This experimental drug could help young people with a rare and aggressive form of ALS
Known as 'ulefnersen,' the therapy showed promise in treating patients with FUS-ALS, which is caused by a genetic mutation in a gene called FUS. The gene provides instructions for making a protein and plays a crucial role in cellular processes. While the mutations are only responsible for between one and two percent of ALS cases, they cause some of the most aggressive forms of ALS that begin in adolescents and young adults.
Fewer than 30,000 people in the U.S. are living with ALS, which is also known as amyotrophic lateral sclerosis or Lou Gehrig's disease, while 5,000 people are diagnosed each year. The nervous system disease affects nerve cells in the brain and spinal cord, leading to a loss of muscle control that gets progressively worse. The exact cause of the disease remains unknown, but a small number of cases are genetic. There is no cure and it is, eventually, fatal.
'When testing new drugs for ALS, we do not expect to see clinical improvement,' neurologist and scientist Neil Shneider said in a statement. 'What we've seen in one patient is really unprecedented functional recovery. It's surprising and deeply motivating for us, the ALS research community, but also the community of ALS patients.'
Shneider, who developed the drug in collaboration with the biotech company Ionis Pharmaceuticals, published his findings in the peer-reviewed journal The Lancet.
Starting as an effort to help a single patient, the case series was small, including just 12 patients who were treated with the drug.
Two of the patients showed a remarkable response to the drug. A young woman who had received injections of ulefnersen since late 2020 regained the ability to walk unaided and to breathe without the use of a ventilator, both previously lost to ALS. The medical center said she has lived longer with this disease than any other known patient with this juvenile-onset form of FUS ALS.
A man in his mid-thirties was asymptomatic when he began treatment, but tests of electrical activity in his muscles indicated that symptoms would likely emerge imminently. However, after three years of treatment, he has yet to develop FUS-ALS symptoms and the abnormal electrical activity in his muscles has improved.
Lastly, the researchers found patients experienced an up to 83 percent decrease in a protein called neurofilament light — a biomarker of nerve damage — following six months on the drug.
'These responses show that if we intervene early enough and go after the right target at the right time in the course of disease, it's possible to not only slow disease progression, but actually reverse some of the functional losses,' said Shneider. 'It's also a wonderful example of precision medicine and therapeutic development based on science and an understanding of the biology of disease.'
While most of the other symptomatic patients did not survive their disease, Shneider said that 'several apparently benefited from the treatment. The progression of their disease slowed, and they lived a longer life as a consequence.'
There were no serious adverse events related to the drug and a global clinical trial is now in progress. At least 25 patients have been treated with the drug around the world, including the dozen in Shneider's case studies. It was first tested six years ago in an Iowa patient named Jaci Hermstad. The drug was originally named for her.
'Now we are eagerly awaiting those results, which we hope will lead to the approval of ulefnersen,' Shneider said.

Senate appropriators defend the NIH
WASHINGTON WATCH Senate appropriators came out hard in support of the National Institutes of Health on Thursday, giving the agency a $400 million funding boost for the 2026 fiscal year. How so: The Senate Appropriations Committee upped the agency's budget to $48.7 billion in the 2026 funding bill that cleared the panel with a 26-3 vote Thursday. If the bill becomes law, it would increase cancer research by $150 million; Alzheimer's research by $100 million and amyotrophic lateral sclerosis, or ALS, research by $25 million. The NIH's National Institute of Allergy and Infectious Diseases and the Office of Research on Women's Health would each get a $30 million boost. Research on maternal mortality, diabetes and rare diseases would also see an increase, among others. Why it matters: The funding boost is a rebuke from both Republicans and Democrats to the Trump administration's demand to decrease the NIH funding in the next fiscal year by as much as 40 percent, or $18 billion. Senate Appropriations Chair Susan Collins (R-Maine) said the legislation 'prioritizes funding to help make Americans healthier and supports life-saving medical research.' Sen. Patty Murray (D-Wash.), the top Democrat on the panel, said the budget increase was a message to 'the scientists wondering if there will even be an NIH by the end of this administration. This committee's resounding message is: 'Yes, Congress has your back.'' Murray urged scientists to continue their research in the U.S. despite the efforts of other countries to lure them away. The appropriators also adopted an amendment Thursday that would limit the Trump administration's control over NIH research funding. An amendment in the bill's manager's package limits the administration's plan to shift funding for most NIH grants from a multiyear schedule to an upfront single-year payment. The amendment states that no funds appropriated in the fiscal 2026 spending bill can be used to increase the proportion of grants fully funded in the first year of the