After-hours boost details awaited

The devil will be in the details for regional health centres around the South following the announcement of an increase in government funding for after-hours services.
The government on Sunday announced more than $164 million over four years would be spent to expand urgent and after-hours healthcare services across the country.
Health Minister Simeon Brown said the initiative would mean 98% of New Zealanders would be able to receive in-person urgent care within one hour's drive of their homes.
It would result in a new urgent and after-hours service in Dunedin and other services would be offered in other parts of the South.
A document outlining changes said the Invercargill after-hours service would next year expand to also provide daytime services.
Improved services would also be rolled out across rural and remote communities in places such as Balclutha and Stewart Island.
Clutha Health First chief executive Gary Reed said the Balclutha facility welcomed the announcement, although details remained scant at this early stage.
"Any new monies are warmly received in these financially constrained times. We are waiting for further information from the ministry with respect to how and when the monies will be allocated, as well as what the ministry's expectations are with regards to it," Mr Reed said.
The Invercargill after-hours clinic closed in March last year as it struggled to get doctors but reopened within a few days.
WellSouth Primary Health Network chief executive Andrew Swanson-Dobbs said he was looking forward to receiving more details about the after-hours healthcare package and what it would mean for Invercargill.
"This news paves a way for a more sustainable service in Dunedin as well as the Invercargill after-hours service," he said.
"We all know the pressure on health practitioners and other staff to support after hours, with clinicians often working day jobs as well as after hours to sustain the service.
"It's a good result and now we need to get on and plan the work."
Up to 30 rural and remote South Island services will be improved, alongside a further seven provincial and urban services in 2027.
This includes places such as Haast and Gore.
Gore Health chief executive Karl Meltzer said he was glad to hear of the increase in funding but, like everything in health, "the devil is in the details."
He said potentially it was exciting, and it could help alleviate urgent-care issues.
"But we do not know what it is going to be. I have been making a few calls to Wellington but have not had any calls back," he said.
Gore Health had a round-the-clock emergency department which allowed Gore GPs to be able to go to bed by 9pm, he said.
That sort of coverage was great for a population of 20,000. He said the interesting thing would be whether the money was tagged to the Gore Health emergency department.
HealthCentral Alexandra general manager Jenaya Smith said HealthCentral began lobbying for adequate and equitable after-hours and urgent-care funding prior to opening its urgent-care centre in July last year.
"We are currently providing an extremely well-utilised walk-in service for both residents of Central Otago and visitors to the area and are open from 8am to 6pm, seven days a week," he said.
She was hopeful any funding boost would benefit patients accessing urgent-care/after-hours service but had not yet seen any information.
Central Otago Health Services Ltd was contracted to provide urgent GP services between 6pm-8am for HealthCentral-enrolled patients. Comparatively small numbers of its patients accessed this service, she said.

Waitaki District Mayor Gary Kircher welcomed the news Oamaru would have its after-hours services improved.
"The after-hours emergency department has always been a critical service provided by Oamaru Hospital, so it is great to see that Health NZ is making further improvements to support our wider Waitaki community. A significant proportion of our population will benefit and, in a time when government funding is under huge pressure, it's awesome to hear that this service will be improved."

