Spending cuts to the health insurance program for the poor and disabled partially offset revenue losses from tax cuts in the measure and are a crucial demand of GOP fiscal conservatives.
The revision helps Republicans shore up the spending cuts they need to fund the bill, but it could also alienate three crucial senators — Susan Collins of Maine, Lisa Murkowski of Alaska and Thom Tillis of North Carolina — who have been pushing to scale back the Medicaid cuts.
Senate Republican Leader John Thune is trying to navigate competing demands from conservatives and moderates as he rushes to pass the massive tax and spending package to meet a July 4 deadline Trump has set for congressional approval.
The Senate's legislative rules-keeper had judged a series of key health care provisions in the legislation ineligible for a special procedure Republicans are using to bypass the Senate's normal process so they can avoid making concessions to Democrats.
That earlier decision swept aside $250 billion in spending cuts fiscal conservatives had sought.
But Senate Parliamentarian Elizabeth MacDonough ruled acceptable a revised provision that would limit states' ability to tax health care providers to help fund Medicaid, Senate Budget Committee Democrats said in an email on Sunday.
It wasn't immediately clear how much budget savings the new version would produce.
The Congressional Budget Office has estimated that the legislation could lead to millions of people losing health coverage. The scorekeeper found that an earlier iteration of the Senate bill would lead to 11.8 million people losing health benefits by the end of the decade.

The revised provision is likely bad news for HCA Healthcare Inc. and Tenet Healthcare Corp., as hospitals are again facing potential cuts to Medicaid funding.
States often use the provider taxes, within some already existing rules, to draw down federal funding and increase payments to facilities like hospitals.
The House version of the provider tax provision, which is less aggressive than the Senate's draft, would have saved the federal government $89 billion over a decade, according to congressional budget analysts.
Alaska, Biden Rules
The Senate's rules-keeper also nixed measures that would've boosted some hospital outpatient payments and Medicaid federal matching rates for Alaska and Hawaii, according to Democrats on the Budget Committee. The measures had been included as Thune sought to win over support, including from Alaska's Murkowski, who eventually helped advance the legislation after expressing hesitations.
The parliamentarian also said provisions aimed at blocking efforts to streamline federal health care enrollment and a Biden-era nursing home minimum staffing requirement weren't in compliance with chamber rules. That could threaten billions in savings for the tax bill.
The Congressional Budget Office, when considering a similar House-passed measure aimed at blocking implementation the enrollment regulations until 2035, estimated $167 billion in savings over the next decade, while placing a moratorium on the nursing home staffing rule would save $23 billion.
The Senate version of the legislation, the centerpiece of Trump's economic agenda, makes permanent individual and business tax breaks enacted in 2017, while adding temporary new breaks for tipped and overtime workers, seniors and car-buyers.
The bill would add hundreds of billions of dollars in new spending for the military, border patrol and immigration enforcement. To partly pay for the revenue losses, the bill reduces spending on Medicaid, food assistance for low-income Americans and financial aid to college students.

The Department of Justice has filed a lawsuit against Mayor Karen Bass and the city of Los Angeles over sanctuary city policies, claiming they "interfere with the federal government's enforcement of its immigration laws," the department says. In the lawsuit, the DOJ alleges that days after President Trump won the November 2024 election, the city of LA and its officials worked to "thwart the will of the American people" by beginning to codify sanctuary city policies into law. The DOJ claims that LA's sanctuary city ordinance, Prohibition of the Use of City Resources for Federal Immigration Enforcement, signed by Bass on Dec. 9, 2024, prevents Immigration and Customs Enforcement (ICE) and U.S. Customs and Border Protection agents from carrying out their obligations under federal law. "Today's lawsuit holds the City of Los Angeles accountable for deliberately obstructing the enforcement of federal immigration law," said U.S. Attorney Bill Essayli for the Central District of California. "The United States Constitution's Supremacy Clause prohibits the City from picking and choosing which federal laws will be enforced and which will not." Court documents name the city of Los Angeles, Mayor Karen Bass, the Los Angeles City Council and Council President Marqueece Harris-Dawson as the defendants. CBS Los Angeles has reached out to Bass, Dawson and the city attorney for a comment and is awaiting a response. The lawsuit adds that the U.S. is currently facing a "crisis of illegal immigration" and claims the government's efforts to address the crisis are "hindered" by LA's sanctuary city policies. The lawsuit comes weeks after immigration operations across Southern California began ramping up, prompting demonstrations that mainly started peacefully but escalated into clashes between protestors and law enforcement. As a result, Mr. Trump ordered members of the National Guard and U.S. Marines to deploy to the region. A section of downtown LA was also affected by the violent demonstrations, including businesses being looted and public property being vandalized. Bass issued a curfew for a portion of downtown, which was lifted seven days after when safety conditions started to improve.

