
#HEALTH: ADHD — disorder or disruptive behaviour?
He tries to wait his turn but can't stop himself from blurting out answers during lessons and interrupting his friends.
He's been told over and over again that he's naughty and not trying hard enough to be better, but he does.
He struggles every day to be like his friends but fails.
Joe is a typical case of a child with attention deficit/hyperactivity disorder (ADHD) and while symptoms and specific challenges may vary from person to person, the helplessness and the feeling of being misunderstood are a shared struggle for those diagnosed with the condition.
In 2020, the Health Ministry estimated that 1.6 per cent to 4.6 per cent of the population had ADHD.
However, like many other disorders, it tends to be under-reported, says IMU University clinical psychologist and senior lecturer Dr Serena In.
Statistics show that it is more prevalent in boys, who are three to four times more likely to have ADHD compared with girls.
However, these figures may not be entirely accurate as girls tend to adapt their behaviours in socially desirable ways, thus leading to fewer diagnoses.
WHAT IS ADHD?
ADHD is a neuro-developmental disorder that starts in early childhood. It affects how our brain develops and processes things.
"It is not a behavioural problem. What happens in the brain affects the way a child functions," says In.
There are three types of ADHD — the predominantly hyperactive-impulsive type, the predominantly inattentive type, or a combination of both.
The hyperactive-impulsive types have difficulty staying on a task, are always on the go and fidgety.
They can't sit still through a lesson, or even a movie. They may also display a lack of self-control.
For example, a typical child would be able to learn behaviours such as lining up and taking turns, but a hyperactive-impulsive type will find it very difficult to do so.
On the other hand, the inattentive types tend to forget things or lose things, are easily distracted and find it difficult to concentrate.
They don't appear to be listening well and lose interest, appear bored or seem like they are daydreaming.
"Whichever type of ADHD a child has, they are often dismissed as children with behavioural and discipline issues, and unfortunately, go through life labelled as rude, uncontrollable and naughty."
WHAT CAUSES ADHD?
Many different factors play a part in how our brains develop.
Genetics play a role, with children having a predisposition to ADHD if it runs in the family.
There are also environmental factors, such as toxins they were exposed to as children, as well as prenatal factors like medication consumed by a pregnant mother that can expose a foetus to potential hazards.
"Expert groups have also advised parents to delay screen time in children for as long as possible.
"The increasing speed of movies, constant advertisements and notifications can be overwhelming and over-stimulating, especially for young malleable brains that are still developing."
Excessive screen time also means less exercise, less quality sleep and less time for homework, which affects a child's ability to be at their best, she says.
Once a diagnosis has been made, doctors can write reference letters to advocate for the accommodation of certain needs, such as longer exam times, to be allowed sit where he or she can get up and move during class without disrupting others, or to be allowed to use fidget toys in class.
"We need to manage and support these children effectively instead of just scolding or disciplining them every time," she says.
Outside of school, psychological therapy is also essential for addressing related issues like depression or anxiety.
Therapy often involves teaching children coping mechanisms, such as expressing their emotion constructively, as well as managing behaviours by using timers and reminders that can help a child stay focused.
At home, parents can relook how they organise family life and set up routines and habits, such as structured meal times. Ensuring the house is clean, neat and orderly also helps.
"These are little routines and structures that help everyone, but they are especially beneficial for individuals with ADHD."

More protection against pneumonia with newly-approved vaccine
Young children are one of the high-risk groups for developing serious and potentially fatal pneumonia. — Canva Malaysians can now be protected against 20 strains of the bacteria Streptococcus pneumoniae , which is one of the main causes of pneumonia. This follows the recent announcement that the Health Ministry has approved the 20-valent pneumococcal conjugate vaccine (PCV20) by American pharmaceutical company Pfizer. This new vaccine offers protection against an additional seven serotypes of S. pneumoniae , also known as pneumococcus, compared to its 13-valent predecessor (PCV13). These include newer strains associated with antibiotic resistance and high death rates. Professor Dr Mark van der Linden, who heads the national reference centre for Streptococci in Germany, notes that scientists have been racing to keep up with the evolving strains of pneumococcus. 'There are currently 107 known serotypes. 'We're not creating new ones; they evolve naturally. 'Pneumococcus reproduces every 30 minutes, so it adapts far faster than humans,' he shares. The first conjugate vaccine, which was developed specifically for use in young children, targeted the seven serotypes that caused the majority of pneumococcal disease. This 7-valent pneumococcal conjugate vaccine (PCV7) was introduced in Malaysia over two decades ago. As vaccination reduced these strains, others emerged – a process called serotype replacement. 'We saw a 40% drop in cases, but new strains emerged. 'That's why vaccines must continuously evolve. 'We can't cover all 107, but expanding coverage is essential, especially for high-risk groups,' says Prof van der Linden. ALSO READ: How vaccines are developed Meanwhile, consultant clinical microbiology and infection physician Dr Teresa Wang Kin Fong from Hong Kong explains that based on data from the Chinese territory, invasive pneumococcal disease is now more common among adults than children. 