Province, nurses must take charge after study finds racism exists in Winnipeg ERs

If you want to know why we never seem to make progress in battling systemic racism, events this week provide some compelling answers.
On Tuesday, a landmark study by Shared Health showed adult African/Black and Indigenous patients wait longer for treatment in Winnipeg emergency rooms than patients of other races and ethnicities.
Black patients waited the longest on average at Shared Health-run Health Sciences Centre (5.5 hours) and Indigenous patients waited the second longest (5.3 hours). White patients, as a point of comparison, on average waited 4.1 hours, a full hour less.

As well, 36.5 per cent of adult Indigenous patients, and 30.4 per cent of African/Black patients left the HSC ER without having been examined or treated. About 20 per cent of white patients left before being seen.
The report was prepared by Shared Health in collaboration with the University of Manitoba's Ongomiizwin Indigenous Institute of Health and Healing, and the George & Fay Yee Centre for Healthcare Innovation. It was based on analysis of more than 600,000 patients who agreed to self-identify their race and ethnicity.
The findings are not necessarily seismic in their essence; research into the racial bias in the health-care system has been going on for decades. This is, however, one of the most precise studies of its kind.
Report author Dr. Marcia Anderson correctly noted the report contains 'hard truths.' Although not mentioned in the report, among those hard truths is the undeniable fact that the health-care system is horrible at responding to the issue of systemic racism.
Health Minister Uzoma Asagwara greeted the report with a recommendation that all emergency department staff undergo cultural safety and anti-racism training. However, the minister declined to say which training, when it would start and did not indicate whether the affected personnel would be allowed to undertake it during normal working hours.
That drew immediate criticism from the Manitoba Nurses Union.
MNU president Darlene Jackson said the collective agreement clearly says that nurses must be given time off from normal duties to undergo any required educational programming. Jackson warned that because Asagwara's comments were only a recommendation, there is an implication the nurses should do the training on their own time. She said she was concerned about consequences for those who don't.
If we take a hard look at what both Asagwara and Jackson are saying, a rather unflattering picture comes into focus.
If the minister takes no issue with the findings, and agrees that the issue is fundamentally important, then why wasn't more done to formulate an actual plan to address gaps in anti-racism training? And why wouldn't Asagwara make this kind of training mandatory, even if the fine details of delivering it have yet to be worked out?
It should be said that 'how' and 'when' and 'paid for by whom' are all good questions. However, Jackson and the MNU need to ensure their concerns do not obscure their commitment to combating systemic racism. Right now, the union has created the impression it is only interested in building a list of reasons why they shouldn't get involved.

Overall, it's a bad look for a health-care system that had the courage to, at the very least, dig into patient data to uncover the hard truths. But the mad dash for details, and lack of any sense of collaboration between the province, the health authorities and groups like the MNU, makes everyone look rather incompetent.
There is also a renewed concern about whether this is just another provincial directive unleashed upon nurses without adequate negotiation.
The former Progressive Conservative government rattled the very foundations of Winnipeg's hospital system by force-feeding nurses a major restructuring of emergency rooms and specialty surgical programs.
Apparently unaware, or unconcerned, that nurses had fought long and hard to have some control over where and when they worked, the PC government changed the entire structure of Winnipeg's hospital network. Nurses that had worked in departments and programs in hospitals for years were suddenly told they were relocating.

Nurses at suburban hospitals were suddenly faced with the prospect of moving to one of the larger, core-area hospitals. Carefully structured family lives were thrown into chaos.
Not surprising, then, that the nurses refused to get with the Tory program. Older nurses retired early; younger nurses fled the public system for private nursing agencies where they were paid better and had total control over where and when they worked. The same agencies that provide staff back to an understaffed public system at a premium rate.
Premier Wab Kinew and his health minister have repeatedly promised they would manage health care differently. Although more details need to come out, the haphazard response to the issue of systemic racism and the conflict with the MNU suggest the New Democrats may be repeating tragic history.
The province and its unions have acknowledged that this is a real and important issue. What Manitobans need now is someone to take charge and show real progress, not just acknowledgement.