The civil grand jury in San Mateo County has indicted embattled Sheriff Christina Corpus for alleged misconduct, prosecutors announced Monday, as she faces potential removal by the Board of Supervisors. District Attorney Steve Wagstaffe's office said the grand jury on Friday returned an accusation against Corpus that includes one count of conflict of interest in violation of the County Charter. The conflict of interest allegation stems from the hiring of Victor Aenlle, whom she allegedly had a close personal relationship with. The grand jury also accused Corpus of three counts of retaliation over the termination of Assistant Sheriff Ryan Monaghan, the transfer of Capt. Brian Phillip and the arrest of Deputy Carlos Tapia, who is the president of the Deputy Sheriff's Association. Prosecutors said the grand jury returned an indictment following a month-long hearing in which 32 people, including Corpus, testified under oath. The hearing was conducted by Wagstaffe's office, which acted as an advisor to the civil grand jury. Corpus has been facing months of scrutiny and calls to resign following the release of an independent report by retired judge LaDoris Cordell into allegations of abuse of power at the sheriff's office. Following the report, the Board of Supervisors called for a special election to temporarily amend the charter to remove a sheriff from office for cause. In March, 84% of voters approved the charter amendment. Earlier this month, the board initiated the process to remove Corpus from office, a process which is expected to take several months. The sheriff has repeatedly denied the allegations against her and has refused multiple calls to resign from local leaders and the Deputy Sheriffs' Association. Corpus has also filed a $10 million lawsuit against the county alleging discrimination and harassment. Prosecutors said Corpus is scheduled to appear in court on July 15 to answer the accusation, in which she is entitled to a trial by jury.

At the heart of the debate over the 'Big, Beautiful Bill' stands one little-known official: the Senate parliamentarian. The low-profile congressional official has been thrust into the spotlight after issuing a procedural decision on the GOP-backed spending bill, drawing sharp responses from some Republicans. President Donald Trump himself has even weighed in, calling for the 'unelected senate staffer' to be ignored. Who is the Senate parliamentarian? What powers do they have? And why are Republicans up in arms? Here is what to know. Who is the parliamentarian? The Senate parliamentarian is a nonpartisan advisor who makes recommendations to lawmakers regarding the interpretation of rules and precedents in the upper chamber. The role was established in 1935 amid the passage of a slew of New Deal-era laws, which 'expanded opportunities for procedural confusion and mischief,' according to Senate records. The parliamentarian is appointed by the Senate majority leader and serves at their pleasure, according to the National Constitution Center. The official's rulings are not necessarily final. The presiding officer of the Senate — typically the vice president or the president pro tempore — can simply ignore their advice, according to the Bipartisan Policy Center. This has happened a few times in recent history, including in 2017, when GOP lawmakers changed Senate rules to allow Supreme Court nominees to be confirmed with a simple majority. That said, the Senate typically adheres to the parliamentarian's guidance, according to Time. This included in 2021, when the staffer rejected Democrats' attempt to include a pathway to citizenship for illegal immigrants in the 'Build Back Better' bill. Currently, the office of the parliamentarian is held by Elizabeth MacDonough, who has served in the position since 2012. She was appointed by then-Senate Majority Leader Harry Reid, a Democrat. MacDonough formerly worked at the Senate library and as a trial attorney, according to the Wall Street Journal. She also advised former Vice President Al Gore during a recount dispute following the 2000 election. Why are Republicans upset? Some Republicans expressed outrage at the parliamentarian after she issued rulings on the 'Big Beautiful Bill,' which is currently being considered by the Senate, following its passage in the House in May. In late June, MacDonough ruled that a series of provisions in the bill violate the Senate's Byrd Rule, which prohibits 'extraneous' provisions from being included in budget reconciliation bills. One such provision would have charged immigrants a $1,000 fee to apply for asylum in the U.S., according to The Hill. MacDonough also ruled against several provisions that affect Medicaid. One was a proposed cap on provider taxes, which states use to fund Medicaid, according to CNBC. Another would block noncitizens from accessing Medicaid in addition to other health programs, according to USA Today. In response to these proposed changes, multiple GOP lawmakers telegraphed their frustration with MacDonough. 'The WOKE Senate Parliamentarian, who was appointed by Harry Reid and advised Al Gore, just STRUCK DOWN a provision BANNING illegals from stealing Medicaid from American citizens,' Sen. Tommy Tuberville, a Republican from Alabama, wrote on X, formerly Twitter, on June 26. 'THE SENATE PARLIAMENTARIAN SHOULD BE FIRED ASAP.' Rep. Greg Steube, a Florida Republican, also singled out MacDonough for criticism. 'How is it that an unelected swamp bureaucrat, who was appointed by Harry Reid over a decade ago, gets to decide what can and cannot go in President Trump's One Big Beautiful Bill?' he wrote on X. He then called on Vice President JD Vance — using his power as the presiding officer in the Senate — to ignore the proposed changes. Trump appeared to concur with this view. 'Great Congressman Greg Steube is 100% correct,' he wrote on Truth Social on June 29. 'An unelected Senate Staffer (Parliamentarian), should not be allowed to hurt the Republicans Bill. Wants many fantastic things out. NO!' However, Senate Majority Leader John Thune, a South Dakota Republican, poured cold water on the push to sideline MacDonough. When asked by Politico about overruling her on June 26, Thune said, 'No, that would not be a good option for getting a bill done.' Meanwhile, other Republicans have defended MacDonough. North Carolina Sen. Thom Thillis, who recently announced he will not seek reelection, called her a 'straight shooter,' according to Reuters. And Louisiana Sen. John Kennedy told reporters, 'Nah, never overrule the parliamentarian.'