'We used to give adults the polysaccharide vaccine PPV23, but it had limited effectiveness. 'Pfizer later developed PCV7, and at one point, we [in Hong Kong] implemented a combined adult programme using both PPV23 and PCV7. 'Now with PCV20, we can skip PPV23 entirely. 'PCV20 offers broader protection and simplifies the process to a single dose, making it more convenient for patients and doctors alike.' In Malaysia, vaccine uptake among adults remains low. 'This is largely due to the outdated perception that vaccines are only for children,' notes Malaysian Society of Infectious Diseases and Chemotherapy past president Prof Dr Zamberi Sekawi. Adults with chronic conditions like diabetes, high blood pressure and lung disease, are especially vulnerable to developing severe pneumonia, which can be life-threatening. 'When one family member brings home an infection, the whole household is at risk. 'Vaccinating adults helps break that chain,' Prof Zamberi adds. This is important as both young children and the elderly are also vulnerable to developing severe pneumonia. 'Children are born with immature immune systems. 'As we age, our immune system weakens – a process called immunosenescence. 'That's why both groups need the most protection,' says consultant paediatrician Prof Datuk Dr Zulkifli Ismail. Since the Covid-19 pandemic, parental concerns about vaccination have shifted from cultural or religious questions to fear of side effects. 'I've had parents who declined the pneumococcal vaccine, only to return later after their child was hospitalised. 'Sadly, by then it's too late,' shares Prof Zulkifli. Hartal Doktor Kontrak renews call to end bullying culture in public healthcare

KOTA KINABALU: A medical pressure group has recently revealed that a trainee doctor at a hospital in Tawau suffered severe mental health issues after enduring prolonged workplace bullying. According to Hartal Doktor Kontrak (HDK), the junior doctor was repeatedly targeted by the same senior colleague during his or her housemanship. The group claimed that the situation left the doctor deeply depressed and isolated, confined to his or her room for years and unable to return to the medical field. In a Facebook post, HDK shared a video showing the condition of the doctor's room, which they say reflects the long-term impact of the trauma he experienced. HDK said the junior doctor's case is one of many stories they have gathered since launching their #stopworkplacebullying campaign three years ago. They noted that some healthcare workers have suffered such extreme distress from workplace bullying that they contemplated suicide. The Health Ministry had since set up a special task force – HWCITF – to investigate workplace bullying and recommend improvements, with HDK invited to contribute input. HDK voiced support for Health Minister Datuk Seri Dr Dzulkefly Ahmad's planned launch of anti-bullying guidelines for public hospitals and clinics this October. The group said they hoped such policies would help create safer, more respectful workplaces not just within the Health Ministry, but across all sectors. Youths urged to get screened for diabetes to avoid premature death
KUALA LUMPUR: Youths are urged to undergo diabetes screening as an early measure to detect and treat the disease in order to reduce the risk of premature death. Premature death refers to passing away before the age of 75, which is the people's average life expectancy in this country. This follows the revelation by the Health Ministry yesterday that 84 per cent of young people aged 18 to 29 are unaware that they have diabetes. Universiti Kebangsaan Malaysia medical faculty public health specialist Professor Dr Sharifa Ezat Wan Puteh said it is time for diabetes screening to begin at a younger age, especially for those at high risk. "I support screening tests being conducted at a young age, as many are unaware that they already have diabetes mellitus (DM) or prediabetes. "This recommendation is also directed at those who are at high risk, such as individuals with symptoms, a family history of diabetes, those who are overweight or have central obesity (abdominal obesity)," she said. Sharifa said early screening serves as a crucial "first warning" to alert individuals to seek treatment promptly and make lifestyle changes. "This will help improve self-care management, such as practicing healthy eating, quitting smoking and striving to achieve an ideal body weight. "Subsequently, it can help prevent chronic complications such as the need for dialysis," she said. She cited a study from Cambridge University which stated that individuals with diabetes generally die 14 years earlier than those without the disease. "Early screenings is also the most cost-effective and affordable intervention compared to providing treatment when it's already too late," she added. Diabetes screening costs are not too high, said Sharifa, and can be carried out widely, unlike specific tests for the kidneys, eyes and liver, which are more expensive. "In the United States, public health screening has now been lowered to age 35. "The World Health Organisation (WHO) also recommended that screening begin at the age of 30, but it can be done at any time for those at high risk," she said. The Health Ministry revealed yesterday that 15.6 per cent of adults in Malaysia, or one in every six people, have diabetes, with two out of every five of them unaware that they have the disease. Findings from the 2023 National Health and Morbidity survey also revealed that individuals with diabetes are at high risk of developing complications such as ischemic heart disease, stroke, retinopathy, kidney disease, diabetic foot and nerve damage. These complications reduce life quality when organ damage occurs, such as blindness, amputation, heart failure, stroke with full dependency, impotence and end-stage kidney damage requiring renal replacement therapy.