award, compared with fiscal 2024. The NIH can only increase that proportion of forward-funded grants if the agency ensures it isn't cutting grants to do so. What's next: The bill is cleared for floor action. But congressional leaders haven't started bipartisan negotiations toward overall government funding totals, increasing the odds that lawmakers will again resort to a stopgap funding patch before the next fiscal year starts on Oct. 1. As a physician who once treated pain, I now endure the unimaginable. Amyotrophic lateral sclerosis (ALS) has left me quadriplegic, dependent on a tracheostomy to breathe and a feeding tube to eat. Diagnosed at 35, my career ended abruptly. My family's story reveals the scale of the coming storm—my father battles Alzheimer's, my uncle succumbed to Parkinson's, and my grandmother to Lewy body dementia. We are not outliers; we represent a silent epidemic. Neurodegenerative diseases are surging, yet our nation slumbers, unaware of the devastation ahead. The public's complacency is by design, built on a statistical illusion. ALS, fatal since 1869, exemplifies this peril. With a median survival of 2-3 years, it strikes about 1 in 300 people in their lifetime, which predicts that over 1 million people now alive in the U.S. will succumb to ALS. Yet, under federal law, it is labeled "rare" because that definition is based on prevalence—a static snapshot of how many people are living with a disease at one time. Because ALS kills its victims so quickly, the number of living patients stays below the 200,000-person "rare" threshold. Its very lethality ensures it is never treated like the mainstream public health crisis it is. This paradox obscures a terrifying forecast: a projected 69 percent global increase in ALS cases by 2040. This illusion of rarity perpetuates a deadly inaction that extends to all brain diseases. Alzheimer's already affects over 7 million Americans and is projected to strike nearly 13 million by 2050, costing our economy $384 billion in 2025—and projected to nearly $1 trillion annually by mid-century. Getty Images The failure to confront this crisis stems from a Tale of Two Agencies within the Food and Drug Administration (FDA). In 2017, Congress established the Oncology Center of Excellence (OCE), a dynamic hub that has revolutionized cancer treatment, accounting for 85 percent of all accelerated approvals in the last decade. This success is the result of focused will and resources; National Institutes of Health (NIH) funding for cancer has topped $7.2 billion, compared to $2.8 billion for neuroscience. Neurology has no such center. Lacking an institutional home, it is fragmented, slow, and characterized by a risk aversion unthinkable in oncology. This disparity persists because of the tragic nature of these diseases. In the 1980s, ACT UP activists staged "die-ins" to force a reluctant government to fight AIDS. Patients with ALS, Huntington's, or Alzheimer's cannot mount a similar protest—we are physically silenced and immobilized, unable to "seize the FDA." This vulnerability places a unique moral obligation on our leaders to act proactively on behalf of the voiceless. The new FDA leadership now arrives with bold promises of change, posing a question that haunts everyone touched by an untreatable neurological disease: Will this time be different? Health and Human Services (HHS) Secretary Robert F. Kennedy Jr. vows to "sweep away barriers" and to "figure out new ways ... of accelerating approvals for drugs and treatments that treat rare diseases." FDA Commissioner Marty Makary has several times asked why it takes 10 years for a drug to get to market and proposes a "conditional approval" pathway based on a "plausible mechanism." Center for Biologics Evaluation and Research (CBER) Director Vinay Prasad promised to "take action at the first sign of promise for rare diseases." For the ALS community, these words are now a crisis of credibility. In a disease that has a median survival of 2-3 years, we are asking for regulatory flexibility for a treatment that began its Phase 1 trial in 2011 and was granted FDA Fast Track designation in 2014. As a petitioner on the July 3 citizens' petition for a stem cell therapy known as NurOwn, my community has presented the FDA with a clear test. Ultimately, we are seeking accelerated approval based on new, unprecedented survival data from an expanded access program—data that exceeds the extension of survival of many approved cancer therapies. But the initial request is simpler: invite the sponsor to resubmit its application for a full review. Ours is a request for due process to give a voice to the voiceless—the lowest possible bar for the Trump administration to demonstrate its promised flexibility. A clear, bipartisan solution has already failed once. The Neuroscience Center of Excellence Act, introduced in 2021 to replicate oncology's success for brain disease, stalled in committee. It is time for our leaders to find the political will that has been so catastrophically absent. Congress must immediately revive and pass the Neuroscience Center of Excellence Act. The FDA, in turn, must match its leaders' promises with action by granting our petition a review. The science is poised for breakthroughs, but it is being shackled by a broken system. For those of us on a deadline, this is not a policy debate. It is a death sentence. Awaken now, before this silent storm engulfs us all. The voiceless can't wait. A year later, researchers found that 73% of those who followed up with echocardiograms were indeed diagnosed with SHD, a rate far above average.
