exercising regularly, and getting tested for H. pylori if you have any symptoms that may indicate infection.
The vast majority of stomach cancer cases have been linked to a common bacteria that often doesn't produce any symptoms.
A new study published in the journal Nature Medicine reports that 76% of stomach cancers worldwide may be attributed to the Helicobacter pylori (H. pylori) bacteria.
The scientists found that nearly 16 million people born between 2008 and 2017 are expected to develop gastric cancer sometime during their lifetime. Of those cases, around 12 million will be caused by H. pylori bacteria.
They noted that about 8 million of those bacteria-related cases are expected to occur in Asia. Another 1.5 million are predicted for North America and South America.
The researchers said their findings may help explain the rise in gastric-related cancers in younger people in recent years.
They also noted that a vaccine to prevent H. pylori bacteria 'would be a powerful tool for gastric cancer prevention,' but there appears to be 'little momentum at present to advance its development.'
Experts not involved in the study told Healthline the findings are important and should be noted by medical professionals.
'This is truly a fascinating, practice-changing study,' said Anton Bilchik, MD, a surgical oncologist as well as the chief of medicine and director of the Gastrointestinal and Hepatobiliary Program at Providence Saint John's Cancer Institute in Santa Monica, CA. 'This tells us that … it is a preventable cancer.'
'We have known that H. pylori infection increases an individual's risk of developing stomach cancer for many years,' added Joo Ha Hwang, MD, a professor of medicine at Stanford University.
'This study quantifies the estimated number of deaths that would likely occur if we do not start screening individuals who are at moderate to high risk for H. pylori infection and gastric cancer. This study is a call to action to address a preventable cancer,' Hwang noted.
H. pylori bacteria driving stomach cancer rates
Alan Venook, MD, a professor of medical oncology and translational research at the University of California San Francisco, and program development director at the Helen Diller Family Comprehensive Cancer Center, said the connection between the H. pylori bacteria and gastric cancer has been known for more than 30 years.
He cited older research from 1991 published in the New England Journal of Medicine that first made the connection, and said the findings in the new study aren't surprising. Still, he expressed concern over the persistent nature of the bacteria's connection to cancer.
'The most interesting thing in this study to me is that this is still a problem,' Venook said, adding he is 'mystified' as to why progress hasn't been made. 'I think the message to medical professionals is that this remains a terrible problem,' he noted. 'This is a wake-up call.'
Bilchik, who had attended the 1991 presentation of the original study, said the research was initially dismissed by the medical community. Since then, he said those early findings have proven to be correct.
'This is a clear message to both patients and professionals that stomach cancer is not necessarily caused by smoking and alcohol,' he said.
Nilesh Vora, MD, a hematologist and medical oncologist and medical director of the MemorialCare Todd Cancer Institute at Long Beach Medical Center in California, has some cautions about the new research.
He called it 'thought-provoking,' but added that more research is necessary to confirm the data and the conclusions.
What to know about H. pylori bacteria
The Helicobacter pylori bacteria grow in the digestive tract and tend to attack the stomach lining. It has adapted itself to live in the highly acidic world of the human gastrointestinal system.
H. pylori often develops in a person's stomach during childhood. The bacteria are typically harmless and don't cause symptoms. However, in some cases, they may lead to stomach ulcers and some diseases, including gastric cancer.
Many people with H. pylori don't have symptoms. However, an estimated 30% of people with the bacteria develop peptic ulcers and other conditions.
Some common symptoms from the bacteria include:
burning stomach pain
feeling full quickly
nausea
burping
unexplained weight loss
The bacteria can be transmitted via saliva, contaminated food, and other methods. It can be diagnosed with tests involving breath, stool samples, and blood antibodies. Doctors sometimes order an endoscopy, which involves placing a tube down a person's throat.
The bacteria are treated with antibiotics. Some people may be prescribed proton pump inhibitors to help the stomach heal.
The American Cancer Society (ACS) notes there are currently no recommendations for routine stomach cancer screenings for those with average risk.
'Given the relatively low prevalence of gastric cancer in the United States, there is currently no screening recommendation for H. pylori in patients without symptoms,' said Anu Agrawal, MD, vice president of Global Cancer Support at the American Cancer Society.