Previously, medical graduates only had their first year of specialist general practitioner training paid for, but now all three years would be covered. The government would also cover the exam costs for about 200 trainees, and full education costs for about 400 year 2 and 3 trainees each year. President of the Royal New Zealand College of General Practitioners - which delivers the 'General Practice Education Programme' - Dr Samantha Murton, said the changes brought GP training in line with all other medical training across New Zealand and Australia. "This funding will be a gamechanger for current and future trainees. This is a significant acknowledgement for the specialism of the general practice workforce and the vital role we play in healthcare being as important as those of our peers in secondary hospital settings. "Not only will this funding offer the necessary financial support our GP registrars need throughout their training, but we are optimistic that the news will encourage medical graduates who have an interest in general practice but have been put off by the financial barriers to make the step to train as a specialist GP. To them, I say welcome and you won't regret your decision." Chief executive Toby Beaglehole said the college was enthusiastic that primary care funding was heading in the right direction. "We are focused on building a sustainable workforce for the future , which starts with training and the equitability of our program costs to other specialist medical training. "This funding sends a signal to the sector that the expertise of general practice is valued as a vital part of the health system." Photo: Calvin Samuel / RNZ He also announced the government's funding method for GP clinics, known as capitation, would be updated for the first time in more than 20 years, with changes taking effect from 1 July, 2026. "The current model is outdated and doesn't reflect the needs of patients. The revised formula will go beyond just age and sex, to also include multimorbidity, rurality , and socioeconomic deprivation," Brown said. "These changes will better distribute funding to where it's needed most, so that GP clinics with a higher needs population of enrolled patients will receive more funding to care for them." A new national health target would be developed with the primary care sector, proposing to ensure that more than 80 percent of people could see a primary care provider within one week. "People shouldn't have to wait weeks to see a doctor. Delays can lead to poorer health outcomes, more pressure on hospitals, and growing frustration for patients. We're focused on delivering timely, quality care that puts patients first." Cheryl Waaka, a former Black Fern and a māmā to two teenagers, is now facing her toughest opponent yet, stage 4 bowel cancer. She was a fit and strong wāhine Māori hailing from Ngāpuhi and Ngāti Kahungunu, she's won two World Cups in the black jersey and is still dedicated to the game — but her diagnosis in June has left her stricken with sadness. "I was healthy, running around, doing my everyday job. It was King's birthday weekend. There was a Johnny Isaacs Māori tournament in Kaikohe. I was running the line, AR-ing, and three days later, I'm on the floor in excruciating pain." She remembers the day her life was changed forever. "I got my brother to get me to the GP in Kaikohe. Ambulance to Kawakawa. Ambulance to Whangārei and then that night, broke the news that I have a tumour in my bowel and I need to have emergency surgery. ADVERTISEMENT "I suppose I'm very lucky to my surgeon. He got the tumour, but unfortunately, it spread to my liver. In March, Health Minister Simeon Brown announced the National Bowel Screening Age would be lowered for all New Zealanders from 60 to 58, a step he said would save hundreds of lives. The "redirected" funding was previously set aside for a pilot programme that allowed Māori and Pacific people to access bowel cancer screening starting at age 50. While Brown said the age of eligibility for free bowel screening was projected to prevent an additional 771 bowel cancers and an additional 566 bowel cancer deaths over the next 25 years, advocates called the move "institutionalised racism" as half of bowel cancers in Māori occurred before the age of 60. Waaka, who was up to date on all her health checks, said if the screening age was lowered to 50, her diagnosis would not have been stage 4. "If it didn't happen now, would [the tumor] have been still sitting there, and by the time I got to 58, if I got to 58, it might have been too late. "You look at me, strong, fit, healthy. It can happen to anyone. And I don't want it to happen to anyone." ADVERTISEMENT (Source: Photosport) And the effect ripples to her whānau, she recalled the moment she had to break the news to her two children. "I wouldn't like to wish that on anybody. Just myself to be told, and then to actually tell the kids. "I was trying to hold myself to be strong, as we do as wāhine. So that was one of the hardest moments of my life. I wouldn't wish that." The rates of early onset colorectal cancer have risen by 26% per decade on average over the past 20 years, according to University of Otago researcher Dr Oliver Waddell. Researchers and Māori health practitioners have called for the age to be lowered to 45 as it was in many other countries. "If we had [earlier] screening, I most probably wouldn't be costing the country money. ADVERTISEMENT "That's how I'm looking at it. If I was able to get screened at a younger age, you just imagine how much money this government will be saving in health costs," Waaka said. Bowel cancer treatment costs depended on when it was found — stage 1 being the cheapest and stage 4 is the most expensive. For Waaka, her bowel cancer has got into her liver and aside from rounds of chemotherapy, the non-funded drug she needs comes at a cost of $30,000. To start her first round of treatment on August 11, she is having to fundraise to survive. But Waaka said it's a big ask to turn to the community for funds. "And everyone said, you know, you've given so much, it's our time, but deep inside you know everyone's struggling to survive out there, and I'm just so grateful that people are willing to help me to survive and for me to fight this fight... which I'm going to do." General practice owners are delighted that the Government is modernising the way it funds community doctors to better reflect the needs of patients. Cabinet has broadened the range of factors which influence funding from just age and sex, to include deprivation, multi-morbidity, and rurality. 'The capitation system, which determines how general practices are funded, hasn't been meaningfully updated since it was established in 2002,' says Dr Angus Chambers, Chair of the General Practice Owners Association (GenPro). 'Despite numerous reviews and the widely supported recommendations of the 2022 Sapere report, little progress had been made. This lack of action by successive governments perpetuates inequities and undermines the sustainability of primary care.' 'GenPro has been calling for a funding review for years, so hats off to Health Minister Simeon Brown for finally getting it done and implemented from July next year. 'In particular we support the addition of funding criteria such as deprivation – a marker of high health needs – and rurality, as rural areas have less access to health services under the current formula. GenPro also supports regular updates of capitation every five years, starting in 2028. 'GenPro also welcomes Minister Brown's announcement of increased funding to support general practitioners in training, and putting more structure around feedback from the sector. We invite him to work with GenPro to design a health target so we can see patients as soon as we can. 'GenPro is also pleased with the announcement of a group to advise the minister on primary care. We believe that decisions made by successive ministers have suffered from poor advice from the Ministry of Health and Health New Zealand leading to the current workforce and funding crisis affecting General Practice. Better advice will be essential to a high performing system in the future. 'These are all positive signs and, along with recent funding increases announced in the Budget, indicate improvements that will make a real difference for patients.'
