KiwiRail bans 2500 workers from using melatonin, Zopiclone citing safety concerns

KiwiRail has banned approximately 2500 of its workers from using melatonin and Zopiclone, citing safety concerns.
Melatonin will soon be
sold over the counter without requiring a prescription

Rail and Maritime Union health and safety organiser Karen Fletcher said in a recent memo to staff, KiwiRail said workers in safety sensitive roles taking melatonin or Zopiclone would be stood down temporarily.
She said the union was not consulted on the decision.
"We first heard about it when the staff got in touch with us and said, 'What's this all about?' They've been taking melatonin and Zopiclone for years and even decades, prescribed by a doctor, and sometimes the rail medical doctor.
"It came as a shock."
She said the staff included in the ban were those in high-risk, front-line roles, who often operated heavy machinery.
Many were shift workers who relied on melatonin or Zopiclone to help them sleep, she said.
"Shift workers have to sleep at odd times of the day and night, and [sleep medication] has been really helpful for them.
"They take it to be fit for work because if they don't sleep, they're at risk of working in a fatigued state."
She said the workers often had to shift their start time and sleep time, working rotating shifts.
She said the union had not had any issues with workers being impaired from taking sleep aids, and some said taking them made them feel the best they ever had at work.
"They're questioning why now, why the change, is there new evidence?
"And will people be able to take it in certain circumstances, and what will happen to our members if they're unable to transition off these meds that they've been using for decades in some cases?"

In a statement, a KiwiRail spokesperson told RNZ it made the decision based on advice from its chief medical officer Dr Simon Ryder-Lewis who is an occupational medicine physician.
"KiwiRail's chief medical officer has independently reviewed the use of Zopiclone and melatonin by safety critical workers at KiwiRail. This review considered the medical evidence regarding these medications as well as regulatory practices in rail and other comparable industries, both in New Zealand and internationally.
"Based on this assessment, the chief medical officer has determined that neither medication is compatible with safety critical work at KiwiRail."
KiwiRail said it placed the highest priority on safety and under its Fitness for Work Policy, all workers were required to notify their managers or supervisor of any factors which might impair their ability to perform their duties safely, including declaring prescription medications.
The Civil Aviation Authority (CAA) also told RNZ it did not generally recommend melatonin for pilots and crew.
"We have found the use of melatonin by pilots and cabin crew is usually not useful to manage fatigue and roster-related time zone changes. Melatonin may be approved by CAA on a case-by-case basis for pilots without sleep disorders."
Deputy Head of the University of Auckland Department of Anaesthesiology Guy Warman has expertise in sleep, circadian rhythms and melatonin.
He said it was common for shift workers, particularly those who work overnight, to feel tired while on shift and have poor sleep while off work.
"Shift work presents all sorts of challenges because people are working at times when the body is trying to promote sleep.
"People then seek ways of improving their sleep through medication and other means.
He said melatonin was different from other sleep medications that acted as sedatives.
"It's not really a sleep drug, it's a hormone.
"Melatonin is produced by the pineal gland in the brain in very low doses. It naturally acts on receptors in our central biological clock to reinforce our day-night sleeping cycle.
"However, in pharmacological doses, it can be taken to shift our biological clock and reduce sleep onset latency, which is the amount of time it takes to get to sleep."
He said as long as it was taken in appropriate doses and at appropriate times, the safety effects of melatonin were positive.
"The general recommendation is eight hours before doing any work.
"The evidence I'm aware of is that the melatonin will be gone before the eight hours are up."
He was not aware of any evidence that taking a standard 3mg of melatonin could cause impairment the day after it was taken.

Minister Judith Collins hospitalised after 'nasty bout of vertigo'

She was in Wellington Hospital where she remained overnight, but was now recovering at her Wellington home, and said she was "on the mend". Collins posted on social media on Wednesday evening saying she "found out the hard way" that a "virus followed by several flights" can cause vertigo. Vertigo is a sensation of spinning, whirling, or dizziness. Other symptoms can include nausea, vomiting, difficulty walking, and loss of balance. Collins described it on social media as "like seasickness multipled." Over the weekend, Collins visited a range of Defence Force bases using a combination of NZ Defence Force and Commercial flights in her capacity as Defence Minister. Collins thanked the "wonderful, professional care" from Wellington Free Ambulance and Wellington Hospital, and told RNZ she was very grateful for the "excellent initial care" from her colleague Dr Shane Reti. Reti has an office on the same floor as Collins. Collins will eventually travel back to her Auckland home, and suggested people "don't be alarmed" if they see her "with a little support." Lifting advertising restrictions will enable medical conferences in NZ