'The high incidence in Asia particularly could justify screening protocols which, as noted in the publication, have been initiated in a few Asian countries,' he told Healthline.
Bilchik said that people should not ignore gastrointestinal symptoms that linger for an extended period of time. 'If the symptoms are persistent, then go to a physician and be proactive,' Bilchik advised.
Vora added that while screenings are important, there are some logistical issues. Endoscopies, he said, are invasive and expensive. He added that the big questions are whether general screenings should be done for this bacterium and whether everyone with long-term gastrointestinal symptoms should be tested for H. pylori.
Hwang agreed that screening should be selective.
' H. pylori often infects family members, so if one family member is diagnosed with H. pylori, family members who live together should be tested for the infection,' he said.
'We have been advocating for individuals who have immigrated from regions that have a high incidence of H pylori infection and gastric cancer, such as East Asia, Eastern Europe, and the west coast of South America, to be tested for H pylori infection and treated if positive.'
What to know about stomach cancer
The National Cancer Institute (NCI) estimates there will be about 30,000 new cases of stomach cancer in the United States this year. More than 10,000 people are expected to die from the disease.
Those numbers represent less than 2% of all cancer cases and all cancer deaths in the United States.
Almost 38% of people in the United States with stomach cancer are expected to live at least five years after diagnosis.
Stomach cancer usually develops over a number of years. Many times, there are no symptoms until the cancer has spread to other parts of the body.
The disease is more common in adults over the age of 60 and in those who smoke, have obesity, or have a family history of the disease. It's also more common in people of Asian descent and people with a diet high in salty foods, meat, and processed products.
Symptoms of stomach cancer include:
frequent heartburn
loss of appetite
bloating or burping
indigestion
abdominal pain
The disease can be treated with chemotherapy, radiation, surgery, and immunotherapy.
Experts say you can lower your risk of stomach cancer by:
eating a healthy diet
exercising regularly
quitting smoking if you smoke
limiting or avoiding alcohol
treating H. pylori infection if you receive a diagnosis
The American Cancer Society has a list of ways people can lower their risk.
'Obesity, tobacco use, alcohol, diets high in smoked foods or foods preserved by salting, or diets low in fruits and vegetables are risk factors,' said Agrawal. 'If you have a family history of stomach cancer, are a first-generation immigrant from a high prevalence area (e.g., East Asia), or have certain genetic cancer predisposition syndromes, you should be screened for stomach cancer.'

For many of us, gluten is a dietary villain, capable of causing all sorts of gastrointestinal troubles. Research out this week looks to complicate that narrative, however, finding that people are sometimes wrongly blaming gluten for triggering their symptoms of irritable bowel syndrome. Scientists at McMaster University in Canada conducted the study, a small, randomized trial of IBS patients. People became sicker just as often after eating food containing gluten or wheat as they did after eating food without the ingredient, they found. Gluten is likely safer to eat for those with IBS than commonly thought, the researchers say. 'These findings suggest that expectations played a major role in symptom generation and that only some of these patients could benefit from gluten or wheat restriction,' they wrote in their paper, published Monday in The Lancet Gastroenetrology and Hepatology. IBS is a complex, chronic, and relatively common condition, estimated to affect between 5 and 10% of the population worldwide (including up to 45 million people in the U.S.). Its symptoms include stomach pain, bloating, and diarrhea or constipation. Unlike the closely named inflammatory bowel disorder, IBS isn't marked by physical changes or visible damage along the digestive tract, but it can still be a debilitating burden for many, with around 20% of sufferers experiencing severe bouts. The exact causes of IBS remain unclear, though scientists have speculated it can arise from a miscommunication between the nerves found in the gut and the brain. People with IBS tend to report having specific triggers that can cause a flare-up of illness, gluten included, and often manage their condition by avoiding these triggers. The researchers behind the new study aimed to better understand gluten's potential role in IBS, so they recruited over two dozen people with IBS for their randomized, double-blinded trial, all of whom had reported improving after switching to a gluten-free diet. At first, the volunteers were randomly assigned to one of three groups. All the groups were first told to eat cereal bars for a week. One group ate bars made with flour containing gluten, the second ate bars made with wheat flour, and the third ate bars made with gluten- and wheat-free flour. Afterward, the volunteers took a break for two weeks. Then they switched to eating the bars they hadn't eaten yet for another round of testing, a process repeated twice. By the study's end, all the volunteers had eaten all three kinds of bars, though on different schedules and without knowing which bars they had eaten on a given week. A total of 28 people completed the trial. A significant percentage of people reported having worsening IBS symptoms after eating gluten or wheat bars, the study showed, but about just as often as they did after eating non-gluten/wheat bars (roughly a third experienced worsening IBS during each of the three conditions). 