
Why Long COVID May Hit Toddlers Harder Than Thought
A recent study in JAMA Pediatrics systematically investigated the symptoms experienced by children under the age of 5 years and the frequency of long COVID in this age group.
The analysis revealed that 14% of infants and toddlers and 15% of preschoolers in the cohort showed symptoms consistent with long COVID.

Symptom Discrepancy
'This would mean that 1 in 6 children in these age groups develop long-term COVID after a SARS-CoV-2 infection. This does not correspond to the clinical reality we experience in our specialized outpatient clinics,' said Elling.
'In our view, long COVID is primarily a condition of the second decade of life, not the first 10 years. We see almost no cases in toddlers — and that holds true at all university clinics in Baden-Württemberg, not just Freiburg,' he added.
In contrast, findings from a multicenter cohort study suggested a different age pattern. Rachel S. Gross, MD, professor at NYU Grossman School of Medicine in New York City, and co-principal investigator of the NIH RECOVER Pediatric Observational Cohort Study, led the study from 2022 to 2024. The results identified persistent post-COVID symptoms, even among children under the age of 5 years.
The study combined retrospective and prospective data to identify symptoms that are more common in children with prior SARS‑CoV‑2 infection and to develop an age-specific long COVID index. The researchers enrolled 472 children aged 0-2 years and 539 children aged 3-5 years. Among them, 59% (278 of 472) of the younger group and 74% (399 of 539) of the older group had documented prior infections. These findings produced distinct symptom indices for each age group.
Elling noted a key limitation: 'Seroprevalence studies showed that by 2023, over 90% of children had been infected with SARS-CoV-2. As younger children often have asymptomatic infections, many cases may have been missed. No serologic testing was performed; all data relied on parental information.'
On average, parents of younger children were surveyed 10 months after infection, and parents of preschoolers were surveyed 17 months after infection.
Although this helps separate long COVID from short-term symptoms, Elling stated that the long delay makes the data unreliable. 'I cannot reliably ask parents if their child had symptoms lasting 4 weeks over a year ago,' he said.
The questionnaires assessed 41 symptoms in children aged 0-2 years and 75 symptoms in preschoolers aged 3-5 years. These symptoms were grouped into eight clinical areas: general health, ENT, heart and lungs, digestion, skin, muscles and joints, nervous system, and mental health.
According to the Long COVID Symptom Index developed in the RECOVER project, 14% of children aged 0-2 years and 15% of those aged 3-5 years were identified as likely to have long COVID.
Age-Specific Symptom
The most common symptoms in infants and toddlers were sleep disturbances, irritability, loss of appetite, nasal congestion, and a productive cough. According to parents, preschoolers more often experience dry cough, daytime fatigue, and low energy levels. Parents also reported that children with more severe symptoms had a poorer quality of life, reduced overall well-being, and sometimes developmental delays.
'In older children and adolescents, it is neurocognitive symptoms such as postexertional malaise or brain fog that significantly impact daily life — not cough or other respiratory issues,' said Elling.
He noted that these complex symptoms are difficult to detect in young children.
Conclusion
Studies have shown that less than 1% of adolescents develop long COVID, with the risk likely to be even lower in younger children. Elling said, 'Young children cope better with COVID-19 than adolescents and adults.'
Elling agreed with the authors that long COVID presents differently in very young children than in older children and should be defined and studied by age. 'I would go even further,' he said. 'To truly understand the disease, we need to move away from the broad term 'long COVID,'' he said.
Elling emphasized that long COVID should be classified not only by age but also by the organ systems affected. 'It is medically inaccurate to group all symptoms lasting more than 12 weeks — such as headache, anosmia, or dyspnea — under one broad diagnosis. We don't do this for any other disease,' he said.
These symptoms are likely to have different underlying mechanisms; for example, loss of smell after COVID-19 is not the same as chronic headaches or breathing issues.
Despite some methodological limitations, Elling supported the study's approach. 'Systematic assessment of long COVID in early childhood is important. However, we must remain realistic. If the data suggest that four children in every class develop long COVID after a SARS-CoV-2 infection, it would be a serious concern. But I don't think that's actually the case,' he said.