Two of Australasia's largest medical conferences will come to New Zealand next year, and the government is talking up estimates they could add $5 million to the local economy. Seymour said the advertising restrictions were brought in because of fears pharmaceutical companies could try to skirt approvals processes, but his Regulations Ministry found that approach was out of step with other countries. He said advertising medicines at a conference was one of the main reasons to hold one. "We have got rid of the restriction on advertising new medicines, so there's good reasons for doctors to come to New Zealand and learn about them. Before we made this change, New Zealand doctors would go to places like Fiji for a medical conference, which is nice for them, but bad for the New Zealand economy, because the money's going over there," Seymour said. The 2026 conferences are for the Australian and New Zealand College of Anaesthetists (ANZCA) and the Royal Australian and New Zealand College of Ophthalmologists (RANZCO), and could attract an estimated 3300 delegates. "You're going to see a whole lot of mainly doctors but medical professionals interested in learning about new medicines and technologies and that they're going to join together in New Zealand, and it's going to just be a wonderful thing," Seymour said. "It's the kind of activity we should want in this country. You know, I've long said New Zealand should try to be the ancient Athens of the modern world, the centre of medical science and excellence." Seymour in a statement said the government was committed to removing red tape, and the lifting of restrictions would make it easier for health professionals to keep up with the latest products and medicines. "New Zealand's current health regulations can be overly bureaucratic, and this is slowing down access to care, increasing costs, and making it harder for patients to get the services they need." Plea to reopen consultation into plans to close Auckland mental health facility

Photo: Google Maps Street View A union representing public service workers is urging health officials to reopen consultation into proposals to close an Auckland mental health facility , citing concerns about predetermination. The PSA has sent a lawyer's letter to Health NZ Te Whatu Ora with the plea about Rauaroha Segar House, which offer a publicly funded intensive programme for people with long-standing or chronic mental health problems. Health NZ says it is working through feedback on the proposed closure, after staff put forward a plan to keep a service they and many patients say is unique. After the period seeking submissions closed, correspondence obtained by Labour spokesperson for mental health Ingrid Leary and reported by RNZ revealed officials had intended to vacate the Khyber Pass Road facility Segar House operates from in January, before a pre-Christmas move to extend the lease until June. It has since been extended again until 30 September. The correspondence makes no mention of possible alternative venues for the programme, but Health NZ said the building lease was not a factor in its decision making, and the proposal was about providing the best healthcare and value for money. The PSA's letter, from its solicitor Caroline Mayston to Health NZ co-director of mental health and addiction services in Te Toka Tumai, Stacy Silva Garay, last Friday said after learning of the lease information in the media the union had serious concerns about "predetermination of the change process". It said Health NZ breached its duty to provide all relevant information to employees and unions during the Segar House consultation process. "We request that consultation be reopened to allow affected employees to consider and respond to this additional information, as is their right," Mayston wrote. The union's concerns were not alleviated by Health NZ saying lease arrangements had nothing to do with the proposal to close Segar House. "This [the correspondence obtained by Leary] indicates to us that Te Whatu Ora was planning for the disestablishment of Rauaroha Segar House well prior to opening consultation with affected employees and had no intention of maintaining the service elsewhere. "We do not accept this information was not relevant to the change process." Rauaroha Segar House has offered a publicly funded intensive programme for people with long-standing or chronic mental health problems. Photo: 123RF RNZ asked Health NZ if it would allow staff and other parties to have a say on the new information, what its response was regarding concerns about predetermination, and when a decision on Segar House's future would be made. Health NZ northern regional acting deputy chief executive Mike Shepherd said officials were considering the information the PSA provided. "We have received a large volume of feedback on the change process for the facility and we have been carefully reviewing this feedback. "We will be in a position to make a decision shortly and this decision will be communicated to affected stakeholders as soon as possible." Leary said allowing the PSA's suggestion of reopening consultation seemed fair and logical. "Extending the time to ensure proper consultation is happening is the right thing to do. That's what I've said all along and that's what needs to happen." It made sense to give the staff idea of loosening the criteria for people to use the service a go, to see if it worked, she said. But, it looked like officials hadn't entered the process with an open mind as they scrambled to save money, Leary said. Health NZ has cited low referral numbers in its proposal to axe Segar House, although entry requirements were tightened about six years ago.
