Behavioral Therapy as First-Line for Bladder Control in PD?

TOPLINE: Structured behavioral therapy focused on pelvic floor muscle exercise was noninferior to treatment with solifenacin for alleviating overactive bladder symptoms in patients with Parkinson's disease (PD), new research showed. Because it was also associated with fewer side effects, especially falls, researchers recommended the behavioral therapy as a first-line treatment option. METHODOLOGY: This 12-week randomized noninferiority trial was conducted between 2018 and 2023 within four Veterans Affairs healthcare systems in the US. Overall, 77 patients diagnosed with PD and overactive bladder symptoms, as measured with an International Consultation on Incontinence Questionnaire overactive bladder module (ICIQ-OAB) symptom score ≥ 7, were included. All participants (mean age, 71 years; 84% men; 90% White individuals) were randomly assigned to receive either pelvic floor muscle exercise-based behavioral therapy with urge suppression training (n = 36) or 5 mg solifenacin daily with need-based titration up to 10 mg daily (n = 41). The primary outcome was ICIQ-OAB symptom score at 12 weeks. Secondary outcomes were ICIQ-OAB bother and quality of life scores and drug-related adverse events. TAKEAWAY: At 12 weeks, clinically significant improvement in ICIQ-OAB scores was observed across both treatment groups within a noninferiority margin of 15% (drug vs behavioral therapy: mean ICIQ-OAB score, 5.8 vs 5.5; P = .02). Both groups also had a reduction in symptom frequency, which was associated with a reduction in bother scores and an improvement in overactive bladder-related quality of life. Compared with the behavioral therapy group, the solifenacin group had a greater number of adverse events, including dry mouth (P = .002) and pain or burning during urination (P = .03). They also had a greater number of falls (6 vs 0). IN PRACTICE: 'The finding of increased falls in the solifenacin therapy group reinforces the need to carefully consider the risk-benefit ratio of medications for urinary symptoms, especially given the increased risk of falls among persons with PD. Results of this study suggest that behavioral therapy is an effective treatment option for persons with PD,' the investigators wrote. SOURCE: The study was led by Camille P. Vaughan, MD, Division of Geriatrics and Gerontology in the Department of Medicine, Emory University, Atlanta. It was published online on July 14 in JAMA Neurology. LIMITATIONS: The follow-up duration was limited to 12 weeks, restricting insight into longer-term outcomes. The cohort predominantly had male participants, reducing the generalizability of the findings, and the study relied on patient-reported data. Additionally, a higher dropout in the drug group led to nonrandom missing data, potentially biasing comparisons. DISCLOSURES: The study was funded by a grant from Veterans Affairs Rehabilitation Research and Development. Two investigators reported receiving grants from various sources during the conduct of the study. The other nine investigators reported having no relevant financial relationships. Overcoming Anxiety: Methods That Work in a High-Pressure City

Living in a city renowned for its fast-paced lifestyle, ambitious professionals, and high expectations can be exhilarating, but it often comes with a unique set of challenges. Washington DC, for instance, is celebrated for its driven population, numerous opportunities, and constantly evolving cultural scene. However, this environment can also be a breeding ground for heightened anxiety, stress, and feelings of inadequacy—especially when everyone around seems to be excelling. For many adults striving to find balance between personal growth and professional accomplishments, managing anxiety becomes not just important, but essential. In such demanding urban settings, anxiety can manifest in various forms: relentless worries about work performance, the pressure to achieve more, or the fear of falling behind peers. These feelings may be compounded by the social comparison that naturally occurs in competitive cities. The sheer pressure to meet high standards can make it difficult to ever truly relax, leading many to seek out anxiety therapy. For those looking to take control, therapy for anxiety in Washington DC provides both validation and practical solutions to the struggles that often come with city living. Understanding the Urban Effect on Anxiety High-pressure environments like Washington DC amplify anxiety through a mix of constant stimuli and social dynamics. The hustle of daily commutes, the endless networking events, and the pursuit of career milestones combine to create a sense of perpetual motion. Often, there's little time left for genuine downtime or self-reflection. Over time, this lifestyle can wear down even the most resilient individuals, causing symptoms like insomnia, irritability, physical tension, and difficulty concentrating. These urban stressors can make traditional relaxation methods less effective. For some, short walks or a quick meditation might not be sufficient to break the cycle. Instead, a comprehensive, multi-faceted approach tends to yield the best results, integrating both professional support and practical lifestyle adjustments. Popular Therapy Approaches for Anxiety in the City Residents of high-pressure environments often benefit from targeted anxiety therapy. Cognitive Behavioral Therapy (CBT) remains one of the most effective methods for managing anxiety, especially in cities. CBT enables individuals to recognize patterns of negative thinking and teaches practical tools for reframing unhelpful thoughts. This evidence-based approach is popular among adults in Washington DC, who appreciate its structured, goal-oriented design. Mindfulness-Based Stress Reduction (MBSR) is another technique gaining traction in urban settings. By helping individuals anchor themselves in the present moment, MBSR interrupts the persistent worry cycle characteristic of anxiety. Group sessions or classes focused on mindfulness and guided meditation can be especially transformative for those who feel isolated in a competitive environment. Acceptance and Commitment Therapy (ACT) also deserves mention. ACT encourages individuals to accept anxious thoughts without judgment and focus on what truly matters to them. This therapy is particularly relevant in high-pressure cities, where letting go of unattainable standards can help restore a sense of purpose and reduce overwhelm. Stress Management Strategies and Lifestyle Interventions While therapy can be crucial, integrating stress management strategies into daily life is equally important. For urban adults, this often begins with setting clear work-life boundaries. Turning off email notifications after business hours and dedicating time to hobbies can mitigate burnout and create space for recovery. Physical activity remains one of the most accessible and effective ways to combat anxiety. Many Washington DC residents find that taking advantage of city parks, joining fitness classes, or simply walking to work provides a valuable mental health reset. Even brief, regular movement can significantly reduce stress hormones and improve overall mood. Social connection is another cornerstone of anxiety relief. Urban environments can sometimes feel isolating, so intentionally reaching out to friends, joining community activities, or participating in group therapy can foster supportive relationships and a sense of belonging. When to Seek Professional Help Despite these strategies, there are times when self-help just isn't enough. If anxiety is interfering with daily life, hindering relationships, or affecting work performance, it may be time to seek professional support. Washington DC is home to countless skilled therapists who specialize in anxiety treatment. Adult therapy options range from individual counseling to specialized group sessions, many of which can be tailored to fit busy schedules. Choosing therapy is a commendable step toward well-being. Therapy not only provides a safe space to explore the roots of anxiety, but also equips individuals with long-term coping mechanisms. In a high-pressure city, reaching out for help isn't a sign of weakness—it's a courageous act of self-care. Final Thoughts Managing anxiety in a demanding, ambitious city is a journey—one that requires a combination of self-awareness, effective coping strategies, and, when needed, professional guidance. Whether you're juggling career ambitions, family responsibilities, or your own expectations, remember that you're not alone. With the right support, it's possible to not just survive city life, but truly thrive amidst its challenges. TIME BUSINESS NEWS