93% of participants also reported having adverse events after each scenario of bar-eating. Interestingly enough, tests of the participants' stool found that only a third strictly followed their diets as instructed, with likely many fearing they would get sick. The findings are based on a very small sample size, so it's not yet certain that they apply to the general population of people with IBS tied to gluten. There are also, of course, digestive conditions clearly caused by an intolerance to gluten, particularly celiac disease. And the researchers aren't saying gluten can't sometimes be a genuine trigger for people's IBS. But they argue that in many cases, people's negative perception of gluten is causing a nocebo effect, the dark cousin of the placebo effect. In other words, someone's belief that gluten is bad for them can potentially spark or worsen the IBS symptoms seen after eating it, rather than gluten itself. The researchers say better communication and follow-up care from doctors are needed for IBS patients, given the findings. 'What we need to improve in our clinical management of these patients is to work with them further, not just tell them that gluten is not the trigger and move on. Many of them may benefit from psychological support and guidance to help destigmatize gluten and wheat and reintroduce them safely in their diet,' said senior author Premysl Bercik, a professor at McMaster's Department of Medicine, in a statement from the university. That may be easier said than done, though. The researchers note that most patients, upon learning the team's findings, staunchly refused to entertain the idea of gluten not being a trigger for their IBS symptoms. So it looks like gluten may need a promotional campaign to win back its reputation.

For decades, Coke and the makers of other soft drinks have generally used high-fructose corn syrup or artificial sweeteners in their products manufactured in the U.S. But American consumers are increasingly looking for food and drinks with fewer and more natural ingredients, and beverage companies are responding. PepsiCo and Dr Pepper have sold versions of their flagship sodas sweetened with cane sugar since 2009. Coca-Cola has sold Mexican Coke — which uses cane sugar — in the U.S. since 2005, but it's positioned a trendy alternative and sold in glass bottles. Coke with cane sugar will likely be more widely available. Here are some frequently asked questions about the sweeteners in U.S. sodas: What's the difference between cane sugar and high-fructose corn syrup? Many consumers know that consuming too many sweets can negatively affect their health, but soda drinkers sometimes debate if either cane sugar or high-fructose corn syrup is better (or worse) than the other. The short answer is that it doesn't make a difference, said Marion Nestle, one of the nation's top nutrition experts and professor emeritus at New York University. High-fructose corn syrup is made of the simple sugars glucose and fructose in liquid form. Cane sugar, also known as sucrose, is made of glucose and fructose bonded, but quickly split, Nestle explained. Both are still sugars, with about the same amount of calories. Whether a can of Coca-Cola contains one or the other, it will still be a sugary drink with about the same amount of calories and the same potential to increase well-documented health problems from obesity and diabetes to tooth decay. Why did soda companies switch from using sugar to high-fructose corn syrup? High-fructose corn syrup costs less. According to price data from the U.S. Department of Agriculture, the wholesale price of HFCS-55, the type of corn syrup most commonly used in beverages, averaged 49.4 cents per pound last year. The average wholesale price of refined cane sugar was 60.1 cents per pound, while the average wholesale price of refined beet sugar was 51.7 cents per pound. But high-fructose corn syrup has advantages beyond price. According to a 2008 paper in the American Journal of Clinical Nutrition, high-fructose corn syrup is more stable than sugar when added to acidic beverages, and it can be pumped directly from delivery trucks into storage and mixing tanks. Why is high-fructose corn syrup less expensive that sugar? Tariffs are one reason. The U.S. has had barriers on sugar imports almost back to its founding; the first went into place in 1789, according to the Cato Institute, a think tank that advocates free markets. Since the passage of the Farm Bill in 1981, the U.S. has had a system in place that raises duties on sugar once a certain amount has been imported. The U.S. also has domestic production controls that limit supplies, keeping prices higher. But high-fructose corn syrup is also cheaper because of the federal government's billions of dollars in subsidies for corn farmers. Loans, direct payments, insurance premium