Drug Level Changes Predict Renal Transplant Success in Kids
TOPLINE: In paediatric kidney transplant recipients, high variability in tacrolimus trough blood levels was significantly associated with allograft rejection occurring beyond 12 months post-transplant; however, rapid tacrolimus metabolism was significantly associated with rejection episodes occurring between 6 and 12 months. METHODOLOGY: Tacrolimus remains essential in contemporary immunosuppressive regimens, yet its narrow therapeutic window and wide interpatient and intrapatient variability complicate dosing. Although high variability in tacrolimus blood levels and a low concentration-to-dose (C/D) ratio — an approximation of tacrolimus metabolism — predict kidney transplant rejection in adults, comparable paediatric data are scarce. Researchers in Germany conducted a retrospective study to assess the predictive value of tacrolimus intrapatient variability (TacIPV) and C/D ratio for transplant outcomes in children receiving kidney transplantation. They analysed 13,159 tacrolimus trough blood levels in 255 paediatric kidney transplant recipients (median age at transplantation, 11 years; 59% boys) who received a tacrolimus-based immunosuppressive regimen; the median follow-up duration was 60 months. TacIPV was quantified during months 6-12 post-transplant, and the C/D ratio was evaluated during both early (first 6 months) and late (6-12 months) periods. Measured outcomes encompassed the incidence of allograft rejection, the occurrence of opportunistic infections, and the degree of graft dysfunction. TAKEAWAY: Overall, allograft rejection occurred in 45% of patients — 9% within the first 6 months post-transplant, 6% between 6 and 12 months, and 30% after 12 months. High TacIPV during months 6-12 post-transplant was associated with an increased risk for allograft rejection beyond 12 months post-transplant (hazard ratio [HR], 1.04; P = .002); a TacIPV threshold of more than 23% could distinguish patients who did vs did not experience allograft rejection. A low C/D ratio during the first 6 months post-transplant was correlated with a higher risk for allograft rejection between months 6 and 12 (inverse HR, 3.13; 95% CI, 1.05-9.09; P = .04); a C/D ratio cutoff of less than 1.0 could distinguish patients who did vs did not experience allograft rejection. Opportunistic infections and graft dysfunction occurred in 42% and 10% of patients, respectively. IN PRACTICE: "TacIPV and C/D ratio may serve as cost-effective, non-invasive predictive markers for early identification of patients at risk for rejection," the authors wrote. Monoclonal Antibody Shows Promise in Cardiogenic Shock
Treatment with a novel monoclonal antibody against circulating dipeptidyl peptidase 3 (cDPP3) led to complete reversal of cardiogenic shock in three compassionate use cases in Germany, according to a new research letter. The cases mark the first time the intervention has been used to treat cardiogenic shock in humans. Only one of the patients survived, however. 'The findings are highly promising and represent a significant step forward in causally addressing cardiogenic shock, a condition with limited treatment options and high mortality,' Mahir Karakas, MD, of the University Medical Center Hamburg-Eppendorf, Hamburg, Germany, told Medscape Medical News . He is the chief technology officer of 4TEEN4 Pharmaceuticals GmbH, the pharmaceutical company developing the monoclonal antibody called procizumab. 'Out-of-the-Box' Approach The mortality rate of cardiogenic shock exceeds 50%, and current interventions treat only symptoms rather than addressing the root cause, said Alexandre Mebazaa, MD, PhD, of the Université Paris Cité in Paris, France, and a coauthor on the letter. Recent research has revealed a strong link between elevated cDPP3 and higher mortality rates in patients with cardiogenic shock. Procizumab targets DPP3, an enzyme released during cell death that breaks down angiotensin II and disrupts the renin-angiotensin-aldosterone system. By blocking this enzyme's activity, procizumab 'is intended to rapidly stabilize hemodynamics and therefore cardiovascular and renal function,' Mebazaa and his colleagues wrote. 'With these three cases, we [were] really thinking outside of the box, saying, 'There is a protein that is toxic inside the blood: let's inactivate it and see what we can do,'' Mebazaa told Medscape Medical News . The research letter, published online on July 10 in the European Journal of Heart Failure , documented the treatment of three patients in critical condition with signs of multiorgan failure due to refractory septic shock. Patient 1 (age 64 years) and patient 3 (age 80 years) were women; patient 2 (age 84 years) was a man. All conventional therapies had been tried in these patients 'with no further options for escalation of conventional therapy in the presence of a high chance of immediate mortality,' the authors wrote. All three patients had elevated DPP3 activity, acute kidney injury, and needed renal replacement therapy. Patients 1 and 2 were intubated and received mechanical ventilation. Procizumab was administered at a 10 mg/kg dose over 2 hours, and patients were followed for 48 hours. Efficacy was assessed by shock reversal, defined as a norepinephrine dose of 0.2 µg/kg/min or less, or a reduction by at least half of the initial dose. A decline in DPP3 activity over 2 days was accompanied by a marked reduction in norepinephrine dose and lactate concentrations. All three patients demonstrated improved arterial oxygen partial pressure/fractional inspired oxygen ratios, a measure to assess hypoxemia, compared with baseline. Early, but 'Impressive' Findings 'These very early findings support the potential of this specific anti-DPP3 antibody, procizumab, which has previously been shown in animal studies in septic and cardiogenic shock,' said Holger Thiele, MD, director of the Heart Center Leipzig at the University of Leipzig, in Leipzig, Germany, who was not involved with the case study. Both Thiele and the authors acknowledged the uncontrolled design and small sample size preclude any firm conclusions around the potential efficacy of the treatment. 'This is far too early to draw conclusions [about whether] all these effects are related to procizumab, although the drop in [interleukin 6] IL-6 and the improvement in renal function are impressive,' Thiele said. 'However, like always with promising devices or drugs, initial results are often too good to be true. We have to wait for the phase 2, and more importantly, the phase 3 trials to see if this drug in cardiogenic shock or septic shock is able to reduce the mortality.' Two of the patients who received the treatment died less than 2 weeks later: one from recurrent shock likely due to papillary muscle rupture from untreated myocardial infarction (MI), and another from refractory shock due to exacerbation of right heart failure. Patient 2 recovered organ function and was transferred to rehab at 8 weeks despite a new MI at week 4, the researchers reported. 'That two of the three patients had refractory shock again is the typical nature of these severely ill patients,' Thiele said. 'This probably will also be one of the challenges to show a possible benefit of this drug in the subsequent phase 2 and phase 3 trials.' Ongoing Trial Mebazaa is now leading a phase 1b trial to evaluate the safety and tolerability of procizumab as well as the ideal dose for a phase 2 trial. The study, PROCARD 1b, aims to enroll 130 patients with cardiogenic shock due to acute coronary syndrome or sepsis with elevated DPP3. 'It is most likely that specific cutoffs for DPP3 are required,' Thiele said; while too low of a concentration will probably show no benefit, 'too high with too many organ dysfunctions may also result in neutral results. Therefore, a well-designed trial for phases 2 and 3 is of paramount importance.'