Manitoba is 'forging ahead' with plans to establish its first supervised consumption site, in Winnipeg, this year, says the minister leading the charge. 'This is something that's still top of mind and something we're forging ahead on,' Housing, Addictions and Homelessness Minister Bernadette Smith said Monday. The NDP campaigned on a promise to establish a supervised consumption site in the 2023 election because the Tory government at the time refused to consider it. 'This is very personal for me,' the minister said in an interview. 'I've met with far too many loved ones who've lost someone to an overdose and this is about getting it right.' The province hasn't yet 'landed' on a proposed site but is 'moving toward' on having a site up and running by year's end, Smith said. On July 12, 2024, the Manitoba government announced $727,000 to support the development of an Indigenous-led supervised consumption site in partnership with the Aboriginal Health and Wellness Centre of Winnipeg. The Indigenous organization applied in November to Health Canada for a licence to operate the harm-reduction service in Point Douglas; its staff would be trained to respond to accidental overdoses and other emergencies, and help patients access health and social services. In April, community members at a public consultation meeting raised concerns about a proposed site at 200 Disraeli Fwy., and how it could affect the safety of their neighbourhood. 'We're listening to everyone that's coming to the table with ideas, whether that's for or against. The NDP government's spring budget includes $1.3 million for Shared Health to contract with the Aboriginal Health and Wellness Centre to operate the site. It would be the first Indigenous-led facility of its kind in Canada, the province says. Each proposed supervised consumption site requires an exemption under section 56.1 of the federal Controlled Drugs and Substances Act, Health Canada says. A completed application must include key information related to policies and procedures, personnel, local conditions, community consultations and finances. Health Canada lists the Winnipeg application as being in the 'review' stage and 'awaiting key information before decision can be taken.' Union sounds alarm over gaps in ambulance service in Thompson

The head of a Manitoba firefighters union says communication has broken down between the union and the city of Thompson, and he believes that is keeping ambulances off the streets, and putting public safety and lives at risk. "Public safety should be the number 1 concern of everybody, it should be the number 1 concern of the mayor, the number 1 concern of council," Thompson Professional Firefighters Association President Eric Campbell said. On June 29, the union posted on its Facebook page that Thompson Fire & Emergency Services would not be able to staff either of its two ambulances between 8 a.m. and 6 p.m., due to what they said was a "critical" staffing situation. In a second post on July 3, the union once again said the department would not be able to run its two ambulances, and that only one ambulance would be available that day between 8 a.m. and 6 p.m. The union said Shared Health EMS did have one ambulance staffed in Thompson during those days, but added they do not believe that's enough to adequately serve the city. "It is not a matter of if tragedy strikes as a result of this, it's a matter of when," the union posted. "For a city with as high of a demand for EMS services as Thompson, one staffed ambulance during this time is nowhere near adequate." Ambulance services moved to Shared Health According to Campbell, issues with ambulance services in Thompson have been brewing for years, and currently the department only has 10 of the department's 24 jobs filled. "The service is in need of a gigantic overhaul, and we've been preaching that, saying that this model is not sustainable," he said. "The problem that we face in Thompson is that wages are significantly lower than any other service in the province that provides the same kind of service. It makes it really tough to recruit people and retain people." But he said those issues have been amplified since last fall, when the city announced it would be handing over ambulance services in the city to Shared Health in February, after provincial underfunding and staffing shortages stretched resources. As of Feb. 1, ambulance services in Thompson have technically been a Shared Health responsibility, but Campbell said Shared Health only has one available ambulance in Thompson, so local fire department still runs its two ambulances in a city that has one of the highest emergency call volumes per capita in Canada. According to Shared Health, just over 9,000 calls were answered by the city's emergency department in the 2023-24 fiscal year. "We have always thought based on our experience and call volumes we need three full-time ambulances running day and night for a city with as high of a demand for emergency services as Thompson," he said. Campbell fears staffing issues will continue to keep ambulances off the road. He said he is also worried about the staff who make up the department, as low staffing levels are forcing some to work shifts sometimes as long as 38 hours at a time. "People are becoming physically and mentally exhausted. This job is mentally trying at the best of times and it's not like we are at a slow place, we are constantly busy, so that's a grave concern." Campbell is now asking for Thompson's mayor and city council to work with the union to come up with a new agreement, and he hopes that ultimately emergency services in Thompson can be returned to being the sole responsibility of the department. "We have been lobbying to get the ambulance service back," Campbell said. "We need a point blank statement as to what the plan is, and what we are doing moving forward." Not a city issue: Mayor Thompson Mayor Colleen Smook argued any issues with emergency services in Thompson should be directed to Shared Health first and not the city. "It's all Shared Health for ambulance services, it's no longer the City of Thompson," Smook said. "And those posts from the union are making it sound like we're letting the citizens down. "The ambulance is not the City of Thompson's issue, and to say that the mayor and council is not doing anything is not fair." Smook says the agreement with Shared Health to run ambulance services will run out at the end of July, and said the city will continue discussions with the province and Shared Health on how to continue after that deadline passes. Both the province and Shared Health confirmed last week they have been in discussions with the City of Thompson as they look to establish a new model for ambulance services in the city. But Campbell is also accusing the city of keeping the union out of discussions regarding the future of emergency services in Thompson. "We've been trying to come to the table," he said. "We've been trying to be part of the solution, but unfortunately when it comes to being privy to these meetings we are either ignored completely, or told no. "I think for us to be completely left in the dark on these matters is unacceptable, and completely inappropriate."

The symposium is a response to the urgent need to address the compounding effects of racism and sexism in sexual and reproductive health outcomes.