subsidies and surplus crop purchases all lower farmers' costs – and the price of the corn they grow. Are sugar replacements used in diet sodas safe? While cutting back on added sugars has documented benefits, replacing them with artificial sweeteners is complicated, too. Coca-Cola Zero Sugar, introduced in 2017, uses the artificial sweetener aspartame and the natural sweetener stevia in its recipe. But research suggests that aspartame may be linked to cancer. In 2023, a committee for the World Health Organization determined that aspartame should be categorized 'as possibly carcinogenic to humans.' While that doesn't mean that diet soda causes cancer, the scientific committee concluded that there may be a possible link between aspartame and liver cancer, and that the issue should be studied further. The U.S. Food and Drug administration disagreed with the WHO panel, citing 'significant shortcomings' in the research that backed the conclusion. FDA officials noted that aspartame is one of the most studied food additives and said 'FDA scientists do not have safety concerns' when it is used under approved conditions. How President Kennedy's Vision Drove Apollo 11's Triumph

Nations, and organizations, can accomplish great things when guided by clarity of purpose. On July 24, the country celebrates the successful conclusion of the Apollo 11 lunar mission; perhaps the most significant technological triumph in American history. You don't need to be a senior citizen to recognize its relevance. The Apollo 11 triumph reflects the exceptional contributions of a disparate group of scientists, engineers, technicians, astronauts and test pilots. But it was very much born of the bold, clear vision of a young president, and of the extraordinary national commitment that vision generated. And there's a lesson in that for today's organizational leaders. Early in the John F. Kennedy presidency, the country's global prestige had been buffeted by the unanticipated achievements of the Soviet space program, which far outpaced similar efforts of the United States. Soviet spacecrafts were orbiting the earth while American rockets were exploding on the launch pad. The 'space gap' became an unexpected yet powerful weapon in the Cold War. In turn, President Kennedy viewed national leadership in space as critical to preserving the country's world leadership position. This, he felt, could be achieved by pursuing aggressive goals for space exploration. To that end, in a bold address to a joint session of Congress on May 25, 1961, he called for a national commitment 'to achieving the goal, before this decade is out, of landing a man on the moon and returning him safely to the earth.' (Note Kennedy's reference to both 'landing' and 'returning'). In this call before Congress, he pulled no punches, articulating both the promise and the 'heavy burden' associated with his challenge. No single space project in this period will be more impressive to mankind, or more important for the long-range exploration of space; and none will be so difficult or expensive to accomplish. Nations, and organizations, can accomplish great things when guided by clarity of purpose. Kennedy further galvanized public support for this vision with his famous September 12, 1962 speech at Rice University in Houston: We choose to go to the Moon. We choose to go to the Moon in this decade and do the other things, not because they are easy, but because they are hard, because that goal will serve to organize and measure the best of our energies and skills, because that challenge is one that we are willing to accept, one we are unwilling to postpone, and one which we intend to win, and the others, too. Of course, Kennedy did not live to see the success of Apollo 11. Yet his simple, clear and stirring vision for the nation was strong enough to sustain the public commitment and support the scientific effort through the challenges and tragedies of the intervening years. To address the continuing technical barriers of space flight. To confront the loss of confidence from a great tragedy ‒ the Apollo 1 fire. To overcome the significant distractions of an unpopular foreign war and growing national racial strife. Indeed, Apollo 11 returned safely to earth on July 24, 1969; 'in this decade', just as Kennedy had called for. Simply amazing. All of this is a reminder that words do indeed matter. That a thoughtful purpose can breed meaningful commitment. And that a clearly articulated vision can motivate an organization, just as a nation, to pursue and achieve noble goals. With that, the elements of vision-making become more apparent: it's the ability of leadership to succinctly express where the organization needs to go in the future, the reasons why it needs to go in that direction, how it expects to reach the specific destination, the plan for the organization when it gets there, and why the effort will ultimately be worth it. Easier said than done, of course. But President Kennedy's skill in formulating, presenting and advocating for his space exploration goals made it easier for the public to understand and support them. More broadly, it helped them become more acquainted with the young president, and the principles for which he stood. That's the force of an effective vision; a force should be attractive to all leaders ‒ no matter the industry sector.
